Does the current panorama remain the same?</div></div><div><h3>Objective</h3><div>To investigate the changes in volume and quality of RCT of neurological physical therapy in adults and paediatrics indexed in the PEDro database between 1958 to 2021. In addition, to investigate if there is a relationship between journal impact factor and methodological quality.</div></div><div><h3>Methods</h3><div>All RCTs of neurological physical therapy in adults and pediatrics indexed in PEDro between 1958–2021 were included. Descriptive statistics described trial volume, quality, and trends over the years. Spearman's rho correlation test assessed the association between methodological quality and journal ranking (Journal Impact Factor (JIF) and Journal Citation Indication (JCI)).</div></div><div><h3>Results</h3><div>A total of 6291 RCTs of neurological physical therapy in adults and pediatrics were indexed in PEDro between 1958–2021, with a mean PEDro scale score of 5.3/10 (SD 1.6). The quality of RCTs improved over time, with a mean score of 5.7/10 (SD 1.4) for RCTs published between 2018–2021, compared to 1/10 between 1962–1965. A weak and significant relationship was found between methodological quality and JIF (r=0.153; p&lt;0.001) and JCI (r=0.146; p&lt;0.001).</div></div><div><h3>Conclusion</h3><div>There is a large and growing volume of RCTs on neurological physical therapy in adults and pediatrics indexed in PEDro, with increasing quality over time, though still moderate. Exercise progression provides continuous stimulation and assists physical therapists in offering challenges to patients.</div></div><div><h3>Objective</h3><div>To investigate the effectiveness of an exercise program with systematic progression of difficulty for older individuals.</div></div><div><h3>Methods</h3><div>Randomized clinical trial, with 22 older individuals allocated to experimental (EG, <em>N</em> = 12) or control (CG, <em>N</em> = 10) group. In EG, individuals performed an exercise program with progressions of difficulty for 12-weeks (2 days/week, 1 h/session). In the control group the participants performed the same program without progressions of difficulty. The Berg Balance Scale (BBS), Timed up and Go (TUG), and modified Dynamic Gait Index (mDGI) were assessed after and before the 24 exercise sessions. An intention-to-treat approach and multiple imputation by chained equations were utilized. Raw data were transformed into standardized individual differences (SID) and analyzed using one-way ANCOVA to test group effects, with baseline and age as covariates. A one-sample <em>t</em>-test was used to compare SIDs against zero. Effect sizes were estimated using partial eta squared (ηp²) and Cohen's d.</div></div><div><h3>Results</h3><div>ANCOVA revealed no significant group effect across any of the variables. Baseline values emerged as significant predictor of changes in BBS (<em>P</em> = 0.038, η<sub>p</sub>²=0.219), TUG (<em>P</em> = 0.042, η<sub>p</sub>²=0.210), and mDGI (<em>P</em> &lt; 0.001, η<sub>p</sub>²=0.545), suggesting a substantial differences among participants with lower baseline values. Age also emerged as a significant predictor of change for mDGI (<em>P</em> = 0.002, η<sub>p</sub>²=0.431). Chronic symptoms, high-level pain intensity, and disability are associated with high levels of pain catastrophizing in this condition. Although the Pain Catastrophizing Scale (PCS) is widely used to assess pain catastrophizing in individuals with chronic symptoms, its measurement properties are still unknown for assessing individuals with chronic RCRSP.</div></div><div><h3>Objectives</h3><div>To assess construct validity, reliability, and responsiveness of the PCS in individuals with chronic RCRSP.</div></div><div><h3>Methods</h3><div>Eighty-three adult individuals with chronic RCRSP were included in this study. The assessment of construct validity was based on hypothesis testing. Spearman's rank correlation coefficient was used to verify the correlation of the total score and rumination, magnification, and helplessness subscales of the PCS with pain intensity, disability, and fear of movement. Test-retest reliability was analyzed with Intraclass Correlation Coefficient (ICC)<sub>(3,1)</sub> and internal consistency was analyzed with Cronbach's alpha. Responsiveness was analyzed by effect sizes and the area under the receiver operating characteristic curve (AUC).</div></div><div><h3>Results</h3><div>The PCS showed significant moderate correlation (rho ≥ 0.40) with fear of movement (95% confidence interval [CI]: 0.23, 0.70), pain intensity (95% CI: 0.14, 0.64), and disability (95% CI: 0.17, 0.66), except the rumination and magnification subscales, which showed significant weak correlation with pain intensity (95% CI: 0.14, 0.58) and disability (95% CI: 0.12, 0.56), respectively. The Lower Extremity Functional Scale (LEFS) was developed in the framework of the ICF to define the functional status of the lower extremities as an important indicator of the health, activity, and participation of the elderly.</div></div><div><h3>Objective</h3><div>To analyze the psychometric properties of the Turkish version of LEFS (LEFS-T) in older individuals.</div></div><div><h3>Methods</h3><div>A total of 214 older individuals were included in the study and the LEFS-T, Five-Times-Sit-to-Stand Test (FTSTS), and Falls Efficacy Scale-International (FES-I) were administered. Reliability and validity were evaluated according to Cronbach's alpha internal consistency coefficients (ICC), spearman correlation analysis, and confirmatory factor analysis (CFA).