The causes for this discrepancy have not yet been researched in this target group.</p><p><strong>Objectives: </strong>Aim of this study was to get insight into the perspectives of OPLA in the community on planning ahead for the end of life.</p><p><strong>Materials and methods: </strong>(Secondary) Analysis of 26 qualitative interviews. 17 were from the Austrian Survey on the very old (ÖIHS), and 9 additional interviews were conducted with a specific focus on advance care planning to enrich the data base. Verbatim transcripts were analysed by applying coding procedures.</p><p><strong>Results: </strong>Not all people had specific concerns or a desire to deal with the end of life. In contrast to others, concerns about avoiding (unnecessary) life-prolonging measures resulted in a formal regulation, usually a living will. Staying at home or issues regarding burial and inheritance were often equally important concerns for OPLA. Numerous obstacles to formal arrangements were identified.</p><p><strong>Conclusion: </strong>The heterogeneity of OPLA with regard to their willingness and form of planning ahead for staying at home shows the limits of standardized procedures. Ophthalmological care in nursing homes is often inadequate, leading to poor detection and treatment of visual problems. The prevention program \"Good vision in care facilities\" (\"Gutes Sehen in Pflegeeinrichtungen\") of the Institute for the Blind Foundation (Blindeninstitutsstiftung) aims to promote the eye health of residents through targeted measures and the training of qualified vision officers.</p><p><strong>Objective: </strong>The aim was to evaluate the prevention program from the perspective of the vision officers, focusing on their role, the performance of vision assessments, the improvement of the living environment and the qualification measures.</p><p><strong>Method: </strong>A qualitative evaluation study was conducted through guideline-based interviews with eight vision officers from seven nursing homes. The data were analyzed using Mayring's structured content analysis.</p><p><strong>Results: </strong>Vision officers consider their role to be important but the success of implementation depends on the available resources. The performance of vision assessment varies due to cognitive impairment and motivation of the residents. Barriers to accessibility were primarily addressed when minimal effort was required. Face to face training sessions contribute significantly to awareness due to their practical content and interaction.</p><p><strong>Conclusion: </strong>The sustainable implementation of the prevention program requires resource allocation by nursing home management. The qualification of several vision officers in each facility could increase the success of the program. {"title":"[Improvement of oral health of older people in hospitals: a scoping review].","authors":"Damian Pazdziernik, Harald Stummer","doi":"10.1007/s00391-024-02339-8","DOIUrl":"10.1007/s00391-024-02339-8","url":null,"abstract":"<p><strong>Background: </strong>The current evidence regarding interventions to improve oral health in older hospital patients is unclear.</p><p><strong>Objective: </strong>This scoping review assesses the scope and nature of research activities and identifies gaps in the existing literature.</p><p><strong>Methods: </strong>The inclusion criteria for this study included adults over the age of 65 years who were treated as inpatient outside of intensive care units and covered all interventions aimed at promoting oral health or hygiene.</p><p><strong>Results: </strong>The systematic search yielded 12 final studies, focusing on oral healthcare interventions in various settings, primarily in high-income countries. The studies employed diverse designs including randomized controlled trials and prospective studies, with interventions mainly provided by multidisciplinary teams. The interventions aimed to improve oral health or prevent pneumonia. Overall, the studies highlighted a potential effectiveness of multidisciplinary approaches in improving oral health and preventing pneumonia in geriatric populations.</p><p><strong>Conclusion: </strong>This scoping review shows a limited and heterogeneous evidence base for oral health interventions for older patients in hospitals. Frailty, cognitive impairment and sarcopenia are predictors for inadequately performed lung function testing as well as presenting challenges in subsequent disease management. Underdiagnosis and suboptimal treatment are often the result of incomplete assessment of older COPD patients. Problems associated with older COPD patients are not adequately addressed in international guidelines, although numerous evidence-based strategies are available. A key aspect is the management of the various inhaler devices. Up to 60-80% of older patients with COPD do not use the inhalers correctly. Even when the inhaler technique is correct, there is undertreatment if the required inspiratory flow cannot be achieved. Given the high rate of suboptimal inhaler treatment in older patients with COPD, an analysis of the pitfalls is important. An objective measurement of inspiratory flow and assessment of cognition and coordination are essential. At the same time there is an increase in pulmonary diseases that lead to restrictions in mobility and autonomy.</p><p><strong>Research question: </strong>What are the underlying changes in lung ageing? To what extent do they affect lung function and are there factors that can be influenced?</p><p><strong>Method: </strong>Literature search.</p><p><strong>Results: </strong>Ageing of the lungs is associated with a loss of elasticity and distensibility. Senescence-associated factors play an important role at the molecular level. Accumulation of damaged DNA and proteins, oxidative stress and chronic inflammation are major factors. Avoidance of harmful environmental factors can reduce the disease burden.</p><p><strong>Conclusion: </strong>Age-related pathophysiological changes lead to increased work of breathing with decreasing muscle strength. We conducted a pragmatic trial to test whether staff-directed interventions can improve chronic pain management in older community-dwelling adults and thereby improve their pain situation. Participants of 22 ambulatory nursing services (clusters) were allocated to 3 study arms: I1 (individual intervention with recommendations for each participant's physician and a newly trained pain nurse), I2 (digital training offered for participants' physicians and nursing staff) and CG (control group). Survey-based face to face interviews were held at the participants' homes. Descriptive statistics, ANOVA, and χ<sup>2</sup>-tests were utilized for data analysis. At baseline, 190 and at follow-up 144 participants were analyzed (24% dropout). Overall, the interventions were only implemented by a small proportion of involved staff. We found significant changes in documented nursing care in I1 but improvements regarding pain medication appropriateness or pain situations of participants could not be achieved. Structural challenges like time and staff shortages as well as the lack of billing options in ambulatory care were identified as major obstacles to substantially improve pain care. The Engagement of a Person with Dementia Scale (EPWDS) is a valid and reliable tool to assess behavioral and emotional expressions and responses of engagement in people with dementia through observation; however, the EPWDS is not yet available in the German language.</p><p><strong>Objectives: </strong>1) Translation and cross-culturally adaptation of the original English version of the EPWDS into the German language (EPWDS-GER) and 2) to gain insights into assessing data with the newly developed instrument.</p><p><strong>Material and methods: </strong>International recommendations were followed to cross-culturally adapt the English original version of the EPWDS into the German language in 5 steps: translation by three independent translators, synthesis, back translation, expert committee review (N = 10) and test of the prefinal version in nursing practice (N = 22) on a 5-point Likert scale to assess comprehensibility, practicability and suitability of the EPWDS-GER.</p><p><strong>Results: </strong>The EPWDS-GER achieved high ratings for the five subscales on ease of understanding, ease of answering and importance of single items for assessing engagement. Average agreement for all items ranged from 3.86 to 4.43 (SD = 0.68-1.29).
