Kirov","doi":"10.5272/jimab.2024303.5658","DOIUrl":"https://doi.org/10.5272/jimab.2024303.5658","url":null,"abstract":"Introduction: Despite the progress of additive manufacturing, there are different opinions regarding microbial adhesion on the surface of 3D printed dentures and the biocompatiability of the materials. The purpose of this in-vivo study was to determine the presence of Candida species in the saliva, oral mucosa and on dentures of patients wearing heat-cured polymethylmethacrylate (PMMA) and 3D-printed prosthetic appliances after a three-month period of observation. Materials and methods: The clinical study included 40 edentulous patients, divided into two groups - first group (A) –wearing conventional dentures and second group (B) – wearing 3D printed dentures. It was used chrom agar Candida (Bio Merieux) for identification and quantity assessment of the presence of Candida spp. and spectrophotometric analysis using VITEK MS (Bio Merieux). Results: After three months of wearing the dentures, Candida spp was not isolated in the saliva in 60% of patients in group A and 70% of group B, with no statistically significant difference between the results before the treatment and after the third month. In the third month, Candida spp was isolated from the oral mucosa of 20% of group A and in group B – 10%, with no significant difference between the groups (p<0.05). No Candida spp was isolated from 90% of denture surfaces in both groups. Conclusion: The adhesion of Candida spp. to 3D printed dentures is not higher than conventional dentures made from heat-curing resin. Material and methods: Material: 15 men (mean age 25.16) were observed in the span of 14 weeks (three and a half months) after anterior cruciate ligament reconstruction. They underwent physiotherapy, including analytical kinetic chain exercises in hospital, ambulatory and at home conditions. Research, measurement, and evaluation methods include range of motion in the knee joint, manual muscle testing (MMT), static and dynamic balance tests, and postural control tests. Statistical methods include statistical data grouping, Cronbach’s test, descriptive method, testing of statistical hypotheses, and correlation analysis. The data was processed with SPSS v.24.0 for Windows. Results: The results show a change in the static postural control with a 1/4-point deviation from the standard. This requires the inclusion of additional sensorimotor exercises in combination with other specific and general strengthening exercises. Conclusion: From the performed observation and the obtained results, compared to those of 20 healthy men, the following conclusion has been drawn: the analytical exercises for lower extremities lead to the overcoming of dysfunction and increased stability in the extremities. Correct diagnosis is crucial. The purpose of this study was to apply Femoflor-16 to study the vaginal microbiome in women with genital tract discharge. Material/Methods: A total of 45 women were included in the study. Vaginal/cervical swab - one for routine tests and Gram staining and another for PCR Femoflor-16, were tested. Clinical diagnosis of bacterial vaginosis (BV) was done by using Amsel's criteria. Microbiologically, BV was assessed by applying a Nugent score system and Femoflor-16. Results: A total of 45 women with genital tract discharge were included in the study. Based on Amsel's clinical criteria, 9 (20%) of them were diagnosed with BV. Based on Nugent's score system, 11(24,4%) were categorized as having BV. Only 40 samples were compared, and the results were tested using Femoflor-16 and Nugent score system. Of them, 33 (83%) were in agreement as a result of using both methods. Femoflor-16 detected species such as aerobic bacteria (Enterobacteriасeae, Streptococcus spp., Staphylococcus spp.), Mycoplasma spp., Ureaplasma spp. and yeast-like microorganisms (Candida spp.). Conclusions: A reliable, comprehensive, and on-time diagnosis of BV is needed. Nucleic acid amplification tests nowadays complement the \"classic\" laboratory methods. The real time multiplex PCR test Femoflor-16 can be effectively used in vaginal microbiota evaluation in women with discharge. {"title":"TABES DORSALIS: IS THE DIAGNOSIS A CHALLENGE NOWADAYS?","authors":"M. Danovska","doi":"10.5272/jimab.2024303.5642","DOIUrl":"https://doi.org/10.5272/jimab.2024303.5642","url":null,"abstract":"Purpose: To present a rare case of neurosyphilis, the tertiary form of syphilitic infection, following adequate but belated treatment and manifesting with a variety of clinical signs and further progression to tabetic neurosyphilis. Known as the \"great imitator\", neurosyphilis is often misdiagnosed and underestimated. Materials/methods: A 48-year-old male patient was admitted to the Neurology clinic of UMHAT \"Dr Georgi Stranski\" Pleven, Bulgaria, with progressive neurological deficit including numbness in the lower limbs, imbalance and wide-based gait. The T2W- MRI showed abnormally high longitudinal signals at Th12-L1 level, consistent with transverse myelitis. CSF analysis revealed leucocytic pleocytosis, elevated proteins and normal glucose. The nerve conduction studies confirmed sensory motor polyneuropathy. Results: After treatment with intravenous aqueous crystalline (IV) penicillin, 2 million intravenously every 4 hours for 20 days, the sensory symptoms reduced. The coordination disturbances persisted, but the gate showed moderate improvement. Conclusions: The diagnosis of syphilitic myelitis, known as tabes dorsalis, is always complex, not only because of its rarity but because of its unique clinical presentation with multiple clinical symptoms, mimicking other more common neurological disorders. Interstitial pregnancy has an incidence of 2-4% of all ectopic pregnancies. We present a clinical case of interstitial pregnancy. On 20.10.2023, a woman visited the University Hospital \"St. Marina\" - Pleven, Bulgaria, because of a positive morning pregnancy test. The patient was a 30-year-old woman. The last menstrual period was on 12.09.2023. Two consecutive vaginal ultrasonography were made every 10 days. No fetal sac was visualized in the uterine cavity. The second