Can the APACHE II score be used to assess the risk of mortality in the elderly with hip fractures?

The aim of this study was to establish the capacity of the APACHE II score to predict the survival of patients with hip fracture, in people aged over 65 years, and compare it with the existing score used to predict the survival of these patients.</p><p><strong>Methods: </strong>This was a prospective, cohort study conducted in a tertiary care teaching hospital including 410 consecutive patients with hip fracture aged ≥65 years. As part of the preoperative preparation, general health of the patients was classified according to the ASA score. The Charlson Comorbidity Index (CCI), the Nottingham Hip Fracture Score (NHFS), the POSSUM-P and the APACHE II scores were analysed.</p><p><strong>Results: </strong>The sensitivity and specificity of the P-POSSUM score were 78.3 and 73.0, respectively, which was better than the APACHE II whose sensitivity and specificity were 56.6 and 89.7. The area beneath the ROC curve for P-POSSUM was 0.809, and for APACHE II, 0.803. However, there was no statistically significant difference between the APACHE II and P-POSSUM scores. University education lacks research programs, hindering students from developing critical evaluation skills.</p><p><strong>Methods: </strong>This cross-sectional study examined knowledge, attitudes, experience and perceived barriers towards scientific research among biomedical students and recent graduates at the University of Sarajevo in B&H. A self-reporting questionnaire was distributed via social networks and among students from all years of the Medical, Dentistry, Pharmaceutical faculties, the Faculty of Health Sciences, and the Faculty of Science, as well as recent graduates.</p><p><strong>Results: </strong>The survey was completed by 243 participants. The mean knowledge score of 4.3/11 was noticed; 216 (96%) believed research was necessary for healthcare improvement. Although 147 participants (65.3%) attended courses on research methodology, only 63 (28%) engaged in scientific research at their faculties. Only 5 (8.5%) employed graduates participated in research at their jobs. Almost half, 99 (44%) participants consulted scientific papers only upon recommendation. The majority of participants, 199 (82.2%), reported they experienced barriers to conducting research in B&H.</p><p><strong>Conclusion: </strong>Biomedical students and graduates demonstrated limited research knowledge, but had positive attitudes and acknowledged significant barriers. Group one (G1) stands for the negative control group. The second one (G2) was injected with gentamycin only, while the third one (G3) was given an Arabic gum orally 10g/kg. The 4th group (G4) was injected with gentamycin for 8 days, and then followed by Arabic gum orally for 8 days. The fifth group (G5) was injected with gentamycin plus oral administration of Arabic gum at the same time. A scoring system (+1-+4) was used to grade the histological features in the kidneys.</p><p><strong>Results: </strong>The results revealed a significant increase in body weight in G3 group only; this group presented a significant decrease in the level of both urea and creatinine. Group G2 showed a marked elevation in both serum urea and creatinine. Acute tubular necrosis (+4), in both G2 and G5. In G3, kidney sections presented mild cell swelling of the tubular epithelium in addition to the normal architectures (score +1), while G4 sections revealed tubular hyalin cast, expansion of glomerular tufts.</p><p><strong>Conclusion: </strong>The research indicates that gentamycin leads to kidney damage in mice. {"title":"Coexistence of aerobic vaginitis caused by Enterococcus faecalis and human papilloma virus (HPV) infection as a risk factor for the development of cervical intraepithelial lesions.","authors":"Amna Baraković, Mahira Jahić, Anis Cerovac, Fatima Numanović","doi":"10.17392/1846-22-01","DOIUrl":"https://doi.org/10.17392/1846-22-01","url":null,"abstract":"<p><strong>Aim: </strong>To determine the prevalence of aerobic vaginitis (AV) caused by Enterococcus faecalis (E. faecalis) in human papilloma virus (HPV)-positive women with pathological Pap test and to determine the most prevalent HPV type associated with E. faecalis infection.</p><p><strong>Methods: </strong>This prospective study was conducted at the Gynaecology Centre \"Dr. Mahira Jahić\" Tuzla and Primary Health Care Centre Tešanj (Bosnia and Herzegovina) in the period between February 2023 and March 2024. The research included 200 women aged 25 to 50 years. The examined group consisted of 100 women with a pathological (examined group) and 100 with a normal (control group) Pap test result.</p><p><strong>Results: </strong>Pathological Pap smears were found in 60 (out of 100; 60 %) women in the examined group: cervical intraepithelial neoplasia (CIN) 1 and CIN 2 in two women, respectively, CIN 3 in seven, atypical squamous cells of undetermined significance (ASCUS) in 29 and atypical squamous cells-high-grade cannot be excluded (ASC-H) in two women. Overall (both groups) prevalence of E. faecalis was 25.5% (51women); in 45 (22.5%) women E. faecalis was the only bacterial isolate, of which 42 (21%) in the examined group and three (1.5%) in the control group. High-risk HPV types were found in 62 (out of 100; 62%) women with the pathological Pap smear test. An inductive thematic analysis generated themes and subthemes.</p><p><strong>Results: </strong>Sixteen studies were included in this research. The three main themes were access to healthcare, professional treatment, and feeling significant in care. Key findings revealed that immigrant women struggled with trauma, difficult interactions with midwives, communication issues, interpreter problems, lack of detailed information, the role of doulas, and future concerns.