{"title":"The Spiritual and Religious Dimensions of Holistic Perinatal Care.","authors":"Kalliopi Papadopoulou, Theodoros Theodoridis, Francine Jellesma, Vasileios Kalliakmanis","doi":"10.1177/08980101261456473","DOIUrl":"https://doi.org/10.1177/08980101261456473","url":null,"abstract":"<p><p><b>Purpose:</b> The purpose is to integrate existing knowledge about spiritual needs and interventions during the perinatal period, including cultural and contextual variation, to inform how spirituality may be further integrated<b>.</b> <b>Study Design:</b> Critical narrative literature review evaluated against the Scale for the Assessment of Narrative Review Articles (SANRA) criteria. <b>Analysis:</b> The thematic synthesis was conducted to identify analytical themes and evidence levels across diverse cultural contexts. <b>Findings:</b> The review was based on 34 diverse sources (ranging from randomized controlled trials [RCTs] and systematic reviews to phenomenological and ethnographic studies). Synthesis revealed a dual nature of spirituality, in which it can act as a protective factor (contributing to meaning-making and coping) or, conversely, increase stress when interpreted as divine punishment or when spiritual needs go unmet. <b>Conclusions:</b> Recognizing and integrating spirituality into perinatal care contributes to holistic, person-centered midwifery and nursing. Reframing ACP as a holistic, relational nursing practice may enhance its integration into acute care. Guided by Watson's Theory of Human Caring, the Brief Care Preferences Conversation Framework (BCPCF) was developed as a theory-informed nursing intervention to facilitate value-centered, whole-person conversations in time-limited encounters. This study aimed to develop and evaluate the feasibility and clinical utility of the BCPCF in the ED. Development included literature review, expert consultation, and iterative pilot testing. Final evaluation used a practice innovation approach incorporating field observations, workflow assessment, and clinician feedback. The BCPCF supported structured yet flexible conversations integrating physical, psychosocial, and spiritual dimensions of care, with an average duration of 25.3 min and minimal workflow disruption. Beyond feasibility, it facilitated narrative expression, meaning-making, and identification of patient-defined quality-of-life priorities. Documentation met Centers for Medicare and Medicaid Services standards and informed clinical decision making. Psychological and spiritual processes contribute to stress regulation and adaptive coping; however, quantitative synthesis of psychospiritual interventions within holistic nursing remains limited. This meta-analysis evaluated the effects of psychospiritual interventions on systolic blood pressure (SBP) and diastolic blood pressure (DBP) in adults with hypertension. A systematic review and meta-analysis of randomized controlled trials was conducted following PRISMA 2020 guidelines using databases covering 2000-2025. Twelve studies were included in the qualitative synthesis, with six studies analyzed for SBP and five for DBP. Psychospiritual interventions were associated with a significant reduction in SBP (SMD = -0.91; 95% CI: -1.54 to -0.29; <i>p</i> = .004), but not DBP (SMD = -0.20; 95% CI: -0.77 to 0.36; <i>p = .</i>48). Health care provisions should be grounded with respect to the patient and characterized by mutual trust, respect for autonomy, maintenance of confidentiality, and delivery of the highest standard of care. The aim of this study was to examine the perceptions of individuals with gender dysphoria and TGD individuals among professionally active nursing staff, as well as undergraduate nursing and midwifery students.MethodsThis study was conducted via the diagnostic survey method. The research instrument was a questionnaire comprising a sociodemographic section and items assessing knowledge of and perceptions of transgender identity and gender diversity.ResultsThe level of knowledge regarding transgender identity and gender diversity was most frequently assessed by respondents as satisfactory (31%). More than half of the participants (65.1%) reported knowing a transgender or gender-diverse individual, and 37% declared having provided care to such a patient. According to the majority of respondents (82.9%), TGD individuals constitute a group at risk of discrimination and stigmatization, and 56.2% indicated that the Polish health care system does not adequately recognize or respond to their health-related needs. Most respondents (75.8%) reported perceiving TGD patients in the same way as all other patients did, with perceptions being primarily dependent on individual personality