Model based on clinical characteristics to identify patients with neurogenic oropharyngeal dysphagia.

To improve its detection and characterization, models are emerging that integrate clinical variables to complement the physical examination of swallowing.</p><p><strong>Objective: </strong>Develop an explanatory model to differentiate patients with neurogenic oropharyngeal dysphagia.</p><p><strong>Materials and methods: </strong>Case control study based on a set ofdata derived from the clinical examination of swallowing with neurological emphasis carried out in a sample of patients with neurogenic oropharyngeal dysphagia of neurological and neuromuscular causes (cases), and in healthy people (controls).</p><p><strong>Results: </strong>158 clinical variables were compared between both groups, where those with the greatest classification capacity were identified, integrated into an explanatory binary logistic regression model made up of nine variables: two history, two symptoms, three physical examination signs and two signs after consistency/volume test with food. The dependent variable was the category of being healthy or patient and the covariates were the clinical variables. {"title":"Lifestyle adaptations in cardiometabolic patients after COVID-19.","authors":"Thamires Sales Macêdo, Nelson Miguel Galindo, Luana Eugenia de Andrade Siqueira Parente, Simone de Sousa Paiva, Joselany Áfio Caetano, Lívia Moreira Barros","doi":"10.15649/cuidarte.3841","DOIUrl":"10.15649/cuidarte.3841","url":null,"abstract":"<p><strong>Introduction: </strong>Numerous factors contribute over time to changes in lifestyle behaviors, and the COVID-19 pandemic not only altered individuals' routines but also influenced the factors affecting their chronic conditions.</p><p><strong>Objective: </strong>To understand the perception of patients with cardiometabolic diseases regarding adaptations to their lifestyle after contracting COVID-19.</p><p><strong>Materials and methods: </strong>This exploratory study with a qualitative approach was conducted with nine patients diagnosed with cardiometabolic diseases who tested positive for COVID-19. Data collection involved semi-structured audio-recorded interviews, which were transcribed and processed using IRAMUTEQ software.</p><p><strong>Results: </strong>The collective discourse revealed that mass contamination by the coronavirus and subsequent Long COVID triggered significant fears and anxieties, as well as increases in muscle pain, fatigue, and difficulties in maintaining blood pressure and/or glycemic control. These issues directly impacted the daily routines of infected patients. Utilizing these resources, healthcare professionals can enhance patient care practices.</p><p><strong>Objective: </strong>This study aims to assess the efficacy of information and communication technologies in enhancing the knowledge of hospitalized patients.</p><p><strong>Materials and methods: </strong>A systematic review was conducted to address the research question: \"Which information and communication technologies effectively improve the knowledge of hospitalized patients?\" The search encompassed electronic databases such as Scopus, the National Library of Medicine, Web of Science, Cumulative Index of Nursing and Allied Health Literature, Scientific Electronic Library Online, Cochrane, and Excerpta Medica Database. Gray literature was sourced through Google Scholar. The exposure factor was educational interventions employing information and communication technologies, with the outcome measured as improved knowledge. Only randomized clinical trials in any language were included.</p><p><strong>Results: </strong>A total of 1,153 articles were initially identified, from which four were deemed eligible for inclusion. These studies demonstrated effectiveness in enhancing knowledge through educational interventions utilizing technologies such as video, applications, and concept maps.</p><p><strong>Discussion: </strong>Information and communication technologies have been shown to enhance patient knowledge. {"title":"The quality of life of people with multiple sclerosis in Portugal.","authors":"Rosa Martins, Sara Gomes, Patricia Vicente, Nélia Carvalho, Susana Batista","doi":"10.15649/cuidarte.2841","DOIUrl":"10.15649/cuidarte.2841","url":null,"abstract":"<p><strong>Introduction: </strong>Multiple sclerosis significantly affects the quality of life of those suffering from this specific condition.</p><p><strong>Objective: </strong>To assess the quality of life of people with multiple sclerosis and analyse the correlation between the disease and its associated effects and different sociodemographic, clinical, and functional variables.</p><p><strong>Materials and methods: </strong>An observational, cross-sectional, descriptive-correlational and quantitative study conducted using a non-probabilistic convenience sample composed of 70 patients suffering from multiple sclerosis registered with the Multiple Sclerosis Association of the Central Region of Portugal. The data collection protocol included sociodemographic and clinical questions, the Family Apgar Scale, and the Barthel Index. Descriptive and inferential statistics were used to process the data. Data collection took place between April and July 2021.</p><p><strong>Results: </strong>The majority of participants reported a moderate overall quality of life (M=51,78 ± 24,09). Higher scores were observed in the social relationships and environmental health domains, while lower scores were recorded for the physical domain. Better quality of life was found to be positively associated with being under 45 years old, having higher educational qualifications, living in functional families, and experiencing greater functional independence in activities of daily living.</p><p><strong>Discussion: </strong>The variables with the strongest association were those capable of influencing the physical and social domains. Consecutive sampling was used, and data were collected with a structured questionnaire. Data analysis involved the calculation of frequencies, percentages, and inferential tests.