The large proportion of systematic reviews that do not reach a definitive conclusion might discourage clinicians from engaging with and interpreting systematic reviews.</div></div><div><h3>Objectives</h3><div>To determine the percentage of conclusive Cochrane reviews in physical therapy and to investigate whether this percentage has increased over time.</div></div><div><h3>Methods</h3><div>In this meta-research study, we performed a systematic search of the Physiotherapy Evidence Database (PEDro) for Cochrane reviews. We extracted a random sample of 200 published systematic reviews, with 50 reviews from each of the periods: 2001 to 2005, 2006 to 2010, 2011 to 2015, and 2016 to 2020. Two independent assessors extracted information. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) data for the primary outcomes was used to assess conclusiveness. Reviews were considered conclusive when at least one primary outcome provided high certainty of evidence.</div></div><div><h3>Results</h3><div>Outcomes with very low certainty of evidence represented 21 % of outcomes and increased from 14 % in 2001–2005 to 34 % in 2016–2020. Outcomes with low certainty of evidence comprised 55 % of outcomes and remained consistent over time. Moderate- and high-certainty outcomes remained consistent, composing 22 % and 2 % of outcomes, respectively. The proportion of outcomes with high certainty of evidence never exceeded 4 % per period. For the formal assessment, 117 Brazilians with TMD completed the translated version of the TMD Pain Screener, the Symptom Questionnaire, and the Numeric Pain Rating Scale for orofacial pain intensity assessment. Subsequently, a standardized clinical examination was performed. Fifty-one participants were examined by two raters for interrater reliability, and 53 participated on a second assessment day for intrarater reliability analyses. TMD Pain Screener criterion validity was assessed through sensitivity, specificity, and ROC curve. Kappa and intraclass correlation coefficient (ICC) were used to analyze reliability for categorical and continuous data, respectively.</div></div><div><h3>Results</h3><div>The translated version achieved language equivalency. High accuracy was found for the Brazilian Portuguese version of the TMD Pain Screener short form (AUC = 0.95) compared to the long form, as well as high intrarater reliability (ICC 0.88). Regarding the health of women, vulvovaginal symptoms (e.g., itching, burning, pain, irritation, dryness, and vaginal odor) need consideration.</div></div><div><h3>Objective</h3><div>To verify the prevalence of vulvovaginal symptoms, their association with contextual factors, and evaluate their impact on the emotional well-being, quality of life, and sexual function of Brazilian women.</div></div><div><h3>Methods</h3><div>This cross-sectional study was conducted from October 2021 to August 2022 using Brazilian women aged 18 or older. Health and sociodemographic data were collected, and the women were divided into two groups according to the Vulvovaginal Symptoms Questionnaire: with and without vulvovaginal symptoms. The association between contextual factors and vulvovaginal symptoms was verified using logistic regression.</div></div><div><h3>Results</h3><div>The study included 313 women (34 ± 11.5 years), physically active (71 %), non-smokers (9 %), and employed (72 %). Of those, 227 (72.5 %) presented vulvovaginal symptoms. Low educational level and no pregnancy were associated with vulvovaginal symptoms. The most prevalent symptoms were vaginal discharge (63.4 %) and itching (54.6 %).</div></div><div><h3>Conclusion</h3><div>The results indicated that vulvovaginal symptoms are prevalent in Brazilian women. Often, however, there has been incomplete reporting or a lack of coherence among study purpose, design, choice of analysis, sample size justification, and reporting of results. Two possible explanations for this oversight are a vaguely written purpose statement and statistical software that does not provide all essential information.</div></div><div><h3>Objective</h3><div>The goal of this masterclass is to provide considerations and resources to assist investigators structure a coherent reliability study design and subsequent presentation of results.