Early initiation of rehabilitation procedures can enable a greater return of neurological function and improve long-term outcomes and quality of life. In this study, we aimed to compare the effects of home-based exercise and hospital-based supervised exercise in the early rehabilitation of stroke patients. Methods: This was a prospective observational study and was conducted in the Department of Physical Medicine and Rehabilitation, Dhaka Medical College Hospital, Dhaka, Bangladesh during the period from January, 2014 to June, 2014. In our study, we included 250 patients, and the patients were divided into two groups – Group A (Patients who got home-based exercise) and Group B (Patients who got hospital-based exercise). Result: We found the mean age of the patients of Group A & Group B was 56.75± 9.45 and 55.46± 10.58 years respectively. Among the patients, 41% were female,59% were male and 168 (67.2%) patients had been suffering from ischemic stroke and 82 (32.8%) patients from hemorrhagic stroke. The most common risk factors were HTN, dyslipidemia, and obesity. Mean FIM gain in Group A was 37.97 ± 17.52 and in Group B was 48.56 ± 21.22 which was significantly higher than that of Group A patients. 导言:中风是导致全球死亡和残疾的主要原因之一,在孟加拉国这样的欠发达国家更是如此,因为这些国家的医疗支持系统包括康复系统并不完善。及早启动康复程序可使神经功能得到更大程度的恢复,并改善长期疗效和生活质量。在这项研究中,我们旨在比较家庭锻炼和医院监督锻炼在中风患者早期康复中的效果。研究方法这是一项前瞻性观察研究,于 2014 年 1 月至 2014 年 6 月期间在孟加拉国达卡医学院附属医院物理医学与康复科进行。在研究中,我们纳入了 250 名患者,并将患者分为两组--A 组(在家中接受锻炼的患者)和 B 组(在医院接受锻炼的患者)。研究结果我们发现 A 组和 B 组患者的平均年龄分别为(56.75± 9.45)岁和(55.46± 10.58)岁。其中女性占 41%,男性占 59%,缺血性中风患者 168 人(67.2%),出血性中风患者 82 人(32.8%)。最常见的危险因素是高血压、血脂异常和肥胖。A 组患者的平均 FIM 增量为(37.97 ± 17.52),B 组患者的平均 FIM 增量为(48.56 ± 21.22),明显高于 A 组患者。结论在我们的研究中,我们发现在脑卒中患者早期康复的 4 周内,以医院为基础的监督运动优于以家庭为基础的运动。
