It is essential to understand their knowledge, attitudes and sexual habits, as this is the objective of the study, along with tracking their evolution over time. This is the second expanded edition of a similar study conducted in the same population in 2010.</p><p><strong>Methods: </strong>A cross-sectional descriptive study based on a poll which was performed from April to June 2021. An anonymous survey was carried out with 435 adolescents plus 16 between 12 and 21 in a public school.</p><p><strong>Results: </strong>The main sources of information about sexuality (sex) were: friends (77.6%), Internet (64.7%). 82.7% had their first sexual non coital experience at the average age of 13 years. Masturbation was/is considered to be pleasant and necessary in both sexes. 35.7% state as having maintained their first sexual relations at an average age of 15 years. Main reasons: desire (65.8%), Falling in love 46.6%, curiosity (46.6%). 93.5% consider their partner's consent as being essential. 13.8% of the girls and 4.2% of the boys stated as having had suffered Sexual Abuse. A 20.8% pointed that they had witnessed it in others. A 55.7% had resorted (turned) to pornography at one moment or another. Preservative (condom) is the most known method of contraceptive (79.2%), 35% have used it once. 92.5% consider violent male chauvinism, unacceptable or negative. 88.5% believe that Sexual Education should be included as a school subject in their curriculum.</p><p><strong>Conclusions: </strong>To be worked on: To increase sexual knowledge in our youth, particularly in those fields related to the prevention on the transmission of sexual diseases and contraceptive methods. To improve their affective-sexual education, an aspect in which family can play a key role. {"title":"The association between testosterone deficiency and metabolic syndrome in middle-aged and elderly men in Korea: a comparative analytical cross-sectional study.","authors":"Nana Zhao, Xiao Han, Yonghwan Kim, Yunfei Lu","doi":"10.22514/j.androl.2025.008","DOIUrl":"https://doi.org/10.22514/j.androl.2025.008","url":null,"abstract":"<p><strong>Background: </strong>The number of elderly patients with metabolic syndrome is on the rise in Korea. A prevalent indication of aging in males is a decrease in testosterone levels. This study was conducted as a cross-sectional study to analyze the prevalence of metabolic syndrome due to testosterone deficiency in middle-aged and elderly individuals.</p><p><strong>Methods: </strong>The participants included 6118 middle-age men aged 40 to 59 and 1542 middle-aged men aged 60 to 79 (7660 participants). A testosterone level of 2.5 ng/mL or less was used as the diagnostic criterion for testosterone deficiency. The study used logistic regression to calculate the odds ratio and find the cut-off value of testosterone for metabolic syndrome. The results showed that 8.3% of middle-aged and 12.9% of elderly individuals had testosterone deficiency, and 39.3% of testosterone deficiency middle-aged and 52.8% of testosterone deficiency elderly individuals had metabolic syndrome.</p><p><strong>Results: </strong>Testosterone deficiency had a significantly increased risk of metabolic syndrome (middle-age odds ratio, 1.58; 95% confidence interval, 1.31-1.94 and elderly odds ratio 2.32; 95% confience interval, 1.71-3.13). The cut-off value of testosterone for metabolic syndrome was 4.2 ng/mL in middle-age and 3.9 ng/mL in older age. Engaging in regular physical activity has a posivive impact on metabolic syndrome, leading to a 36% decrease in the likelihood of developing metabolic syndrome in middle-aged individuals who engage in exercise and a 13% decrease in elderly.