{"title":"Mozambican Food Pyramid","authors":"Ernesto Francisco Maborre Fotinho","doi":"10.1101/2024.08.24.24311155","DOIUrl":"https://doi.org/10.1101/2024.08.24.24311155","url":null,"abstract":"Introduction: In order to support nutritionists and other food and nutrition education actors in Mozambique in improving the approaches of intervention messages to fight for chronic malnutrition and other nutritional related diseases that plague the country, this work brings the adaptation of the American food pyramid published in 1993 to the local reality. Methods: The pyramid was built with the foods distributed in eight groups (Basic Foods, fruits, vegetables, legumes, milk, meats, fats and sugars) and in 5 levels. Each level was presented in minimum and medium portions to be consumed according to the established standard diets. The portions and the food equivalents tables were defined according to the caloric contribution of each food group using an automated spreadsheet of Microsoft Excel, version 19, and based on the principle of linear programming. The pyramid was based on two standard diets (1693 kcal and 2300 kcal) with four meals for both. Results: The percentage distribution of macronutrients in both diets is within the recommended range: Carbohydrates (45 to 65%), lipids (20 to 35%) and proteins (10 to 35%). Special attention was given to the milk group by calculating the bioavailable calcium values. Discussion: Factors such as food availability, dietary reference intake, and eating habits of the local population were elements of adaptation to the local context. Methods: A cross-sectional secondary analysis of baseline data from 1622 parents enrolled in the ENCIRCLE study, a pragmatic cluster-randomized controlled trial to prevent obesity among preschool-aged children from rural lower-income families. Logistic regression models examined associations between parent engagement in health-promoting behaviors (attitude, subjective norm, perceived behavioral control) and food resource management (self-confidence, behaviors) with household food security status.\u0000Results: Parents with greater perceived behavioral control (OR=1.44, CI:1.14-1.82) and food resource management self-confidence (OR=2.08, CI:1.80-2.41) had significantly higher odds of experiencing household food security in adjusted models. Conclusions and Implications: Findings suggest that perceived behavioral control and food resource management self-confidence may help safeguard lower-income families. However, it is also often feared due to proclaimed negative effects on sleep quality, thereby potentially hampering exercise recovery and next-day exercise performance. We recently showed that ketone ester (KE) ingestion beneficially impacted sleep following strenuous, late evening exercise in normoxia, and alleviated hypoxemia. Therefore, we hypothesized that KE ingestion may be an effective strategy to attenuate hypox(em)ia-induced sleep dysregulations. Methods: Eleven healthy, male participants completed three experimental sessions including normoxic training and subsequent sleep in normoxia or at a simulated altitude of 3,000m while receiving either KE or placebo post-exercise and pre-sleep. Sleep was evaluated using polysomnography, while next-day exercise performance was assessed through a 30-min all-out time trial (TT30). Physiological measurements included oxygen status, heart rate variability, ventilatory parameters, blood acid-base balance and capillary blood gases. Results: Hypoxia caused a ~3% drop in sleep efficiency, established through a doubled wakefulness after sleep onset and a ~22% reduction in slow wave sleep. KE ingestion alleviated the gradual drop in SpO2 throughout the first part of the night, but did not alter hypoxia-induced sleep dysregulations. Neither KE, nor nocturnal hypoxia affected TT30 performance, but nocturnal hypoxia hampered heart rate recovery following TT30. Conclusion: We observed that sleeping at 3,000m altitude already impairs sleep efficiency. Although this hypoxia-induced sleep disruption was too subtle to limit exercise performance, we for the first time indicate that sleeping at altitude impairs next-day exercise recovery. This study aimed to identify the drivers of acute malnutrition in Turkana and Samburu counties, Kenya. This was a population-based longitudinal mixed-methods observational study. Qualitative and quantitative data were collected at Wave 1 but only quantitative data were collected during follow-up. Participants were a representative sample of children and their primary caregivers from households with children under 3 years at Wave 1. Anthropometric