traits (82.6%). At the same time, respondents indicated that TGD individuals constitute a group with particular needs related to mental health care (89%) and gender-related health care (60.9%).ConclusionsPerceptions of transgender and gender-diverse individuals among the surveyed nursing personnel were heterogeneous and dependent on sociodemographic variables and levels of knowledge. Within transcultural nursing, integrating spirituality into mindfulness may enhance cultural congruence and support holistic care. This systematic integrative review synthesized interdisciplinary evidence on spiritually integrated mindfulness interventions and their relevance for culturally congruent nursing practice. Guided by PRISMA, 28 studies published between 2015 and 2025 were analyzed. Findings indicate consistent improvements in spiritual well-being, psychological distress, coping, and meaning-making across diverse cultural contexts, particularly within Buddhist, Islamic, and Christian traditions. Spiritual mindfulness operates through interconnected mechanisms, including emotional regulation, spiritual connectedness, relational awareness, and compassionate engagement. Integrative synthesis informed by Watson's Theory of Human Caring and CIMO logic suggests that culturally congruent spiritual mindfulness practices facilitate meaning-centered adaptation and person-centered healing. Scales measuring spiritual struggle, moral injury, post-traumatic growth, turnover intention, and burnout, as well as one item assessing job satisfaction and demographic questions, were administered. Univariate, bivariate, and multivariate statistical analyses were conducted. <b>Findings:</b> The participants (<i>n</i> = 244) included 30% men and 83% who identified as Roman Catholic. Moral injury and spiritual struggle were associated with higher levels of burnout and increased turnover intention. Post-traumatic growth was negatively associated with job satisfaction and turnover intention, but unrelated to burnout. Age, work setting, and hours worked per week were associated with some nurses' spiritual responses. <b>Conclusion:</b> Nurse spirituality can be expressed through spiritual struggle, moral injury, and post-traumatic growth. These spiritual responses are related to work outcomes. Data were collected through semistructured in-depth interviews and analyzed using Colaizzi's seven-step phenomenological method.FindingsNine themes were identified, covering pain management, safety and environmental needs, daily care, privacy and respect, psychological support, rehabilitation guidance, continuity of care, caring attention and support, and communication. Beyond these specific needs, the findings revealed a core lived experience of vulnerability, dependence, and threatened dignity during hospitalization. Patients sought not only symptom relief, but also humanistic care that helped them feel respected, understood, supported, and gradually move toward healing.ConclusionsOrthopedic inpatients have multidimensional humanistic care needs that reflect a holistic caring experience of body, mind, and spirit. Anxiety was measured using a validated scale. Paired <i>t</i>-tests and effect sizes assessed pre-post changes.FindingsAnxiety scores decreased significantly from 22.3 ± 3.1 at baseline to 7.7 ± 2.1 after the intervention, indicating a large effect size (Cohen's d = 0.90). While reductions were substantial, causal inference is limited due to the absence of a control group. Ninety-one percent of participants transitioned to the \"no anxiety\" category, with improvements across all cognitive, somatic, and autonomic subdomains. Participants also reported enhanced psychospiritual comfort, consistent with the domains of relief, ease, and transcendence.ConclusionsA brief, spiritually integrated nursing intervention using Dhikr Istighfar promotes immediate reductions in preprocedural anxiety and supports psychospiritual well-being, reflecting the holistic principles of mind-body-spirit care. Key antecedents include access to basic needs and services, social and community support, cultural familiarity and integration, legal status stability, and culturally competent services. Consequences include improved well-being, social integration, reduced reliance on social services, intergenerational bonds, empowerment, and cultural identity preservation. <b>Conclusions:</b> Resourceful aging among migrants and refugees involves adapting to aging in a new country by embracing changing circumstances and utilizing available resources, enabling well-being, personal agency, and resilience. This process preserves cultural heritage while adapting to new environments, balancing adaptation with identity.