</div></div><div><h3>Discussion</h3><div>This masterclass highlights the importance of framing a study purpose that clearly distinguishes between a hypothesis testing and parameter estimation study and appropriately labelling the study design. Primary outcome measures were the number of repetitions, total work (J), total vertical displacement (cm), and peak height (cm) upon the single-leg HRET, assessed using the validated Calf Raise Application. A multiple quantile regression model was developed, incorporating covariates (personal characteristics) which significantly impacted HRET metrics. Median (50.0th percentile) and 95 % reference intervals (2.5th-97.5th percentiles) were derived.</div></div><div><h3>Results</h3><div>55 % of the participants were female and 88 % participated in physical activities. Median (dominant/non-dominant leg) number of repetitions was 25/24, total work was 1374/1325 J, vertical displacement was 192/186 cm, and peak height was 9.3/9.6 cm. There was no significant difference between the dominant and non-dominant leg for any HRET metric. Lower physical activity levels, female sex, and higher body mass index (BMI) were associated with lower HRET metrics.</div></div><div><h3>Conclusions</h3><div>Normative calf muscle strength-endurance metrics (number of repetitions, total work, total displacement, and peak height) were developed. Personal characteristics influence HRET outcomes, with female sex, higher BMI, and lower physical activity levels being associated with lower HRET metrics. An openly accessible calculator for estimating normative HRET metrics was developed to help healthcare providers monitor personalized recovery trajectories and provide well-informed rehabilitation guidance. However, the association between hip ROM and symptom severity in these individuals is unclear.</div></div><div><h3>Objective</h3><div>Explore associations between symptom severity and hip flexion and rotation ROM in patients with FAI syndrome and determine if ROM measures can discriminate those with worse symptoms.</div></div><div><h3>Methods</h3><div>Data from 150 participants with FAI syndrome were analysed. A digital inclinometer was used to measure hip flexion, internal rotation, and external rotation ROM. Symptom severity was quantified using the symptoms subscale of the international Hip Outcome Tool questionnaire (iHOT-Symptoms). Multivariable fractional polynomial analyses explored associations between hip ROM measures and symptom severity. Receiver operator characteristic curves explored the ability of ROM measurements to discriminate participants with different symptomatic states.</div></div><div><h3>Results</h3><div>Smaller hip flexion ROM values were associated with worse iHOT-Symptoms scores (<em>p</em> &lt; 0.01; R<sup>2</sup>=0.242); with the polynomial concave association attenuated at approximately 120° of hip flexion ROM. Hip internal rotation was weakly associated with iHOT-Symptoms score (<em>p</em> = 0.01; R<sup>2</sup>=0.033). Hip external rotation ROM was not associated with iHOT-Symptoms score (<em>p</em> = 0.06). A hip flexion value of 107° best discriminated mild to moderate and severe symptom states (sensitivity 92%, specificity 52%).</div></div><div><h3>Conclusion</h3><div>Less hip flexion ROM was associated with worse symptoms in patients with FAI syndrome. Patients with hip flexion ROM ≥ 107° had a 15-fold decrease in the likelihood of having severe symptoms. The Start Back screening Tool (SBT) and Central Sensitization Inventory (CSI) classify people with LBP into subgroups. It is currently unknown whether linear trends exist regarding somatosensory changes, psychological characteristics, and physical disability across severity levels of the SBT and CSI in patients with LBP.</div></div><div><h3>Objective</h3><div>To investigate whether linear trends in psychological variables (kinesiophobia, pain catastrophizing), disability, and somatosensory characteristics exist in a sample of people with acute and chronic LBP in primary care across severity levels based on the SBT and CSI.</div></div><div><h3>Methods</h3><div>Participants with LBP were recruited in primary care. Demographic, psychological, and disability data were obtained. A comprehensive Quantitative Sensory Testing (QST) protocol was followed. Linear contrast analysis was conducted.</div></div><div><h3>Results</h3><div>Kinesiophobia and pain catastrophizing show significant positive linear trends across the subgroups based on the SBT (<em>p</em> &lt; 0.001) and CSI (<em>p</em> &lt; 0.001 to <em>p</em> = 0.01). Heat pain threshold at the lower leg (<em>p</em> = 0.005) and pressure pain threshold at the lumbar region and lower leg (<em>p</em> = 0.02 and <em>p</em> = 0.04, respectively) show significant negative linear trends within the SBT. Negative linear trends in sensory changes exist within CSI for all pressure pain thresholds and a positive linear trend was seen in δ conditioned pain modulation at the thumb (<em>p</em> = 0.03).