Novices (n=56) and experienced HNUS operators (n=22) were then invited to take the full MCT. Based on their answers, an item-response analysis selected the MCT items with the best performance. Generalizability theory determined the number of MCT items sufficient for certification. An ANOVA test examined the MCT's ability to distinguish novices from experts and contrasting groups' standard setting was used to establish a cut-off test score. A group of physicians (n=23) was tested at a formal ultrasound course, and pass-fail consequences for the final test were evaluated.</p><p><strong>Results: </strong>Over three Delphi rounds, 64 items were revised and 21 were excluded, yielding 106 items. The item-response analysis found nine items with low discrimination that were excluded based on the MCQ answers from 78 novices and experienced physicians who had taken the test. The final 97 test items had a high internal consistency reliability of 0.97, and an MCT with 15 items was found sufficient for certification purposes. The MCT could significantly discriminate between novices (mean 51.1, SD 13.8) and experienced participants (mean 92.0, SD 3.1), p < 0.001. A pass-fail score of 83 was established. {"title":"IgG4-related disease involving distinctive areas of skull base: our experience.","authors":"Sharmistha Chakravarty, Kritika Bhardwaj","doi":"10.1007/s00405-025-09479-6","DOIUrl":"10.1007/s00405-025-09479-6","url":null,"abstract":"<p><strong>Background: </strong>Immunoglobulin G4-related disease (IgG4-RD) is a rare, systemic fibro-inflammatory condition characterized by lymphoplasmacytic infiltration with IgG4-positive plasma cells, storiform fibrosis, and elevated serum IgG4 levels. It can affect multiple organ systems, often mimicking malignancies or infections. Skull base involvement in IgG4-RD is uncommon and presents diagnostic and therapeutic challenges. We present three topographically distinct areas of skull base involvement patterns in IgG4-RD.</p><p><strong>Case presentation: </strong>This case series describes three patients with IgG4-RD affecting distinct skull base regions. The first case involves a 57-year-old female presenting with chief complaint of headache, sudden deterioration of vision in the right eye over the past two months, with ptosis and saddle nose deformity. Imaging revealed a heterogeneous multiloculated cystic lesion in intraconal compartment of right orbital apex with polypoidal mucosal thickening in ethmoid and sphenoid sinus. Biopsy done from posterior ethmoid and sphenoid mass confirmed IgG4-RD. The patient improved with corticosteroid therapy. The second case is a 50-year-old male with right-sided ear pain, decreased hearing, hoarseness of voice and facial nerve palsy for 3 months. Imaging showed a heterogeneous, enhancing soft tissue lesion involving right parotid, carotid, masticator, parapharyngeal and pharyngeal mucosal space with few lytic lesions involving petrous part of right temporal bone with adjacent sclerosis. Biopsy from nasopharynx confirmed IgG4-RD and corticosteroid therapy led to clinical improvement. Our third case is a 60-year-old male with IgG4-RD Oof nasopharynx, nasal obstruction and discharge for 3 months. Imaging revealed an ill defined soft tissue thickening in nasopharynx, bilateral Fossa of Rosenmuller and disease spreading to clivus. Biopsy from nasopharynx confirmed the diagnosis and patient started on oral corticosteroids, initially improved but the disease progressed, leading to multiorgan failure and death.</p><p><strong>Conclusion: </strong>These cases highlight the diverse clinical presentations and outcomes of IgG4-RD involving the skull base. Early diagnosis through histopathology and initiation of corticosteroid therapy is critical for improving outcomes. Furthermore, it compares the diagnostic approach and management of this case with those outlined in the limited existing literature.</p><p><strong>Methods: </strong>A 10-year-old male patient presented with progressively worsening dysphonia. Clinical evaluation included nasofibroscopy, contrast-enhanced CT imaging, and histopathological examination of resected tissue. The histological diagnosis was confirmed using Congo red staining. Additional tests were conducted to rule out systemic amyloidosis, including serum biochemistry and echocardiogram. The patient was followed for three years postoperatively.</p><p><strong>Results: </strong>The patient was diagnosed with localized laryngeal amyloidosis, confirmed through histopathological analysis. Surgical management included a partial excision using cold dissection, followed by complete macroscopic resection with CO2 laser. Postoperative recovery showed minimal dysphonia with no dyspnea. Follow-up evaluations, including nasofibroscopy, revealed no significant recurrence of disease after three years, with only a small right posterior leukoplakia, interpreted as an impact-related lesion.</p><p><strong>Conclusion: </strong>Localized laryngeal amyloidosis is an rare condition in pediatric patients. Early diagnosis is critical, and surgical management via transoral endoscopic resection is effective in improving symptoms. However, the surgical technique has not been adequately validated. The primary goal of treatment is improving health-related quality of life (HRQoL) and maintaining clinical control. The aim of this study was to assess clinical control in CRS patients more than 5 years after endoscopic endonasal surgery (EES).</p><p><strong>Methods: </strong>In this observational cohort study, 123 patients with CRS were included and they completed the Endoscopic Endonasal Sinus and Skull Base Surgery Questionnaire and updated European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) criteria. Three clinical control groups were obtained according to the EPOS criteria and corresponding EES-Q and domain scores were analyzed. Univariate analyses were performed to identify variables significantly associated with outcome.</p><p><strong>Results: </strong>Symptoms of 25.2% of CRS patients were 'controlled', 26.8% were 'partly controlled' and 48.0% were 'uncontrolled' according to the EPOS criteria. Nonsteroidal anti-inflammatory drug - exacerbated respiratory disease, allergic rhinitis, revision surgery, smoking, asthma, and nasal polyps were all significantly associated with partly controlled or uncontrolled CRS. The physical domain played the most significant role in disease control, followed by the social and psychological domain.</p><p><strong>Conclusion: </strong>According to the updated EPOS criteria, only 25% of patients had controlled CRS more than five years following EES. This emphasizes the importance of unravelling the factors that contribute to disease control to improve treatment. A significant increase in power spectra values was found within the middle-/high-frequency interval ANK when compared to CG. ANK participants demonstrated significant reduced cervical spine mobility and higher values in all CRT positions as well as significant higher values along DHI and JFLS-20 sub-item and total score.
