{"title":"The effect of different solutions in tracheal suctioning on the incidence of pneumonia in patients on the ventilator.","authors":"Chunyan Chai, Xiangzeng Liu, Yuan Zhao","doi":"10.14715/cmb/2022.68.2.28","DOIUrl":"https://doi.org/10.14715/cmb/2022.68.2.28","url":null,"abstract":"<p><p>Ventilator-associated pneumonia (VAP) is a common nosocomial infection in patients admitted to intensive care units (ICU), leading to prolonged stay in the ICU, increased hospital costs, and mortality. This study aimed to compare the effect of using normal saline with eucalyptus in endotracheal suctioning on the rate of ventilator-dependent pneumonia. For this purpose, a randomized clinical trial study was performed on 120 patients under a ventilator in the hospital ICU. Patients were randomly divided into control and intervention groups. The control group consisted of 60 patients who used 0.9% normal saline to dilute endotracheal secretions. The intervention group also included 60 patients treated with 0.9% normal saline with 5% eucalyptus to dilute endotracheal secretions. The sensitivity of isolated microbes was determined by the diffusion susceptibility test Kirby-Bauer disk protocol. The CDC protocol was used to diagnose VAP. 100 CFU/ml of endotracheal aspiration was considered the differentiation number between the microbe responsible for VAP and colonization. Whenever a VAP guess was made, a blood culture was done. Finally, the incidence of ventilator-associated pneumonia in the two groups was compared. The results showed that the incidence of VAP during using normal saline and normal saline with eucalyptus as a diluent for pulmonary secretions was different between the two groups (P = 0.042). Also, among infected patients with VAP, there was a difference between the types of microorganisms in the two groups (P = 0.019). Seven cases of Klebsiella pneumonia were observed in the control group, while no case of this bacterium was observed in the intervention group. In terms of the prevalence of Pseudomonas (two cases), both groups had a similar situation. From January 2018 to January 2019, 30 fresh tissues of benign epithelial ovarian tumors, 30 fresh tissues of borderline tumors, 30 fresh tissues of metastatic ovarian were collected in The First Affiliated Hospital of Fujian Medical University, and 30 fresh tissues of normal ovarian tissues were collected as the control group. Federation of gynecology and obstetrics (FIGO) staging criteria: 25 cases of stage I, 26 cases of stage II, 16 cases of stage III, and 23 cases of stage IV. The relative expression of KRT7 was detected by real-time fluorescence quantitative PCR, and the relationship between KRT7 expression and epithelial ovarian cancer grading was analyzed. The results showed that the positive expression rate of KRT7 was 12.1% in normal ovarian tissues, 28.4% in benign epithelial ovarian tumors, 53.5% in borderline tumors, and 24.2% in metastatic ovarian cancer. With the increase of tumor stage malignancy, the relative expression of KRT7 decreased significantly, but there was no significant difference between stage I and stage II, stage III and stage IV (P > 0.05). The difference between stage I and stage III, and stage IV was significant (P < 0.05). Patients with epithelial ovarian cancer had a significant difference compared with the control group (P < 0.05). In summary, compared with the control group, the expression of KRT7 in patients with benign epithelial ovarian tumors and borderline tumors was significantly decreased. The expression level of KRT7 in benign epithelial ovarian tumors was lower than that in borderline tumors. For this aim, 126 children admitted to our hospital for elective temperature-controlled radio-frequency of tonsils and adenoids from October 2020 to September 2021 were randomly divided into an observation group (n=63) and a control group (n=63). The observation group was anesthetized with propofol in combination with remifentanil, while the control group underwent propofol combined with ketamine. The mean arterial pressure (MAP), heart rate, serum C-reactive protein (CRP), interleukin-6 (IL-6), tumour necrosis factor-α (TNF-α), epinephrine, cortisol (Cor), CD3+ T lymphocytes, CD4+ helper T lymphocytes, CD8+ suppressor T lymphocytes and CD4+/CD8+ ratio were compared between the two groups before induction of anaesthesia (T1), upon intubation (T2), at the beginning of surgery (T3), at the end of surgery (T4) and 5 min after extubation (T5). -(TNF-α). The recovery time from anaesthesia and adverse reactions after extubation were observed in the two groups. Results showed that the MAP and heart rate in both groups increased significantly at T2 compared to T1, but the observation group had lower values than the control group after the maintenance of anaesthesia (P<0.05). Serum CRP, IL-6 and TNF-α levels increased with time in both groups, and the increase was considered significant (P<0.05). In addition, serum epinephrine and Cor levels gradually rose from T1 to T4 in both groups, and then decreased at T5. The difference was statistically significant (P<0.05) between any two-time points. CRP, IL-6, TNF-α, epinephrine and Cor in the observation