Success in reducing morbidity with oncologic diseases depends on many factors with the essential one being the development of the legal framework for organization of medical care for patients with precancerous conditions and MNPs of the oral mucosa and lips. A content-analysis of the sources of the regulatory legal information was performed based on the following keywords and their combinations: dentistry, neoplasm, screening, oral mucosa. The sources were studied by regular review of the electronic Garant database (http://base.garant.ru). Sixteenth regulatory legal acts were identified from 2017 to the present. CONCLUSION: Based on the studied regulatory legal acts for providing care to patients with malignant neoplasms (MNPs) and precancerous conditions of the organs and tissues of the oral cavity in the Ryazan region, it can be concluded that the management takes an active part in the organization of prevention, diagnosis, treatment of patients with MNPs of different organs and systems, including the maxillofacial region, as well as of their rehabilitation. The implementation of the regional program to combat oncological diseases, on the one hand, reflects their high prevalence, and on the other hand, the active participation of the government and the leadership of the Ryazan region in solving this problem. The documents regulating the procedure for provision of medical care in the profiles ‘oncology’ and ‘dentistry’ detail the actions of medical personnel when treating a patient with a MNP. Special attention is paid to the issues of prevention and early detection of oncological diseases in the population of the Ryazan region, as well as dispensary monitoring of patients with oncopathology. Shustov","doi":"10.23888/hmj2023112257-270","DOIUrl":"https://doi.org/10.23888/hmj2023112257-270","url":null,"abstract":"INTRODUCTION: One of the important disorders that can be induced by using synthetic cathinones (SC) is delirium, differentiated from other psychotic complications in ICD-11. The article presents a systematic review of the literature with the aim to describe the symptoms, the effectiveness of empirical approaches to therapy and probable outcomes of delirium induced by SC. The analysis of scientific publications in eLibrary, CNKI, PubMed, Scopus databases was performed using search by keywords. In accordance with the recommendations of PRISMA, 23 articles were selected. CONCLUSION: Most of the articles found were about MDPV-induced delirium. No exact dose range of SC that can cause delirium is specified. Delirium is more often described after prolonged use of SC (for 1 month), in more rare cases after a single use. In most cases delirium develops within a few hours after intake of SC and is associated with intoxication, rather than with withdrawal syndrome. The clinical picture of the studied conditions, besides the triad of the main symptoms of delirium, includes psychomotor excitement with pronounced sympathicotonia, in the vast majority of cases psychotic symptoms (acute sensory delirium with paranoid syndrome, true visual hallucinations, insomnia, aggressiveness expressed in the form of hetero- or autoaggression) are also present. Of highest effectiveness is therapy with benzodiazepines and antipsychotics with the use of detoxification and general resuscitation measures. It is important to study changes in the cardiovascular system induced by exposure to industrial noise. AIM: To evaluate the effect of industrial noise on the parameters of volume sphygmography in men of working age. MATERIALS AND METHODS: The study included 205 men of the working age undergoing periodic medical examinations at the polyclinic of the Nizhny Novgorod Research Institute of Hygiene and Occupational Pathology of Rospotrebnadzor. Two groups of the subjects were identified: the main group (150 people) — working under the influence of industrial noise exceeding the maximum permissible level, and the comparison group (55 people) — working out of contact with hazardous industrial factors. To assess the stiffness of the vascular wall, all patients underwent volume sphygmography on VaSera VS-1500N device (FukudaDenshi, Japan). RESULTS: The formed groups were comparable in the main cardiovascular risk factors. However, of attention was the trend towards an increase in the share of smoking patients in the control group. The analysis of the parameters. of volume sphygmography showed a statistically significant increase in the stiffness index of the vascular wall on the right and left, in the estimated age of the arteries and also frequent (7.5 times) detection of syndrome of early vascular aging in the main group. CONCLUSION: The identified deviations of sphygmography parameters are a marker of early changes in the vascular wall in people working with noise. Leonov","doi":"10.23888/hmj2023112178-188","DOIUrl":"https://doi.org/10.23888/hmj2023112178-188","url":null,"abstract":"INTRODUCTION: Drug-resistant epilepsy (DRE) is associated with a high incidence of comorbidities, psychiatric comorbidity, and early mortality in patients, which permits to consider epilepsy a systemic dysfunction of the body as a whole. AIM: Was to describe the features and relationships of clinical, neurophysiological and psychometric parameters of patients with DRE from a systemic standpoint. MATERIALS AND METHODS: A comprehensive clinical and neurophysiological study was conducted in 60 patients diagnosed with epilepsy, who were divided into 2 groups: 30 patients with DRE and 30 patients with remission of seizures. A clinical study was carried out including electroencephalography (EEG) with the calculation of the spectral power of the α-rhythm, the coherence coefficient and the cross-correlation coefficient for each pair of leads. The psychometric study included Eysenck, Strelau tests, the Taylor Anxiety Scale, the Freiburg Personality Inventory (FPI), the Toronto Alexithymia Scale, and the Self-Destructive Tendency Test. An analysis of correlations between clinical, psychometric and neurophysiological parameters in the initial state and with functional load was carried out. Based on the data obtained, graphical models were constructed. RESULTS: In patients with DRE, the clinical features of the disease, consisting in a longer duration of the disease and higher frequency of attacks, negatively correlated with the parameters of the spectral power of the α-rhythm, and with the levels of neuroticism and self-destructive behavior, and the low values of these characteristics can be associated with an unfavorable course of the disease. Rigid, systemic organization of psychological functions in patients with DRE, deficit of inhibitory processes in the nervous system were manifested in numerous correlations between psychometric parameters that negatively affect the quality of life, and also of