The effectiveness of providing this type of medical care is influenced by a significant number of medical, social and organizational factors. Achieving a positive result during IVF largely depends on the obstetric history of the expectant mother. Purpose. To determine the medical and social characteristics of the obstetric and gynecological profile of patients at the Center for Reproduction and Family Planning who had a positive result in the treatment of infertility using IVF. Materials and methods. Using a specially developed statistical form “Questionnaire for medical and social examination of women suffering from infertility,” an anonymous survey was conducted of 398 women whose pregnancy resulted from the IVF procedure. The significance of differences in indicators was assessed using Student’s t-test. Differences were considered significant at p<0,05. Statistical data processing was carried out using the MS Office‑2016 and StatSoft-Statistica 10.0 software packages. Results. With an average age of onset of sexual activity of 18,82± years, sexual debut before 18 years of age was observed in 32,7% of patients. Most often, for the first time, women sought medical help for infertility within 2–5 years after the start of regular sexual activity (65,4%) (m=3,38±0,17 years) to obstetrician-gynecologists at antenatal clinics and planning centers families (83,0%). Despite the fact that the average age of initial diagnosis of infertility was 28,86±0,34 years, 38,2% of women were diagnosed with infertility after 30 years of age, and 55.3% of all requests for medical help for problems with conception came from private medical organizations. In most cases, before being referred for IVF, women underwent treatment at antenatal clinics and family planning centers (91,3%). The obstetric and gynecological profile of women who have overcome infertility with the help of ART is characterized by: a history of negative IVF results (50.4%), absence of children before this pregnancy (87,0%), low detection of gynecological diseases in childhood (13,1%) and the presence of abortions (27,1%). 38,9% of patients who had a history of pregnancy terminations had two or more abortions (m=1,61±0,03 abortions). In the study group, a high frequency of abortions for medical reasons and spontaneous terminations of pregnancy was revealed (31,5% and 24,1%, respectively). Conclusion. The purpose of the study was an attempt to create a unified and harmonious theoretical foundation for conducting an experiment in order to find a prototype of a medical organization implementing projects at the regional level. In order to reveal the potential for solving technological problems facing project stakeholders, formalization of approaches was carried out for experimentally finding the parameters of a prototype project for a medical organization implementing a regional project. Materials and methods. The research materials were the works of Russian and foreign scientists related to the issues of mathematical modeling and prototyping. In order to study the research question, more than 30 literature sources were analyzed, directly or indirectly addressing the problems of practical application of object prototyping methods. Methods of system analysis, comparison, systemic and integrated approaches, as well as mathematical modeling methods were used. Results. Complex, multi-level systems when implementing projects in the regions are poorly structured and, accordingly, not fully formalized, so the experimental approach is especially relevant when searching for prototyping tools. The degree of formalization of approaches to searching for a prototype of a project object depends on the completeness (saturation) of processes, their structure and the degree of correlation of process data in a medical organization to process performers, operations, and management objects. Prototyping is designed to optimize the procedures for planning, analyzing and processing the results of research data based on a mathematical foundation. The prototyping technique is primarily aimed at saving resources (labor, financial, time) as much as possible, therefore, justification of the need and mastery of mathematical foundations in formalizing approaches to prototyping is a fundamental factor and the purpose of this discussion. Findings. The survival rate of the patients with prostate cancer depends on clinical and organizational factors. The network of express-diagnostic urological offices (EDUO) has been deployed in medical organizations of the Sverdlovsk region, designed for early diagnosis of urological and oncourological diseases, and determination of patient routing schemes. The purpose of the study: to determine the organizational and clinical factors of prostate cancer survival based on the data form the Sverdlovsk regional segment of the Russian oncological information system “ONCOR». Materials and methods. Data on all the 20087 newly identified cases of prostate cancer in the Sverdlovsk region in 2005–2022 were exported from the ONCOR database. One- and five-year tumor-specific survival rates were estimated by the life tables. The cumulative survival function was calculated by the Kaplan-Meyer method. Simple and multiple Cox regression analysis was used to find the relative risks of death of patients with prostate cancer depending on the clinical and organizational prognosis factors. Results. In 2005–2022, in the Sverdlovsk region, the level of primary morbidity rate of the male population with prostate cancer increased by +445.8%; 42.3% – 49.5% of cases were newly identified on the III and IV stages depending on the patient’s place of residence. The five–year tumor–specific survival rate of patients with I stage of prostate cancer was 87.4%, II stage – 77.4%, at the third III stage – 65.1%, IV stage – 23.6% (p < 0.001). The five–year tumor–specific survival rate of patients with prostate cancer confirmed in EDUO was 78.8%, in other medical organizations – 57.7%. The relative risk of death of the patient with prostate cancer depended on the place of residence, age, stage of the disease, as well as the availability of EDUO in the regional medical organization. All elements of the transformational change process are based on a patient-centered system of medical care that is sustainable in the long term. The urgent problem for outpatient clinics to become improve effectively using a patient-centered approach. The aim of this study was to describe the strategies on patient-centred outcomes used in primary care in Russian regions. Materials and methods. A systematic literature review was conducted in Google Scholar and Elibrary databases using keywords. According to the inclusion criteria, the studies described the experience of implementing new strategies in the Russian primary health care system between 2017 and 2023, resulted in a total of 2874 articles being found. After reviewing the abstracts of the retrieved articles, 211 were retained for further analysis. Another 175 articles were excluded after reading the full text. Results. In this article, we describe a set of priorities strategies related with a new models in primary health care: expanded role of nurses in primary health care, improvement of Ambulatory complementary technologies, use of new organizational structures of medical care, using remote technologies. Findings. The