Despite their use, the combined oral contraceptives (OCS) don't supress the ovarian follicular development, completely. The level of follicular activity during OC use depends on factors such as the type and dose of steroids administered, the administration schedule, the user's adherence to the regimen, and the individual's response to the hormones. Aim of the study: To evaluate the relationship between functional ovarian cysts and users of combined oral contraceptive pills and Medroxyprogesterone acetate (MPA). Subjects and Methods: A Prospective observation study conducted in the Department of Obstetrics and Gynecology, Faculty of Medicine, Fayoum University, involving 200 women divided into two groups: Results: Our results showed that 9% of patients in group A have follicles 10–30 mm and 4% have cysts ≥ 30 mm, while in group B, 17% have follicles 10–30 mm and 12% have cysts ≥ 30 mm, with a significant difference between both groups ( P < 0.001 ). Also, the mean cyst diameter in groups A and B were 47.6 mm and 59.3 mm, respectively, without any significant difference. {"title":"Comparative evaluation of ultrasound strain elastography and diffusion weighted magnetic resonance imaging in differentiating malignant from benign thyroid nodules","authors":"Ashraf Huseein, A. The key to successfully treating malignant thyroid nodular lesions is an early, expert, and accurate diagnosis. Aim of the study: To distinguish between benign and malignant thyroid nodular lesions by comparing the diagnostic performance of quantitative diffusion-weighted MRI, elastic score, TI-RADS, and strain ratio. Subjects and Methods: The current prospective study recruited 59 individuals with Thyroid nodular lesions. All patients were assessed by conventional ultrasonography, ultrasonic strain elastography (elasticity score + strain ratio), ADC value, and diffusion-weighted magnetic resonance imaging. The results of the histology were typical. Result: The TI-RADS score was 84.9% accurate, 89.8% specific, and 80% sensitive. An elasticity score with a cutoff value of 3 had (91.8%) specificity, (70% sensitivity), and (80.9%) accuracy using Asteria's 4-point criterion. When a strain ratio cutoff value of 1.65 was chosen, there were significant improvements in diagnostic accuracy (91.8%), sensitivity (90%), and specificity (71.4%). It was found that (95.9%) represents specificity, (80%) sensitivity, and overall accuracy determined with (98%) were observed For the ADC value, the cutoff value of 1.45 x 10-03 was used. Multi-parametric analysis improved the sharpness of TI-RADS grading in our experiment. By combining the ADC diffusion value with the TI-RADS rating, which had specificity of 95.9%, sensitivity of 100%, and accuracy of 98%, the best diagnostic approach was achieved. Conclusion: both strain ratio and the value of ADC diffusion performed accurately, while the US TI-RADS rating for describing thyroid nodular lesions was not of the same accuracy. {"title":"A Meta-analysis on the management options of Idiopathic Inflammatory Punctal Stenosis","authors":"Ahmed Seif, O. Said, M. Acquired punctal obstruction with profuse epiphora is a rather common illness, with a prevalence ranging from 8.3% to 54.3%. Aim of the study: Aim of the study: To perform a meta-analysis on the management options for idiopathic inflammatory pyloric stenosis. Subjects and Methods: This meta-analysis follows the PRISMA flow diagram. We searched PubMed, Cochrane, Web of Science, Embase, and local databases for relevant clinical trials evaluating the management of idiopathic inflammatory punctal stenosis. The analysis was performed using the risk ratio (RR) for dichotomous data and the mean difference (MD) for continuous data, in addition to the 95% confidence interval (CI). The data were considered homogenous if the I2 was 50% and heterogenous if the I2 was > 50%. Results: Results showed significantly improved punctal staging in groups (I) and (III) compared to baseline, ( P = 0.007 and 0.017, respectively). However, no significant improvement was observed in group II. The relative risk (RR) across all groups was 1.68 (95% CI: 1.03, 2.75, P = 0.04 ), indicating a significant improvement in the treatment at the assessment time. The analysis of individual stages showed that the treatment effect varied