Although vitiligo most often runs in isolation, being limited to a pathological process in skin, this disease can lead to the development of a number of comorbid disorders. Alopecia areata also being an autoimmune disorder of skin appendages, is one of the most common comorbid disorders in vitiligo, manifesting along with the main disease, against the background main disease, or as the main disease itself. In the article, a clinical case of association of vitiligo and alopecia areata is presented, similarities and differences in their pathogenesis are discussed, and other comorbid disorders are described. CONCLUSION: Determination and identification of comorbid disorders with the underlying chronic dermatosis is an important task aiming to prevent the development of probable autoimmune and autoinflammatory disorders. Bubley","doi":"10.23888/hmj2023112159-168","DOIUrl":"https://doi.org/10.23888/hmj2023112159-168","url":null,"abstract":"INTRODUCTION: The data of SARS-CoV-2 pandemic studies indicate a pronounced influence of overweight and obesity on the course of the new coronavirus infection and the risk of lethal complications. In this regard, of particular interest are the changes in the parameters of the main routine diagnostic complex identified in patients with SARS-CoV-2 infection having excess body weight and obesity, and also the search for pathophysiological relationships between these changes and the excess body weight. AIM: To evaluate the changes in the parameters of systemic inflammation and coagulogram, as well as of the peripheral blood leukocyte count in patients with COVID-19 with excess body weight and obesity. MATERIALS AND METHODS: 73 patients with PCR-positive result for SARS-CoV-2 in the age group of 44–65 years old admitted to the infectious diseases department of Semashko Republican Clinical Hospital were examined. The patients were divided into 3 clinical groups depending on body mass index (BMI): Group 1 — patients with BMI < 25 kg/m2, Group 2 — patients with excessive body weight, and Group 3 — patients with 1st degree obesity. On admission to the hospital, the patients underwent the general blood analysis, analysis of C-reactive protein (CRP) level, erythrocyte sedimentation rate (ESR), ferritin, coagulogram parameters, as well as a computer tomography (CT) of the chest organs. RESULTS: All the patients with new coronavirus infection had significantly higher parameters of systemic inflammation (CRP and ferritin) compared to the control group (p < 0.05). The group of patients with 1st degree obesity recorded the highest absolute numbers of band and segmented neutrophils, ferritin, and D-dimer (р1–3 < 0.05; р2–3 < 0.05). The levels of ESR, CRP, and fibrinogen were significantly higher in the overweight and obese groups, but no significant differences between groups 2 and 3 were found in the intergroup comparisons (р1–2 < 0.05; р1–3 < 0.05; р2–3 > 0.05). The values of activated partial tromboplastin time and thrombin time did not differ significantly between the clinical groups (p > 0.05). It is also worth noting that the percentage of pulmonary tissue lesions according to CT data varied significantly depending on BMI, with the highest rate in the group of patients with 1st degree obesity (p < 0.05). Angiogenesis is a key factor in wound healing. It is known that the optimal blood supply provides the tissue with nutrients, growth factors, immune cells necessary for the complete repair of skin defect. Insufficient ability to restore functional blood supply to the wound area can lead to chronic non-healing wounds. Therefore, the identification and validation of various factors influencing angiogenesis in the wound bed is of great interest for the development of potential therapeutic agents. The aim of this review is to highlight the current understanding of the mechanisms and morphological manifestations of skin wound angiogenesis, as well as the functional significance of stromal cells involved in this process. The contribution of macrophages, pericytes, mast cells, telocytes to the mechanisms of formation of microcirculatory vessels is shown. The search for literary sources was carried out in the electronic systems eLIBRARY.RU and PubMed by the main keywords of the topic: histology, skin wound regeneration, angiogenesis, stromal cells, macrophages, mast cells, pericytes, telocytes. The publications were analyzed over the last 12 years. The criteria for including a research in the review were the simultaneous mention of several keywords in the article. CONCLUSION: Analysis of the literature demonstrates the relevance of the study of angiogenesis. Modern morphological methods permitted to describe complex intercellular interactions providing the growth of the vascular network. Despite the large number of studies in this area, morphological data are constantly being supplemented to expand the potentials of histological and immunohistochemical studies. Cellular interactions during wound healing are being actively studied and confirmed to be used in the study of the mechanisms of action of drugs and in the development of new methods for treating wounds of various origin. The basis of effective therapy for irritable bowel syndrome (IBS) and functional dyspepsia (FD) is a combined use of symptom-reducing drugs. AIM: To assess the dynamics of the intensity of abdominal pain and changes in the motility of stomach and intestine after treatment of patients with IBS, and with IBS in combination with FD, with STW 5 preparation. MATERIALS AND METHODS: 28 patients with IBS, including patients with a combination of IBS and FD, were taking STW 5 orally, 20 drops 3 times daily before meals for 