</p><p><strong>Conclusions: </strong>Middle-age and elderly individuals with testosterone deficiency had an increased risk of metabolic syndrome, and exercise had a preventive effect even if they had testosterone deficiency. High exercise and low alcohol consumption reduced metabolic syndrome prevalence even in the presence of testosterone deficiency. {"title":"Safranal ameliorates testicular ischemia-reperfusion injury in testicular torsion-detorsion rat model.","authors":"Si-Ming Wei, Yu-Min Huang","doi":"10.22514/j.androl.2024.028","DOIUrl":"https://doi.org/10.22514/j.androl.2024.028","url":null,"abstract":"<p><strong>Background: </strong>Testicular torsion-detorsion damage is a common ischemia-reperfusion injury brought on by an excess of reactive oxygen species. Reactive oxygen species may affect cellular differentiation by regulating gene expression. The <i>heat shock protein 70-2 (HSP70-2)</i> gene expression in the testis is essential for spermatogenesis. Safranal, the main bioactive ingredient isolated from <i>Crocus sativus</i> L., has potent antioxidant properties. Our current investigation examined the potential mechanism by which safranal could shield the testis from ischemia-reperfusion damage.</p><p><strong>Methods: </strong>Sixty Sprague-Dawley male rats were randomly assigned into the sham-operated control group, testicular ischemia-reperfusion group, and safranal-treated group. Testicular ischemia was achieved by twisting the left testis 720° counterclockwise and maintained for two hours. Reperfusion was created by counter-rotating the torsional left testis to its natural position. Rats in the safranal-treated group received an intraperitoneal injection of safranal at the onset of reperfusion. Testes were excised to examine the quantity of malondialdehyde (a sensitive indicator of reactive oxygen species), expression of the HSP70-2 protein, and the testicular spermatogenic activity.</p><p><strong>Results: </strong>Unilateral testicular ischemia-reperfusion significantly increased the levels of malondialdehyde in the ipsilateral testes compared to the control group. It also significantly reduced the expression of HSP70-2 protein and spermatogenic activity (<i>p</i> < 0.001). {"title":"Laptop and tablet use and their influence on total motile sperm count parameters: are laptops linked to infertility in Jamaican men?","authors":"Lanceford Sterling, Kamali Carroll, Louis-Ray Harris","doi":"10.22514/j.androl.2024.027","DOIUrl":"https://doi.org/10.22514/j.androl.2024.027","url":null,"abstract":"<p><strong>Background: </strong>This study aims to determine the impact of laptop and tablet use on total motile sperm count (TMSC) in men being investigated for assisted reproduction.</p><p><strong>Methods: </strong>A cross-sectional study was conducted on 156 men attending a fertility clinic in Jamaica. Routine semen analyses were performed and parameters specific to TMSC assessed. All data analyses were performed using SPSS Version 26. Logistic regression analyses were performed to independently predict the impact of quantifiable measures of laptop and tablet use. The main outcome measures were the parameters associated with TMSC.</p><p><strong>Results: </strong>Overall, 64% of the participants reported using laptops and 36% reported using tablets. There was a significant relationship seen with time spent on laptops and time trying to conceive (<i>p</i> = 0.015). Regression analyses showed that persons who used their laptops for 2 to 5 h daily were approximately 16 times (adjusted Odds Ratio (aOR) = 15.9; 95% Confidence Interval (CI), 2.5-103.3, <i>p</i> = 0.004) more likely to be diagnosed with low semen volume (hypospermia). Even smoking cessation is often suggested for infertility patients by clinic doctors, while the impact of smoking cessation on semen quality in patients with oligoasthenospermia is uncovered.</p><p><strong>Methods: </strong>Ninety oligoasthenospermia patients with long tobacco smoking history were directed by andrology doctors to cease smoking, and their cessation was followed up for 3 to 6 months. The changes of semen quality after cessation in patients with at least 70% cessation ratio were evaluated by Paired-<i>t</i>-test for the changes in sperm concentration (SC), sperm progressive motility (PR), sperm volume (SV), sperm acrosin activity (SAA), total sperm count (TSC), normal sperm morphology rate (NSMR) and DNA fragmentation index (DFI). The cessation index was calculated for each patient, and its correlation with changes of semen parameters was analyzed.