across stages, with a significant improvement observed in stage 3. The study also found a substantial increase in Outer Punctal Diameter (OPD) in the groups (I) and (III) immediately after the treatment compared to baseline, with a pooled effect size of -58.22 (95% CI: -108.50, -7.93, P = 0.02 ). It worsens the patient's condition and develops as a result of a variety of underlying diseases. Understanding the etiologies is necessary for effective management. Aim of the study: The goal of the work was to evaluate SAAG and AFTP's sensitivity and specificity in identifying the ascites-causing factor. Understanding the various causes of ascites was another goal of the study. Subjects and procedures: The study comprised 100 participants who had ascites. They underwent a thorough clinical evaluation and diagnostic paracentesis. They underwent AFTP and SAAG tests. The specificity, sensitivity, and diagnostic accuracy of AFTP and SAAG were calculated individually after several diagnostic techniques were used to determine the etiology of ascites. Results: The studied patients had an average age of 57.44 ±10.5 years, and 57% were men. 91% had ascites associated with portal hypertension (PHT). The most common etiologies were liver cirrhosis (LC) associated with hepatitis C virus (HCV) (78%) and malignant and cardiac ascites (4%). The SAAG has a 96.7% sensitivity, 100% specificity, and 98.4% diagnostic accuracy compared to the AFTP's 94.6%, 100%, and 97.3%, respectively. Conclusions: LC linked to HCV was the most frequent etiology of ascites. SAAG and AFTP had high diagnostic accuracy in determining ascites' etiology. The development of markers with enhanced test characteristics is an important challenge because of the high falsely positive rate. Aim of the study: The current study aimed to determine the value of cervical length (CL) measurements (as determined by the single-and two-line techniques) in predicting of preterm birth (PTB). Subjects and Methods: The current study was a prospective cohort study carried out on 120 asymptomatic primigravida women at low risk of preterm labor (PTL), attending the Gynecology and Obstetrics department at Fayoum University Hospital, between February 2022 to August 2022. the study was designed to evaluate the role of the Cervical length (CL) measurements in Preterm birth (PTB) prediction. cervical length was measured by trans-vaginal ultrasound. Pregnancy data and maternal history were documented, then, delivery-related statistics were obtained. Results: The average age of included women was (21.79 ±3.3) years old, an average BMI (24.6 ±5.8) kg/m 2 , and an average GA at delivery of (38.46 ±1.98) weeks. Out of the studied 120 women fifteen (12.5%) experienced a preterm. birth Cervical length was estimated by single and two-lines methods; CL was significantly shorter among pre-term women as compared with term. There was a positive linear moderate correlation between GA and CL-one line (r= 0.260, P =0.004 ). Also, a positive linear strong correlation between GA and CL-two lines (r= 0.716, P <0.001 ). Using ROC curve analysis, the sensitivity and specificity of CL for prediction of pre-term birth was evaluated. Conclusions: The CL (single and two-lines) approaches are effective at predicting preterm birth. Subjects and Methods: A cross-sectional analytical study conducted at the Gynecology and Obstetrics department at Fayoum University Hospital saw 120 infertile (primary or secondary infertility) women who had verified prior COVID-19 infection as part of this cross-sectional analytical study. For all the study's female participants Results: In the current study, we included 120 infertile women with confirmed COVID-19 infection who had an average age of (26.96 ± 5.68) years, an average BMI of (21.8 ± 7.62) kg/m2, an average duration of (3.95 ± 1.16) months, a mild form of the disease reported by 86 (71.7%), a severe form reported by 30 (25%), and only a moderate form reported by four women (3.3%). According to the severity of the disease, the AMH, AFC, FSH, and LH mean serum levels measured after COVID-19 infection did not differ statistically significantly from the mean serum levels examined before COVID-19 infection ( P >0.05 ). before COVID-19 infection in the three groups according to disease severity ( P >0.05 ).
