Burhan , Mohammad Khursheed Alam","doi":"10.1016/j.ejwf.2023.08.005","DOIUrl":"10.1016/j.ejwf.2023.08.005","url":null,"abstract":"<div><h3>Background</h3><p>Investigating the possible changes in patients’ expectations of and satisfaction with the orthodontic treatment outcomes when they were given the three-dimensional digital prediction of their teeth alignment before the beginning of treatment<strong>.</strong></p></div><div><h3>Methods</h3><p>A prospective non-controlled single-group clinical trial was conducted on 28 (18 females, 10 males, mean age: 20.68 ± 1.91 years) patients with Class I malocclusion and moderate dental crowding who required a nonextraction orthodontic treatment. Patients were given the expectations questionnaire on their first visit (T0). Then, patients were shown a three-dimensional digital setup–created by Orthoanalyzer software (3Shape, Copenhagen, Denmark)–of the proposed treatment results before orthodontic treatment (T1) and received two questionnaires, the second expectations questionnaire and the satisfaction with the proposed changes questionnaire. Treatment was then initiated using the fixed appliances and completed. After debonding, a question about patients’ satisfaction with the achieved changes was completed (T2).</p></div><div><h3>Results</h3><p>Patients’ expectations level significantly increased after watching the predicted alignment of teeth compared with the initial levels in terms of chewing (<span><math><mover><mi>x</mi><mo>¯</mo></mover></math></span> = 5.54 and <span><math><mrow><mover><mi>x</mi><mo>¯</mo></mover><mspace></mspace></mrow></math></span>= 6.71), speech (<span><math><mover><mrow><mi>x</mi><mspace></mspace></mrow><mo>‾</mo></mover></math></span> =5.93 and <span><math><mrow><mover><mi>x</mi><mo>¯</mo></mover><mspace></mspace></mrow></math></span>= 6.93), and oral hygiene improvement (<span><math><mover><mrow><mi>x</mi><mspace></mspace></mrow><mo>‾</mo></mover></math></span>= 7.93 and <span><math><mover><mi>x</mi><mo>¯</mo></mover></math></span>=8.61 ± 1.06; at T0 and T1, respectively). The remaining items showed no significant differences between the two assessment times. Patients had a higher level of satisfaction at the end of treatment than after watching the proposed outcome. The aim of this study was to evaluate the content regarding orthodontic retention and retainers on the social media website, Reddit.</p></div><div><h3>Methods</h3><p>A systematic search for relevant content submitted over a 12-month period on the Reddit forum, r/braces, was conducted. Qualitative analysis of the initial posts for themes and subthemes was conducted by two investigators. Responding comments to the initial posts were assessed for supportiveness for each initial poster and alignment with the evidence-base. Quantitative assessment was via descriptive statistics.</p></div><div><h3>Results</h3><p>A total of 271 initial posts and 984 comments satisfied the inclusion/exclusion criteria. All initial posts were by patients. Just 1.12% (n = 11) of the comments appeared to be provided by oral health professionals. Most initial posts were negative (50.18%; n = 136), and most comments were positive (70.42%; n = 693). Alignment with the evidence-base was high among the comments (67.89%; n = 668). Eight main themes were identified with concerns regarding the negative impact of retention and retainers on quality of life, compliance with retention protocols and relapse commonly expressed. Concern regarding fear of relapse when waiting for the initial or renewal of retainers was a novel finding. More negative sentiments about orthodontists were expressed than positive.</p></div><div><h3>Conclusions</h3><p>Reddit is a supportive and reliable environment for patients regarding orthodontic retention and retainers. The content evaluation suggested deficiencies in communication processes between clinicians and patients. Digital setups were generated using OrthoAnalyzer Clear Aligner Studio software to serve as the reference standard. Both pretreatment scans and setups were converted to STL files and exported to Geomagic, OrthoAnalyzer-Model Set Compare, and Compare model registration software packages. The amount of tooth movement of the maxillary incisors and canines was calculated in six degrees of freedom.