measurements of all children under 5 years in the sampled households were taken at Wave 1 (May to July 2021), and one child under 3 years was randomly selected for follow-up about every 4 months over 2 years for six data collection waves. The study also collected data on socio-demographics; child feeding practices and morbidity, household water and food insecurity; shocks; coping strategies, social safety nets and economic safeguards; water, sanitation, and hygiene; decision making of women; and food consumption. Qualitative data were collected through community dialogues, focus group discussions, in-depth interviews, photovoice and key informant interviews with mothers and fathers with children under 5 years, community leaders, county officials and staff of nongovernmental organizations. The data were coded and analyzed thematically. Data collection is complete, and analysis is ongoing. Design: Tweet Archiver collected tweets including the word nutrition on seven randomly selected days in 2021. Tweets were screened for URL inclusion and grouped based on retweet status. URLs were assessed using the OQAT. Rasch measures defined quality levels (low, satisfactory, and high-quality), while mean differences of retweeted and non-retweeted data were assessed by the Mann-Whitney U test. The Cochran-Mantel-Haenszel test was used to compare information quality by source. Results: In total, 10,573 URLs were collected from 18,230 tweets. After screening for relevance, 1,005 articles were assessed (9,568 were out of scope) sourced from: professional-blogs (n=354), news-outlets (n=213), companies (n=166), personal-blogs (n=120), NGOs (n=60), magazines (n=55), universities (n=19), government (n=18). Rasch measures indicated the quality levels; 0-3.48, poor, 3.49-6.3, satisfactory and, 6.4-10, high quality. Personal and company-authored blogs were more likely to rank as poor quality. There was a significant difference in quality of retweeted (n=267, sum of rank, 461.6) and non-retweeted articles (n=738, sum of rank, 518.0), U = 87475, p=0.006, but no significant effect of information source on quality. Conclusions: Lower-quality nutrition articles were more likely to be retweeted. We compared the extent to which WHZ, MUAC, MUAC-for-age Z-score (MAZ) identify the same children as wasted and assessed the predictors of discordance and concordance in wasting diagnosis by these indicators using data from a longitudinal study of children younger than 3 years at recruitment in Turkana and Samburu counties. Wasting prevalence was consistently lower based on MUAC than WHZ and MAZ. Compared to WHZ, MAZ had higher sensitivity than MUAC, with the sensitivity of MAZ increasing and MUAC decreasing with age. Both indicators had high specificity. WHZ had a better agreement with MAZ than MUAC in wasting diagnosis. Older children were less likely to be classified as wasted by MUAC alone or by both MUAC and WHZ but were more likely to be classified as wasted by WHZ alone, MAZ alone or by both MAZ and WHZ. Compared to girls, boys were less likely to be classified as wasted by MUAC alone but more likely to be classified as wasted by WHZ alone. Stunted children were more likely to be classified as wasted by MUAC alone, MAZ alone, both MUAC and WHZ, and both MAZ and WHZ but not by WHZ alone. Classifications of wasting based on WHZ, MAZ, and MUAC are age, sex, and stunting status dependent. The survey covered 85 stores through on-site visits, identifying 251 unique PBMA products from 42 brands. The PBMAs were categorized into nine groups: Burgers/Patties, Coated Meat, Luncheon Meat, Minced Meat, Pastries, Pieces/Chunks/Fillets/Strips, Prepacked Cooked Meals, Sausages, and Seafood Balls/Cakes/Meatballs. The survey indicated widespread availability of PBMAs in Petaling, with premium supermarkets offering the most extensive selection. The median prices of PBMAs across categories were below MYR 7 (~USD 1.6) per 100 grams, with Pieces/Chunks/Fillets/Strips being the most affordable. Nutritionally, PBMAs exhibited lower energy, total fat, saturated fat, sodium, and protein levels than meat equivalents, while showing higher carbohydrate content. Nutri-Score profiling revealed generally favourable scores (Grade A to C), indicating moderate to good nutritional quality. These findings suggest that PBMAs offer a viable alternative for consumers in Malaysia seeking healthier and more sustainable food options, particularly for those aiming to reduce their intake of fat and sodium. However, PBMAs may not be recommended for individuals seeking a higher protein and lower carbohydrate diet. Ketone-based diets improve the disease course in MS animal models and health outcomes in different pilot studies of neurodegenerative diseases.