A bibliometric analysis of penile cancer research. 阴茎癌研究的文献计量分析。

{"title":"A bibliometric analysis of penile cancer research.","authors":"Peng Zhao, Chengquan Ma","doi":"10.22514/j.androl.2025.007","DOIUrl":"https://doi.org/10.22514/j.androl.2025.007","url":null,"abstract":"<p><strong>Background: </strong>The purpose of this study is to investigate the collaboration among countries, institutions and authors in the field of penile cancer research, as well as to identify the current hot topics in this area.</p><p><strong>Methods: </strong>Papers about penile cancer were retrieved from the Web of Science Core Collection (WoSCC). Bibliometric analyses were conducted using VOSviewer and CiteSpace to evaluate collaborations between different keywords, institutions, authors and countries in network maps.</p><p><strong>Results: </strong>2891 articles related to penile cancer were included. The number of publications were gradually increasing the from 1991 to 2023. Journal of Urology (196 papers) owned the highest number of publications and Journal of Urology was the most co-cited journal. Simon Horenblas was identified as the top productive author with 98 papers and received a large number of citations (5383 co-citations) over the past decades. The United States emerged as the leading publisher in this field with 932 papers published. Cooperation between countries and between institutions was observed. Involvement of testis and epididymis can occur in acute and chronic forms of systemic brucellosis. If the diagnosis and treatment is delayed, young men may suffer with complications resulting in permanent damage to reproductive system and infertility. This study gives an insight into clinical and therapeutic approaches used in brucellar endemic area and also highlight clues that can help in early detection of Brucellar epididymo-orchitis.</p><p><strong>Methods: </strong>This is retrospective, hospital record-based study. After ethical approval and record de-identification, data from Urology clinic of King Abdullah hospital, Bisha was collected for 2 years. All cases presenting with swelling or pain in scrotal region with suggestive radiological features of epididymo-orchitis and brucellar antibody titre ≥1:160 were included in our study.</p><p><strong>Results: </strong>Brucella contributed to 18 (30.51%) of epididymo-orchitis cases during study period. Young men were affected with mean age ± standard deviation (SD) 36.78 ± 7.93. History of animal contact and ingestion of unpasteurized dairy products was positive in all cases. Most common presenting symptoms were scrotal pain (18, 100%), swelling (10, 55.56%) and fever (8, 44.44%) with an average duration of 20 days. Leukocytosis with lymphocytic predominance was observed. All patients were treated medically with no complications and unremarkable follow up.</p><p><strong>Conclusions: </strong>Brucellar epididymo-orchitis must be considered by physicians in patients with prolong duration of illness, systemic symptoms and relative lymphocytosis. Despite its significance, previous studies have not adequately explored preventive strategies for DVC bleeding. This study aimed to determine whether applying an artificial erection (AE) before transecting the DVC in a rat model could effectively reduce bleeding.</p><p><strong>Methods: </strong>This study was conducted using 10 male Wistar albino rats, aged 16 weeks and weighed between 300 and 350 grams. These rats were randomly divided into two groups: one group underwent artificial erection (AE) before transecting the dorsal venous complex (DVC) (AE+, n = 5), while the other group did not (AE-, n = 5). At the end of the study, we compared the volume of aspirated blood around the transected DVC between the two groups.</p><p><strong>Results: </strong>In the AE- group, an average of 4.28 mL of blood was aspirated (range: 3.8 mL to 4.7 mL), while in the AE+ group, an average of 1.54 mL of sero-hemorrhagic fluid was aspirated (range: 1.4 mL to 1.7 mL). The difference between the two groups was statistically significant (<i>p</i> = 0.009).</p><p><strong>Conclusions: </strong>The results of this study suggest that DVC bleeding caused by DVC transection can be significantly reduced by inducing penile erection using the AE technique in rats. The aim was to quantify the prevalence and some associated predictors of EF in a large sample of the university students from the largest National University in Kazakhstan.</p><p><strong>Methods: </strong>At Kazakh National University, 1556 male students aged 16 to 23 years filled in anonymous self-administered web-based questionnaire on their sexual and reproductive behavior, including the International Index of Erectile Function (IIEF-5) in 2023. We report age-specific prevalence of erectile dysfunction (ED), defined as IIEF-5 below 22 and the association of IIEF-5 score with selected predictors in adjusted regression models.