14 days, to relieve clinical symptoms. Before and after treatment, patients completed a survey using VAS and ‘7 × 7’ questionnaires and underwent peripheral electrogastroenterocolography (EGECG). RESULTS: The level of abdominal pain before treatment in patients with predominating constipation corresponded to a moderate degree of intensity, in patients with diarrhea to a mild degree of intensity. During treatment, 32.1% of patients noted relief of abdominal pain in the second week of treatment; bloating was less common and/or did not occur in 71.4% of patients; 42.9% of study participants noted normalization of stool frequency and/or stool shape. Improvement of clinical condition in terms of FD symptoms was observed in 76.9% of patients with IBS and FD. The use of STW 5 was associated with a 51.3% reduction of pain intensity (p < 0.001) after a week of treatment. Mean changes in the colon coefficient rhythmicity (Crhythm) according to the results of EGECG after treatment in patients with constipation reliably differed from the level of change in Crhythm in patients with diarrhea (p = 0.047). This article describes a clinical case of elimination of the stricture of the parotid salivary gland duct and compensation for its expanded section behind the gland with intraoperative visualization using ultrasound. Under US control, the expanded portion was punctured from external access; a conductor was inserted, which passed into the oral cavity through the natural orifice. Through the formed opening, a catheter was installed, which expanded the narrowed area and prevented the reappearance of the stricture. Control examinations in 1, 6 and 12 months showed the functioning of the orifice with release of clear saliva in the satisfactory amount. The patient noted absence of enlargement of the parotid salivary gland and absence of pain at rest and in chewing. Control US in 6 and 12 months showed absence of any signs of stricture and reduction of the orifice diameter to 2 mm. The parasite is the original factor that triggers productive processes due to the mutation of numerous proliferative genes. Stem cells are initiated with their subsequent differentiation along the lines of various differons. AIM: to identify proliferative processes and processes of neoangiogenesis of adipose tissue in type 2 diabetes mellitus and superinvasive opisthorchiasis. MATERIALS AND METHODS: sectional material (n = 58): group 1 — healthy individuals (n = 10), group 2 — deceased with type 2 diabetes mellitus from accidental causes (n = 22), group 3 — persons with superinvasive opisthorchiasis died by violence, with duration of invasion more than 10 years (n = 26). The material was subjected to routine histological examination, including histochemical and immunohistochemical staining methods. Further, microscopy was carried out with subsequent statistical analysis. RESULTS: reduction of the proliferative activity of adipose tissue cells in type 2 diabetes mellitus and increase in the parameters in opisthorchiasis invasion compared with healthy individuals were revealed; in diabetes mellitus, the hypertrophic variant of adipose tissue prevailed, the volumetric density of blood vessels decreased, while in patients with superinvasive opisthorchiasis, lipolysis and angiogenesis increased. CONCLUSION: predictors of type 2 diabetes mellitus were assumed to be low proliferative activity of adipocytes, low volumetric density of blood vessels and hypertrophic variant of adipose tissue. AIM: To assess the quality of life (QoL) of women in peri- and postmenopausal periods, to identify factors producing a negative effect on him during these periods. MATERIALS AND METHODS: А cross-sectional study was conducted through a survey of 353 women above 40. To assess QoL, the SF 36 questionnaire was used. Sociodemographic factors and peculiarities of health status of the participants were considered risk factors. The data were introduced and analyzed using SPSS 23 statistical package. Statistical significance of the results was evaluated by Kruskal–Wallis test with the subsequent a posteriori analysis, and Mann–Whitney test. RESULTS: Peri- and post menopause had negative impact on the parameters of physical component of QoL and all its domains. Participants with regular menstruation had a reliably higher score of physical component of QoL compared to peri- and postmenopausal women (p < 0.001). The scores of physical component of QoL were higher in women at the age under 50 compared to women of older age groups. The scores were higher in women with higher and secondary special education compared to participants with general secondary education (p < 0.05). Higher scores of physical and psychological components of QoL were recorded in working women compared to the retired group (p < 0.05). The parameters of psychological component of QoL were the highest among married and single women. Parameters of physical and mental components of QoL were higher in groups with the income above and at the level of the living minimum compared to those who reported the income below the minimum wage (p < 0.001). A reliably higher scores of physical and mental components of QoL were recorded in participants with favorable microclimate in families compared to the group with an unfavorable one (p < 0.01). A reliable relationship was found between the evaluation of the health and parameters of both components of QoL by the participants. Higher scores were recorded in women who evaluated their health as excellent and above average compared to all other groups (p < 0.05). The presence of chronic diseases had a negative impact on both components of QoL. The work presents the aspects