Despite the widespread use of instrumental methods of examination, timely diagnosis of placental in-growth is a challenging issue. Here, we reviewed the existing biochemical markers used for early detection and confirmation of placental in-growth, their specificity and sensitivity, and correlation with gestational age. Significant results were found for the following substances: pregnancy-associated plasma protein A (PAPP-A) in the first trimester, alpha-fetoprotein (AFP) and human beta-chorionic gonadotropin (Beta-hCG) in the second trimester, brain natriuretic peptide, antithrombin III, plasminogen activator inhibitor type I, soluble Tie-2 receptor (endothelial cell-specific tyrosine kinase receptor), and soluble vascular endothelial growth factor receptor-2. Ishchenko","doi":"10.17816/2313-8726-2022-9-2-103-109","DOIUrl":"https://doi.org/10.17816/2313-8726-2022-9-2-103-109","url":null,"abstract":"The article discusses the effectiveness of the combination of statins and metformin for hyperandrogenism treatment in polycystic ovary syndrome (PCOS). \u0000This study aimed to analyze the current scientific data on the effect of the combination of statins and metformin on the clinical and laboratory indicators of hyperandrogenism, ovarian volume, and menstrual cycle regularity and assess their use in the treatment of women with PCOS. \u0000Results of retrospective and prospective studies, meta-analyses, and systematic reviews at ClinicalTrials.gov, PROSPERO, among the publications of the International Endocrinological Society, as well as in open electronic databases of scientific literature Google Scholar, PubMed, and Cochrane Library were analyzed. Randomized controlled trials (RCTs), meta-analyses, and systematic reviews that involve women with PCOS who received metformin and statins alone or in combination for androgen level reduction and menstrual cycle regularity improvement were selected. \u0000These studies have shown certain effectiveness of statins in reducing the laboratory manifestations of hyperandrogenism. The positive effect of their combination with metformin on insulin resistance and, generally, the high effectiveness of such treatment regimen to reduce laboratory and clinical manifestations of hyperandrogenism in women with PCOS, reduce ovarian volume, and increase menstruation frequency was also demonstrated. \u0000Their isolated use cannot be recommended for patients with PCOS due to the possible negative effect of statins on insulin resistance and an increased risk of developing diabetes. In recent years, a large number of works have been published on the relationship of hyperinsulinemia with ovarian hyperandrogenism. Most authors recognize that insulin resistance and hyperinsulinemia contribute to polycystic ovary syndrome formation. The formation of menstrual disorders in overweight girls is based on the violation of gonadotropin secretion, insulin resistance, and hyperinsulinemia, which over time lead to hyperandrogenism. A decreased level of somatotropic hormone and insulin-like growth factor and an increased level of leptin leads to malfunctions in the correct functioning of the hypothalamic-pituitary system. These changes disturb the ovulatory function and, consequently, adolescent reproductive health. The relationship between obesity and depression is actively discussed by foreign authors, and the obtained results often contradict each other. Most studies suggest that excessive weight gain during adolescence can lead to depression, negative mood, and low self-esteem. \u0000This study analyzes the results of research by domestic and foreign authors on the issues of diagnosis and treatment. Menstrual disorder therapy against the background of overweight adolescent girls is a matter of dispute among clinicians. In recent years, the arsenal of drugs that can stimulate ovarian function has significantly expanded. A prerequisite for the use of drugs in this group is the presence of a follicular apparatus in the ovaries. Ovulation inducers contribute to the maturation of several follicles at once, and their ability to influence the process of steroidogenesis is beyond doubt. However, the long-term risk assessment when using these drugs, namely the risk of developing malignant female reproductive system neoplasm, remains poorly understood despite their long-term use. This problem is multifaceted and quite difficult to study since the etiological causes of infertility are considered a risk factor for developing malignant female reproductive system neoplasms. Oncological diseases are