The main aspects investigated by analyzing the results of the questionnaire survey were the attitude of Moscow residents to their health, their personal experience of overcoming the pandemic, and its impact on the perception of medical professionals. According to the results, the experience of overcoming the pandemic had a positive impact on changing the perception of the doctor's image and of the medical community in general. Survey participants noted the heroization of medical professionals' actions during the fight against the novel coronavirus infection, which resulted in a general rise in the level of trust in doctors and an increase in the status of the medical profession. Muscovites believe that part of the responsibility for their health lies with medical professionals. However, taking into account that behavioral strategies of a substantial number of the respondents are marked by low activity of observing the principles of healthy lifestyle, the presence of bad habits, low medical aid appealability at the occurrence of primary signs of disease, and non-compliance with doctor's prescriptions, it significantly complicates the possibility of providing medical care and preserving health.","PeriodicalId":500680,"journal":{"name":"Glavvrač (Chief Medical Officer)","volume":"15 8","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140441158","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and methods. The study was based on the analysis of 51 patients with cervical spine hernias (30 (58.8 %) men, 21 (41.2 %) women aged 23–72 years, with mean age of 42.8 years). All patients underwent discectomy of the cervical spine in the self-supporting department of the National Center for Rehabilitation and Prosthetics of the Disabled during the period of 2018–2022. The patients were divided into two groups. The first group consisted of 31 (60.7 %) patients who underwent discectomy of cervical hernias followed by TMS in the early postoperative period. The second group included 20 (39.3 %) patients who underwent discectomy without using TMS. Results and discussion. The results of open surgical interventions were assessed in two groups, according to the surgical intervention performed and the use of TMS. The first (study) group of 31 (60.7 %) patients underwent «discectomy with decompression of the spinal cord and subsequent stabilization with an improved cylindrical cage and the use of TMS in the early postoperative period». The second group of the patients underwent discectomy using cylindrical cages, laminectomy from the posterior approach, and laminectomy using transpedicular or transarticular stabilization without subsequent TMS. Conclusions. The use of an improved cylindrical cage for herniated cervical spine is justified and timely; the application of transcranial magnetic stimulation in the early postoperative stage in patients with a herniated disc of the cervical spine restores neurological deficits and awakens long-lost functions of a group of muscles and, in some cases, of entire organs.","PeriodicalId":500680,"journal":{"name":"Glavvrač (Chief Medical Officer)","volume":"73 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140438252","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

According to the obtained data, this period is characterized by a significant increase in academic load, disruption of regular routines, and loss of social connections, which can lead to destabilization of students' psycho- emotional state. The majority of survey participants reported experiencing symptoms of stress, including nervousness, irritability, excessive fatigue, changes in eating habits, etc. which negatively affect their ability to absorb new material. In the opinion of high school students, the psychological support provided in schools often has a formal nature, limited to collective measures such as workshops and training sessions.","PeriodicalId":500680,"journal":{"name":"Glavvrač (Chief Medical Officer)","volume":"13 9","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140438474","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Bacterial vaginosis (BV) is one of the factors of increased risk of tubo-peritoneal infertility. Presumably, there are two mechanisms of tubo-peritoneal infertility associated with BV: firstly, the increased levels of proinflammatory cytokines, resulting in the attack of seminal fluid, and secondly, the cleavage of cervical mucus by bacterial mucinases, which improves adhesion by pathogenic microorganisms, contributing to colonization with a further tendency to diseases of the upper reproductive tract, including infertility. Women who have BV are more likely to suffer from inflammatory diseases of the pelvic organs caused by C. trachomatis and N. gonorrhoeae, which leads to tubo- peritoneal infertility. Therefore, timely detection and treatment of BV is crucial.","PeriodicalId":500680,"journal":{"name":"Glavvrač (Chief Medical Officer)","volume":"80 5","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-01-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140500624","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Rapid diagnosis and appropriate treatment are necessary. There are three categories of antithrombotic agents: antiplatelet agents, anticoagulants, and fibrinolytics. Anticoagulants are first-line therapy for the prevention and treatment of venous thrombi. Dabigatran etexilate is a direct thrombin inhibitor. The effectiveness of dabigatran has been demonstrated in several clinical studies for the prevention of venous thromboembolism in patients undergoing total hip or total knee arthroplasty, for the prevention of stroke in patients with non-valvular atrial fibrillation, and in the treatment of acute venous thromboembolism. Some studies have demonstrated that the use of dabigatran leads to an increased risk of bleeding. Therefore, the purpose of this review is to examine the safety of dabigatran in patients over 18 years of age. We identified potentially relevant studies by searching the electronic databases PubMed, Web of Science, Cochrane Library, MEDLINE, and Google Scholar. Maksimov","doi":"10.33920/med-03-2401-02","DOIUrl":"https://doi.org/10.33920/med-03-2401-02","url":null,"abstract":"Scientific relevance. According to the WHO data, cardiovascular diseases rank first as the cause of human death. In 2022, 18,601,923 people died from cardiovascular diseases, which amounted to 33 % of the total mortality in the world. Malignant neoplasms rank second with 9,958,133 deaths, representing 17.6 % of total mortality. Among cardiovascular diseases, ischemic heart disease (IHD), the pathogenesis of which is associated with impaired cholesterol and lipoprotein metabolism, is leading. The use of statin drugs is recommended as a first-line lipid-lowering therapy. As an alternative to HMG-CoA reductase inhibitors (statins), proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9) were discovered in 2003, however, they are less popular due to insuf ficient data on these drugs and their costliness. Purpose of the work: to study the safety of using proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors in patients with dyslipidemia and compare the advantages and disadvantages in relation to the ef fects of HMG-CoA reductase (statins). Discussion: The analysis of literature data has shown that the most frequent manifestations of intolerance to statins are muscle symptoms. The main mechanisms of statin-associated complications are mediated by inhibition of HMG-CoA reductase, as well as by the effect of drugs on cellular and subcellular processes, skeletal muscles and genetic features of patients. PCSK9 inhibitors have a fundamentally different mechanism of action — they bind and inactivate PCSK9, which leads to a decrease of low-density lipoprotein cholesterol in the blood level. A small number of undesirable reactions have been identified for drugs of this group, the most important of which is immunogenicity. However, even if patients have antibodies to PCSK9 inhibitor, the efficacy of the drug in reducing the level of low-density lipoproteins remains high.Conclusion. In medical studies, PCSK9 inhibitors have been shown to perform better compared to HMG-CoA reductase inhibitors; meanwhile, patients continue to use statins as lipid-lowering medications.","PeriodicalId":500680,"journal":{"name":"Glavvrač (Chief Medical Officer)","volume":"39 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-01-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140500420","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
