This cross‑sectional function offers great potential for technologic and organizational innovation to influence health care as a whole. In recent years, a variety of technologies have emerged and entered the field of medical research, or even medical care. A new generation of biosensors enables laboratory tests to be carried out at the point of care and allows for faster medical decisions. Modern devices allow for patient‑centric blood sampling, which eliminates the need for painful blood draws, patient traveling, and limits the workload of health care professionals. Analytical techniques, such as metabolomics, lipidomics, or proteomics can identify biomarkers extremely sensitively, even down to individual cells. Pharmacogenomics allows for determination of genetic polymorphisms that predict a response to chemotherapeutic agents. Machine‑learning approaches can handle large amounts of multilayered data for diagnostic applications. However, this enormous diagnostic potential is far from being utilized and only very few applications have been implemented in clinical practice. Why is this the case? In this article, we describe the key technologic fields, discuss their medical potential, and list obstacles to their implementation. {"title":"Validation of the Fibromyalgia Rapid Screening Tool in patients with axial spondyloarthritis: Polish version adaptation and value in clinical practice.","authors":"Jakub Wroński, Hanna Rozenek, Dorota Włodarczyk","doi":"10.20452/pamw.16753","DOIUrl":"10.20452/pamw.16753","url":null,"abstract":"<p><strong>Introduction: </strong>Fibromyalgia frequently co‑occurs with axial spondyloarthritis. The Fibromyalgia Rapid Screening Tool (FiRST) is a well‑recognized screening tool for fibromyalgia and has been translated into multiple languages. Yet, it has not been adapted into Polish, nor has it been validated in the context of axial spondyloarthritis.</p><p><strong>Objectives: </strong>This study aimed to create a Polish version of the FiRST, evaluate its psychometric properties, and conduct its validation among patients with axial spondyloarthritis.</p><p><strong>Patients and methods: </strong>We translated and performed a cross‑cultural adaptation of the FiRST into Polish, followed by its validation in a cohort of 174 patients with axial spondyloarthritis. For criterion validity, we employed the 2016 American College of Rheumatology fibromyalgia diagnostic criteria as the gold standard for fibromyalgia diagnosis.</p><p><strong>Results: </strong>The Polish version of the FiRST demonstrated marginally acceptable internal consistency with the Cronbach α coefficient of 0.644, but exhibited high test‑retest reliability, with a global score correlation coefficient of 0.75 (P <0.001). Receiver operating characteristic analysis indicated a good performance of the translated questionnaire (area under the curve of 0.803). The accuracy of the derived cutoff value for the global score (5+ points, consistent with the original instrument) was 75.3%, featuring higher specificity (82.6%) than sensitivity (63.1%), and a fair level of diagnostic agreement, as indicated by the Cohen κ coefficient of 0.46.</p><p><strong>Conclusions: </strong>Our study provided a validated Polish version of the FiRST. Characteristics of the skin lesions, their clinical course, and impact on treatment are of high importance.</p><p><strong>Objectives: </strong>The aim of this study was to assess the prevalence, risk factors, and clinical sequelae of dermal lesions in IBD patients treated with anti-TNF‑α antibodies.</p><p><strong>Patients and methods: </strong>This retrospective, single‑center study evaluated 541 IBD patients treated with anti-TNF‑α drugs and 688 IBD individuals with no history of anti-TNF‑α treatment.</p><p><strong>Results: </strong>Higher prevalence of dermal lesions was noted in the patients on anti-TNF‑α therapy than in the individuals not receiving such treatment (30.9% vs 16.4%; P <0.001). Risk factors for dermal lesions included higher body mass index (BMI), Crohn disease located in the small intestine, and longer duration of therapy. Some types of dermal lesions were associated with anti-TNF‑α therapy; these included infusion reactions and injection site reactions, cutaneous infection, psorasiform reactions, and lupus‑like symptoms. Overall, 5.9% of the patients on anti-TNF‑α therapy required treatment change or discontinuation due to dermal lesions (alopecia, lupus‑like symptoms, melanoma, and psoriasis).</p><p><strong>Conclusions: </strong>We observed a higher prevalence of dermal lesions in patients with IBD undergoing anti-TNF‑α therapy than in the treatment-naive group, although development of such lesions rarely necessitated a change in or discontinuation of treatment. Patients with IBD should regularly undergo follow-up dermatologic evaluation, which may improve detection of dermal lesions.
