Seminskiy","doi":"10.23888/hmj2022102225-236","DOIUrl":"https://doi.org/10.23888/hmj2022102225-236","url":null,"abstract":"INTRODUCTION: Socket alveolitis is the most unpleasant and frequent complication of the tooth extraction operation. According to the data of numerous studies, it most often occurs after extraction of the third mandibular molar. The causes of inflammation of the extracted tooth socket are well known. They include absence (“dry socket”) or decay of a blood clot which forms immediately after the operation and performs protective and reparative function during physiological healing of the socket. In turn, the risk factors provoking “dry socket” formation or blood clot fibrinolysis are mechanical injury of the socket, use of high concentration of adrenaline in local anesthetic solution, inadequate socket curettage, bleeding in the postoperative period, technically complicated surgical procedure and many others. However, all the mentioned factors leading to alveolitis, equally influence the inflammatory activity in the socket of any extracted tooth, including the lower third molar. Consequently, the anatomical location of the lower wisdom tooth, its germ and formation, have some peculiarities. The data analysis was carried out in the following Internet resources: eLibrary, Cyberleninka, disserCat, National Library of Medicine, ResearchGate (48 sources for the time period from 1967 to 2021). In the review article, the collected data of different studies on the frequency of alveolitis after tooth extraction, after the third lower molar extraction, on the main causes of the socket inflammation are given; generalized information of the anatomical characteristics of third molar location, its germ and formation is presented. {"title":"Endovascular Occlusion in Combined Treatment of Arteriovenous Erectile Disfunction","authors":"K. A. Burdin","doi":"10.23888/hmj2022102165-174","DOIUrl":"https://doi.org/10.23888/hmj2022102165-174","url":null,"abstract":"BACKGROUND: The vasculogenic nature of the erectile dysfunction (ED) prevails over other causes and accounts for 70%, of which 30% is arteriogenic, 20% to 40% is venogenic, and 30% is mixed arteriovenous ED. The main importance of shock wave therapy (SWT) in the treatment of ED is a complex positive effect on the blood supply of the penis. However, there is not enough information in the literature about the use of SWT in patients with arteriovenous ED, which determines the relevance of research in this direction. AIM: To evaluate the results of combined treatment (SWT and endovascular embolization of the deep dorsal vein (DDV) of the penis) in patients with arteriovenous ED. MATERIALS AND METHODS: In the Federal State Research Center of the Federal Medical and Biomedical Agency named after AI Burnazyan of Russia 45 patients aged 26 to 45 years (35.5 ± 9.5) years with a confirmed clinical diagnosis of arteriovenous ED, who underwent combined treatment were examined. The treatment was performed in 2 stages: stage I included endovascular embolization of the dorsal penile vein; stage II consisted of SWT with the use of BTL-6000 SWT TOPLINE device (BTL Corporate) in 2 weeks after surgical treatment. The condition of patients was assessed before treatment and 1 and 12 months after it. The assessment was based on the data of the questionnaire of the International Index of Erectile Function, fifth revision (IIEF-5), and the Erection Rigidity Scale (ESR), as well as instrumental diagnostic methods: computer dynamic cavernosography and pharmacodoplerography. RESULTS: An improvement of blood flow velocity parameters, a positive dynamic according to IIEF-5 and SER in 1 year following treatment. Povarov","doi":"10.23888/hmj2022102175-186","DOIUrl":"https://doi.org/10.23888/hmj2022102175-186","url":null,"abstract":"INTRODUCTION: Despite the intensive study of the pathophysiology, molecular and cellular mechanisms of atherosclerosis, the development and introduction of a number of new treatment methods, to date, the disease of the arteries of the lower extremities is one of the most pressing problems of modern vascular surgery and medicine in general. AIM: To evaluate the influence of cilostazol on the functional status of endothelium and pain-free walking distance in patients with lower limb artery diseases of atherosclerotic etiology. MATERIALS AND METHODS: The study was performed on 80 patients with obliterating atherosclerosis of lower limb arteries. Depending on the type of the given conservative