{"title":"Déficience intellectuelle et oncologie : les connaissances des soignants suffisent-elles à une prise en charge adaptée de la douleur chez ces patients ?","authors":"Aurélie Curnier ,&nbsp;Sébastien Mas ,&nbsp;Sarah Habib-Hadef ,&nbsp;Daniel Satgé ,&nbsp;Florence Cousson-Gélie","doi":"10.1016/j.prps.2024.06.001","DOIUrl":"10.1016/j.prps.2024.06.001","url":null,"abstract":"<div><h3>Introduction</h3><div>The life expectancy of people with intellectual disabilities is increasing thanks to medical advances, and the risk of developing cancer in this population is high.</div></div><div><h3>Objectives</h3><div>This study focuses between oncology caregivers’ training in intellectual disability and the impact this has on their daily practice, particularly in the assessment and treatment of painful symptomatology.</div></div><div><h3>Method</h3><div>Semi-structured interviews were conducted with 13 caregivers, all of whom had cared for intellectually disabled cancer patients at least once in their careers.</div></div><div><h3>Results</h3><div>Thematic content analysis revealed a lack of knowledge of intellectual disability on the part of caregivers, and a concern on the part of professionals to care for these patients. This lack of knowledge hampers the care given to these patients, and interferes with the assessment and treatment of pain.</div></div><div><h3>Conclusion</h3><div>This type of study enables us to understand the obstacles to care with this specific population, in particular the fear of caregivers of not recognizing pain and not being able to relieve it adequately due to difficulties in communicating with patients. We want to provide accuracies with the prototypical theory and by a language approach.</div></div><div><h3>Objective</h3><div>The aim of the present research is to determine whether those different persons belong to a homogeneous whole or to several categories with interchangeable (or not) members.</div></div><div><h3>Method</h3><div>Five hundred forty-two responses have been collected using the method of hierarchical evocations. We randomly presented to respondents a stimulus word (transgender, non-binary, androgynous or genderfluid). Then, participants had to give the first five words occurred to them following the presentation of the stimulus and to classify these words induced from the most to least important to the stimulus.</div></div><div><h3>Results</h3><div>Several analyses were carried out and showed that semantic categories of induced words were more or less similar between the stimuli (whole or not of categories). Next, we studied the types of word (adjectives, nouns, and verbs) which were different between the stimuli (inclusion level of categories and homogeneity). This statement is made without clear evidence that these states are indeed similar. We address this gap in the literature by explicitly linking diminished voice behaviors and the phenomenon of learned helplessness. Building on the learned helplessness literature, we explored how perceived non-instrumentality of voice in unfair working environments contributes to the emergence of helplessness feelings. We hypothesized that helplessness feelings, in turn, contribute to resignation through reduced engagement in voice behaviors and an increase in depressive symptoms, in accordance with learned helplessness theory. Respondents were 305 workers who completed an online survey of justice perceptions, voice instrumentality, helplessness feelings at work, voice behaviors, and depressive symptoms. Results corroborated the proposed framework in that perceptions of low voice instrumentality in unfair working contexts were significant predictors of workers’ helplessness feelings. Subjectivation is considered in relation to the institution. The research fields used for this article are analyzed through the prism of clinical psychology and work psychodynamics. Through two clinical vignettes, the aim is to understand how the institution influences and affects the subject. In one case, the user is confronted with the prescription issued by the structure's professionals; in the other, it is the professional who is put to the test. We then we explore the negative emotional interactional impasse in the adolescent/parent relationship. We present a sequential deconstruction of anger and its transformation into a more adaptive emotion, based on clinical cases.</div></div><div><h3>Results</h3><div>Three steps are described: (1) individual preparation to reconnect with adaptive emotions (adolescent and parents), (2) enactment in the dyadic adolescent–parent emotional and relational reconnection, (3) emotional transformation in the dyadic relationship.</div></div><div><h3>Conclusion</h3><div>EFT components enhance and deepen the emotional work in the dyadic adolescent/parent relationship. Yet, the diagnosis’ impact during this sensitive developmental period remains little explored. The aim was to assess whether self-perception and adjustment to illness differ depending on age (emerging adults versus young adults) and time since diagnosis (&lt;<!--> <!-->2 years vs &gt;<!--> <!-->2 years), and explore associations between self-perception, clinical and psychosocial variables.