diagnosed several years after infertility treatment, thus proving a causal relationship is difficult, on the one hand, and dictating the need for a long period of observation in the group of women who underwent infertility treatment, considering the characteristics of each specific method, on the other hand. Gynecologists must consider all aspects of the patients condition during treatment, including the blood coagulation system status. Features of hemostasis affect the disease development, features and outcomes of the operation, and the postoperative period course, as well as the possibility of carrying out the necessary drug therapy and its results. This literature review reflects the results of research by domestic and foreign authors regarding the hemostasis system status in patients with uterine fibroids, including the effect of hormonal, antioxidant, and disaggregate therapy. Their relevance increases due to the increasing abdominal delivery frequency. Numerous proposed approaches and ways to stop such bleeding indicate the complexity and versatility of the problem, as well as its unresolved present stages. \u0000AIM: This study aimed to evaluate the effectiveness of a new method of bleeding control in placenta previa. \u0000MATERIALS AND METHODS: Herein presented the results of a prospective study that included 58 cases of bleeding control for placenta previa in patients who gave birth in 20172020 at G.G. Kuvatov Republican Clinical Hospital in Ufa. Two groups were formed depending on the method of bleeding control. The main group included 34 patients who used a new method for bleeding control. The control group included 24 patients who used traditional methods of bleeding control following the clinical protocol. \u0000RESULTS: The proposed method of bleeding control in placenta previa using drainage silicone tubes and a circular suture is used during cesarean section surgery for placenta previa without signs of placenta accreta spectrum. Despite more than a century of history of study, POP remains one of the important problems of modern gynecology. \u0000AIM: This study aimed to identify the most significant risk factors for pelvic organ prolapse and their associations in women of reproductive age. \u0000MATERIALS AND METHODS: The study involved 211 women of reproductive age (1845 years). Based on the clinical and anamnestic data, all patients were divided into two groups: the main group (n=97), which included patients with pelvic organ prolapse, and the control group (n=114), which included women without prolapse. \u0000RESULTS: The following are the identified significant risk factors: body mass index of 23.03 kg/m2 (odds ratio [OR]: 6.69; 95% confidence interval [CI]: 3.3613.34), secondary education (OR: 2.41; 95% CI: 1.374.21), lifestyle associated with lift (OR: 7.15; 95% CI: 3.4115.00), history of 3 pregnancies (OR: 4.48; 95% CI: 2.728.76), 2 vaginal birth count (OR: 9.12; 95% CI: 4.8117.28), 1 abortions (OR: 4.34; 95% CI: 2.407.85), maximum newborn body weight of 3700 g (OR: 7.67; 95% CI: 4.1214.28), and perineal obstetric injury (OR: 39.27; 95% CI: 4.79321.70). Adequate risk factors were childbirth (OR: 8.22; 95% CI: 2.0133.58), obstetric trauma (OR: 9.16; 95% CI: 1.3661.68), and weight lifting-associated lifestyle (OR: 37.56, 95% CI: 3.42412.40). \u0000CONCLUSION: Significant independent risk factors for pelvic organ prolapse in women of reproductive age have been identified as follows: vaginal childbirth, perineal injury in childbirth, and weight lifting-associated lifestyle. Berg","doi":"10.17816/2313-8726-2022-9-1-49-55","DOIUrl":"https://doi.org/10.17816/2313-8726-2022-9-1-49-55","url":null,"abstract":"AIM: This study aimed to evaluate the effectiveness of the choice of surgical method for treating genital prolapse, depending on the intensity of pelvic floor tissue perfusion. \u0000MATERIALS AND METHODS: The study assessed the relationship of extragenital pathology, medical history of women operated for genital prolapse and medical history of women without genital prolapse during peri- and post-menopause (control group, n=67) with indicators of vaginal perfusion intensity. Overall, 328 patients with pelvic organ prolapse (IIIV degree) underwent surgery according to the Pelvic Organ Prolapse Quantification system. Of these, 287 patients were examined after 2 years to diagnose disease recurrence; these patients were included in the main study group (n=287). For statistical processing of the obtained results, the STATISTICA 10.0 and EViews 12.0 software packages were