therapy, the patients were divided to two groups. Group A (n = 40) included patients who received the baseline therapy in accordance with National recommendations on managing patients with peripheral artery diseases, and also medical drug Aducil® (cilostazol) 100 mg 1 tablet two times a day. Group В (n = 40) included patients who received only baseline therapy in accordance with National recommendations on managing patients with peripheral artery diseases. From all the patients, venous blood samples were taken for determining a number of markers of the endothelial dysfunction: endothelial nitric oxide synthase, prostacyclin, endothelin-1, interleukine-6. RESULTS: In 6 and 12 months after the start of the treatment, the level of endothelial nitric oxide synthase was reliably higher in group А patients (6 months — 12.91 (11.23–14.57) ng/ml; 12 months — 13.86 (12.42–16.25) ng/ml) in comparison with group В (6 months — 11.62 (8.63–16.73) ng/ml; 12 months — 11.76 (8.32–18.43) ng/ml) (р < 0.001). The level of prostacyclin was statistically significantly higher in group А after 6 months (group А — 18,532.88 (17,458.85–20,905.71) ng/ml; group В — 17,592.5 (15,542.56–19,721.52) ng/ml) and 12 months (group А — 18,452.27 (17,567.31–20,780.05) ng/ml; group В — 17,013.05 (15,090.1–19,517.58) ng/ml) in comparison with group В (р < 0.05). After 1 month from the start of treatment, the level of interleukin-6 was lower in group А (group А — 2.8 (2.23–3.4) ng/ml; group В — 3.65 (2.65–4.7) ng/ml), in 3 months — (group А — 2.4 (2–2.95) ng/ml; group В — 3.6 (2.55–4.5) ng/ml), 6 months (group А — 2.15 (1.73–2.88) ng/ml; group В — 3.45 (2.73–4.4) ng/ml), and 12 months (group А — 2.2 (1.93–2.95) ng/ml; group В — 3.3 (2.6–4.2) ng/ml) in comparison with group В (р < 0.001). No statistically significant difference in the levels of endothelin-1 was seen between group А and group В (p > 0.05). AIM: To study social and hygienic risk factors in children with bronchial asthma and comorbid pathology. MATERIALS AND METHODS: To study medical and social risk factors in children with bronchial asthma with comorbid pathology, a sociological survey of their parents was conducted. The survey involved the parents of 368 children diagnosed with bronchial asthma. To obtain clinical and statistical characteristics of children, primary medical documentation was selected and analyzed — medical histories and outpatient cards. This is a single-center, non-randomized and uncontrolled study. RESULTS: It was found that in children of the early age with comorbid pathology, diagnosed with bronchial asthma, the latter mostly takes uncontrolled and partially controlled course. The most common comorbid diseases are diseases of the respiratory organs (except for bronchial asthma), digestive organs, endocrine system, eating disorders, metabolic disorders, a combination of several diseases of allergic origin. Among the socio-hygienic factors potentially affecting the health of children, the most common were smoking in the family, low income, and poor housing conditions. The disease affects the gastrointestinal tract along the whole length from the mouth to the anus. More than 50% of patients with CD within the first 10 years following established diagnosis develop strictures or fistulas. These complications lead to intestinal obstruction which is a common indication for surgery in the volume of resection of the affected bowel. Surgery for CD complications does not rid the patient of the disease. Endoscopic and clinical recurrence of CD is a common phenomenon after surgical resection of the gut. The surgical approach to treatment of CD have not gained a wide spread since the first description of the disease. It is evident that to reduce the frequency of recurrences, it is necessary to improve surgical tactics. Clarification and better understanding of the anatomy and function of mesentery in recent years has led to the suggestion of its probable role in the pathogenesis of CD. Extended mesenteric resection may improve the outcomes of surgery in CD. Rather than to mobilize and excise the mesentery as in colorectal cancer, the mesentery is usually transected at the level of the intestine and remains in an organism. This review provides the data of recent studies of the role of mesentery in the pathogenesis of CD, which permit to suggest that extended resection of mesentery can significantly improve surgical results in CD and to reduce the frequency of endoscopic and clinical recurrences and