Colita","doi":"10.2478/rrlm-2023-0019","DOIUrl":"https://doi.org/10.2478/rrlm-2023-0019","url":null,"abstract":"Abstract Introduction: Over the past few years, the introduction of chimeric antigen receptor (CAR) T-cell therapy by the FDA has shown remarkable success in treating various hematologic malignancies. However, the limited response and resistance observed in some patients have hindered its broader application. Methods: At Fundeni Clinical Institute, we implemented the use of Tisagenlecleucel, a second-generation CAR T cell therapy, in April 2022. This therapy targets CD19, an antigen expressed in all B lineage cells. To assess the cellular kinetics of CAR T cell-treated patients and conduct further research, we developed an 8-color/10-parameter flow cytometry tube. This tube utilizes a biotinylated CD19 CAR Detection Reagent with high sensitivity and specificity for CD19-targeted CARs, enabling us to effectively separate CAR T cells from normal T cells. Results: Through immunophenotyping, we successfully identified circulating CAR T cells and distinguished various subtypes of immune cells in the peripheral blood of infused patients. Furthermore, we validated the accuracy of our flow cytometry panel for monitoring the progress of CAR T cell therapy. Conclusions: This paper highlights the implementation of our flow cytometry monitoring panel for CAR T cells following Tisagenlecleucel therapy at Fundeni Clinical Institute. Our practical solution allows us to identify CAR T cells, assess B cell presence, and characterize different T cell subtypes in our patients. Despite traditional risk factors, asymptomatic individuals with atherosclerosis have been identified, and new biomarkers are being sought to better predict CVD risk. This study aims to investigate the relationship between platelets indices and biochemical variability in the first day after patient admission in the hospital. Methods: A pilot study was conducted to analyze 73 medical records of patients who experienced acute myocardial infarction, to analyze the association among mean platelet volume (MPV), platelet count (PC), creatine phosphokinase (CPK), creatine phosphokinase-MB (CKMB) and ultra-sensitive cardiac troponin I (TnI-us) between the first and second sampling (1-hour intervals), to investigate acute ST-segment elevation myocardial infarction. Results and conclusion: The biochemical analysis showed no significant difference in CPK CK MB, TnI-us, MPV and PC levels between the first and second samples. We analyzed ascitic and blood samples from twenty cirrhotic patients, using the Child-Pugh classification to assess liver cirrhosis severity. Methods: Blood tests evaluated platelets, albumin, INR, fibrinogen, CRP, ferritin, and D-dimers, while ascitic fluid analysis incorporated total protein, albumin, LDH, cholesterol, leukocytes, with a focus on D-dimers and ferritin. Results: The results showed a significant positive correlation between ascitic D-dimers and ferritin concentrations and ascitic albumin, total protein, and cholesterol. A correlation was also observed between ascitic ferritin and LDH. These correlations suggest a potential relationship between D-dimers and ferritin levels, disease severity, and the local production of these markers. Conclusion: This study proposes that evaluating D-dimers and ferritin in ascitic fluid might reveal underlying inflammation, even when other inflammatory laboratory markers are normal. Our findings suggest that ascitic D-dimers and ferritin levels could offer valuable insights into inflammatory processes in ascitic fluid, potentially enhancing diagnostic and prognostic approaches for patients with ascites from liver cirrhosis or other causes. We aimed to analyze the diagnostic values of serum visceral adipose tissue-derived serine protease inhibitor (Vaspin) and secreted frizzle-related protein 5 (SFRP5) for ISS children. Methods: Sixty-five ISS children treated from February 2019 to June 2022 were selected as an ISS group, while another 65 healthy children receiving physical examination in the same period were selected as a healthy group. Their general data, physical development status, levels of serum Vaspin and SFRP5, and levels of serum growth hormone-releasing hormone (GHRH)/growth hormone (GH)/insulin-like growth factor-1 (IGF-1) axis-related indicators were compared. Results: The ISS group had lower body height, body weight, bone age, growth velocity and serum SFRP5 level and higher serum Vaspin level than those of the healthy group (P<0.05). The levels of serum GHRH, GH and IGF-1 were lower in the ISS group than those in the healthy group (P<0.05). Body height, body weight, bone age, growth velocity, and levels of GHRH, GH and IGF-1 were negatively correlated with serum Vaspin level (r<0, P<0.05) but positively correlated with serum SFRP5 level (r>0, P<0.05). The areas under the ROC curves of serum Vaspin and SFRP5 and their combination for the diagnosis of ISS were 0.871 [95% confidence interval (CI): 0.812-0.929], 0.880 (95% CI: 0.824-0.935) and 0.942 (95% CI: 0.907-0.977), respectively. During the process of oncogenesis, genetic and epigenetic changes lead to abnormal expression of genes associated with cellular pathways that coordinate extremely important functions such as cell multiplication, cell differentiation, cell death, and cell cycle. Methods: There are over 200 approved biomarker-driven drugs for various types of cancer. Valuable biomarkers are analyzed to establish their importance in specific therapies. Precision medicine for oncological