group were significantly lower than those in the control group from T3 to T5 (P<0.05). CD3+, CD4+ and CD4+/CD8+ ratio decreased whereas CD8+ went up in both groups at T4 and T5, and which were considered statistically significant when compared with data from T1 to T3 (P<0.05). However, CD3+, CD4+, CD8+ and CD4+/CD8+ ratios did not differ statistically significantly between the two groups at each time point (P>0.05). In the observation group, the time to recovery of spontaneous respiration, the time to resumption of limb movements and the span from discontinuation of anaesthetic to extubation were all significantly shorter than those in the control group, and the incidence of agitation during the awakening period was lower than that in the control group (P<0.05). Then propofol combined with remifentanil is more effective in inflammation, stress response and immune function in anesthetizing children undergoing tonsil and adenoid surgery. Ferroptosis is an iron-dependent form of non-apoptotic cell death that has gained the worthwhile attention of both basic and clinical researchers. Ferroptosis has dichotomous, context-dependent functions both as a tumor suppressor and promoter of carcinogenesis. For this purpose, SKM-1 cells were transfected and three groups were set up according to different transfection protocols: si-NC group (NLRP3 silencing negative control plasmid), si-NLRP3 group (NLRP3 silencing plasmid), miR-223-3p mimic-NC group (miR-223-3p overexpressing negative control plasmid), miR-223-3p mimic group (miR-223-3p overexpressing plasmid), miR-223-3p mimic+oe-NLRP3 group and miR-223-3p mimic+oe-NLRP3 group (NLRP3 silencing combined with miR-223-3p overexpressing plasmid). Normal bone marrow cells were used as the control. qRT-PCR was used to detect relative expressions of NLRP3 and miR-223-3p; and Western blot to detect Ki67, Caspase-1, Gasdermin D, IL-1β, IL-18 and MMP-9 expressions. Cell proliferation detection used CCK-8 assay, cell cycle distribution detection adopted flow cytometry, and cell migration and invasion analyses relied on Transwell assay. Dual-luciferase reporter assay verified the relationship between NLRP3 and miR-223-3p. An animal experiment was finally conducted to confirm the results obtained in cells. Results showed that compared with normal bone marrow cells and K562 cells, there were significantly upregulated NLRP3 expression and upregulated expression of miR-223-3p in SKM-1 cells (all P<0.05). Compared with the si-NC group and mimic-NC group respectively, the si-NLRP3 group and miR-223-3p mimic group showed inhibited proliferation, blocked cells in the G0/M phase, reduced cells in S phase, inhibited cell invasion and migration, decreased expressions of Ki67, Caspase-1, Gasdermin D, IL-1β and IL-18, and MMP-9 (all P<0.05). NLRP3 was the direct target of miR-223-3p. Moreover, compared with the miR-223-3p mimic group, the miR-223-3p mimic+oe-NLRP3 group showed obviously increased expressions of NLRP3, Ki67, Caspase-1, Gasdermin D, IL-1β, IL-18 and MMP-9, promoted proliferation, invasion and migration, and increased cells in S phase (all P<0.05). The animal test revealed that compared with the mimic-NC+ oe-NC group, miR-223-3p mimic+ oe-NC group showed reduced tumor volume and decreased Ki67 expression (both P<0.05); while compared with miR-223-3p mimic+ oe-NC group, miR-223-3p mimic+ oe-NLRP3 group had increased tumor volume and increased Ki67 expression (both P<0.05). The rat model of HF was established, and the rats were randomly divided into model group (HF model, n=20) and ALDH2 group (intervention with ALDH2, n=20), with a normal group (n=20) set. After successful modeling, MRI and ECG were applied to detect the cardiac function indexes of the rats. The myocardial function index creatine kinase (CK) was measured, the status of myocardial tissue injury was determined using hematoxylin and eosin staining, and the apoptosis was observed via terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) staining. The activity of ALDH2 was detected, and the expression levels of genes and proteins were measured through quantitative polymerase chain reaction (qPCR) and Western blotting assay. The model group had notably decreased fractional shortening (FS) and ejection fraction (EF) and remarkably increased left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) compared with the normal group (p<0.05). The activity of ALDH2 declined obviously in the model group. The myocardial tissue injury was severer in the model group, and the number of apoptotic cells in myocardial tissues was greater in the model group than that in other groups (p<0.05). The model group manifested higher expression levels of Caspase-3 and light chain 3 (LC3) than the ALDH2 group (p<0.05) but significantly lower expression levels of PINK1, Parkin and B-cell lymphoma-2 (Bcl-2) (p<0.05). In comparison with those in the model group, the protein expression levels of PINK1, Parkin and Bcl-2 in myocardial tissues were prominently higher in the ALDH2 group (p<0.05). Forty rats were the experimental subjects. They were randomly grouped as a control group (Group C), an endotoxin group (Group E), an inhibitor group (Group I), and