used. \u0000RESULTS: Correlation analysis revealed an inverse relationship between vaginal microcirculation intensity with age (rs=0.187; р=0.025), menopause duration (rs=0.390; р=0.001), genital prolapse duration (rs=0.245; р=0.046), and surgical menopause (rs=0.442; р 0.001). The study of microcirculation intensity in the anterior and posterior vaginal walls, depending on the presence of somatic diseases, revealed an inverse correlation with diseases such as hypertension (rs=0.291; р 0.001), coronary heart disease (rs=0.169; р=0.044), and diabetes mellitus (rs=0.223; р=0.008). The main signs of PCOS include a violation of menstrual and/or ovulatory function and clinical and/or biochemical hyperandrogenism, as well as polycystic ovarian morphology according to the ultrasound results. PCOS leads to infertility, obesity, and cardiovascular system diseases. PCOS is an important symptom complex in the diagnosis, which directly affects the reproductive function of females, hence the percentage of fertility. This study aimed to analyze the features of clinical, laboratory, and instrumental parameters in females with a previously established PCOS diagnosis referred for surgical treatment. The obtained results were studied from the point of view of the clinical guidelines of the American Society for Reproductive Medicine and the European Society of Human Reproduction and Embryology (2003), International PCOS Network (2018), and the Ministry of Health of the Russian Federation (2021). \u0000MATERIALS AND METHODS: The study included 121 females who were admitted to hospitals in Moscow for surgical treatment. Among the gynecological surgical hospitals, the following medical institutions were allocated: the city clinical hospital, the research center, and the commercial clinic we conditionally divided into three levels. \u0000The city clinical hospital had 54 females (1st group) aged 2137 years, the research center with 48 females (2nd group) aged 2242 years, and the commercial clinic with 19 females (3rd group) aged 2541. Before the ovarian surgery for PCOS, all patients underwent an additional examination, including the collection of anamnesis, particularly, features of menstrual and generative function, anthropometric indicators, body weight, and height study. Based on these parameters, the body mass index (BMI) was calculated, the clinical signs of hyperandrogenism (acne, excessive hair growth on the body and face, and the degree of hirsutism) were assessed, and the hormonal profile was determined to detect biochemical hyperandrogenism (free testosterone). Additionally, the level of glucose and insulin in the blood plasma was determined in all patients, and a pelvic ultrasound examination (ultrasound) was performed. \u0000RESULTS: Menstrual irregularities were revealed, of which complaints were presented by a total of 49.5% of patients. The results of the preliminary selection of patients with PCOS for surgical treatment revealed that 108 (89.2%) had primary and secondary infertility, of whom 64 (52.9%) had BMI within the normative values, 29 (23.9%) were overweight, and 25 (20.6%) were obese. Clinical manifestations of hyperandrogenism were present in 61 (50.4%) patients included in the study. The level of free testosterone above the normative values (2.85 pg/ml) was diagnosed only in 9 (7.4%) patients out of 121 who are included in the study. Concurrently, the uterine factor occupies one of the most important places in infertility structure, reaching 50%. Improvement of assisted reproductive technology methods can successfully overcome many causes of infertility, but the possibilities of the method are limited in the uterine factor of infertility. \u0000AIM: To assess the presence of nucleotide polymorphisms of the TP53 signaling pathway genes (LIF rs41281637, c.256GA; LIF rs929271, n.397-2854TG; MDM2 rs2279744, c.14+309TG; MDM4 rs1563828, c.558+572AG; TP53 rs1042522, c.215CG) in the endometrium in patients with primary and secondary infertility. \u0000MATERIALS AND METHODS: In 20182021, the V.F. Snegirev Clinic of Obstetrics and Gynecology examined and treated 54 patients aged 26 to 48 years with primary or secondary infertility, including a genetic endometrial sample examination. The first group consisted of 28 patients with primary infertility aged 26 to 42 years. The 2nd group included 26 patients aged 29 to 48 years with secondary infertility. \u0000RESULTS: The study evaluated the expression of nucleotide polymorphisms of the TP53 signaling pathway genes in patients with primary and secondary infertility.