patients has been recognized as a valuable approach to solid tumors. Results: Various genes and their mutations either have a direct pathogenic effect or can give hints to a certain prognosis regarding the oncological pathology. A comprehensive genetic test for a broad molecular profile and complete characterization of tumor genetic heterogenicity should contain genes that are aligned with professional practice, guidelines and clinical trials, full coding region coverage for each gene and targeting of unique emerging and actionable markers. It is useful to use such a comprehensive test when a broad genomic profile identifies treatment options including immunotherapies and targeted drugs for patient enrollment or when relapse or disease progression has occurred after prior therapies. Conclusions: For patients with solid tumors, personalized medicine has been recognized as a successful strategy treatment, but it is not sufficient to seize cancer growth and progression up to a single molecular alteration due to specific hallmarks such as tumor heterogeneity, clonal evolution, and independent resistance mechanisms. Earlier studies have evaluated the effectiveness of using multigene panel screening methods for personalized cancer therapy, with controversial results. Future research in the field of circulating tumor DNA analysis might be the key to overcoming some of these limitations. This study investigated the prevalence of VRE isolates of clinical specimens from a tertiary hospital in Southern Thailand and their biofilm formation and associated virulence factors. Methods This retrospective study was conducted from February 2011 to March 2021 at a 1,000-bed tertiary care hospital in Songkhla Province in Southern Thailand. Results In total, 95 VRE isolates were collected. Urine had the highest VRE prevalence (38%), followed by pus or secretions (23%) and the digestive tract (19%). VRE infections were most common in medical (45%) and surgical wards (19%). VRE strains were equally resistant (85–100%) to the six classes of antibiotics commonly used in a tertiary care hospital. Biofilms were produced by 73% of the multidrug-resistant strains; however, 93% of VRE isolates lacking hyl were highly capable of producing biofilms. Conclusions No relationship was observed between virulence genes and biofilm formation. Methods The study participants included 200 pregnant women, 100 healthy subjects as the control group, and 100 having GDM as the case group. The enzyme-linked immunosorbent assay (ELISA) was used to examine adiponectin levels. The polymerase chain reaction-restriction length polymorphism (PCR-RFLP) was also used for genotyping examination. Results Findings showed that GDM patients had significantly lower serum adiponectin levels (p=0.001). The T/T genotype ho-mozygotes prevalence for rs1501299 polymorphism was significantly lower in GDM patients. (p=0.047). Conclusion Low serum adiponectin levels are associated with GDM. Methods One hundred patients with sepsis treated from August 2021 to August 2022 were included. They were assigned to a sepsis group (n=65) and an SAE group (n=35), while 50 healthy volunteers physically examined in the same period were enrolled as a control group. The levels of GFAP and NSE were detected by enzyme-linked immunosorbent assay, and that of S100β was determined by transmitted immunoturbidimetric assay. The expressions of GFAP, NSE and S100β in patients with SAE were detected, and their correlations and diagnostic values were analyzed. Results Compared to patients with mild and moderate SAE, those with severe SAE had higher levels of GFAP, NSE and S100β (P<0.05). The levels of GFAP, NSE and S100β were higher in coma patients than those with consciousness disturbance, and they were higher in patients with a poor prognosis than those with a good prognosis (P<0.05). Positive correlations were identified between GFAP and NSE (r=0.573, P=0.001), GFAP and S100β (r=0.468, P=0.005), and NSE and S100β (r=0.540, P=0.001) expression in patients with SAE. Compared with GFAP, NSE and S100β alone, their combination had higher sensitivity and lower specificity for diagnosing SAE (P<0.05). Methods A total of 232 patients with axial spondyloarthritis admitted from January 2020 to December 2021 were selected, among whom 132 were AS patients (AS group) and 100 were non-AS cases (non-AS group). Meanwhile, 100 healthy subjects undergoing physical examination were included as a control group. Peripheral blood was collected to detect the levels of CRP, ESR, HLA-B27, IgA, IgG, and IgM. Results The positive rates and levels of CRP, ESR, HLA-B27, IgG, and IgM in non-AS and AS groups were significantly higher, and the IgA level of the AS group was higher than those of the control group (P<0.05). The AS group had significantly higher positive rates and levels of CRP, ESR, HLA-B27, and IgA than those of the non-AS group (P<0.05). The levels of HLA-B27, ESR and CRP had positive correlations with those of IgA, IgG and IgM (P<0.05). The area under the curve (AUC) of HLA-B27 with a cut-off value of 53.18 U/mL for the diagnosis of AS was 0.879. AUC of ESR with a cut-off value of 23.83 mm/h for the diagnosis of AS was 0.811. AUC of CRP with a cut-off value of 14.35 mg/L for the diagnosis of AS was 0.745. With the cut-off values of 2.75 g/L, respectively, AUC of IgA for the diagnosis of AS were 0.668. AUC of the combined diagnosis was 0.929 (P<0.05). Conclusions AS patients have abnormally elevated levels of HLA-B27, ESR, CRP, and IgA, and the combined detection has a higher diagnostic value.