reoperations. CONCLUSION: Mesentery is involved in the pathogenesis of Crohns disease and the development of relapse. {"title":"Provision of People of Retirement Age with High-Tech Medical Care is One of the Most Important Trends of Healthcare Development","authors":"I. B. First of all, it is associated with a decline in the share of diseases with the acute course and an increasing number of chronic diseases. There is an increasing risk of initiation of diseases that require not only primary medical and rehabilitation care, but also high-tech care. AIM: Analyze the scope and structure of high-tech medical care (HTMC) rendered to people above the working age living in the Astrakhan region. MATERIALS AND METHODS: The study was carried out using a continuous method, with subsequent statistical processing. The observation period covered 2010 to 2019. This study included 21271 cases of HTMC rendered to people of the retirement age. A statistical research method was used with calculation of the relative values and analysis of the dynamic series. RESULTS: Showed 2.8-fold increase in the volume of HTMC rendered to people older than the working age. The main profiles were cardiovascular surgery, traumatology and orthopedics and oncology, which accounted for more than 4/5 of all cases of HTMC. In the total structure of people who received this type of medical care during the study period, the share of rural residents averaged 27.2%. Concerning the structure of outcomes of HTMC, in the absolute majority of patients (96%) despite the advanced age, improvement and recovery were achieved. It should be noted that ¾ of the total number of cases were managed in medical organizations in the city of Astrakhan, the rest of the patients were managed outside the city. CONCLUSION: The negative aspect is poor availability of HTMC for people living in rural areas and a limited number of profiles for which HTMC is provided within the region. However, in practice, sufficient cosmetic effect is not always noted. Study, analysis and improvement of botulinum therapy by a doctor, familiarization of the patient with the correction plan including long-term monitoring, are necessary measures to increase the compliance and the extent of satisfaction with the results, which will permit to further improve the quality of medical care. Despite numerous developments in the field of organization and quality management in healthcare, this problem remains relevant today. The article describes a clinical example of using a computer program for evaluation of the evidence of facial mimic wrinkles after botulinum therapy in different follow-up periods, and evaluation of the effectiveness of botulinum therapy using mathematical model for calculation of the integral quality coefficient and visibility graphs. Shukis","doi":"10.23888/hmj2022102157-164","DOIUrl":"https://doi.org/10.23888/hmj2022102157-164","url":null,"abstract":"INTRODUCTION: Epicardial adipose tissue is usually located on the free wall of the right ventricle, the left ventricular apex, and the atria, but the valvular apparatus of the heart, the interventricular and interatrial septum can be subjected to lipotoxic damage. The accumulation of adipose tissue on the interatrial septum is of particular scientific interest and is described in the literature as lipomatous hypertrophy. AIM: To study the clinical and morphological features of lipomatous hypertrophy of interatrial septum in patients with diseases of the cardiovascular system. MATERIALS AND METHODS: Morphological examination of the hearts of 60 patients (30 men, 30 women), average age 74.6 ± 9.5 years was conducted. Morphometric parameters of the heart muscle and the aorta were evaluated. The studied heart material was stained with hematoxylin-eosin and according to Mallory and was processed by standard histological methods. RESULTS: The causes of death of patients were myocardial infarction, cerebral infarction, postinfarction cardiosclerosis and type 2 diabetes. The analysis of thickness of the interatrial septum in patients of different groups revealed lipomatosis of the interatrial septum in 83% of patients with postinfarction cardiosclerosis, in 80% of patients with myocardial infarction, in 70% with type 2 diabetes, in 66.7% with cerebral infarction. The thickness of the interatrial septum with lipomatous hypertrophy was 1.2 ± 0.12 cm in patients with myocardial infarction, 1.2 ± 0.11 cm in cerebral infarction, 1.2 ± 0.2 cm in postinfarction cardiosclerosis, 1.2 ± 0.15 