of the implantological treatment in the system of public healthcare of the Ryazan region. The article, on an example of the united city dental organization providing dental services to the entire population of the regional center, presents difficulties in identifying the real need for dental implantation at the regional level, which is associated with the fact of receiving dental care by a part of patients in private clinics that do not submit statistical data to the reports of the region. The data of a scientific study conducted in the Ryazan region to identify the loyalty of old-age pensioners to public dental organizations are presented. The influence of preventive measures aimed at combating the pandemic of the new coronavirus infection (COVID-19) on the implementation of the volumes of dental care, including dental implantation, is noted. Arkharova","doi":"10.23888/hmj2023112189-202","DOIUrl":"https://doi.org/10.23888/hmj2023112189-202","url":null,"abstract":"INTRODUCTION: The issue of restoration of the architectonics of the palatine arch remains relevant both due to the widespread use of removable dentures of different design in everyday practice of dentists of private and governmental medical and prophylactic institutions of dental profile, and due to the development of modern techniques and approaches to the total rehabilitation of patients through use of non-replaceable structures with support on dental implants. All these interventions are associated with derangement of the anatomical structure of the front third of the palatine arch either in result of spanning of it with structural elements of dentures, or in result of reduction of bone tissue to achieve the required width for installation of dental implants. AIM: To study architectonics of the hard palate, alveolar process of the upper jaw and the volume of the moth cavity in patients with intact (sanitated) dentitions and with pathologies. MATERIALS AND METHODS: The study involved patients with intact dentitions and with dental pathologies. In the course of the work, morphometric parameters were studied, namely silicon keys taken from the models of the patients. Separation was performed perpendicular to the palatine seam along the teeth, and the degree of tilt was determined on millimeter paper. There were also studied lateral teleroentgenograms of patients obtained in the course of treatment. The volume of the oral cavity was evaluated by measuring the mean value of two height dimensions, two width dimensions, and the distance from the angle of the mandible to the middle of the chin by the method described in RU 2448646 C2 patent of 27/04/2012. RESULTS: The obtained results of morphometric studies in patients with healthy dentitions show variations in the mean angle of tilt of the hard palate in various sections 69 ± 1.2 degrees in male patients, 76 ± 2.4 in female patients, and the maximum angle of tilt of the hard palate in various sections 74 ± 5.2 degrees in male patients, 83 ± 4.4 in female patients. During the analysis of models in patients with secondary adentia, the data were obtained that permit to get an idea about the influence of atrophic processes of the bone-periodontal complex on changes in the architectonics of the oral cavity in general and in the morphometric parameters of the upper jaw in particular. The study of the data obtained in radiation diagnostics evidences the influence of atrophic processes on the parameters of the volume of the oral cavity of patients. CONCLUSIONS: The obtained results show the influence of atrophic processes on the parameters of the volume of the oral cavity of patients; thus, with increase in the number of missing teeth and increase in the volume of bone tissue lost in atrophic processes, and with its densification, the volume of the mouth cavity of the patient increases. Kamayev","doi":"10.23888/hmj2023112241-256","DOIUrl":"https://doi.org/10.23888/hmj2023112241-256","url":null,"abstract":"INTRODUCTION: An analysis of publications of domestic and foreign literature was carried out and a review on endothelial dysfunction in patients with varicose saphenous veins of the lower extremities was presented. We searched Elibrary and PubMed biomedical publication databases for the required information for the last 5 to7 years using keywords: varicose saphenous veins of lower extremities, endothelial dysfunction, endothelial dysfunction markers. Today, diseases of the cardiovascular system occupy a leading position with the most common of them being chronic venous disease that affects 10% to 30% of the adult working-age population of the Russian Federation. Varicose veins cover a wide range of conditions from minor telangiectasia to pronounced varix dilatation of veins and severe venous insufficiency with venous ulcers. The endothelium is not only a barrier between blood and tissues, but also a kind of endocrine organ that produces a number of vasoactive substances that maintain homeostasis and perform, depending on environmental conditions, both an antithrombotic, anti-inflammatory, vasodilating effects, and a procoagulant, inflammatory and vasoconstrictive effects. Endothelial dysfunction is considered by many domestic and foreign authors as the main link in the development and progression of many diseases, including varicose veins. CONCLUSION: Determination of the dynamics of some markers of endothelial dysfunction and connective tissue dysplasia in patients with varicose saphenous veins of the lower extremities is a priority direction in prevention of disease progression. Despite ongoing research, many aspects of the pathogenesis of varicose veins of the lower extremities are still unclear.
