It is prevalent among women in the age group 30 to 50 years. Other than placental estrogens and progesterone, a range of endocrine and paracrine factors have their impact on fibroid blood supply, growth rate, and risk of degeneration during the gestational and puerperal periods. The current case report is about a 30-year-old primigravida with pregnancy and a large fibroid presenting with complications such as malpresentation and pressure symptoms, ending up in a cesarean section at 36 weeks delivering a live baby with a birth weight of 2.5 kgs and a large fibroid weighing 6.4 kg. During the pregnancy, there was adequate fetal development, without major maternal complications. The patient presented to us with a history of swelling and pain in the left scrotum for one month. There were no other associating symptoms and sexual and traumatic history were not present. Incision and drainage were planned for the patient under our care, however, because of ruptured tunica albuginea and dispersion of seminiferous tubules, left orchiectomy was performed and the specimen was sent for histopathological analysis. Xantho-granulomatous inflammation (XGI) is a relatively benign and infrequent condition and usually affects the 3rd-7th decade of life. The lipid-laden macrophages replace the destructed and necrotic normal tissue. The findings that are the hallmark of granulomatous diseases are necrosis and multinucleated giant cells which are not present in cases of XGI. Comparison of the Efficacy of Duloxetine Versus Pregabalin for Pain Relief of Neuropathy in Diabetics 度洛西汀与普瑞巴林在缓解糖尿病患者神经病变疼痛方面的疗效比较

A wide variety of medications are used for diabetic neuropathy with varying degrees of pain relief reported. The study objective was to compare the efficacy of duloxetine versus pregabalin for pain relief of neuropathy in diabetics.\u0000\u0000Methods: The study was a six-week, single-blind, Randomized Controlled Trial conducted at HBS Medical and Dental College and Hospital in Islamabad. Patients were randomly designated to either of the groups (A or B) with 50 participants in each group. Group A received 150mg of pregabalin twice a day while group B received 60mg of duloxetine once a day. Pain relief was the primary outcome which was considered as a ≥ 50% decrease in pain score on the Visual Analog Scale at 6 weeks from baseline.\u0000\u0000Results: A total of 100 diabetic patients were registered in the study. The mean age in groups A&B was 48.36 and 50.56 (SD± 6.64) years respectively. The majority of the study population were males 39 (78%) and 42 (84%) in both groups. A total of 66% (n=33) in Group A and 74% (n=37) of the patients in Group B achieved pain relief. This may render them unable to maintain proper oral hygiene. The objective of this study was to evaluate oral hygiene in children with cerebral palsy in Karachi’s special schools.\u0000\u0000Methods: A cross-sectional study with a total of 80 CP children was conducted in special schools in Karachi using a purposive sampling technique. CP children aged 12-15, who consented via guardians/school and were able to comply with the procedure were included. CP children on medications or systemic diseases were excluded. Proforma containing demographic details, and dental history questions were asked. Oral examination was done using the Plaque index, Gingival index, and Oral hygiene index simplified was observed.\u0000\u0000Results: In this study, there were 48 (60%) males and 32 (40%) females. The mean ± SD of age was 14.4 ±1.4. Independent t-test showed that there was a significant association of gingival index means with the institutes selected (p=0.004). The other parameters PI and OHIS did not show any association. Mean PI (mean 1.99, SD 0.62) and GI (mean 1.9, SD 0.60) showed moderate plaque and gingival inflammation and poor oral hygiene index (mean 3.31, SD 1.3). This study evaluated the prevalence of NAFLD patients who also had type II diabetes.\u0000\u0000Methods: The demographic data (name, age, body mass index (BMI), gender, occupation, duration of TIIDM, and treatment taking for TIIDM), as well as the results of the tests for these enzymes, were also recorded. Alanine transaminase (ALT) and Aspartate transaminase (AST) levels, as well as abdominal ultrasound findings of the liver, were recorded in a proforma after informed consent for a total of 200 Type II diabetic patients. Using SPSS, the data were analyzed, and stratified groups were subjected to the Chi-square tests.\u0000\u0000Results: There was a significant difference (p<0.05) observed in NAFLD patients of TIIDM concerning age, gender, BMI, treatment, occupation, and duration of diabetes. The sample size consisted of 200 patients of which 111 (55.5%) were men and 89 (44.5%) were women. Patients ranged in the age range of 40-70 years old, with a mean age of 55.08 ± 8.98 years and a mean BMI of 29.69±4.04, respectively. Socioeconomic conditions were also affected. The initial response to the pandemic was total isolation termed a lockdown and the population was confined to their homes. Later on, two models were adopted. The majority of countries adopted mass vaccination and gradually decreased restrictions on movement and return to work. The Chinese government adopted a zero COVID policy resulting in popular unrest and weakening the economy. While zero COVID policy resulted in decreasing morbidity and mortality at the cost of limitations and restriction of movement and declined in economic recovery.