</p><p><strong>Conclusion: </strong>Immigrant women's experiences of Swedish maternity care were marked by information gaps, ignorance, and disrespect, leading to mistrust and delayed help-seeking. Language barriers with midwives caused misunderstandings. A sense of belonging to Swedish society was crucial for a positive experience. Upper respiratory tract infections (URTIs) were in a significantly negative correlation with the frequency of training on weekdays (p=0.041) and weekends (p=0.005). Lower respiratory tract infections (LRTIs) had a significantly positive correlation with the frequency of usage of electronic gaming devices on weekdays (p=0.034), and a significant negative correlation with the frequency of spending time in nature on weekends (p=0.036). ARIs in total were in a significant negative correlation with the frequency of training on weekdays (p=0.025) and weekends (p=0.005).</p><p><strong>Conclusion: </strong>An increase of SB and reduced PA level showed a negative impact on the ARIs increase. Platelet concentrates obtained by apheresis have better content in terms of better functionality and viability of platelets, reduced risk of infection with blood transmissive viruses and HLA-sensitization. The aim of the study was to examine the influence of donors' predonation haematocrit on the value of quality control parameters in the plateletpheresis product, to determine the frequency of adverse events and its relation to the predonation haematocrit value of donors.</p><p><strong>Methods: </strong>A retrospective analysis included the total number of performed plateletpheresis in the period between 1 January 2022 and 31 December 2023 at the Polyclinic for Transfusion Medicine in University Clinical Centre of Tuzla. Two groups of platelet donors with haematocrit <45% and haematocrit ≥45% and quality control parameters in both groups were compared.</p><p><strong>Results: </strong>A total of 73 donors with Hct <45%, and a total of 43 donors with Hct ≥45% was found. The total volume of processed blood was significantly higher in the group of donors with Hct ≥45% (p<0.0001). Prevalence and risk factors of obstetric violence among Jordanian women: a cross-sectional study.

Recruitment occurred in public maternity and children's health centres in the Irbid Directorate in Jordan. Five hundred and fifteen women were enrolled in the study as a convenient sample. An online questionnaire comprised socio-demographic, obstetric and clinical variables and obstetric violence measurement.</p><p><strong>Results: </strong>The prevalence of physical, verbal, and psycho-affective violence was reported in 390 (75.7%), 154 (29.9%), and 191 (37.1%) cases, respectively. Mode of delivery, using induction, and complicated delivery were significantly associated with physical violence (p<0.05). Place of last delivery (OR= 0.51 95% CI: 0.3-0.86), complicated delivery, and primary healthcare provider were associated with exposure to verbal violence (p<0.05). Psycho-affective violence was significantly associated with the mode of delivery, complicated delivery, and primary healthcare providers (p< 0.05).</p><p><strong>Conclusion: </strong>The findings of this study, which showed a high prevalence of physical, verbal, and psycho-affective violence, underscore the urgent need to create possible solutions that guarantee the global goal of respectful maternal treatment. A total of 102 patients in the epilepsy group and 102 in the control group participated in this study. We conducted the data collection approach in three stages: a questionnaire, anthropometric measures and biochemical analysis.</p><p><strong>Results: </strong>Epilepsy patients were more likely to develop metabolic syndrome than the controls. The epilepsy group exhibited higher triglycerides, fasting blood sugar, and lower high density lipoprotein (HDL) values than the control group. The epilepsy group exhibited sedentary behaviour, while the control group had moderate to intense physical activity (plt;0.05). Epilepsy patients who take polytherapy, or several antiepileptic drugs, were 8.43 times more likely to develop metabolic syndrome (OR=8.43; 95%CI=1.45-32.12). Epilepsy patients with body mass index (BMI) >25 had 1.57 times the risk of metabolic syndrome (OR=1.57; 95%CI=1.16-2.34). Epilepsy patients with total cholesterol levels >200 mg/dL have a 5.81-fold higher risk of metabolic syndrome (OR=5.81; 95%CI=1.23-23.32).</p><p><strong>Conclusion: </strong>Our study found that more than a quarter of epilepsy patients have metabolic syndrome. Electrocardiogram (ECG), point of care ultrasound (POCUS) of the heart and tripple rule out computed tomography (CT) were performed to differentiate the cause of acute chest pain.</p><p><strong>Results: </strong>ECG showed no ischemia. POCUS of the heart showed aneurysmal ascending aorta (4,5 cm), suspected intimal flap, mild aortic regurgitation, minimal pericardial effusion, right heart dilatation with impaired function. CT confirmed TAAAD with extensive intramural hematoma around the central pulmonary arteries propagating to intrapulmonary branches. Additionally, there was dilatation of the right ventricle and right atrium indicative of right heart pressure overload along mild haemorrhagic pericardial effusion anterior to the right ventricle. No evidence of pulmonary embolism was found. Emergency surgery included the replacement of the supracoronary ascending aorta and aortic hemiarch.</p><p><strong>Conclusion: </strong>TAAAD is a rare but life-threatening cause of chest pain often mimicking other conditions such as pulmonary embolism. Pulmonary complications including intramural hematoma of the pulmonic trunk due to its shared adventitia with ascending aorta are rare but clinically significant, causing lumen reduction or occlusion of pulmonary arteries, leading to right heart pressure overload.