\u0000Methods: We enrolled 105 individuals with relapsing-remitting MS (RRMS) in an 18-month, randomized, controlled study, and randomized them into 1) standard healthy diet (SD) as recommended by the German Nutrition Society, 2) fasting diet (FD) with 7-day fasts every 6 months with intermittent fasting at 6 of 7 days a week or 3) ketogenic diet (KD) with 20-40 g carbohydrates per day. Primary outcome was the number of new MRI lesions after 18 months in the KD and FD compared to SD and compared to baseline. Secondary outcomes included further MRI outcomes, disease biomarkers as well as metabolic, and clinical MS outcomes.\u0000Results: Eighty-one participants completed the study. The primary endpoint number of new T2 lesions after 18 months did not change in any of the groups (SD 0 (0-(-1)), FD 0 (2-0), KD 0 (2-0)). Compared to baseline, in the FD group, Neurofilament light chain (NfL) -concentrations were lower at 9 months (-1.94 pg/mL, p = 0.042) and depressive symptoms improved slightly at 18 months (p = 0.079). In the KD group, cognition improved at 18 months (symbol digit modalities test +3.7, p = 0.020). In Tanzania, with an HIV prevalence of 4.9% among adults, malnutrition among People Living with HIV (PLHIV) remains high. This study evaluated adherence to nutritional guidelines for PLHIV in Care and Treatment Centers (CTCs) in Dar es Salaam and identified influencing factors. A mixed-method design was used among PLHIV, health facility administrators, and healthcare providers. Data were collected through observational checklists, structured questionnaires, and in-depth interviews. Statistical and qualitative analyses were performed to assess adherence and determinants. Among 478 participants who received care, only 19.46% were managed with fully adherence to the nutritional guidelines. Universally, anthropometric assessments were observed, however, micronutrient supplementation was minimal (1%). Higher education (AOR=5.08, p=0.019) and attendance at referral hospitals (AOR=8.23, p=0.032) positively influenced adherence. Conversely, nurse attendance (AOR=0.34, p=0.038), adequate staffing (AOR=0.31, p=0.010), and urban residence (AOR=0.47, p=0.009) negatively influenced adherence. Key facilitators included consistent training and supportive leadership, while barriers involved financial constraints and high staff turnover. The study reveals a significant gap in adherence to nutritional guidelines among PLHIV in Dar es Salaam, highlight the need for improved resource distribution, staff training, and patient education. Its potential impact on body composition in children, particularly in relation to obesity and undernutrition, is an area of growing interest. However, existing literature presents inconsistent findings regarding the correlation between Vitamin D levels and body composition parameters in pediatric populations. The aim of this study was to explore the relationship between Vitamin D levels and body composition among kindergarten-aged children in Chengdu, Sichuan Province, China, providing insights that could contribute to health promotion strategies for this demographic.A cross-sectional study was conducted among children aged 3 to 6 years across 47 kindergartens in Chengdu. Participants were selected via stratified random sampling to ensure representativeness. Data collection took place during April to June to minimize seasonal effects on Vitamin D levels. Body composition was assessed using bio-electrical impedance analysis (BIA). Vitamin D status was determined through blood tests, and the distribution of Vitamin D levels by sex and age groups was analyzed. The study included 1239 children, with 75.54% having insufficient Vitamin D levels and 24.46% having sufficient levels. There was no statistically significant difference in Vitamin D status between sexes (χ²=0.676, p=0.411) or age groups (χ²=4.700, p=0.095). Correlation analysis revealed that BMI had a weak negative correlation with Vitamin D (-0.028, p=0.320) but no significant correlation with Vitamin A (-0.007, p=0.799). Similar results were observed for other body composition components. This study suggests that a majority of kindergarten children in Chengdu have insufficient Vitamin D levels, regardless of sex or age. While some correlations between Vitamin D levels and body composition were observed, they were weak and not clinically significant. Further research is needed to elucidate the complex interplay between Vitamin D, body composition, and other influencing factors in this age group.
