{"title":"Temporal trends in incidence and mortality from pulmonary tuberculosis: time series study, Sul da Bahia, 2010-2023.","authors":"Marília Caixeta de Araujo, Renata Soares Passinho, Renan Sallazar Ferreira Pereira, Delio José Mora","doi":"10.1590/S2237-96222025v34e20240778.en","DOIUrl":"10.1590/S2237-96222025v34e20240778.en","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the temporal trend of incidence and mortality from pulmonary tuberculosis in Southern Bahia.</p><p><strong>Methods: </strong>Time series study of new cases and deaths from pulmonary tuberculosis conduccted on Costa do Descobrimento and in the Extremo Sul of Bahia. The number of new cases and deaths from pulmonary tuberculosis was obtained from the Notifiable Diseases Information System, between 2010 and 2023, and used to calculate incidence and mortality. Prais-Winsten regression was used to calculate the annual percentage variation (APV) and 95% confidence interval (95%CI) and to classify trends.</p><p><strong>Results: </strong>4,005 new cases and 128 deaths from pulmonary tuberculosis were registrated during the period of the study. The average incidence was 34.91 cases per 100,000 inhabitants, and mortality was 1.11 deaths per 100,000 inhabitants. Incidence and mortality showed stationary trends. Decreasing incidence occurred in females (APV -0.01; 95%CI -0.02; -0.01) and in the age groups of 0-9 years (APV -0.02; 95%CI -0.04; -0.01), 40-59 years (APV -0.01; 95%CI -0.02; -0.01) and 60 years or older (APV -0.01; 95%CI -0.04; -0.01). Costa do Descobrimento showed a decreasing incidence for females (APV -0.02; 95%CI -0.03; -0.01) and for the age group of 40-59 years (APV -0.02; 95%CI -0.03; -0.01). The age-adjusted mortality rate variation was calculated using Joinpoint software and sociodemographic variables were analyzed using the Chi-square test, through the SPSS Statistics.</p><p><strong>Results: </strong>There were 1,071 deaths, with annual percentage change and mean annual percentage variation of -0.81 (95% confidence interval [95%CI -3.30; 1.81] p-value 0.516). The highest number of deaths occurred among women over 64 years of age (32.8%), with incomplete basic education (62.0%), light black skin (62.8%), single (32.3%), working in paid jobs (52.5%), residing in the Central-West/Northwest macro-region (53.9%), who received medical assistance (75.7%) and underwent medical tests (36.8%).</p><p><strong>Conclusion: </strong>In Mato Grosso, cervical cancer mortality presented a stationary trend in the time series. Secondary data was obtained from the Interpersonal and Self-Inflicted Violence Surveillance System, part of the Notifiable Health Conditions Information System. Cases of victims identified as \"transvestites\" and \"transsexual women or men\" were selected from the reports for 2015 to 2022. The reports were characterized according to the profiles of the victim and the perpetrator; characteristics of the violence; temporal trend for transvestite and transsexual victims; and trend by Brazilian macro-region. A descriptive analysis of absolute and relative frequencies was performed.</p><p><strong>Results: </strong>We selected 37,104 reports of interpersonal/self-inflicted violence against transvestites and transsexuals in the study period. Transsexual women (66.1%), heterosexuals (54.1%), adults (49.5%), and individuals of mixed race/skin color (44.5%) were the groups that predominantly suffered some type of violent episode. The percentage increase in the period was greater among transvestites (+182.0%). As for the time series by Brazilian macro-regions, an increasing trend in the number of reports was identified. The Southeast and Midwest regions showed an increase of 155.0% in reports from 2015 to 2022, while there was an 81.0% increase in the Southern region, 72.0% in the North and 69.0% in the Northeast.</p><p><strong>Conclusion: </strong>Over the period, more cases were found for transsexual women victims. However, from 2015 to 2022, a greater percentage increase in cases was identified for transvestites. With eligibility criteria for the clinical guidelines from the Grading of Recommendations, Assessment, Development and Evaluation system and others that addressed ICPHPs hypertension, with no language or date restrictions. Medline, Embase and Scopus were the databases consulted. The Appraisal of Guidelines for Research and Evaluation II tool was used for risk of bias assessment, focusing on methodological rigor. The results were then summarized narratively, grouping the recommendations by outcome and analyzing commonalities and differences between the included guidelines.</p><p><strong>Results: </strong>Eight clinical guidelines were included in this evidence summary, selected from 560 records initially identified. The most common interventions were meditation, yoga, breathing techniques, tai chi and mindfulness, with a positive impact on blood pressure, stress control and patients' quality of life. Perceived stress levels were assessed using the Perceived Stress Scale tool and descriptive and multivariate analyses were performed using simple and multiple linear regression. Stata, version 17.0, was used.</p><p><strong>Results: </strong>The average perception of stress among women living in Vitória was 16.4 with standard deviation of 7.5. Women aged 60 or over presented, on average, 6.0 points less stress perception compared to younger women (18 to 24 years old). The group belonging to the first quartile of family income presented, on average, 1.4 point more stress, and those receiving government financial assistance showed 1.9 more point of stress. Women without religion had, on average, 1.2 more point of stress. Annual prevalence of each marker was calculated, in addition to the following indicators: combined consumption of beans, fruit, vegetables and greens; and ultra-processed food. Prais-Winsten regression was used to calculate the annual increase rate, according to sex, age group (10-14 years, 15-19 years) and macro-regions. Increasing or decreasing trends were denoted by a positive or negative annual increase rate with p-value<0.05.</p><p><strong>Results: </strong>The prevalence rates of consumption of beans, fruit, vegetables and legumes in 2019 were 83.0%, 69.0% and 65.0%. The combined indicator of beans, fruit, vegetables and legumes was found in 48.0% of adolescents in 2019, with a rising trend in Brazil as a whole (annual increase rate: +4.0%, 95% confidence interval [95%CI 0.4; 6.6]), in the 15-19 age group and in the Southeast region. In 2019, markers of ultra-processed food consumption were frequent among adolescents (burgers and sausages: 45.0%, sugar-sweetened drinks: 65.0%, instant noodles/snacks: 46.0%, sweets: 55.0%).