</p></div><div><h3>Results</h3><p>Statistical significance of the obtained results was expressed at <em>P</em> &lt; 0.01 to account for multiple comparisons. The maxillary central incisors showed the highest agreement for torque and rotation as measured by all software programs. Lateral incisors showed the least agreement in linear movements as measured by Geomagic and Compare, and for tip as measured by Geomagic and OrthoAnalyzer. Maxillary canines had the highest agreement for all linear movements as measured by Geomagic and Compare, and tip as measured by Geomagic and OrthoAnalyzer. Geomagic showed excellent agreement for all measurements except for torque, whereas Compare showed excellent agreement only for rotation and linear measurements. OrthoAnalyzer showed moderate agreement for all measurements except for rotation, which showed good agreement.</p></div><div><h3>Conclusions</h3><p>Maxillary central incisor measurements showed higher agreement compared with measurements of the maxillary lateral incisors and canines. En-masse retraction of maxillary anterior teeth was carried out with extraction of upper first premolars with infrazygomatic crest (IZC) mini-implants as anchorage units. Clinical parameters such as success rate, soft tissue thickness, maximum insertion torque, maximum removal torque, pain response, soft tissue response, and cone-beam computed tomography parameters such as embedded angulation, penetration depth, thickness of bone on buccal and palatal aspect of mini-implant, and peri-implant bone density were evaluated.</p></div><div><h3>Results</h3><p>The success rate of IZC mini-implants in adolescents was found to be 96.6% and 98.3% in young adults respectively. There was no significant difference in success rate between the two groups. Intergroup comparison showed a significant difference (<em>P</em> &lt; 0.05) in terms of maximum insertion torque, maximum removal torque, soft tissue thickness, cortical bone thickness, and peri-implant bone density values. Comparison between right and left side revealed a significant difference (<em>P</em> &lt; 0.05) with regards to soft tissue response, soft tissue thickness, total bone thickness, cortical bone thickness, and peri-implant bone density.</p></div><div><h3>Conclusions</h3><p>There was no significant difference in the success rate of IZC mini-implants between adolescents and young adults. The relationship between the microdamage and primary stability was also examined.</p></div><div><h3>Methods</h3><p>Ti6Al4V orthodontic miniscrews and 1.0-mm-thick cortical bone pieces from fresh porcine tibia were prepared. The orthodontic miniscrews had custom-made thread height (H) and pitch (P) size geometries, and were classified into three groups: control geometry; H<sub>C</sub>P<sub>C</sub> (H<sub>C</sub>; thread height = 0.12 mm, P<sub>C</sub>; pitch size = 0.60 mm), geometry with a narrower pitch; H<sub>C</sub>P<sub>N</sub> (H<sub>C</sub>; thread height = 0.12 mm, P<sub>N</sub>; pitch size = 0.30 mm), and geometry with a taller thread height; H<sub>T</sub>P<sub>C</sub> (H<sub>T</sub>; thread height = 0.36 mm, P<sub>C</sub>; pitch size = 0.60 mm). The orthodontic miniscrews were inserted into a pilot hole in the cortical bone, and maximum insertion torque and Periotest value were measured. After insertion, the samples were stained with basic fuchsin. Meade","doi":"10.1016/j.ejwf.2023.05.008","DOIUrl":"10.1016/j.ejwf.2023.05.008","url":null,"abstract":"<div><h3>Background</h3><p>This retrospective study aims to evaluate the predictability of the Invisalign appliance (Align Technology, Santa Clara, CA) in leveling the maxillary curve of Wilson (COW).</p></div><div><h3>Methods</h3><p>53 adult subjects treated by orthodontists who are experienced Invisalign providers, between 2013 and 2019 were selected. All patients had either Angle Class I or II malocclusions and were treated by non-extraction in the maxillary arch with a minimum of 14 Invisalign aligners with no bite ramps or auxiliaries. Initial, predicted, and actual outcomes were analyzed with Geomagic Control X software (3D systems, Rock Hill, SC; Version 2017.0.3).