</p><p><strong>Results: </strong>Women's APC knowledge score obtained a mean of 13.3 ± 3.24 and for practices a mean of 8.13 ± 3.44 scores considered low for both. Knowledge of APC obtained a relationship with schooling (rs.460, p < .05), monthly economic income (rs =.334, p < .05). A \"snowball\" sampling method was employed through social media dissemination.</p><p><strong>Results: </strong>The mean PSQI score was 9.75 ± 4.36 points. The poorest sleep quality was identified in participants without dependents (p=0.031; p=3.329; 95% CI=0.035-6.354) and those with dependents other than children (p=0.022; p=4.121; 95% CI=0.575- 7.667). However, having a Ph.D degree (p=0.001; p=-3.406; 95% CI=-5.503- 1.309) or specialist degree (p=0.021; p=-1.639; 95% CI=-3.031- -0.247) was associated with better sleep quality. Higher job satisfaction was found among women (p=0.034; p=0.104; 95% CI=0.007-0.201) who did not work with COVID-19 patients (p=0.049; p=-0.085; 95% CI=-0.174- -0.003).</p><p><strong>Discussion: </strong>Improving working conditions, such as the nurse-to-patient ratio and distribution of work shifts, is crucial to enhancing nurses' sleep quality.</p><p><strong>Conclusions: </strong>Spanish nurses reported poor sleep quality. Not having dependents or having dependents other than children were risk factors for poor sleep quality. Job satisfaction was higher among women who did not work with COVID-19 patients. The association between the sociodemographic and personal characteristics and the consumption of PS was assessed using the chi-square test. Prevalence ratios were calculated along with their 95% confidence intervals. For the multivariate analysis, a Poisson regression with a log link and robust estimator was employed to explore the factors associated with PS use.</p><p><strong>Results: </strong>The prevalence of consumption of PS in the last year was 87%, prevailing the consumption of alcohol, marijuana, and poppers. To achieve this purpose, it is essential to identify the needs that arise within the teaching processes and to clearly establish how the pedagogical use of technologies can improve learning environments.</p><p><strong>Objective: </strong>To investigate and critically evaluate the contribution of technology to the strengthening of the humanization of care in the field of nursing.</p><p><strong>Materials and methods: </strong>The methodological approach for systematic literature reviews defined by Okoli, which involves following a rigorous and standardized process to systematically and explicitly identify, evaluate, and synthesize the existing body of research. Initially, 51 articles were selected for analysis. After applying exclusion criteria, 26 studies were extracted and reviewed, identifying categories that highlight the positive influence of technology on cognitive, psychomotor, and affective competencies. Subsequently, the document with the main conclusions was drafted.</p><p><strong>Results: </strong>The findings reveal the effectiveness of various technological environments in nursing education, highlighting the prioritization of competencies linked to knowing and doing. However, there is an observed tendency to underestimate affective competencies crucial for humane care.</p><p><strong>Discussion: </strong>The results revealed a diverse landscape regarding the impact of various technologies on the development of nursing competencies, highlighting both strengths and limitations. {"title":"Analysis of health outcomes in acute myocardial infarction: a value-based healthcare approach.","authors":"Anderson Bermon, Maricel Licht-Ardila, Edgar Fabián Manrique-Hernández, Alexandra Hurtado-Ortiz","doi":"10.15649/cuidarte.3796","DOIUrl":"10.15649/cuidarte.3796","url":null,"abstract":"<p><strong>Introduction: </strong>Understanding the impact of value-based healthcare and various healthcare payment models on the health outcomes of patients with acute myocardial infarction (AMI) is pivotal for guiding clinical strategies and decisions.</p><p><strong>Objective: </strong>To compare health outcomes and costs associated with healthcare for AMI patients under insurance prospective global payment (PGP) and fee-for-service models.</p><p><strong>Materials and methods: </strong>A retrospective cohort study encompassing AMI patients was conducted from 2021-2023. Convenience sampling of participants over 18 years of age diagnosed with type 2 myocardial infarction was conducted. Analysis was based on Colombian healthcare system payment models: PGP and fee-for-service.</p><p><strong>Results: </strong>The study involved 2134 patients, 657 (31%) under PGP and 1477 (69%) under fee-for-service. Length of hospital stay was associated with the payment model (coefficient -0.68, CI 95%: 0.40 to 0.98, p=0.037). Payment models also correlated with costs (845 USD, CI 95%: 87.92 to 1601; p=0.02). In-hospital mortality is not associated with either of the two contracting models. Hypertension management must be carried out by health professionals, whether or not treatment involves medication. By controlling drug treatment, especially adherence, serious health problems for older people can be avoided.</p><p><strong>Objective: </strong>To determine the factors associated with adherence to arterial hypertension treatment in older adults who live at home.</p><p><strong>Materials and methods: </strong>A quantitative and cross-sectional study was conducted in La Libertad Region, Peru, with 342 older adults living at home. For data collection, a sociodemographic profile form, anthropometric measurements, blood pressure measurements, the Mini-mental State Examination (MMSE) test, the Geriatric Depression Scale (GDS), and the Morisky Green Levine (MGL) adherence scale were used. In addition, descriptive and analytical statistics were used.</p><p><strong>Result: </strong>57.60% of the participants did not adhere to the pharmacological treatment, and, in most of the sociodemographic variables examined, they did not adhere to pharmacological treatment in most cases. Likewise, a relationship between retirement in older adults and the MGL adherence scale score was identified.
