Postoperative caesarean section infections remain a public health problem because of their high frequency, the cost of treating them and their seriousness. Aim: The aim of this study was to determine the frequency, risk factors and germs responsible for post-caesarean section surgical site infections in the obstetrics and gynaecology department of the commune I referral health centre in Bamako. Materials and methods: We conducted a cross-sectional, descriptive and analytical study over a 12-month period from 1er January to 31 December 2019. All women who underwent caesarean delivery in the department during the study period were included. Results: Out of 9158 deliveries we recorded́1793 caesarean sections, i.e. 19.58%. Among the 1793 cases of caesarean section we recorded 50 cases of surgical site infection or 2.78%. Long labour was the most common infectious risk factor (46%). The germs found were Escherichia coli, staphylococcus aureus, streptococcus agalactiaes, proteus mirabilis and proteus vulgaris. Conclusion: Bacterial infections of the post-caesarean surgical site are frequent in our department. Cyanocobalamin is a man-made form of vitamin B12 used to prevent and treat low blood levels of vitamin B12. Most people receive enough vitamin B12 from their diet. However serious vitamin B12 deficiency in some cases may result in a low number of red blood cells (anemia), stomach/intestine problems and permanent nerve damage. B12 deficiency could be seen in certain health conditions such as intestinal/stomach problems, poor nutrition, cancer, HIV infection, pregnancy, old age, alcoholism and in people who follow a strict vegetarian (vegan) diet. Accordingly early diagnosis and treatment of vitamin B12 deficiency is important to avoid any serious health problems. Traditionally, appendectomy has been the standard treatment. However, recent literature suggests that conservative antibiotic treatment could be a viable option, particularly for uncomplicated cases in children. Aim of the study: The aim of the study was to evaluate the outcomes of conservative treatment for uncomplicated acute appendicitis in children. Methods: From July 2017 to March 2019, a prospective observational study was conducted at Dhaka Medical College Hospital, Bangladesh, involving 62 children with uncomplicated acute appendicitis. They received conservative treatment, including supportive care, for five days. Patients who responded positively within 24 hours continued for at least five days before discharge and underwent a 6-month follow-up. Results: The study encompassed 62 patients, with a mean age of 8.95 years, and a slight male predominance (61.3% male, 38.7% female). Clinical findings revealed a notable decrease in pain intensity (9.42 to 0.77), temperature (101.02º to 98.90º), and inflammatory markers (CRP: 28.13 ± 11.56 mg/L to 9.78 ± 7.50 mg/L) from admission to day 5. The mean duration of hospital stay was 5.26 ± 0.63 days. Remarkably, 95.1% of patients successfully recovered with conservative treatment, while 4.9% required surgery as part of their treatment course. Conclusion: This study found some potential benefits of conservative treatment for uncomplicated acute appendicitis in children. Despite CSU being commonly associated with adults, the selected studies consistently demonstrate its presence in pediatric populations. Prevalence rates vary by region, with Western countries reporting figures between 0.1% to 0.3% among children, while some Asian populations exhibit higher rates. These discrepancies may stem from genetic, environmental, and ethnic factors. Additionally, the review emphasizes the substantial impact of CSU on children's lives, disrupting sleep, daily activities, and emotional well-being. Comorbidities, including other allergic diseases, are also prevalent among pediatric CSU cases, necessitating comprehensive clinical assessments and integrated management. The review addresses diagnostic challenges, stressing the importance of accurate differentiation from other urticarial conditions and systemic disorders. It explores evolving insights into CSU's pathophysiology in children, with emerging research implicating autoimmunity and chronic inflammation. In this review reaffirms that CSU affects children, with varying prevalence rates worldwide. This systematic review and meta-analysis aimed to assess the relationship between melasma and thyroid function indicators, including thyroid-stimulating hormone (TSH), thyroxine (T4), triiodothyronine (T3), and thyroid antibodies, particularly anti-thyroid peroxidase (anti-TPO) and antithyroglobulin antibody. A comprehensive search across multiple databases identified 39 relevant studies for analysis. The systematic review and meta-analysis were ultimately conducted based on the data extracted from these 39 selected studies. The meta-analysis revealed significant differences in thyroid function indicators between individuals with melasma and control groups. Melasma patients exhibited higher TSH levels (SMD = 0.33, 95% CI: 0.18, 0.47) and lower T4 levels (SMD = -1.50, 95% CI: -2.96, -0.04). However, no significant difference in T3 levels was observed (SMD = -0.01, 95% CI: -0.20, 0.19). Notably, the presence of anti-TPO antibodies was significantly associated with melasma (Fisher = 26.80, P = 0.020). Gender-specific analyses revealed more pronounced thyroid function differences among women with melasma, suggesting potential hormonal influences. Melasma predominantly affects women and often exhibits exacerbations during pregnancy, indicating a complex interplay between sex hormones and thyroid function in pigmentation regulation. These findings have clinical implications for dermatologists and endocrinologists. Dermatologists should consider thyroid evaluation in melasma patients, especially in cases with treatment resistance. For endocrinologists, melasma could serve as a cutaneous marker for underlying thyroid dysfunction, particularly in women. This procedure aims to relieve spinal pressure and stabilize the affected vertebral segments. Aim of the study: The aim of this study was to assess the clinical and functional outcomes of posterior decompression &amp; transforaminal lumbar interbody fusion (TLIF) using cage and bone graft combined with stabilization in lumbar spondylolisthesis. Methods: This prospective observational study was carried out at NITOR, Dhaka, Bangladesh, during the period from January 2020 to December 2021. Total 15 patients with degenerative lumbar spondylolisthesis were included in this study. Result: The study assessed 