We report a case of severe corrosive injury following ingestion of chlorine bleach (200 mL), an alkaline agent. Initial endoscopic evaluation revealed severe esophageal and gastric injuries with necrosis (Zargar grade 3b), without any signs of perforation. The treatment plan included prolonged fasting with total parenteral nutrition, and enteral feeding was initiated via jejunostomy 1 month after hospitalization. The patient was hospitalized and discharged on several occasions until we observed complete healing of the esophagus and stomach. Follow-up endoscopy performed 4 months after the injury showed improvement in the esophageal wall; however, severe stenosis prevented oral intake. Therefore, the patient underwent surgical intervention, including esophagectomy, total gastrectomy, and esophagocolojejunostomy, 7 months post-injury. Since the initial surgery, the patient underwent repeat wound revision procedures, percutaneous drainage, reconstruction, and bougienation owing to anastomotic leakage, infection, and stenosis for approximately a year. The patient received oral and jejunostomy tube feeding for approximately 2.5 years after the initial injury. Early endoscopy was useful to confirm the severity and predict the prognosis of corrosive injuries in this case. Severe corrosive injuries following ingestion of alkaline substances are associated with poor clinical prognosis, as anticipated. The aim of the study was to investigate the feasibility of <i>H. pylori</i> isolation and antibiotic susceptibility testing using rapidly frozen biopsy specimens collected from various institutions.</p><p><strong>Methods: </strong>Eight institutions located in various regions of Korea participated in the study. Patients requiring upper endoscopy and <i>H. pylori</i> testing were screened. Two biopsy samples were taken from the stomach. One was placed in a sterile Eppendorf tube and then immediately placed in a vacuum bottle containing dry ice, which was stored at -80°C. The other was used in a rapid urease test. Collected samples were delivered to a central laboratory. The bacteria were isolated from the frozen samples under microaerophilic conditions. The agar dilution method was used to determine the minimum inhibitory concentration (MIC) of amoxicillin, clarithromycin, metronidazole, tetracycline, ciprofloxacin, and levofloxacin for each <i>H. pylori</i> isolate.</p><p><strong>Results: </strong>Patients with a positive rapid urease test result (n=113) were enrolled. The mean age was 56.6±12.3 years. The male:female ratio was 64:49. The overall culture success rate was 77.0% (87/113). MIC values were determined using isolated 87 <i>H. pylori</i> strains. Theoretical and experimental evidence suggests that oral microbiota may also be associated with non-oral diseases, such as gastrointestinal and extra-gastrointestinal diseases. This review highlights studies demonstrating microbial alterations in the oral cavity associated with malignant tumors including gastric, colorectal, esophageal, and lung cancers, implying that these alterations may serve as early indicators for non-invasive diagnosis and risk assessment of cancer development. Furthermore, we addressed the implications of oral microbial co-occurrence with malignant tumors, such as <i>Streptococcus anginosus</i>, <i>Fusobacterium nucleatum</i>, and <i>Veillonella parvula</i>, which are recognized as tumor-enriched oral pathogens involved in the development and progression of cancers in the stomach, colon, and lungs, respectively. In 1997, the Korean <i>H. pylori</i> Study Group (currently the Korean College of <i>Helicobacter</i> and Upper Gastrointestinal Research) designed a nationwide multicenter study to assess <i>H. pylori</i> seroprevalence and identify risk factors in the asymptomatic Korean population. Sera were collected from 2687 females and 3049 males (mean age: 29.1 years; range: 1 month to 79 years) across Korea from March 1998 to October 1998. All asymptomatic participants completed an assessment questionnaire. An enzyme-linked immunosorbent assay (GENEDIA® <i>H. pylori</i> ELISA) was performed to detect immunoglobulin G antibodies to <i>H. pylori</i>. The seroprevalence rates were 17.2% in children (neonates to 15 years) and 66.9% in adults (16-79 years). A multivariate analysis indicated that variables such as sex, age, geographic area, crowding (number of persons per room) during childhood, economic status during childhood, and types of housing during childhood were significantly and independently associated with <i>H. pylori</i> seroprevalence in adults. For children, significant risk factors included age, geographic area, family income, maternal education, and drinking water sources. Socioeconomic conditions and close person-to-person contact during childhood were significant determinants of <i>H. pylori</i> infection in adults. Additionally, the source of drinking water emerged as a crucial risk factor for <i>H. pylori</i> infection in children, indicating fecal-oral transmission in Korea. Female endoscopists experience higher rates of musculoskeletal injuries and burnout. In this study, we highlight the need for improved training systems and supportive work environments and also the effects of gender concordance on patient satisfaction and outcomes. Furthermore, we recommend the establishment of policies to enhance gender diversity in healthcare. In this study, we investigated the association between oral health and gastric cancer in Korean adults using data from the Korea National Health and Nutrition Examination Survey.</p><p><strong>Methods: </strong>We analyzed data of 79501 patients with gastric cancer and 41856805 individuals without gastric cancer (control group) using the 7th and 8th Korea National Health and Nutrition Examination Survey (2016-2019) records. Layer and colony variables and weights were used for the complex sample design. We performed logistic regression analysis of complex samples to analyze factors that affect gastric cancer development.</p><p><strong>Results: </strong>Patients with gastric cancer were older and had a higher prevalence of hypertension, hyperlipidemia, and diabetes and a higher rate of current smoking and alcohol consumption than individuals without gastric cancer (<i>p</i><0.001). Regarding oral health-related factors, the prevalence of very uncomfortable chewing difficulty was significantly higher in patients with gastric cancer (14.4% vs. 3.6%, <i>p</i><0.001). On multivariate analysis of factors associated with gastric cancer, chewing difficulty showed the highest odds ratio (5.351, 95% confidence interval 2.128-8.982). Patients with very uncomfortable chewing difficulty had high rates of previous dental nerve treatment, gum disease treatment, tooth extraction or intraoral surgery, and prosthetic repair (<i>p</i><0.001).</p><p><strong>Conclusions: </strong>Oral health-related chewing difficulties were associated with gastric cancer, which may be attributable to poor oral hygiene and degradation of oral microbiota. Therefore, as a primary prevention method, the early diagnosis of <i>H. pylori</i> gastritis and eradication of these bacteria can prevent gastric cancer. As a secondary prevention method, the early diagnosis of gastric cancer and endoscopic treatment can also contribute to reducing overall mortality. Gastric mucosal atrophy and intestinal metaplasia are important findings in patients with <i>H. pylori</i> gastritis. The accurate diagnosis of other characteristic findings is also important to comprehensively assess gastric cancer risk. The identification of precancerous lesions and early gastric cancer by evaluating endoscopic gastric cancer risk scores, based on the Kyoto classification of gastritis, is similarly important.