</p><p><strong>Results: </strong>Among 90 study cases, 81 were followed up successfully. Upon andrology doctors' routine instructions, only 19 (23.5%, 19/81) achieved a minimum requirement of 70% cessation ratio. In such a cessation level, only SAA has a significant difference after smoking cessation and a significant correlation was observed between the cessation index with the changes in SAA. Inceased glutathione (GSH) level and decreased reactive oxygen species (ROS) level were found in sperm cells and seminal plasma after smoking cessation.</p><p><strong>Conslusions: </strong>Andrology doctors' routine instructions could not accomplish an ideal level of smoking cessation. The intervene of tobacco smoking cessation for patients of oligoasthenospermia only resulted in a significant improvement of SAA by ameliorating redox imbalance. {"title":"Can the severity of erectile dysfunction be predicted by the change in plasma viscosity.","authors":"Salim Zengin, Muhammet Guzelsoy, Abdullah Gul, Metin Kilic, Ozgur Ekici, Caglar Boyaci, Deniz Barali, Yunus Karakoc","doi":"10.22514/j.androl.2024.029","DOIUrl":"https://doi.org/10.22514/j.androl.2024.029","url":null,"abstract":"<p><strong>Background: </strong>We aimed to contribute to the literature by exploring the possible relationship of PV with erectile dysfunction (ED), as in atherosclerosis-based vascular diseases.</p><p><strong>Methods: </strong>Between October 2021 and December 2022, 99 patients who applied to the urology polyclinic with a complaint of ED were included in the study (Group 1). Fifty-two patients who applied with a complaint other than ED constituted the control group (Group 2).</p><p><strong>Results: </strong>While 52 participants with normal erectile function were included as controls, 17 participants had mild ED, 24 had mild-moderate ED, 31 had moderate ED, and 27 had severe ED. PV (<i>p</i> < 0.001), age (<i>p</i> < 0.001), fasting blood glucose (<i>p</i> < 0.001), glycated hemoglobin level (<i>p</i> < 0.001), and rate of coronary artery disease (<i>p</i> = 0.028) were significantly higher in Group 1. High density lipoprotein (HDL) cholesterol was significantly lower in Group 1 (<i>p</i> = 0.001). A negative correlation was found between IIEF score and PV (<i>p</i> < 0.001), whereas a positive correlation was found between PV and age (<i>p</i> = 0.023). Logistic regression analyses revealed that plasma viscosity (<i>p</i> < 0.001) and age (<i>p</i> < 0.001) were predictive of ED. The cut-off value for PV was 1.12 mPas with area under the curve (AUC) of 0.808 (95% confidence interval (CI): 0.734-0.883) and it predicted ED with 72% sensitivity and 74% specificity. The cut-off value for age was 47.5 years and it predicted ED with 56% sensitivity and 90% specificity (AUC: 0.807, 95% CI: 0.738-0.877).</p><p><strong>Conslusions: </strong>According to the results of this study, PV is higher in patients with ED, negatively correlated with IIEF score, and predictive of ED. Vitamin D3, recognized for its antioxidant and protective properties, might offer a potential protective effect against tramadol-induced testicular damage. This study observed the effects of co-administration of vitamin D3 and tramadol on serum kisspeptin levels, testicular histology, semen parameters, testosterone levels, and oxidative stress markers in male rats.</p><p><strong>Methods: </strong>Fifteen male rats weighing between 150 and 250 g were randomly divided into three groups (n = 5 per group). Group A was the control, receiving only distilled water. Group B was administered 20 mg/kg body weight of tramadol daily, while group C received both 20 mg/kg body weight of tramadol and 25 μg/kg body weight of vitamin D3 daily. The treatments were administered orally for 14 days. Post-administration body weight, relative testicular weight, serum kisspeptin levels, semen parameters, testosterone levels and oxidative stress markers (catalase, glutathione and malonaldehyde) were measured. Testicular histology was also examined using photomicrography.</p><p><strong>Results: </strong>No significant differences were observed in body weights, relative testicular weights, serum kisspeptin levels, semen parameters, testosterone levels, or oxidative stress markers among the experimental groups (<i>p</i> > 0.05). Histological analysis in the tramadoltreated group exhibited significant degradation of spermatozoa, which was not mitigated by vitamin D3 co-administration compared to the control group.