cm in type 2 diabetes. There were no reliable differences in the thickness of the interatrial septum between the study groups. The correlation was established between the thickness of the interatrial septum with lipomatous alterations and hypertrophy of the anterior wall of the left ventricle (r = 0.7; р < 0.0001), fibrosis of the anterior wall of the left ventricle (r = 0.6; p = 0.03), thickness of epicardial fat (r = 0.5; p = 0.0002), as well as the interrelation with the thickness of the interventricular septum (r = 0.7; p < 0.05). The existence of epicardial fat in individuals with lipomatosis of the interatrial septum positively correlated with the thickness of the interventricular septum (r = 0.5; p = 0.01) and the thickness of the posterior wall of the left ventricle (r = 0.6; p = 0.001). The correlation relationship was also established between the thickness of the interatrial septum with lipomatous alterations and laboratory markers of cardiovascular risk: the level of total cholesterol (r = 0.8; p = 0.01), triglycerides (r = 0.6; p = 0.001), low-density lipoproteins (r = 0.7; p = 0.001). The molecular basis of pathogenesis in hypoxia can serve a basis for the development of methods of treatment and correction. AIM: development of a model of chronic normobaric hypoxia in laboratory animals. MATERIALS AND METHODS: The study involved 32 mature male rats of Wistar line of 200–280 g mass. The animals were divided to 2 experimental groups and their control groups. In the first experimental model, a model of acute normobaric hypoxia with hypercapnia was used one time according to the method of M. V. Korableva and P. I. Lukienko (1976) in modification of N. D. Avseenko, in the second — a model of chronic hypoxia according to the method of acute normobaric hypoxic hypoxia with hypercapnia in our modification: the animals were placed in a hermetically closed chamber of 1.2 liter volume connected to the gas analyzer, and stayed there until the content of oxygen in the air decreased to 10%. Animals of the control groups were placed in ventilated chambers. The experiment was performed daily for 14 days. The activity of cytochrome oxidase (CO), lactate dehydrogenase (LDH) and superoxide dismutase (SOD) in the mitochondrial fraction of seminal vesicles and epididymis was determined. RESULTS: In modeling of acute normobaric hypoxia, the activity of the studied enzymes did not show statistically significant changes. In an experiment with chronic normobaric hypoxia, the activity of SOD and CO was significantly reduced in all the studied tissues, and the activity of LDH — only in the tissues of the epididymis head. Novikova","doi":"10.23888/hmj2022102203-212","DOIUrl":"https://doi.org/10.23888/hmj2022102203-212","url":null,"abstract":"INTRODUCTION: Zudek syndrome (neuroreflex algodystrophy, accompanied by osteoporosis, limited range of motion in the wrist and interphalangeal joints, impaired blood flow and associated trophic alterations) accounts for up to 62% of complications of fracture of the distal metaepiphysis of the radius. It may be a consequence of primary action of pathological factors (I type), or may result from compression of peripheral nerves (II type). The article describes a clinical case illustrating occurrence of almost all critical errors in the treatment process (non-performance of local anesthesia of the fracture and of reposition of fragments; fixation of the hand in a vicious (for this type of fracture) flexion position for up to 3 weeks; lack of local physiotherapy, anti-inflammatory and kinesitherapeutic treatment; use of “aggressive” mechanotherapy in passive restoration of the volume of movements and of “hard” massage against the background pain syndrome) that contributed to the development of the given II type complication. As a result, a surgical correction was required — corrective osteotomy of the radius to eliminate the cause of compression of the ulnar and median nerves and restore the interrelations between the articular surfaces in the wrist joint. CONCLUSION: Non-observance of the principles of treatment of a fracture of the radius in a “typical place” (emergency care; adequate anesthesia; immediate sparing reposition; subsequent clinical and radiological control, functional management; sufficient period of immobilization; early physiotherapy and kinesitherapy; etc.) may provoke such a threatening complication as Zudek syndrome characterized by a prolonged disability and inflicting physical and moral suffering.
