{"title":"[Heat dome in Germany and how well we are prepared for it].","authors":"Clemens Becker, Thomas Griebe, Christian Weingart","doi":"10.1007/s00391-025-02459-9","DOIUrl":"10.1007/s00391-025-02459-9","url":null,"abstract":"<p><strong>Background: </strong>In the last 4 years some countries, such as Arizona (USA), India, Saudi Arabia, Australia and British Columbia (Canada), have experienced extremely long heat waves of 40 °C and more. The duration of the episodes ranged from 14 days in Canada to more than 3 months in Arizona. The heat dome in the Vancouver region showed that similar events can occur in our latitudes but these are not adequately taken into account in the planning in Germany.</p><p><strong>Methods: </strong>So far only peer-reviewed data from Canada are available and data on other events have not yet been comprehensively processed. Governmental information and other trustworthy sources were used for the research. The aim was to identify areas which have so far not been sufficiently considered in the ongoing planning discourse in Germany.</p><p><strong>Results: </strong>Numerous measures were identified that have not or only insufficiently been considered in Germany, such as the establishment of crisis management teams, determination of prior planning of a communications strategy and predefined responsibilities for measures, such as bans on outdoor activities, evacuation and holiday restrictions for healthcare workers. Extreme heat events must be defined as natural disasters in order to initiate these and other measures in Germany due to legal requirements. Many of the measures that are now being planned on a voluntary basis would then have to be mandated and, if necessary, made compulsory.</p><p><strong>Conclusion: </strong>The planning of measures for extreme heat events must be revised in many places in Germany. These are not stipulated in the 25 action plans for heat events published so far. Many regions are not adequately prepared. {"title":"[Theory-driven examination of available data on age(ing) and climate change: an explorative contribution].","authors":"Hans-Werner Wahl, Andrea Budnick","doi":"10.1007/s00391-025-02453-1","DOIUrl":"10.1007/s00391-025-02453-1","url":null,"abstract":"<p><strong>Background: </strong>The socioemotional selectivity theory and the concept of generativity lead to two contradictory predictions, namely (1) a decrease in the significance and importance of climate issues with age, but (2) a similarly high level of engagement at the behavioral level, at least compared to younger age groups.</p><p><strong>Method: </strong>Marking survey studies from German-speaking and European countries are used to provide a descriptive test of the two predictions.</p><p><strong>Results: </strong>On the one hand, the selected studies confirm a decrease in the significance and importance of climate issues as people get older. On the other hand, the age segment of people between around 60 and 75 years old in particular shows a similar or even higher level of commitment in many climate-relevant areas of behavior compared to younger people.</p><p><strong>Conclusion: </strong>We apply the theory of planned behavior to explain the at first glance inconsistent findings driven by our predictions. Successful international prevention initiatives show the importance of involving primary target groups early in the process of program development.</p><p><strong>Objective: </strong>The aim of this study was to explore (1) the knowledge about dementia/brain health, (2) offers of activities related to dementia prevention in the community and (3) the feasibility of introducing the experience of art as a brain health-promoting method in a sample of community-dwelling older persons.</p><p><strong>Material and methods: </strong>Senior volunteers from the community were invited to a workshop and a follow-up telephone interview. During the workshop, participants received expert input on dementia/brain health followed by several group discussion rounds using a \"World Café\" approach. After 6 weeks, participants were invited to a structured telephone interview.</p><p><strong>Results: </strong>A total of 26 persons participated in the workshop, 20 of whom (mean age: 70.60 years) took part in the telephone interview. The workshop data revealed 4 main needs: (1) more information on dementia/brain health (2) a broader offer of activities for physical/mental stimulation in the community (3) the definition of a \"brain health strategy\", and (4) development of specific services to experience art. The telephone interviews revealed a high motivation to start with dementia prevention but appropriate services are missing in the communities.</p><p><strong>Conclusion: </strong>Our findings provide first insights into attitudes towards dementia prevention/brain health in an Austrian sample of senior citizens. It is not obvious whether this means they have to cope with other challenges than caring relatives without a professional background.</p><p><strong>Objective: </strong>This article aims to gain a better understanding and an overview of how double duty caregivers experience their dual role and the challenges they face.</p><p><strong>Methods: </strong>The systematic literature search was conducted in 11 specialized databases as well as via a snowball system. Included in the study were 32 articles published in German and English between January 1995 and October 2023.</p><p><strong>Results: </strong>A total of 6 topic complexes were identified that deal with the experience and challenges of double duty caregivers: emotional and personal aspects, demands and expectations, personal needs, role identification, nursing expertise and interaction with professionals in the healthcare system.</p><p><strong>Conclusion: </strong>The challenges for double duty caregivers are complex. The results show that finding their own role in the nursing arrangement is particularly challenging as the professional and private nursing roles are closely intertwined. The preference for one of these roles varies depending on the personality, type of relationship or care situation. This problem is often not recognized or respected by the environment, which can reinforce existing feelings of stress. The aim of the current analysis within the Berlin Aging Study II (BASE-II) was to investigate the influence of abdominal obesity and muscle mass on pulmonary function in subjects with T2D and MetS.</p><p><strong>Material and methods: </strong>A prebronchodilator pulmonary function test was carried out in 1369 subjects from the BASE-II (mean age 69 ± 4 years, 51.6 % women) where T2D was defined according to the German Diabetes Association (DDG) criteria, MetS according to the criteria of the International Diabetes Foundation (IDF), American Heart Association (AHA) and National Heart, Lung and Blood Institute (NHLBI) criteria from 2009 and pulmonary obstruction (obstructive lung disease, OLD) by the Global Initiative for Chronic Obstructive Lung Disease (GOLD) criteria of a forced expiratory volume in 1 s (FEV1 and forced vital capacity (FVC) ratio < 70 %.</p><p><strong>Results: </strong>Of the subjects 50.9 % achieved a sufficient level of quality according to the GOLD guidelines and were analyzed with respect to the research question. The FEV1 and FVC were decreased in study participants with T2D and MetS and the lung volume decreased with an increasing number of MetS criteria. Parameters of body composition, such as waist circumference and muscle mass had a significant influence on lung volumes, independent of MetS or T2D.</p><p><strong>Discussion: </strong>In this study MetS and T2D were associated with decreased lung volumes; however, muscle mass and abdominal obesity proved to be the most important factors influencing pulmonary function and could thus form the link between pulmonary function and MetS or T2D. Air pollution not only leads to an increased rate of cardiovascular and respiratory diseases it also increases the risk of sarcopenia and frailty. Therefore, healthy ageing is inconceivable without sustainable environmental protection. Under study conditions traditional approaches to cooling during heat waves (e.g., foot baths and fans) have been proven to be inadequate for older persons. Identifying people at risk and introducing preventive measures, including medication adjustments, are of great importance in this context.