ultrasonography visualized a hyperplastic endometrium, 21 mm thick. A fetal sac located interstitially was found high in the right uterine horn. The ß-hCG values were 412 mIU/ml and 12490 mIU/ml, respectively. The patient was referred for a pelvic MRI, which confirmed the diagnosis. After discussion and informed consent signed by the patient, on 30.10.2023 at University Hospital \"St. Marina\" - Pleven, Bulgaria, under general anesthesia on 30.11.2023, a Laparotomia a modo Pfannenstiel. Resectio cornualis dextra. Extirpatio graviditas interstitialis dextra. Salpingectomia dextra. Sutura uteri. Lavage. Drainage. Abrasio probatoria was performed. The patient tolerated the surgical intervention well. The histological result proved interstitially located ectopic pregnancy. Material/Methods: The opinion of nurses working in therapeutic, surgical and intensive care units was studied (n=128). Questionnaires were used to assess the type of aggression displayed by the patient and the nurses' behavior. Documentary, sociological and statistical methods were used. Results: Verbal aggression by patients or their relatives occurs more often than physical aggression. Nurses indicate that they accept the patient's aggression as part of the nature of work, not paying attention to the patient's aggression (62.5%). The strategies applied by nurses after being the object of aggression were mainly sharing with family members, using food products, smoking, keeping silent/not discussing the event. The emotional feelings that aggression in the workplace causes are apathy, irritation/anger, and desire to leave as a reaction to aggressive behaviour. A relationship was established between the support received from the team and job satisfaction. Conclusion: Workplace aggression has a negative impact on the quality of professional activity and professional satisfaction. It is a negative factor in providing health care. In cases of closed femoral fractures, artery damages could be missed due to their rarity, as surgeons might not anticipate them on the first encounter with the injured person. Damage Control Orthopaedics (DCO) provides a pathway to managing patients with severe orthopaedic injuries, particularly when the trauma is complicated by vascular injuries, such as femoral artery rupture. The DCO approach prioritizes stabilizing the patient before definitive bone fixation. This way, it provides a stable environment to reconstruct damaged arteries, prevent the secondary inflammatory response, provide hemodynamic stability, and prevent other complications associated with prolonged surgeries. In this case, a 48-year-old man experienced high-energy trauma on his left leg and arrived at the emergency department eight hours after the incident. Upon his arrival, a noticeable deformity was observed in the lower thigh area. X-ray imaging disclosed a distal femoral fracture. Further examination revealed a lack of knee and ankle function, full paraesthesia below the knee region, and absent pedal pulses. Following a CT angiography, a complete rupture of the superficial femoral artery was confirmed. This case was treated with temporary external fixation, femoral artery reconstruction, and prophylactic fasciotomy. {"title":"THE ROLE OF KINESITHERAPY AS PREVENTION AND MOTOR CONTROL OF POSTURAL DISORDERS IN ADOLESCENTS","authors":"P. Kasnakova, B. Tornyova, T. The purpose of the study is to conduct a screening for the detection of postural disorders and determine the correct approach for correction through a kinesitherapeutic program to improve motor control in case of impaired posture and musculoskeletal imbalance. Material and methods: The study was conducted in 2022-2023 in Plovdiv, 244 adolescents aged between 7 and 18 years, 57.8% (n = 141) girls and 42.2% (n = 103) boys, were studied. Special tests for spinal dysfunctions were applied, and prophylactic measures for neuro-postural remodeling were recommended. Statistical methods were used to illustrate the obtained results. Results: An analysis of the posture was made as a result of the condition of the muscles, fascia, mobility and type of spinal dysfunctions. We found increased thoracic kyphosis in 32.3%, kyphoscoliosis – in 26.2% and lumbar lordosis – in 13.5% of the examined. A kinesitherapy program was drawn up and approved in accordance with the findings of the functional examination. Original rehabilitation algorithms for the prevention of impaired posture have been promoted, including physical exercises for the correction of the deformity, which depends on local factors and is subject to neuro-humoral regulation. Conclusion: Successful prevention and correction of impaired posture depends on early detection establishing the causes, type, degree and severity of the dysfunction. {"title":"STUDY OF THE EFFECT OF THE COMBINATION OF HALOTHERAPY AND BREATHING GYMNASTICS IN CHILDREN WITH BRONCHIAL ASTHMA","authors":"I. Karaganova, S. To date, there is no developed program and in-depth analysis of the complex impact of the effect of the combination of halotherapy and respiratory gymnastics in children with bronchial asthma, and the effect of its application on the disease has not been tracked. For this reason, referring to the successful results of the combined application of physiotherapeutic techniques, natural and reshaped physical factors in the recovery process in bronchial asthma in childhood, the following research goal was set: to implement the combined application of respiratory gymnastics and halotherapy for more effective rehabilitation program and optimal functional recovery in children with bronchial asthma and to track the effect of its impact. Material/Methods. The study was conducted from October 2018 to March 2020 in the city of Razgrad, Bulgaria, in the Resonance after-school training center. Results. The results show that properly conducted respiratory rehabilitation, combining the application of respiratory gymnastics and aerosol therapy, can contribute to the improvement of clinical indicators, reducing the frequency and severity of paroxysmal conditions, improving ventilation indicators, some gas parameters, physical capacity and quality of life. Conclusions.
