Osteoarthritis is also a common disease in elderly patients and is characterized by irreversible destructive and dystrophic damage to the joints. Decompensated stages of osteoarthritis of the hip joint often require surgical treatment. It has been noted that the majority of patients requiring total hip replacement for osteoarthritis also suffer from sarcopenia. Considering the clinical and biomechanical role of the muscular system together with the musculoskeletal system, sarcopenia can affect the recovery process of patients after surgery and the success of the operation itself. However, scientific research on the influence of sarcopenia on the results of total hip arthroplasty is insufficient. This article presents the results of a study of the negative impact of sarcopenia in patients who underwent total hip replacement. It is noted that during the pandemic, consumers of online pharmacies began to use the drug delivery service more often, which prompted the active development of this area. In the post-pandemic period, the upward trend in the use of online pharmacies has continued, however, a significant proportion of consumers still prefer pickup from a conventional point of sale when ordering via digital technologies. To evaluate the clinical and economic efficacy of the drug complex of beta-iron (III) oxyhydroxide, sucrose, and starch (Velphoro) in comparison with sevelamer in patients with stage 5 chronic kidney disease (CKD) receiving phosphate binding therapy during renal replacement therapy (RRT) in the conditions of the healthcare system of the Russian Federation. Materials and methods. An analytical decision-making model, which allows estimating the costs of managing patients with stage 5 CKD receiving phosphate binding therapy along with RRT, was built in MS Excel. The model included the costs of phosphate binding therapy with beta-iron (III) oxyhydroxide-sucrose-starch complex or sevelamer, as well as the cost and incidence of non-RRT hospitalizations in patients with CKD. The frequency of hospitalizations was determined according to a retrospective analysis of the data from End Stage Renal Disease Seamless Care Organizations (ESCOs), USA (2016-2018). The potential economic impact of using comparison strategies was calculated in 19,617 estimated patients with stage 5 CKD receiving RRT and phosphate binders. Results. The use of beta-iron (III) oxyhydroxide, sucrose, and starch complex in comparison with sevelamer is associated with a reduction in costs by 10,222 rubles or by 7.4 % on average per patient, which allows fully reimbursing the costs of more expensive drug therapy (the difference in costs minus the cost of drug therapy is 896 rubles in favor of the beta-iron (III) oxyhydroxide, sucrose, and starch complex), and reducing the number of hospitalizations in the target population by more than 6,000 cases. When used in the target cohort of patients, savings will amount to 200.5 million rubles (17.6 million rubles minus the difference in the cost of drug therapy). Conclusion. This makes it possible to control and manage many pathological conditions. However, each drug should be comprehensively studied, as well as its systemic action, side effects, toxicity, effect when interacting with other drugs, and especially when it accumulates in the body, must be taken into account. There are groups of patients in which clinical trials of drugs are difficult and limited. These are pregnant women, children, and newborns, especially premature ones. However, an analysis of the current legislation revealed the need to regulate the organization of the proper storage of archival medical documents. Moreover, in the context of the development of a medical information system (MIS), it is essential to realize that its effective functioning and successful integration into medical activities require not only technical solutions, but also an appropriate legislative and regulatory framework. In addition to the MIS creation and implementation, it is necessary to pay due attention to the improvement of legal and regulatory documents that govern the procedure for working with documents in a mixed workflow.
