Objective: To evaluate the association between serum zinc levels and zinc supplementation with topical retinoid-treated in acne vulgaris patients. Method: A clinical trial was carried out at the Department of Pharmacology and Department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University, spanning from March 2020 to January 2022, with participant enrollment commencing in April 2021. This study encompassed 122 recently diagnosed mild to moderate acne vulgaris (AV) patients who received 20 mg of zinc sulfate or 20mg of placebo tablet twice daily orally with topical retinoid treatment. Notably, nine participants withdrew from the study for various reasons, resulting in a cohort of 113 individuals for per-protocol (PP) analysis. The investigation featured both baseline and after 8-week follow-up assessments, scrutinizing GAGS acne severity scores and serum zinc levels. Results: After 8 weeks of intervention, serum zinc levels in the intervention arm significantly increased from the baseline (100.39 ± 22.26 vs. 59.03 ± 15.76, P = 0.00), and they were also significantly higher than in the control arm. Likewise, the GAGS score in the intervention arm significantly decreased after 8 weeks compared to the baseline (12.03 ± 5.39 vs. 16.74 ± 6.09, P = 0.00) and was significantly lower than in the control arm. Also shows a correlation between serum zinc level and GAGS score which was observed after 8 weeks of intervention. Conclusion: Our study suggests that combining zinc supplementation with topical retinoids for acne treatment depends on factors like patient traits, baseline zinc levels, formulations, and duration. While findings vary, there's an overall trend toward better outcomes with this combination. It comprises of stable angina, unstable angina, acute myocardial infarction and sudden death. Since the most common cause of IHD is coronary artery abnormality it is also termed as “Coronary artery disease”. Method: This cross sectional observational study was carried out in the department of Cardiology, UCC, BSMMU, Dhaka, during the period of September 2013 to December 2013. A total of 200 patients with primary diagnosis of CHD who were admitted for coronary angiogram were included in this study considering the inclusion and exclusion criteria. Results: Patients aged 25 to 40 years were grouped as group A and a 40 to 70 years were grouped as group B. Baseline clinical history, physical examination and investigations including reports of coronary angiogram were recorded accordingly. There were significant differences between two groups in respect to risk factors and angiographic findings. The prevalence of hypertension and diabetes were found to be more among group B CHD patients compare to group A patient (p<0.001) while, smoking and positive family history of IHD were more prevalent in younger age group (p<0.05). Normal or non-significant CAG findings were more common among group A compared to group B (p<0.05). Multiple vessel disease was observed to be more common amongst group B CHD patients (p<0.01). LAD and RCA involvement was more frequent in group B compared to group A patient (p<0.01). This indicated that risk factors and disease severity was different between two groups. In both the groups male predominance was observed but it was not different between two groups. Other risk factors like obesity and dyslipidemia were not found to be different. The common lesions are being some primary brain tumours, abscess, granuloma, resolving haematoma and infarct. Gadolinium compounds, which are paramagnetic IV contrast media for brain imaging, are especially helpful for identification of extra-axial tumours and intra-axial lesions associated with disrupted blood-brain barrier. The enhancing ring of an abscess is usually thinner and more uniform than that found in the neoplasm. Granulomatous abscesses tend to have a thicker ring than pyogenic abscesses and have less surrounding edema. Demyelinating disorders revealed nodular pattern and incomplete ring-enhancing pattern. Infarcts often show gyral enhancement. Enhancement is uniform in immune competent patients while ring-like in immunocompromised patients. Tuberculomas on contrast administration appear as nodular or ring-like enhancing lesions. Aims: To evaluate the role of magnetic resonance imaging in distinguishing neoplastic from non-neoplastic ring enhancing brain lesions. Material and methods: Forty patients with various ring enhancing lesions in the brain were detected by MRI. The age range of the patients was 13 to 63 years. Routine sequences of T1, T2 and FLAIR spin echo sequences with IV contrast were carried out. Signal intensities on unenhanced T1- and T2- weighted images for enhanced and unenhanced solid portions, enhancement patterns, margins, cortical involvement, mass effect, and presence of vasogenic edema were studied. Result : MRI showed 20% of cases were non-neoplastic ring enhancing brain lesions in comparison with 25% revealed by histopathology.65% cases diagnosed by MRI as neoplastic ring enhancing brain lesions in comparison with =75% cases revealed by histopathology. Sensitivity of MRI in the detection of secondary neoplastic ring enhancing brain lesions was 80% and specificity was 81.8%. This article brings out the relation between a healthy gut & conditions like Atopy, Colorectal Cancer (CC) & Auto-immune, Metabolic & Neuro- Psychiatric Disorders. These disorders occur due to gut microbial dysbiosis in the humans. Here, Atopy manifests through skin & respiratory issues, Metabolic manifests through Obesity & Type 2 diabetes, Auto-immune manifests through Irritable Bowel Disease/Irritable Bowel Syndrome (IBD/IBS), Rheumatoid Arthritis (RA), Celiac Disease (CD) & Neuro-Psychiatric manifests through Autism Spectrum Disorder (ASD) [6- 29]. Human gastrointestinal tract hosts a complex ecosystem of approximately 300-500 bacterial species. All these encompass nearly 2 million genes & this is known as micro- biome. The number of bacteria within the human gut is roughly 10 times more than that of all the cells in the human body. The cumulative bacterial genomic content is immensely greater than the human genome [4- 30]. A generalized approach to Auto-immune Disorders (AD), Metabolic Disorders (MD), Neuro Psychiatric Disorders (NPD), Atopy & Colorectal Cancer (CC) has failed & modern medicine has already realized that new avenues to restore personalized interventions to restore microbial balance and improve overall health & that too all the related disorders as well [4- 29]. It is here that the article delves into Homoeopathy as it is a personalized system of medicine. Further, it has a range of medicines called ‘Bowel Nosodes’ that are prepared from the bacteria of the gut. Here ‘Bowel’ means the gut and ‘Nosodes’ means medicines prepared from disease products or organisms. As the set of medicines are prepared from the bacteria of the gut, these are called ‘Bowel Nosodes’ [14, 15]. Unfortunately, the digital world is not accessible equally to all, and digital inequality continues to widen the gap between those who have and those who don't. ChatGPT, an advanced language model created by OpenAI, has the potential to completely change how people communicate and share information, but it also emphasizes how urgently the problem of digital inequality has to be addressed. The potential of such advanced technologies can only be optimally utilized if they are made accessible to all individuals, regardless of their socioeconomic background, geographical boundaries, and gender biasing. Males are more affected than the females. It can be associated with significant morbidity and mortality especially as it increases the risk for malnutrition, and pneumonias due to aspiration etc. Case Presentation: We present this rare case of worsening dysphagia and episodes of choking in a 63year old male due to large anterior cervical osteophytes. X-ray and flexible laryngoscopy revealed a large bulge over the posterior pharyngeal wall in the midline. Conclusion: Dysphagia in the elderly can lead to increased morbidity and life-threatening complications. Anterior osteophytes are found in nearly 20% to30% of the elderly population but rarely cause dysphagia. However, in some cases especially when associated with diffuse idiopathic skeletal hyperostosis (DISH/ Forestier’s disease) and ankylosing spondylitis these can cause severe dysphagia. In many cases of symptomatic anterior osteophytes surgery is considered an ultimate measure to relieve symptoms, decrease mortality, and improve the quality of life. We did a thorough literature search to know the prevalence and management of dysphagia due to anterior osteophytes.","PeriodicalId":471520,"journal":{"name":"Scholars journal of applied medical sciences","volume":"39 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135785098","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
