A qualitative study <b>Abstract:</b> <i>Background:</i> Sound levels in intensive care units exceed internationally recommended limits. This can have negative effects on patients, relatives, and staff. There is a lack of evidence on noise-reducing measures. <i>Aim:</i> The aim of the study was the implementation and evaluation of ward-specific noise management in intensive care units. In the present article, the question of the noise and stress experience of intensive care patients and parents of neonatal intensive care patients was explored. <i>Methods:</i> 33 semi-structured interviews with intensive care patients and parents of neonatal intensive care patients in three intensive care units were conducted and analysed. <i>Results:</i> Sounds in intensive care units can be stressful (especially alarms) but also be interpreted as meaningful. This is not only related to the context of the ICU, but also to individual experiences and the social aspects of the sounds. In order to avoid or reduce stress, participants apply individual noise (stress) management. In contrast, ward-related noise management had no specific effect on the participants. <i>Conclusions:</i> The findings offer indications for suitable measures to reduce the noise exposure of intensive care patients and parents of neonatal intensive care patients. Existing studies have focused on personal HL, which does not entirely align with professional HL. Therefore, a new concept and questionnaire for professional HL have been developed and implemented for the first time. The results obtained for nurses in Germany are analysed with this article. <i>Research question:</i> It is asked how professional health literacy (HL) is distributed among nurses and which factors are associated with it. <i>Methods:</i> Data were collected through an online survey where nurses were asked about the difficulties they encounter in managing tasks in four dimensions: (1) information and knowledge management, (2) explaining and conveying information, (3) patient-centered communication, and (4) professional digital health literacy. Descriptive and multivariate, linear regression methods were used for analysis. <i>Results:</i> The 624 respondents rated the fourth dimension as the most difficult. The first and second dimensions were rated as similarly difficult, while the third dimension was considered least difficult. In June 2021, digital care applications (DiPA) were introduced as a new service in the statutory long-term care insurance in Germany. Their aim is to counteract the increased care need and/or support independence in the home. <i>Aim:</i> This project systematically identified and reported on national and international evidence on DiPA effectiveness, as described by the Federal Institute for Drugs and Medical Devices (BfArM). <i>Methods:</i> The systematic review was conducted in accordance with the PRISMA statement by means of systematic searches in the databases Medline and Cinahl, and supplemented by an extensive manual search (08/2022). Study quality was assessed using the Risk of Bias Tool (RoB2). The results were synthesized narratively. <i>Results:</i> Eight randomized controlled studies were included; improvements in cognition, activities of daily living, and mobility in home care patients, were reported. Study quality was low to moderate, due to mostly unblinded study designs and low case numbers. <i>Conclusions:</i> The currently available evidence fails to provide a reliable basis for assessing the benefits of DiPA, to neither DiPA manufacturers, nor to responsible parties in the healthcare system. A qualitative descriptive study <b>Abstract:</b> <i>Background:</i> To ensure orientation towards needs existing in the realm of practice, the dual competence profile is mandatory at universities of applied sciences (UAS): in addition to academic qualifications, entrenchment in professional practice is demanded. However, it is unclear how needs, attitudes, and expectation of clinical practice regarding the interface between UAS and practice looks like. Thus, it is necessary to shed light on the cross-institutional and cross-divisional cooperation as well as on its participants. <i>Aim:</i> This article shows what needs and attitudes people from clinical practice have with regard to cooperation with the UAS and what they expect from persons with a dual competence profile. <i>Methods:</i> Guided individual and focus group interviews with 24 selected representatives from acute, rehabilitation and geriatric care institutions took place. The interviews address the following five block themes: \"current state\", \"need for change\", \"areas for action\", \"needs\" and \"sustainability\". <i>Results:</i> Practice institutions demonstrated a clear need for networking and knowledge circulation with the UAS, with a bilateral influence of four subcategories: overall goal, staff development, resources and general regulations. <i>Conclusions:</i> Persons with dual competency profiles provide a valid way for interconnecting higher education and practice institutions in a concrete manner. {"title":"[The physical and mental health of home care recipients: A quantitative secondary data analysis].","authors":"Azra Karabegovic, Esther Indermaur, Katharina Fierz","doi":"10.1024/1012-5302/a001008","DOIUrl":"10.1024/1012-5302/a001008","url":null,"abstract":"<p><p>The physical and mental health of home care recipients: A quantitative secondary data analysis <b>Abstract:</b> <i>Background:</i> Although more and more people are being supported by home care services, there is a lack of information regarding the health limitations and needs of this group. <i>Aim:</i> The objective of this study was to examine the health status of people with home care needs in Switzerland. <i>Method:</i> A secondary data analysis based on the HomeCareData database, which contains routine data on people with home care requirements in Switzerland, was conducted. All cases with a fully completed Resident Assessment Instrument (RAI-HC) were included. Data on various items of the RAI-HC and other standardized scale scores with reference to physical or mental health were analyzed using descriptive statistics. <i>Results:</i> In total, 74,674 data records were evaluated. Physical limitations most frequently manifested in the form of fatigue (40.6%), pain (29.7%) or within the scope of impaired hearing (21.9%). Around a third of individuals sampled had experienced a fall in the last 90 days. With regard to mental health, the most frequent signs were fear of falling (33.5%), loneliness (13.9%), depression (12.8%) and anxiety (4%). The relatives' perspective: An integrative review <b>Abstract:</b> <i>Background:</i> Unbearable states of suffering can cause a premature wish to die. Voluntary stopping of eating and drinking (VSED) offers a way to autonomously realize this wish. Relatives play a significant role in the process of realizing VESD. So far, it is unclear how relatives experience the supporting process. <i>Aim:</i> This review intends to systematically record experiences of accompanying relatives. <i>Method:</i> Within the framework of an integrative review, we conducted research in MEDLINE<sup>®</sup>, CINAHL<sup>®</sup> and PsychINFO<sup>®</sup>, as well as supplementary research. The selection was based on defined inclusion and exclusion criteria. For the synthesis, we used a thematic model. <i>Results:</i> From the four included studies, it emerged that the accompanying relatives accepted the family members' wish to die. They defended the realization of this wish with responsibility and advocacy until the death of the person wishing to die. As a result of their caring commitment, they neglected their own needs. Most relatives assessed the accompaniment as peaceful and dignified. <i>Conclusion:</i> Accompanying the VSED process represents an emotional tightrope walk with unfamiliar challenges for relatives. The results can contribute to a deeper understanding of relatives' needs. The effects on the allocation of nursing resources in clinical practice are unclear. <i>Aim:</i> In order to derive implications for future pandemic situations, we analyzed the extent to which decisions were made about the distribution of resources in acute inpatient care and how bottlenecks were dealt with. <i>Methods:</i> We conducted semi-structured interviews with nursing staff (n = 16) and nursing team leaders (n = 10) in five acute-care clinics in Germany and analyzed them inductively using thematic framework analysis. <i>Results:</i> In acute inpatient care under pandemic conditions, prioritization and rationing decisions were made, such as the postponement or reduction of nursing measures, in order to overcome pandemic-specific challenges. These decisions affected certain patient groups and certain near- and distant-patient services, in particular fundamental care measures, such as support with personal hygiene and nutrition, as well as access to acute inpatient care. The decisions were made implicitly rather than according to evidence-based criteria. <i>Conclusions:</i> Nursing prioritization and rationing decisions were made during the pandemic.