</p></div><div><h3>Results</h3><p>Despite planning mean (SD) COW leveling of 0.25 mm (1.91), the curve became significantly more pronounced by 1.15 mm (0.85) (<em>P</em> &lt; .001). There was a significant overexpression of buccal inclinations between mean prescribed versus actual values from 0.10 mm (0.39) (<em>P</em> = .007) to 0.29 mm (0.47) (<em>P</em> &lt; .001) from the first premolars to the second molars, respectively. These differences were more pronounced posteriorly. The differences between the predicted and actual arch width values were underexpressed for all teeth except the second molars; a mean underexpression between 0.28 mm (0.92) and 0.60 mm (1.14). The second molars were the only teeth to overexpress with a mean of 0.42 mm (1.02).</p></div><div><h3>Conclusions</h3><p>Overall mean maxillary COW was not predictably controlled with Invisalign. The appliance tends to overexpress the buccal crown tip regardless of prescribed direction, especially posteriorly. The soft tissue values were integrated into the profiles, and profile preferences were identified for each location. An ANOVA with post hoc Tukey test was used to compare the differences in mean preference in each location. A multivariable regression model was used to assess the effect of the demographics of the evaluator on preference.</p></div><div><h3>Results</h3><p>Thirteen profiles were ultimately analyzed. The mean for profile preference was significantly different across locations (<em>P</em> &lt; 0.0001). For evaluators in the United States and Lebanon, the most preferred profile had the original lip and original nose. In Switzerland and South Africa, retrusive lips, and a small and less upturned nose was most preferred. In Japan and Saudi Arabia, the most preferred profile had the original lip and a protrusive nose that was less upturned. A protrusive lip with a small, upturned nose was preferred in Turkey. Profile change (<em>P</em> &lt; 0.0001), location (<em>P</em> &lt; 0.0001), sex (<em>P</em> &lt; 0.0001), and race (<em>P</em> = 0.02) were significant confounders; in contrast, age, education, and income were not significant.</p></div><div><h3>Conclusions</h3><p>Profile preference is different among the seven locations. For the most part, lay people prefer profiles within one SD from white norms. Also, an upturned nose is the least favored in most of the locations. Sex and race are also significant confounders.</p></div><div><h3>Clinical Relevance</h3><p>An orthodontic treatment plan decision is affected by an individual's preference for their facial appearance. Clinicians are encouraged to develop specific goals of treatment based on a precise diagnosis and to design appliances that will deliver the desired force system. In this article we emphasize the importance of a thorough analysis of the force system needed to achieve the desired orthodontic tooth movement and avoid any of the undesired side effects of treatment mechanics. Methods: For a comprehensive understanding of the three-dimensional circummaxillary suture micromorphology, human midpalatal suture (MPS) and pterygomaxillary articular complex from eight subjects’ (five males, three females, 72–88 years old) autopsies were longitudinally scanned with microcomputed tomography. Additional histology was performed for hematoxylin and eosin staining. Sutural micromorphology was assessed by interdigitation index (II), obliteration index (OI) and obliteration number. Intergroup comparisons were performed with Kruskal-Wallis and Mann-Whitney <em>U</em> with Bonferroni correction (α = 0.005). Correlation with anteroposterior and craniocaudal gradients was assessed with Spearman's correlation test (α = 0.05). Results: Maxillary region of MPS presented a higher II 1.50 (0.61) and obliteration number per slice 8 (9) (<em>P</em> &lt; 0.005). OI was increased in palatomaxillary 35% (47%) followed by pterygopalatine suture 25% (49%) (<em>P</em> &lt; 0.005). The II and OI of the MPS exhibited only a weak anteroposterior gradient, with relatively low correlations. Obliteration areas were found sporadically along the entire MPS.