</div></div><div><h3>Conclusion</h3><div>Kinesiophobia, pain catastrophizing, disability, and pain intensity are positively related with the severity levels based on the SBT and CSI in our participants. While physical therapists are considered well placed to engage in weight management alongside exercise (not replacing dietician care), evidence surrounding physical therapist-delivered weight loss interventions is sparse.</div></div><div><h3>Objective</h3><div>To understand the experiences and perceptions of physical therapists delivering a very low energy diet (VLED) and exercise intervention to people with knee OA and overweight or obesity in a randomized controlled trial (RCT).</div></div><div><h3>Methods</h3><div>Qualitative interview study involving six physical therapists (mean age 34 years, median experience 3.5 years) who completed 20 hours of training and delivered a six-month diet+exercise intervention via telehealth to people with knee OA and overweight or obesity in the POWER RCT. Physical therapists participated in semi-structured individual telephone interviews on completion of RCT involvement. Data were audio-recorded, transcribed, and independently thematically analysed by two researchers.</div></div><div><h3>Results</h3><div>Three themes emerged. Physical therapists felt that delivering a weight loss intervention alongside exercise <em>enabled holistic OA management.</em> Enacting this role within a supportive research environment was considered unique and physical therapists felt the <em>training and resources facilitated care</em>. Women are prescribed exercises as a part of lifestyle intervention programs prior to artificial reproductive techniques (ART) to lose weight, to improve their chances of conception. However, adherence to exercise programs has been observed to be poor and the reasons for this remain less explored.</div></div><div><h3>Objective</h3><div>To explore the perceptions and practices about performing exercises among women with infertility.</div></div><div><h3>Methods</h3><div>A qualitative explorative study was conducted based on the interpretive framework. Face-to-face semi-structured in-depth interviews were conducted with 20 women diagnosed with primary or secondary infertility. Data were analyzed using thematic analysis.</div></div><div><h3>Results</h3><div>A total of 20 women, mean age 30.9 ± 4.8 years, body mass index 22.4 ± 1.9 kg/m<sup>2</sup>, participated in the study. There is a similar need to standardize physical therapy related PF terminology in Brazil.</div></div><div><h3>Objective</h3><div>To propose a guideline in Brazilian Portuguese language on the terminology related to the female PF function and assessment.</div></div><div><h3>Methods</h3><div>The Brazilian Association of Physical Therapy in Women's Health (ABRAFISM) established a working group (Steering committee) responsible for conducting a systematic review on PF terminology, propose definitions in Portuguese, and manage the developmental stages. A systematic review of international consensus was conducted and the terms related to the PF were identified, listed, and defined in Portuguese. A Delphi method was used to reach a consensus on the definitions by querying a panel of expert professionals with clinical and research experience in the PF field. This panel included 19 physical therapists from all Brazilian regions and two physicians. The Delphi process included the following steps: evaluation of proposed terms by the panel of experts; review of suggestions made by the panel of experts, modification of terms based on those suggestions, and a new round of evaluation of the modified terms until reaching an 80 % agreement.</div></div><div><h3>Results</h3><div>Two rounds were necessary for terms related to anatomy and signs/symptoms of the PF, while a 90 % consensus for terms related to assessment of the PF muscles was achieved in the first round.</div></div><div><h3>Conclusion</h3><div>A final Brazilian-Portuguese guideline with consensus terminology on anatomy, symptoms, signs, and terms related to assessment of PF muscles was developed.</div></div>","PeriodicalId":49621,"journal":{"name":"Brazilian Journal of Physical Therapy","volume":"29 2","pages":"Article 101173"},"PeriodicalIF":3.1,"publicationDate":"2025-02-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143103576","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