15 patients undergoing surgery for spondylolisthesis, primarily aged 40-49, with a mean age of 46.4 years. Gender distribution was 40% male, 60% female. Pre-operative spondylolisthesis was mostly at L4/L5 (60%) and L5/S1 (40%). Post-op data showed significant reductions in slip angle and VAS scores for back and leg pain (all P<0.05). Foraminal and disc space height increased. Fusion rate was 86.66% per Hackenberg criteria. Motor deficit improved from 33.3% to 6.7% post-op. According to Macnab criteria, 73% had excellent functional outcomes and 26.66% experienced minor complications; no major complications were recorded. Conclusion: As observed from the result of this study, degenerative lumber spondylolisthesis can be treated with posterior decompression and transforaminal lumbar interbody fusion by using cage and bone graft with spinal stabilization. This replacement has some adverse effects and complications. In this study, the complications and functional outcomes of THA has been discussed. Aim of the study: The aim of this study was to observe the complications that may arise after total hip arthroplasty. In this study, functional outcomes of THA was also another concern. Methods: This prospective observational study was conducted in the Department of Orthopedics in Community Based Medical College, Bangladesh and a private clinic during the period from January 2018 to July 2023 jointly. This study had institutional review board approval and informed consent for evaluation of study patients’ reports and outcomes. A general health questionnaire is used for data collection, the SF-8 Health Survey (SF-8), and a disease-specific health questionnaire, the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) to assess functional outcomes after THA. Result: The result to date show a general occurrence of both in-hospital and post-release difficulties of around 7% in THA patients. Fractures (0.6%) and cardiac events (0.4%) are the most common complications that occur in the hospital for THA patients. The most well-known post-release complications in THA patients are reoperation because of bleeding, wound necrosis, wound infection, or different causes. For both situations, the incidence of bleeding complications was lower than that of other adverse events. Conclusion: According to the observers, in both groups, in-hospital or after 3 months of surgery, the result was in expected improvements in functional outcomes. An early diagnosis and treatment with ATT (Anti Tubercular Therapy) is important to prevent adverse sequlae and can improve reproductive outcome. Aims and Objectives: The aim of the study is to evaluate the reproductive outcome of infertile patients with GTB. Methods: The present prospective observational study was conducted in Reproductive Endocrinology and Infertility Department of BSMMU from January 2018 to December 2021. Thirty (30) infertile women suspected of Genital Tuberculosis (GTB) who undergone hystero-laparoscopy as a part of infertility work up were included in our study. Results: GTB is the disease of young women with the mean age 27.5 ± 5.49 years in our study and most commonly present with primary infertility (63.33%). After completing antitubercular treatment, 90% of the study group had resumed normal menstrual cycles. A comparable proportion (16.67%) of GTB patients receiving ATT conceived, all without the aid of IVF. Among those who were conceived, 60% pregnancy remained uneventful, 20% had abortion, and 20% had ectopic pregnancy. The median length of time to conception after initiation of ATT was 4 months, stressing the role of ATT in conception in women with GTB. Conclusions: In low-income countries like Bangladesh where IVF is costly, Anti Tubercular Therapy may be started even on the basis of positive DNA PCR result or on the basis of laparoscopic and hysteroscopic findings. Dyslipidemia and hypomagnesemia are common in T2DM patients and may contribute to the development of complications. This study aimed to investigate the relationship between serum magnesium level and lipid profile in T2DM patients with and without foot ulcer. Methods: This cross-sectional study was conducted at the Department of Biochemistry and Molecular Biology, BIRDEM Academy, Dhaka, Bangladesh, involving 120 T2DM patients. Participants were divided into two groups: Group A (T2DM patients with foot ulcer, n=60) and Group B (T2DM patients without foot ulcer, n=60). Serum magnesium level and lipid profile were measured, and their correlations were analyzed. Result: Significant differences were observed between the two groups in terms of serum magnesium level and lipid profile. Group A had a higher prevalence of hypomagnesemia (69.6%) compared to Group B (30.4%). Elevated level of total cholesterol, triacylglycerol, and LDL cholesterol were also observed in Group A. A significant positive correlation was found between serum magnesium level and total cholesterol and LDL cholesterol in Group A, while no significant correlation was observed in Group B. Conclusion: The study revealed that T2DM patients with foot ulcers are more likely to have hypomagnesemia and dyslipidemia compared to those without foot ulcer. A significant correlation between serum magnesium level and lipid profile was observed in patients with foot ulcer. Through a comprehensive investigation, we aim to contribute valuable insights that could aid in designing more targeted and effective strategies for promoting the health and well-being of postmenopausal women. Objective: To assess serum uric acid in postmenopausal women. Methodology: This cross-sectional descriptive study was carried out at Department of Physiology of Rajshahi Medical College from July 2018 to June 2019. Where 200 healthy postmenopausal women aged 50-70 years in Rajshahi city were included as sample size. The healthy adults who were fulfill the inclusion criteria was enrolled in this study. After taking informed consent, complete history taking and physical examination was done and recorded in a preformed data sheet. Results: Out of 200 postmenopausal women, 19.5% have hyperuricemia. Remaining 80.5% of postmenopausal women have normal serum uric acid level. The mean serum uric acid level among group-I of postmenopausal women were 6.06±0.94 mg/dl and group-II of postmenopausal women were 6.35±1.02 mg/dl, which does not differ significantly. Conclusion: Heightened serum uric acid levels were detected among postmenopausal women, reinforcing the finding that as women in this group age, there is a noteworthy rise in their serum uric acid levels.