</div></div><div><h3>Method</h3><div>In this cross-sectional study, participants were emerging adults (18–25 years old) and young adults (26–35 years old) with Crohn's disease, recruited via an association and communities of patients online. They answered an online survey assessing their disease activity, illness perception, coping strategies, social support and self-perception.</div></div><div><h3>Results</h3><div>Fifty-two participants (18–35 years old) were included. Mann-Whitney's U test showed that compared to those diagnosed for over two years, those recently diagnosed perceive symptoms as more severe and frequent, with a greater emotional impact, and rely more on catastrophizing and distraction when coping with pain. Multiple regressions showed those reporting higher emotional response to symptoms, less perceived guidance and support in their self-esteem, present poorer self-worth. Those relying more on catastrophizing, less on ignorance, reporting severe joint pain and less concern about their disease present an altered perception of their appearance.</div></div><div><h3>Conclusion</h3><div>Crohn's disease may have a significant impact on young adults, particularly the first years after diagnosis. Transactional and self-regulation variables seem to essentially be associated with self-perception's outcomes. Boudoukha","doi":"10.1016/j.prps.2024.11.007","DOIUrl":"10.1016/j.prps.2024.11.007","url":null,"abstract":"<div><h3>Introduction</h3><div>Coercive control refers to a set of behaviors used by an individual to dominate, intimidate, or manipulate another person through various methods such as isolation, threats, surveillance, and deprivation of needs. This complex phenomenon, popularized by Stark (2007), has been the subject of studies for several decades (Dutton et Goodman, 2005; French et al., 1959; Okun, 1986). If coercive control is not detected or if it is identified too late, the insufficiently protected victim will then suffer physical and psychological health consequences. The repetition and cumulative effect of violence worsen over time, hindering victims in the judicial processing of the violence they have experienced.</div></div><div><h3>Objective</h3><div>This study aims to provide a clinical illustration of coercive control and its somatopsychic consequences in women victims of intimate partner violence perpetrated by their partner. We conducted a qualitative analysis of clinical interviews from four psychological follow-ups representing women victims of intimate partner violence followed within a psycho-trauma unit in the Île-de-France region.</div></div><div><h3>Results</h3><div>The synthetic restitution of the clinical vignettes presented highlights the establishment of coercive control early in the relationship between the perpetrator and the victim, with a range of behaviors triggering early micro-management of verbal and psychological violence, establishing a continuum from the beginning and evolving throughout the relationship, including after separation.</div></div><div><h3>Conclusion</h3><div>Training professionals in identifying mechanisms of coercive control could enable early systematic screening in the medical-psycho-social-judicial sector and propose concrete preventive actions against intimate partner violence and femicides. These were a trauma-focused cognitive behavioural therapy (CBT-T) protocol and a group protocol adapted from Eye Movement Desensitization and Reprocessing (EMDR), called G-TEP (Group Traumatic Episode Protocol).</div></div><div><h3>Methods</h3><div>The study was conducted in the Central African Republic on adult men and women with symptoms of PTSD. After a psychoeducation session about the traumatic symptoms developed following traumatic episodes, they were randomly assigned to receive either CBT-T or G-TEP treatment. Participants’ PTSD symptoms, anxiety and depression were assessed before and after 6 treatment sessions.</div></div><div><h3>Results</h3><div>The results showed that both protocols were effective in significantly reducing PTSD, depression and anxiety scores. However, the state of knowledge and practice of psychologists in French speaking Europe is still unknown.</div></div><div><h3>Objectives</h3><div>The aim of this study was to find out what these psychologists know about EBP and whether they use it in their daily practice.</div></div><div><h3>Method</h3><div>A questionnaire of 50 multiple-choice questions for French-speaking Belgian psychologists was posted online on social networks. The questions focused on psychologists’ knowledge and beliefs about EBP and the information that guides clinicians in their practice.</div></div><div><h3>Results</h3><div>A total of 110 psychologists responded to the survey. Only one third of the psychologists surveyed knew what PBA was. In their daily decision making, clinicians rely on past experience, accumulated knowledge and the difficulties presented by the patient, but scientific literature, patient preferences and opinions about treatment and expert opinions provide little guidance in their practice. Furthermore, perceived barriers, such as lack of knowledge and know-how, seem to prevent psychologists from using EBP in their daily practice.</div></div><div><h3>Conclusion</h3><div>A lack of knowledge of EBP among psychologists was revealed. Furthermore, the behaviours implemented in daily practice were not fully in line with the EBP.
