When capacity to consent is impaired, autonomy can be preserved through a clear, legally valid advance directive. Cognitive impairments, such as dementia, increase the risk of compromised capacity to consent. This study examines the prevalence of advance directives among patients at German memory clinics and explores predictors influencing their presence, focusing on compliance with informed consent standards.Using an observational cross-sectional design, 289 outpatients were assessed. Data included the presence of advance directives, circumstances of their creation and the patient characteristics age, gender, education, cognitive status, physical and mental health, need for autonomy in medical decision-making and health literacy. Descriptive statistics were calculated, and multivariable logistic regression models were applied to analyze the data.The results indicate that age is the strongest predictor for the presence of advance directives. As older lesbian, gay, bisexual, transgender, and intersex (LGBTI*) people frequently live alone and have children less often, non-kinship relationships take on a particularly important role. The extent to which non-heterosexual individuals develop (potentially compensatory) strategies to cope with episodes of needing care has, to date, rarely been the focus of research on aging.</p><p><strong>Objective: </strong>To investigate the role of social support for older LGBTI* people with long-term care needs, including any implications for how this should be approached.</p><p><strong>Methods: </strong>The international scoping review was conducted using the scientific databases: APA PsycInfo, CINAHL, Psychology and Behavioral Sciences Collection, PSYNDEX literature with PSYNDEX tests, BASE, Livivo, Pubpsych, PubMed, and Google Scholar. A total of 19 articles were identified for analysis.</p><p><strong>Conclusion: </strong>This article highlights three key themes related to social networks and care for older LGBTI* individuals: sociodemographic challenges, support resources, and structures for care of older persons and strategies for good quality of care. Family of choice connections are the backbone of social support for LGBTI* individuals but are also perceived as fragile when it comes to long-term care. Strategies for good quality of care include visibility of non-conforming sexual or gender identity, stigma management in long-term care facilities and proactive care planning. In order to support the compensatory effect of chosen family relationships among LGBTI* seniors, the development of diversity sensitive care and work with caregivers is needed. This article aims to compare older people with and without PG recommendations in terms of sociodemographic characteristics, independence in various areas of life as well as medical diagnoses.</p><p><strong>Methods: </strong>The analysis is based on n = 122,478 initial care assessments of adults over the age of 50 years, which were carried out by the Bavarian Medical Service (MD) in 2019. Descriptive analyses are used to compare people with and without PG recommendations.</p><p><strong>Results: </strong>Of those assessed, 19.5% were not given a PG recommendation. These people are younger, more often female and living alone than people with PG recommendation. Overall, those assessed without PG recommendation are less restricted in their independence than those with a PG recommendation. Nevertheless, they also have significant limitations in activities relevant to everyday life (especially in mobility outside the home, shopping, cleaning the home). In both groups, a limitation of gait and mobility is the most common diagnosis that leads to an application for a care assessment.</p><p><strong>Conclusion: </strong>Routine data from long-term care assessments provide information about older people who are on the threshold of needing long-term care. Each item uses a 5-point scale to assess how independently the person can perform the described function. The grading ranges from 'independent/ability present' to 'dependent/ability absent'.</p><p><strong>Material and methods: </strong>1593 rehabilitants were included in the study. The FIDA and Barthel Index (BI) assessments were used to measure functional independence at the beginning and at the end of rehabilitation. Item difficulty, discriminatory power (r<sub>it</sub> = item total correlation) and internal consistency (Cronbach's α) were calculated. The factor structure was tested using a confirmatory factor analysis. Convergent validity was tested by correlating the sum values of the FIDA and the BI.</p><p><strong>Results: </strong>The item difficulties (p<sub>i</sub>) were in the range between 0.17 ≤ p<sub>i</sub> ≤ 0.89. The internal consistency was α = 0.94 with item-scale correlations between 0.45 < r<sub>it</sub> < 0.80. The agreement between FIDA and BI at the start of rehabilitation was r = 0.75. The standardised comparison of the sensitivity to change showed slight advantages for the FIDA. The functional areas that were more severely impaired at the start of rehabilitation showed greater improvements.</p><p><strong>Conclusion: </strong>The FIDA is a change-sensitive assessment instrument that is suitable for assessing functional independence in geriatric patients. It is known that nursing homes with a dementia care unit can provide further training or an expert on person-centredness compared to those with a traditional care unit. It is unclear how the mission statements of these nursing homes differ in terms of their wording of person-centredness. The aim is to investigate how mission statements of nursing homes that provide different types of care units describe person-centred care.</p><p><strong>Methods: </strong>We conducted a document analysis of mission statements from German nursing homes from a dataset of the survey study on specialized care for people living with dementia in nursing homes (BeStaDem Survey Study). The nursing homes that provided the mission statements had different care units, including traditional care units and dementia care units. The mission statements were deductively-inductively analysed using content analysis.</p><p><strong>Results and conclusions: </strong>We analysed 60 mission statements of different nursing homes and identified main categories that include aspects of person-centred care. In particular, mission statements of nursing homes with dementia care units describe combinations of central aspects of person-centred care that mission statements of nursing homes with a traditional care unit without dementia-specific focus do not describe. The variation of different understandings or concepts often becomes evident in mission statements. Additionally, it examined the challenges and desired changes in the intersectoral communication and collaboration for prevention of POD.</p><p><strong>Methods: </strong>This multicenter study was carried out with a qualitative research approach. To explore the experiences of HCPs a total of 21 semistructured expert interviews were conducted with general practitioners, geriatricians, anesthesiologists, surgeons and nurses across the 2 study regions Salzburg County and South Tyrol. The interviews were analyzed using content analysis.</p><p><strong>Results: </strong>The results show that the interviewed HCPs already implement measures to prevent POD but these predominantly focus on identifying patients at risk in the inpatient sector. Information about risk factors is not systematically shared between disciplines or across the intersectoral continuum of care. Structures and processes for intersectoral communication and collaboration in the prevention of POD over the continuum of care are largely missing.</p><p><strong>Conclusion: </strong>To improve the prevention of POD in older adults efforts should be made to enhance intersectoral and interprofessional collaboration and communication between the HCPs involved. A low socioeconomic status means high risks for health and poverty, disadvantageous housing situations and fewer opportunities for social participation.</p><p><strong>Aim/question: </strong>In social space-oriented approaches, the possibilities for strengthening the social participation of marginalized older people still appear to be limited. What should a professional concept look like in order to make weak interests heard?</p><p><strong>Concept: </strong>The concept of social space development and social space organization as an instrument for professional promotion of highlighting and realization of own and mutual interests is presented. It was developed in social work in the context of participatory social space research and examples are presented in this article. Social space development aims to enable groups of people who are connected by mutual interests to meet not only in special places created for the care of older people or community work but also in public places in the spaces that are important for themselves and to appropriate spaces for the representation of their own life designs. In the second step, different interest groups are brought into a moderated negotiation process.