</p><p><strong>Results: </strong>At least one sexual intercourse was reported by 1306 (84%) students with the first coitus at the median age 17 (interquartile range (IQR) 17; 19) years; and 10% students reported only one ever intercourse. The median number of partners in the preceding 12 months was 1 (IQR 1; 2), but 27% reported they planned to have a child in the next 2 years (100% of 16-17-year-old and 0% of 19-20-year-old students). IIEF-5 score ranged from 21 to 25 (median 25, IQR 21; 25), and was negatively associated with age, independent of ethnicity, place of residence, number of partners and other predictors. No ED was reported by 63% (N = 827), whereas mild ED prevalence was 37% (N = 479) among students who had at least one intercourse.</p><p><strong>Conclusions: </strong>The overall prevalence of ED was very low and associated with age. Due to the existence of studies showing that there is a relationship between ABO blood group antigens and vascular diseases and that the risk of varicocele increases in men with various vascular diseases, this study aimed to investigate the association between ABO blood groups and varicocele.</p><p><strong>Methods: </strong>This multicenter study involved 6466 participants. The varicocele group included 2295 patients (1799 unilateral and 496 bilateral), while the control group consisted of 4171 individuals without varicocele. All participants visited urology outpatient clinics at 8 different hospitals in Turkey between 2018 and 2023. The groups were compared in terms of demographic data and blood groups.</p><p><strong>Results: </strong>The distribution of the blood groups in the control group and unilateral varicocele group was similar with the Turkish population. On the other hand, in bilateral varicocele group O blood group was found to be significantly lower than the control group (<i>p</i> = 0.0001), and non-O blood groups were found to be significantly higher than unilateral varicocele (<i>p</i> = 0.022) and control groups (<i>p</i> = 0.008).</p><p><strong>Conclusions: </strong>In conclusion, blood groups do not increase the risk of unilateral varicocele. However, patients without blood type O had a significantly higher likelihood of bilateral varicocele compared to those with blood type O. Accordingly, we assessed the level of seminal angiotensin II (Ang II), which is suggested to be linked to sperm motility and count, and its relation to spermatogenic activity.</p><p><strong>Methods: </strong>A prospective case-control study included three groups: Group I, consisting of 30 male patients with NOA; Group II, consisting of 30 male patients with obstructive azoospermia (OA); and Group III, consisting of 30 healthy fertile males as the control group.</p><p><strong>Results: </strong>Seminal Ang II levels were significantly lower in OA patients compared to NOA patients (<i>p</i> < 0.001) and controls (<i>p</i> < 0.001). Additionally, angiotensin II levels were significantly lower among NOA patients than controls (<i>p</i> < 0.001). However, little is known about the alleviating effect of Que on Cd-induced testicular cell apoptosis. This experiment aims to investigate the alleviating effect of Que on Cd-induced testicular cell apoptosis in rats.</p><p><strong>Methods: </strong>Twenty-four four-week-old male Sprague-Dawley rats were randomly divided into control group, Cd group, Cd + Que group and Que group. Following the assigned treatments, the testicular tissues of the rats were examined 28 days later. The levels of malondialdehyde (MDA) and reducing glutathione (GSH) in testicular tissue were measured via colorimetry, with HE (hematoxylin and eosin) and TUNEL (a TdT-UTP nick end labeling) staining employed to assess tissue damage and cell apoptosis. Total mRNA (messenger RNA) was extracted from testicular tissue using the Trizol method, and reverse transcription was followed by quantitative real-time PCR (Polymerase Chain Reaction) to measure the expression levels of relevant genes. The expression of related proteins was assessed using Western blot.</p><p><strong>Results: </strong>The data revealed that Cd exposure led to a decrease in body weight and a significant increase in MDA and GSH content in testicular tissue. In addition, histopathological examination of the tissue revealed extensive pathological changes, and TUNEL staining showed significant cell apoptosis in the tissue. Furthermore, Cd treatment promoted the expression of relevant genes, including <i>IRE1α</i> (inositol-requiring kinase 1), <i>Caspase-12</i>, <i>XBP1</i> (X box-binding protein 1), <i>GRP78</i> (glucose-regulated protein 78), <i>Bax</i> and <i>Caspase-3</i>, in the <i>IRE1-XBP1</i> apoptosis pathway. Meanwhile, the anti-apoptotic gene <i>Bcl-2</i> was significantly decreased. Their nonspecific symptomatology (urethrorrhagia, purulent discharge, or urinary retention) often delays diagnosis. The objective of this paper is to analyze the clinical and therapeutic profile of urethrocavernous fistulas through a case report and systematic literature review to establish evidence-based management insights.