</div></div><div><h3>Results</h3><div>LEFS-T was feasible, had good internal consistency (0.93), good reliability (ICC = 0.98), good construct, and discriminant validity, and showed no floor or ceiling effects. The results of CFA are at excellent levels (Root Mean Square Error of Approximation = 0.004, Goodness of Fit = 0.974, Comparative Fit Index = 0.991, Non-Normed Fit Index = 0.990). For construct validity, LEFS-T showed a better correlation with FTSTS (<em>r</em> = -0.555, <em>p</em> &lt; 0.001) and FES-I (<em>r</em> = -0.756, <em>p</em> &lt; 0.001).</div></div><div><h3>Conclusion</h3><div>The Turkish version of LEFS has good psychometric properties to evaluate functional capacity in older adults without lower extremity musculoskeletal disorders. Therefore, capturing the flexed trunk posture on radiograph is difficult.</div></div><div><h3>Objective</h3><div>To analyze the interrater reliability in different positions reflecting the dynamic phenomenon of camptocormia using a non-invasive visual three-dimensional capture tool.</div></div><div><h3>Methods</h3><div>Sample size calculation was at least 31 participants. 34 patients who had camptocormia and were monitored in our clinical practice were included. Two different examiners performed measurements using SAM3D tool in the three following positions: Spontaneous Standing Position (SSP), Upright Standing Position (USP), and Walking-like Standing Position (WSP). Sagittal parameters were measured: arrow in C7, kyphosis, lordosis, and knee flexion. The interrater reliability was assessed using Lin's Concordance Correlation Coefficient (CCC).</div></div><div><h3>Results</h3><div>Regarding the arrow in C7, the Lin's CCCs were excellent, respectively 0.96 (SSP), 0.98 (USP), and 0.97 (WSP). All the CCCs for other parameters were at least fairly good (&gt;0.87). However, protective behavior that is ongoing in the absence of tissue damage may contribute to the persistence of disability in people with musculoskeletal pain. Current evidence highlights the importance of addressing pain-related fear, fear of movement-related pain, and avoidance behavior in the management of people with chronic musculoskeletal pain. But, physical therapists find it challenging to make sense of and implement the evidence in their clinical practice. This issue partly stems from the pervasiveness of the biomedical model, which fails to address important psychological factors such as fear of movement-related pain and avoidance behavior and their role in chronic musculoskeletal pain. The experiences of physical therapists and the difficulties they face are critical to the effectiveness of telerehabilitation.</div></div><div><h3>Objective</h3><div>To examine in depth the technical challenges, patient interaction strategies, and session management processes faced by physical therapists who use telerehabilitation for patients with neurological conditions and to assess the opinions of physical therapists about the advantages, disadvantages, technological infrastructure needs, and support strategies of telerehabilitation.</div></div><div><h3>Methods</h3><div>In this study, phenomenological qualitative research method was used. In-depth interviews were conducted with eight physical therapists who treated patients with neurological disorders with telerehabilitation in our country. Interview data were analyzed using content analysis.</div></div><div><h3>Results</h3><div>The analysis revealed seven key themes and 26 sub-themes, which were categorized into two main areas: telerehabilitation session management and perceptions of telerehabilitation sessions. In the area of session management, physical therapists reported challenges related to the adaptation of traditional rehabilitation techniques to the online environment, including difficulties with assessing patients' physical conditions remotely, ensuring patient engagement, and managing technical issues such as internet connectivity and software limitations. They also highlighted the importance of clear communication, structured session planning, and the need for additional training to effectively conduct telerehabilitation sessions.</div></div><div><h3>Conclusions</h3><div>Considering the advantages and disadvantages stated by physical therapists using telerehabilitation with patients with neurological conditions, solutions should be developed to increase the efficiency of telerehabilitation. Reis","doi":"10.1016/j.bjpt.2025.101192","DOIUrl":"10.1016/j.bjpt.2025.101192","url":null,"abstract":"<div><h3>Background</h3><div>The Choosing Wisely campaign (CWC) aims to reduce low-value care by encouraging healthcare professionals and patients to discuss unnecessary tests and treatments. While this campaign has been adopted in various medical fields, its implementation in physical therapy is still emerging.</div></div><div><h3>Objectives</h3><div>To (i) identify physical therapy associations participating in the CWC and (ii) characterize the content of their recommendations.</div></div><div><h3>Methods</h3><div>We identified physical therapy associations affiliated with World Physiotherapy and conducted a search on their websites or contacted them via email to identify existing Choosing Wisely recommendations. The identified recommendations were categorized into musculoskeletal, neurology, cardiorespiratory, women's health, and mixed themes.</div></div><div><h3>Results</h3><div>Out of 127 physical therapy associations, seven (5.5 %) had Choosing Wisely recommendations, representing associations from Brazil, the USA, Norway, Italy, Australia, Spain, and Switzerland. We identified 62 recommendations, with the majority (48.4 %) related to musculoskeletal physical therapy, followed by mixed themes (27.4 %), women's health (14.5 %), cardiorespiratory (6.4 %), and neurology (3.2 %).</div></div><div><h3>Conclusion</h3><div>The adoption of the Choosing Wisely campaign among physical therapy associations appears to be limited. Most recommendations focus on musculoskeletal physical therapy, indicating an opportunity for broader engagement with the campaign.