strategies of new organizational models in primary health care are based on the successful experience of reforms. One of the problems of providing specialized care for ischemic stroke is to achieve high rates of intravenous thrombolytic therapy (IVT) in areas characterized by low population density and large distance from the nearest stroke unit (delivery time more than 1 h). In Sverdlovsk region, in order to solve this problem in such territories, three teleconsulted stroke units equipped with CT scanners and having resuscitation beds were organized in 2021. Objective. To study the dynamics of the rates of intravenous thrombolytic therapy for ischemic stroke in the territories with a teleconsulted stroke units (in addition to the stroke unit). Materials and Methods. We retrospectively analyzed the data of 85 patients treated in three teleconsulted stroke units (teleSU) of the Sverdlovsk region in 2022 and compared the dynamics of the increase in the IVT rate (R parameter,%/year) in these territories in comparison with the rest of the municipalities of the Sverdlovsk region. Results. According to the results of the study, it was found that the rate of IVT in teleSU was 9,0%, which is generally higher than the average value in the Sverdlovsk Region (5,1%) in the analyzed period. The analysis of the statistical data from 2020 to 2022 revealed that establishment of additional teleconsulted stroke units in the territories with low population density in 2022 has led to the increase of the number of procedures of IVT for ischemic stroke. Dynamics of growth of this indicator, became equale of that in the rest of the municipalities of the region. Conclusions. Problems of population reproduction and reproductive health of women of fertile age [1] attract much attention from both domestic and foreign researchers. Important demographic indicators that characterize population reproduction are fertility and mortality, the level of which significantly affects the population size, natural growth and determines the demographic situation not only of the territories, but also of the country as a whole [2]. Purpose of the study: to analyze literary sources and compare with official statistical data of the Krasnoyarsk Territory, both for the region with low population density, and separately for the western medical district of the Krasnoyarsk Territory, fertility rates, women’s reproductive health, the frequency of abortions and the number of births. Material and methods. For an objective assessment, information was taken from Rosstat for the Krasnoyarsk Territory and data from dashboards of the situation center of the Krasnoyarsk Territory, data from the state report on the state of public health and health care activities in the Krasnoyarsk Territory in 2022, a retrospective analysis of medical documentation and state statistics materials was carried out with copying data. Results of the study. Over the past three years, negative changes have emerged in the demographic development of the Russian Federation, expressed in a decrease in the number of births and an increase in the number of deaths. Total fertility rate in Russia in 2020. amounted to – 9.8‰, in 2021. – 9.6‰, in 2022 – 8.9‰. In the Krasnoyarsk Territory in 2020. – 10.1‰, 2021 – 10.0‰, 2022 – 9.9‰ [3]. The indicator both for the country as a whole and for the Krasnoyarsk Territory did not reach the following values: 1980. – 15.9‰ and 1990 – 13.4‰. In the western medical district of the Krasnoyarsk Territory, the birth rate in 2020–12.4‰, in 2021–12.2‰ and in 2022 amounted to 12.0‰. The average age of women giving birth in Russia is 26 years in 2022, in the Krasnoyarsk Territory in 2022 it was 29.2 years and in the western medical district of the region – 29.5 years. The share of premature births among women who completed pregnancy increased over 5 years by 11.2% and in 2022 amounted to 5.8% in the Krasnoyarsk Territory as a whole, and in the western medical district of the region 5.4%. Population reproduction is largely related to women’s reproductive health. Conclusions. In recent years, the total fertility rate has decreased, there has been an increase in the average age of the mother at birth, the birth rate is decreasing under the age of 20 and at the age of 20–24, it is increasing in the group of 30–34-year-olds and older. Despite the decrease in the mortality rate, the indicators of the Kostroma region exceed the national average. Life expectancy has a pronounced upward trend, but remains lower than the average for the Russian Federation and the Central Federal District. The information base consisted of various scientific publications and regulatory documents on strategic management in the territory of the Republic of Buryatia and the Far Eastern Federal District. The analysis was also carried out using statistical data from the database of Rosstat and the Ministry of Health of Russia and on the basis of content analysis of draft strategies that were available on the official websites of government bodies, constituent entities of the Russian Federation, etc. The analysis methodology included the method of studying and generalizing experience, the mathematical-statistical method and content analysis. Results. Based on available statistical information, the publication analyzes current trends in morbidity and mortality from diseases of the circulatory system in the Republic of Buryatia for 2017–2021. and compared with the target indicators of the regional program adopted within the framework of the federal project “Combating Cardiovascular Diseases”. Conclusions. {"title":"Modeling the process of attracting personnel to medical organizations providing medical care to the rural population.","authors":"A. O. According to the Russian Federal State Statistics Service, the shortage of medical personnel as of 2022 is 84.7 thousand people, of which 26.5 thousand are doctors; compared to 2021, the problem of shortage of nursing staff is less acute, but the shortage of doctors remains. An integral part of personnel policy is the process of retaining qualified specialists. The complex motivations of employees to stop or continue working in an organization interact with the personnel decisions of a healthcare organization, which in turn requires a joint study of both the characteristics of the employees and the characteristics of the organization. Purpose of the study is to model the process of attracting personnel to medical organizations providing medical care to the rural population. Materials and methods. Data on 151 students of targeted training who entered the Orenburg State Medical University in 2011 were studied. Using machine learning methods, a mathematical model was built that allows identifying students of targeted training with a higher probability of attraction to the medical organization. Results. The presented data set is unbalanced in terms of the target variable, since only 16.8% of students continued to work in the organization. Among all the options, the most optimal in cross-validation was the use of a decision tree using weights to balance the classes. All studied medical organizations can be divided into 3 groups: organizations with a low level of attraction of specialists, organizations with an average level of attraction of specialists and organizations with a high level of attraction of specialists.