</p><p><strong>Case: </strong>A 22-year-old male presented with recurrent urethrorrhagia during erection and acute urinary retention secondary to clots. The initial urethroscopy and CT angiography were inconclusive. Subsequent arteriography revealed a right internal pudendal artery-cavernosal fistula. After two non-permanent embolizations failed, definitive occlusion was achieved via non-absorbable coils, resolving symptoms without recurrence or other complications at 23 months.</p><p><strong>Conclusions: </strong>Urethrocavernous fistulas are rare with limited literature descriptions. Systematic review of 17 cases reveals marked heterogeneity in the clinical presentations and the therapeutic outcomes, emphasizing the imperative for standardized diagnostic and therapeutic protocols. They are predominantly diagnosed incidentally through retrograde urethrography and urethroscopy. Conservative management with urinary diversion may be considered as the initial approach, while surgery is reserved for refractory cases or those with associated injuries. {"title":"Breaking misconceptions: assessing the quality of YouTube videos on penile fracture with validated scoring tool.","authors":"Suleyman Sagir, Mehmet Şirin Ertek, İzzettin Toktaş","doi":"10.22514/j.androl.2025.003","DOIUrl":"https://doi.org/10.22514/j.androl.2025.003","url":null,"abstract":"<p><strong>Background: </strong>The main objective of this research was to evaluate the effectiveness of YouTube videos related to penile fracture by applying established rating systems.</p><p><strong>Methods: </strong>A descriptive investigation was carried out by browsing YouTube for videos related to \"Penile fracture\". Out of a total of 108 videos that were found, 47 were deemed eligible for inclusion in the analysis. The features and substance of the videos were assessed utilizing the adjusted DISCERN tool, the criteria from the Journal of the American Medical Association (JAMA), and the Global Quality Scale (GQS). Statistical analysis was performed using SPSS 22.0 software, with a significance level of <i>p</i> < 0.05.</p><p><strong>Results: </strong>High-quality videos, as classified by GQS, had more views, likes, and comments than lower-quality videos; however, these differences were not statistically significant (<i>p</i> > 0.05). In contrast, videos with detailed and accurate content had significantly higher view counts (approximately tenfold) and more likes (approximately twofold) than poor-content videos (<i>p</i> < 0.05). A strong positive correlation was found between content analysis scores and DISCERN (<i>r</i> = 0.815, <i>p</i> < 0.001), JAMA (<i>r</i> = 0.781, <i>p</i> < 0.001), and GQS (<i>r</i> = 0.722, <i>p</i> < 0.001). Videos made by urologists and surgeons demonstrated superior quality across all measured criteria in comparison to those crafted by individuals without healthcare backgrounds (<i>p</i> < 0.001).</p><p><strong>Conclusions: </strong>The quality of YouTube content addressing penile fracture exhibits a wide range, with content generated by healthcare experts standing out for its high quality. {"title":"Meta-analysis of the efficacy and safety of L-carnitine and N-acetylcysteine monotherapy for male idiopathic infertility.","authors":"Xiaohong Ma, Yingying Yang, Shiguo Liu, Yuanshan Cui, Jitao Wu","doi":"10.22514/j.androl.2025.004","DOIUrl":"10.22514/j.androl.2025.004","url":null,"abstract":"<p><strong>Background: </strong>Oral antioxidants especially L-carnitine (LC) and N-Acetylcysteine (NAC) are commonly used as the drug treatment method for idiopathic male infertility (IMI).</p><p><strong>Methods: </strong>Randomized controlled trials (RCTs) of LC and NAC monotherapy for IMI were searched systematically by using MEDLINE, EMBASE and the Cochrane Controlled Trials Register. The reference lists of retrieved studies were also perused. We analyzed the sperm concentration, normal morphology, sperm motility, and ejaculation volume.</p><p><strong>Results: </strong>Seven Randomized controlled trials were included. Four trials compared the efficacy of LC with placebo, and three trials compared the efficacy of NAC with placebo. In the efficacy analysis, LC increased sperm concentration (<i>p</i> < 0.001), normal morphology (<i>p</i> = 0.03), and sperm motility (<i>p</i> = 0.02); NAC improved the first three indicators while also increasing ejaculation volume (<i>p</i> = 0.002).