an activator group (Group A), with 10 rats in each group. The endotoxin-educed ALI rat model was built. Arterial Blood Gas Test (ABGT) was performed, and the Wet/Dry (W/D) ratio of lung weight was determined. The pathological variations in rat pulmonary tissues were scrutinized and scored. PMN in peripheral blood was isolated; its apoptosis was assessed, and its total NF-κB p65 and p-NF-κB p65 expressions were assessed. The expression of SNHG11 mRNA in pulmonary tissues was assessed. Results: Compared to Group C, the W/D ratios and pathological scores of Group E, Group I, and Group A boosted notably (P <0.05), while their ABGT indicators and PMN apoptosis rates dropped (P <0.05). Compared to Group E and Group I, the W/D ratio and pathological score of Group A dropped notably (P <0.05), while its ABGT indicators and PMN apoptosis rate boosted (P <0.05). Compared to Group C, the p-NF-κB p65 and SNHG11 expressions were boosted in Group E, Group I, and Group A (P <0.05); compared to Group E and Group I, the p-NF-κB p65 and SNHG11 expressions in Group A dropped (P <0.05). A role for vitamin D related to the immune system has been established. Vitamin D modulating effect is on the adaptive and innate immune responses. Our study aimed to compare serum levels of vitamin D in patients having different clinical symptoms of OLP (symptomatic or asymptomatic) with healthy individuals. Also, in this study, for further evaluation, the expression level of interleukin-17A and interleukin-6 (IL-17A and IL-6) was evaluated because the presence of active vitamin D reduces the expression of these pro-inflammatory factors. This study was included three groups with 30 volunteers in each. The first group included asymptomatic oral lichen planus patients (reticular or plaque-like lesions). The second group consisted of symptomatic oral lichen planus patients (atrophic or bullous-erosive lesions). In contrast, the third group consisted of healthy control subjects. The serum 25-hydroxyvitamin D was measured between the three groups and then correlated with clinical manifestation of oral lichen planus, either symptomatic or non-symptomatic. The Real-Time PCR technique was used to evaluate the expression of IL-17A and IL-6. Patients with symptomatic OLP (second group) had statistically significantly lower Vitamin D levels than asymptomatic OLP patients (first group). Healthy Controls (third group) exhibited statistically significantly higher vitamin D levels than OLP groups. The results of IL-17A and IL-6 genes expression showed that the presence of vitamin D had a statistically significant effect on reducing the expression of these two pro-inflammatory cytokines among symptomatic and asymptomatic OLP patients. Also, the results showed that there was a statistically significant difference between OLP patients (group I and II) and the control group (group III). For this purpose, endotracheal aspirate (ETA) specimens were sampled from ICU patients with clinically suspected VAP in the hospital between August 2020 and August 2021 and then investigated for microbiological content. This was followed by Kirby-Bauer testing for determining drug sensitivity and ERIC-PCR for genotyping. Afterward, microbial culture was performed on cleaned, sanitized and dried ventilator external ventilator pipelines and those stored aseptically for 4 weeks to evaluate the cleaning and disinfection effect and measure the bacterial content. Results showed that in the 64 confirmed VAP cases, Klebsiella was the most frequently isolated organism, followed by P. aeruginosa and Acinetobacter baumannii, while Candida is the most widely isolated fungus. The antimicrobial susceptibility spectrum revealed that 40% of the isolates were multidrug-resistant (MDR). ERIC-PCR showed no genetic relationship between pneumococcal isolates. Through microbial culture, no pathogenic bacteria were detected among cleaned and sanitized ventilator external ventilator pipelines and those stored aseptically for 4 weeks, indicating a 100% pass rate. It was concluded that ventilators in intensive care units (ICU) are susceptible to contamination, exposing patients to bacterial contamination and other comorbidities. Gram-negative bacteria are the main pathogens of VAP, which are mostly multidrug-resistant. In this paper, the detection of vaginal secretions and the positive rate of serum reproductive immune antibody in infertile women and normal pregnant women were used to explore the correlation between infertility and the two, so as to provide an effective scientific basis for the diagnosis and treatment of infertility. In this paper, 80 infertile patients (experimental group) and normal pregnant women (control group) were selected for the experiment. Real-time PCR was used to detect vaginal secretion and enzyme-linked immunosorbent assay. The results showed that the positive rates of UU and CT were 42.5% and 40%, respectively. The positive rate of the control group was 8.8% (Uu) and 7.5% (CT). In addition, the positive rates of ANA and AsAb in the experimental group were 40% and 44%, respectively, and those in the control group were 7.5% and 8.8%, respectively, with a significant difference (P<0.01).
