Intercostal neuromas are a potential source of pain in the thoracic region. In the setting of blunt intercostal trauma, intercostal neuromas should be higher on the differential.</p><p><strong>Case report: </strong>We describe the case of a 69-year-old man with severe thoracic pain for 10 years due to intercostal neuroma who had extensive medical workup over the course of 10 years prior to the diagnosis causing his chronic pain. The first-line treatment for TN is pharmacotherapy, though over 40% of patients may become refractory to medication or experience intolerable side effects. A Gasserian ganglion block is a minimally invasive procedure aimed at alleviating TN symptoms and reducing the reliance on pharmacotherapy.</p><p><strong>Case report: </strong>We present a case of TN that was refractory to 2 times Gasserian ganglion block. Antihistamines are a drug class that are not usually used for analgesia.</p><p><strong>Case report: </strong>We present the case of a 51-year-old woman who presented with low back pain radiating down the right lower extremity. Magnetic resonance imaging of the lumbar spine revealed multilevel degenerative changes, including bilateral foraminal narrowing at L5-S1. Despite conservative treatments, her pain persisted. A trial of 10 mg hydroxyzine 3 times daily led to a 50% reduction in reported pain.</p><p><strong>Discussion: </strong>Hydroxyzine is an H1 inverse agonist not commonly used for analgesia. Clinical reports suggest that a 35kDa hyaluronan fragment (HA35) may effectively relieve this pain with a single local injection. However, evidence of a complete cure is limited.</p><p><strong>Case report: </strong>Two patients were treated-one with acute shingles pain diagnosed 10 days prior, and one with postherpetic neuralgia (PHN) diagnosed a year ago. Pain scores before treatment were 7 for Patient 1, and 4 (average) and 8 (most severe) for Patient 2. Clinicians administered varying doses of HA35 based on pain level and disease duration. There is a wide variation of symptoms, but damage to the neuronal structures can result in chronic pain.</p><p><strong>Case report: </strong>A 31-year-old woman's chronic back and lower extremity pain related to NARP syndrome was successfully treated with dorsal column spinal cord stimulation (SCS).</p><p><strong>Conclusions: </strong>SCS can be used as a means of pain management in patients with genetic etiologies. Facet-mediated neck pain is suspected based on a combination of history, physical examination, and imaging findings. Third occipital nerve (TON) and cervical medial branch radiofrequency ablation (RFA) are effective and safe approaches to facet-mediated pain. Serious complications are rare but can include nerve root, spinal cord, and vascular injury.</p><p><strong>Case report: </strong>This report describes a case of subarachnoid hemorrhage (SAH) that occurred within 24 hours following TON and cervical medial branch RFA. The patient was admitted for symptom management, blood pressure control, and follow-up imaging. This particular case was managed at a specialized pain management center with access to advanced neuromodulation technologies. A 41-year-old woman with ductal mammary carcinoma and pectoral metastasis experienced severe pain in the right arm unresponsive to conventional treatments. A Freedom PNS System was implanted at the brachial plexus using a permanent surgical technique. TTS is a clinical diagnosis, but often involves needle electromyography, nerve conduction studies (NCS), and magnetic resonance imaging. To our knowledge, the use of ultrasound to diagnose tibial nerve compression from synovial cysts (SCs) in the tarsal tunnel has been sparsely reported upon.</p><p><strong>Case report: </strong>A 50-year-old male patient suffering from TTS whose diagnosis of SC-related compression was confirmed via ultrasound. Patient underwent excision confirming an intraneural ganglion cyst secondary to subtalar joint synovial fluid extension. At follow-up, the patient endorsed improvement in painful paresthesias and NCS indicated improving symmetry of tibial nerve function.</p><p><strong>Conclusions: </strong>Our report demonstrates the successful evaluation and treatment of a rare cause of TTS utilizing ultrasound as the primary imaging modality.
