{"title":"[The physical and mental health of home care recipients: A quantitative secondary data analysis].","authors":"Azra Karabegovic, Esther Indermaur, Katharina Fierz","doi":"10.1024/1012-5302/a001008","DOIUrl":"10.1024/1012-5302/a001008","url":null,"abstract":"<p><p>The physical and mental health of home care recipients: A quantitative secondary data analysis <b>Abstract:</b> <i>Background:</i> Although more and more people are being supported by home care services, there is a lack of information regarding the health limitations and needs of this group. <i>Aim:</i> The objective of this study was to examine the health status of people with home care needs in Switzerland. <i>Method:</i> A secondary data analysis based on the HomeCareData database, which contains routine data on people with home care requirements in Switzerland, was conducted. All cases with a fully completed Resident Assessment Instrument (RAI-HC) were included. Data on various items of the RAI-HC and other standardized scale scores with reference to physical or mental health were analyzed using descriptive statistics. <i>Results:</i> In total, 74,674 data records were evaluated. Physical limitations most frequently manifested in the form of fatigue (40.6%), pain (29.7%) or within the scope of impaired hearing (21.9%). Around a third of individuals sampled had experienced a fall in the last 90 days. With regard to mental health, the most frequent signs were fear of falling (33.5%), loneliness (13.9%), depression (12.8%) and anxiety (4%). The relatives' perspective: An integrative review <b>Abstract:</b> <i>Background:</i> Unbearable states of suffering can cause a premature wish to die. Voluntary stopping of eating and drinking (VSED) offers a way to autonomously realize this wish. Relatives play a significant role in the process of realizing VESD. So far, it is unclear how relatives experience the supporting process. <i>Aim:</i> This review intends to systematically record experiences of accompanying relatives. <i>Method:</i> Within the framework of an integrative review, we conducted research in MEDLINE<sup>®</sup>, CINAHL<sup>®</sup> and PsychINFO<sup>®</sup>, as well as supplementary research. The selection was based on defined inclusion and exclusion criteria. For the synthesis, we used a thematic model. <i>Results:</i> From the four included studies, it emerged that the accompanying relatives accepted the family members' wish to die. They defended the realization of this wish with responsibility and advocacy until the death of the person wishing to die. As a result of their caring commitment, they neglected their own needs. Most relatives assessed the accompaniment as peaceful and dignified. <i>Conclusion:</i> Accompanying the VSED process represents an emotional tightrope walk with unfamiliar challenges for relatives. The results can contribute to a deeper understanding of relatives' needs. The effects on the allocation of nursing resources in clinical practice are unclear. <i>Aim:</i> In order to derive implications for future pandemic situations, we analyzed the extent to which decisions were made about the distribution of resources in acute inpatient care and how bottlenecks were dealt with. <i>Methods:</i> We conducted semi-structured interviews with nursing staff (n = 16) and nursing team leaders (n = 10) in five acute-care clinics in Germany and analyzed them inductively using thematic framework analysis. <i>Results:</i> In acute inpatient care under pandemic conditions, prioritization and rationing decisions were made, such as the postponement or reduction of nursing measures, in order to overcome pandemic-specific challenges. These decisions affected certain patient groups and certain near- and distant-patient services, in particular fundamental care measures, such as support with personal hygiene and nutrition, as well as access to acute inpatient care. The decisions were made implicitly rather than according to evidence-based criteria. <i>Conclusions:</i> Nursing prioritization and rationing decisions were made during the pandemic. A particular challenge is the education of the growing group of people with limited literacy skills. <i>Aims:</i> The aim of our project was to develop and evaluate a teaching concept that would gradually enable prospective nursing professionals to independently carry out nurse-led education for people with low literacy. <i>Methods:</i> First, a teaching concept for a nurse-led education for patients in plain language was developed, which was implemented with prospective nursing professionals of the Bachelor of Nursing programme at Charité - Universitätsmedizin Berlin and tested with a pre-post evaluation. Self-developed items were used to measure experience, confidence, competence and learning increase. <i>Results:</i> Before starting the teaching concept, 56% (n = 14) of the participants already had some experience with patient education. The relevance of the topic was rated as very high (Median = 9; IQR = 10-8; n = 25). This raises the question of how nurses organise their individual educational pathways. <i>Methods:</i> Narrative interviews were conducted with nursing professionals, which were analysed using the documentary method as a hermeneutic procedure. By reconstructing practices, it was possible to find central frameworks for organising the educational pathways, which were transferred into a typology. <i>Results:</i> The three types (1) goal-oriented design of the educational pathway, (2) flexible design of the educational pathway and (3) market-oriented design of the educational pathway show that a self-determined design of educational and career paths can be promoted but also restricted by organisational practices. <i>Conclusions:</i> Further training is not just an individual career decision. Healthcare organisations can also be seen as shapers of individual educational and career paths. Standardized instruments for the assessment of PIAD can support clinical decision-making, but no standardized instrument is yet available for German-speaking countries. <i>Aim:</i> Selection, development and agreement of appropriate items for the assessment of PIAD in newborns, infants and young children in German (<i>aIADchild Delphi</i>). <i>Method:</i> Using a Delphi technique, international items were formulated in German by a steering committee, the items were assessed by panellists using an online survey, and a consensus (I-CVI > = 75%, comments) was formed by the steering committee and panellists. <i>Results:</i> 54 experts from four German-speaking countries were recruited. 44 (1st round) and 33 (2nd round) completed the questionnaire in full. Furthermore, these facilities have been significantly impacted by the pandemic and the respective measures with detrimental effects on residents, staff, and their family members. <i>Aim:</i> This study explored issues in working in residential long-term care from the perspective of nurses and their perception on amplifying and additional effects of Covid-19. <i>Methods:</i> It applied a qualitative design focusing on experiences and views of 25 LTC nurses working in Austrian long term care facilities during the pandemic. Data were collected by conducting in-depth interviews either virtually or in presence. The data analysis was conducted by applying a thematic analysis. <i>Results:</i> The four identified themes are: (1) Work motivation, (2) Dealing with vulnerability, (3) Human Resource Planning and (4) External view on long-term care. This paper focuses on the qualitative process analysis of the development and implementation of PRN in an ICU at a German university hospital. <i>Aims:</i> Aim was to record the perception of changes in nursing practice due to the introduction of PRN and obtain information on implementation and further optimization perceived by nurses. <i>Method:</i> A qualitative process analysis was done. Data collection took place at three defined times (immediately before implementation and 6 and 12 months after) and each included a focus group interview (FG) and a 5-day ICU ward process analysis (WA) in the form of participant observation. The analysis of FG and WA was carried out according to Kuckartz's content-structuring qualitative content analysis. <i>Findings:</i> The main categories communication, care planning and integration of patients and relatives in care and 13 subcategories with a cross-sectional category could be identified. Positive effects of PRN were found, particularly concerning relatives. This study aimed to investigate the impact of the Covid-19 pandemic on the daily professional life, the psychosocial situation and the interpersonal relationships of nursing staff in old people's and nursing homes in Brandenburg. <i>Methods:</i> Cross-sectional survey of nursing staff from old people's and nursing homes in Brandenburg in the period from August to December 2020. The questionnaires were analysed using descriptive statistics and qualitative thematic analysis. <i>Results:</i> On the one hand, the increased workload with more difficult working conditions due to Covid-19 hygiene measures or increased working hours led to physical and psychosocial stress. In private life, increased social isolation was evident and changes in interpersonal relationships were stressful. On the other hand, the participants reported recognition and increased professional political interest. <i>Conclusions:</i> The Covid 19 pandemic highlighted pre-existing pressures on caregivers.
