This systematic review aimed to evaluate the efficacy of various pelvic floor rehabilitation techniques in managing post-prostatectomy urinary incontinence. Methods: A comprehensive literature search was conducted using PubMed and Google Scholar, yielding 34 relevant studies for analysis. Results: The review found that pelvic floor rehabilitation, encompassing techniques such as pelvic floor exercises, biofeedback, electrical stimulation, and behavioral management, offers a non-invasive and cost-effective approach to improving continence outcomes. While the optimal timing and duration of rehabilitation remain debated, evidence supports initiating pelvic floor muscle training preoperatively and continuing postoperatively. Combining multiple rehabilitation techniques, particularly manual therapy with biofeedback or functional electrostimulation, appears to yield the best results in accelerating continence recovery. Preoperative rehabilitation showed mixed outcomes, with some studies reporting improved continence and others finding no additional benefit. Postoperative rehabilitation strategies varied, but generally focused on gradually increasing pelvic floor strength and control. The long-term efficacy of rehabilitation at 12 months post-surgery was comparable to no intervention in some studies. Conclusion: While pelvic floor rehabilitation demonstrates modest effectiveness in managing post-prostatectomy urinary incontinence, particularly in the short term, further research is needed to establish optimal protocols and long-term benefits. These findings underscore the importance of a comprehensive, patient-centered approach to post-prostatectomy care.","PeriodicalId":471520,"journal":{"name":"Scholars journal of applied medical sciences","volume":" 6","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141826967","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Diagnosis of tuberculosis in the fistula is still a challenge in the modern era of medical science, even with the advancement of diagnostic modalities. Objectives: The purpose of this study was to determine the incidence of tuberculosis in fistulae- in-ano, with or without evidence of tuberculous foci elsewhere in the body and not responding to conventional surgical treatment. Materials and Methods: This is a prospective study of 50 cases of anal fistula admitted in the surgical unit of 250 Bed General Hospital, Thakurgaon during the period from March 2023 to February 2024. Tissue sent for histopathological examination, Ziehl-Neelsen staining and Tuberculosis culture. All data collected properly are evaluated by standard statistical methods. Result: Out of the 50 cases the majority were males (84%) and the majority of the patients belonged to the 40-60 years of age group. Maximum patients were from lower socio-economic conditions. Only 3(6%) patients are diagnosed with tubercular fistula-in-ano, all are male, and one was under anti-TB treatment previously. Three patients had fistulous tract containing multiple epithelioid cells and granuloma which is suggestive of tuberculosis. Conclusion: Tuberculosis should be suspected in all cases of complex and recurrent fistulae-in- ano. Meticulous clinical evaluation with tissue from anal fistulas should therefore routinely be sent for TB culture as well as histopathological examination and ZN staining in areas where TB is prevalent. The treatment of tuberculous fistula in ano includes not only surgery but also ATT, which is the mainstay of treatment with an adequate duration, owing to the recurrent and relapsing nature of the disease and regular follow-up to cope up with recurrence.","PeriodicalId":471520,"journal":{"name":"Scholars journal of applied medical sciences","volume":"11 s1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141834262","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The study aim to investigate the impact of Soybean phytoestrogen-rich extract on hormonal imbalance and ovarian function on 4-vinylcyclohexene diepoxide-induced menopause in albino rats. Methods: Thirty (30) female albino Wistar rats were employed in the investigation, and each one was induced with 80mg/kg of 4-vinylcyclohexene diepoxide before being treated with either normal estradiol therapy (14ug/kg) or varying concentrations of the soybean phytoestrogen-rich extract (200 mg/kg, 400 mg/kg, and 600 mg/kg). Reproductive hormones (follicle stimulating hormone (FSH), luteinizing hormone (LH), estrogen (E2), anti mullerian hormone (AMH), progesterone and testosterone) were measured by ELISA methods. Statistical software SPSS (IBM) version 23.0 was used to analyze the data. Results: Serum E2 and progesterone levels were observed to be significantly higher (p<0.05) in the soybean phytoestrogen-rich extract treatment group compared to the positive control group. There was non-significant difference (p>0.05) observed in use of standard estrogen therapy compared to phytoestrogen isoflavones but increased serum mean levels of estrogen, progesterone and anti-mullerian hormone was observed. Conclusion: Data from this research clearly demonstrate the hormone regulating effect of soybean phytoestrogen-rich extract therapy in menopause-induced female Wistar rats. Soybean phytoestrogen-rich extract therapy in a high-dose appears to be more effective in management of hormonal imbalance and ovarian function compared to hormone replacement therapy as an alternate source of estrogen.","PeriodicalId":471520,"journal":{"name":"Scholars journal of applied medical sciences","volume":"16 5","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140716306","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It seems to be mostly linked, directly or indirectly, to viral infections. During the COVID-19 pandemic, several cases of thyroiditis were reported in the literature, occurring during or after infection by the SARS-CoV-2, suggesting that this virus is also linked to subacute thyroiditis. Therefore, knowledge of a possible association between these two entities is of the most importance. Several hypotheses explaining the mechanism of action of SARS-CoV-2 on the thyroid gland have been put forward. Clinical symptoms do not appear to be very typical, and thyroid hormonal dysfunction may not be present. Treatment is essentially based on corticosteroids and NSAIDs. The prognosis remains favorable. We report the case of a 46-year-old woman who presented with subacute thyroiditis 25 days after SARS-CoV-2 infection. Her workup showed elevated CRP, multinodular goiter, normal thyroid function and negative antibodies. She was treated with corticosteroids that resulted in improvement of her symptoms.","PeriodicalId":471520,"journal":{"name":"Scholars journal of applied medical sciences","volume":"36 13","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140734368","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