</p><p><strong>Conclusions: </strong>The study demonstrates that vitamin D3 supplementation does not significantly ameliorate tramadol-induced testicular damage. We report our experience with microsurgical VR and identify predictive factors of natural pregnancy.</p><p><strong>Methods: </strong>We retrospectively reviewed all patients submitted to VR by a single surgeon from 2008 to 2021 at our single center. Patency and pregnancy rates were evaluated. The main outcomes after surgery were the patency and natural pregnancy rates. Secondary outcomes were the identification of predictive factors of success and patient satisfaction with the natural pregnancy.</p><p><strong>Results: </strong>Forty VRs were performed with a patency rate of 97.1% and, among those who became patent, pregnancy occurred in 13 of 31 couples by natural conception (41.9%). Two-layer anastomosis was significantly associated with a successful procedure (Odds Ratio of 12.428; <i>p</i> = 0.045). We did not identify a significant association between any of the other variables and a successful outcome. Even without a successful surgery, most of the patients were very satisfied with the results and would do the procedure again.</p><p><strong>Conclusions: </strong>VR is a useful technique for men previously submitted to a vasectomy and who pretend to have children by natural conception. Current therapeutic approaches often struggle to address the diverse needs of all patients. In addition, the efficacy of low-intensity pulsed ultrasound (LIPUS) in improving ED symptoms has been insufficiently investigated. Therefore, this study aims to evaluate the effectiveness of LIPUS and to assess whether combining LIPUS with biofeedback electric stimulation for pelvic floor therapy enhances treatment outcomes.</p><p><strong>Methods: </strong>We retrospectively retrieved and assessed the data of 68 patients treated at West China Fourth Hospital. Of them, 30 patients received LIPUS therapy alone, while 38 underwent combined therapy of LIPUS and biofeedback electric stimulation. Both groups completed eight treatment sessions.</p><p><strong>Results: </strong>After the treatment, the efficacy of the treatments was measured using the International Index of Erectile Function-5 (IIEF-5), Erectile Hardness Score (EHS), and Erection Satisfaction Score (ESS) after 4 and 8 treatments. Significant improvements were observed in the LIPUS-only group in IIEF-5, EHS and ESS scores (<i>p</i> < 0.001 for all measures). The positive response rate was 79.41% after eight treatments. Improvements in IIEF-5 scores were observed in both groups (LIPUS: 11.50 to 16.60; combined therapy: 10.61 to 16.90; <i>p</i> < 0.001), as well as in EHS scores (LIPUS: 2.27 to 3.07; combined therapy: 2.26 to 3.11; <i>p</i> < 0.001). However, no statistically significant differences were found between the LIPUS-only and combined therapy groups (<i>p</i> > 0.05).</p><p><strong>Conclusions: </strong>LIPUS therapy demonstrates potential for alleviating ED symptoms, and the addition of biofeedback electric stimulation for pelvic floor therapy did not result in significantly superior outcomes compared to LIPUS alone. The present study describes the surgical process and establishes a consensus for improving the care circuit for outpatient PP implantation in Spain. A working group composed of a scientific committee with extensive experience in PP implantation and representatives of important scientific societies reached a consensus about the recommendations for outpatient PP implantation. The consensus was based on a structured methodology and evidence extracted from a systematic review, literature review and clinical experience. This study details the consensus reached regarding the profile of patients who are candidates for outpatient PP implantation; the care process in the presurgical, surgical and postsurgical phases; and the quality indicators for monitoring and evaluating the quality-of-care standards for outpatient PP implantation.