psychometric parameters and spectral power of the EEG α-rhythm in different functional conditions. Numerous correlations between parameters may evidence a high level of conjugation of neurophysiological mechanisms with parameters in patients with DRE. These technologies allow not only to update and clarify the understanding of the molecular genetic nature of tumors, but also to develop new directions for the therapy and the course of cancer diseases. The prerequisite for this scientific direction was the decoding of the human genome, as a result of which most human genes have been identified and mapped by now. Microarray technology has been since the late 1980s and has been rapidly developing since then. In a short time, biological microarrays have evolved into an independent field of analysis, including both the study of fundamental problems of molecular biology and molecular evolution, and practical applications in medicine, pharmacology, forensic expertise. AIM: To demonstrate the potential of the microarray expression assay method in clinical oncology, to describe the history of elaboration, evolution, the principle of the method and possibilities of application of expression microarrays at the current stage of clinical oncology development. MATERIALS AND METHODS: The study was carried out at Ryazan State Medical University. The review included sources found in PubMed, Google Scholar, and eLibrary using the query ‘expression microarrays’ and ‘molecular genetic technologies in oncology’. A total, of 126,455 results were found for the query ‘expression microarrays’ in PubMed and Google Scholar. Thirty-five articles were selected, after a relativity analysis of the sources. A total, of 613 results were found in the eLibrary system for the query ‘molecular genetic technology in oncology’, and 15 articles were selected. Thus, 49 articles were included in the final version. CONCLUSION: The advent of microarray technology marked a new stage in molecular genetic analysis. This method allows simultaneous analysis of hundreds of transcription products and even the entire transcriptome, which together with the data on functions of tumor proteins already opens wide horizons for the use of this technology in clinic. Accumulated data allow to see a wide perspective in this method in terms of analysis of expression profile, metastatic potential of tumors, differential diagnosis between different types of malignant neoplasia, and thus individualize the treatment approach for each individual patient. The causative agent of the disease is S. aureus (95%) mostly occurring as a monoculture. AIM: To study the microbial flora of facial skin in military personnel with abscessed furuncle of the maxillofacial region. MATERIALS AND METHODS: In 53 patients with abscessed facial furuncles (the main group) aged from 18 to 25 years, material was taken for bacteriological examination from healthy areas of the skin and from the furuncle with a sterile cotton swab. As a comparison group, a group of 30 practically healthy individuals of the same age was recruited, who also underwent a microbiological examination of facial skin areas. Examinations were performed in the bacteriological laboratory of Kirov Military Medical Academy. RESULTS: Among the representatives of the microflora of healthy skin areas in patients with abscessed furuncles of the face, S. epidermidis was detected in 58% of cases, S. saprophyticus in 33%, S. haemoliticus in 10%. In 30% of admitted patients, Propionibacterium acnes were found. In 4% of cases, S. aureus was detected by us. Fungi of the genus Candida were found in 5% of patients. The microbial composition of the wound discharge in facial furunculosis in 87% of cases was represented by pathogenic S. aureus as a monoculture, in 4% in association with S. saprophyiticus, in 3% with Pseudomonas aeruginoza, in 1% in association with Klebsiella oxytoca, in 5% by S. epidermidis as a monoculture. The species composition of the microflora of the facial skin of healthy people was represented by non-pathogenic staphylococci S. epidermidis (88.1%), S. saprophyticus (40%). In 25.3% of cases, streptococci were also detected. In 10% of cases, yeast-like fungi of Malassezia genus and in 12% of Candida genus were found. In 4% of cases, the pathogenic form of S. aureus was identified. In particular, the causes of chronic postoperative pain associated with the effect of scarring on the spermatic cord have not been fully studied. Testicular flow studies after hernia repair mostly showed different outcomes. AIM: Comparative assessment of blood flow parameters of the testicular, capsular and intratesticular arteries, as well as the diameter of the testicular artery after autoplasty with a displaced aponeurotic flap and Lichtenstein hernioplasty using ultrasound. MATERIALS AND METHODS: A new method of autoplasty was used in 35 patients (Group I). The comparison group (Group II) amounted to 35 patients with Lichtenstein hernioplasty surgery using partially absorbable mesh endoprosthesis (UltraPro). The dynamics of changes in blood flow in the arteries was assessed using color Doppler before surgery, in the early and late postoperative periods. RESULTS: The Wilcoxon paired test showed a significant difference between preoperative and immediately postoperative measurements in both groups. Statistically significant differences between the groups were found in the end-diastolic velocity and resistance index in the testicular, capsular and intratesticular arteries 6 months and 1 year after surgery. The difference in peak systolic velocity was significant in the testicular artery also in the late stages after hernioplasty. The diameter of the testicular artery tended to increase in dynamics, but there was no any statistically significant difference between the groups. CONCLUSIONS: The method of autoplasty in the inguinal canal with a displaced aponeurotic flap does not significantly affect testicular perfusion. Subbotin","doi":"10.23888/hmj202311185-97","DOIUrl":"https://doi.org/10.23888/hmj202311185-97","url":null,"abstract":"INTRODUCTION: The number of patients with implantable cardiac devices (CDs) is steadily growing in the world and, in parallel with this, the number of complications associated with these devices, including infectious endocarditis (IE), arterial embolisms and mechanical damage to the heart valves. A case of patient P., 51 year old, diagnosed with IE of CD, pulmonary embolism and progression of tricuspid regurgitation to the 3rd degree with a previously implanted Sensia SR VVI pacemaker is presented. The peculiarities of this clinical case are highly probable existence of the source of infection (the patient suffered acute paraproctitis with unauthorized opening of the abscess without antibiotic therapy), severe comorbid pathology, the absence of fever throughout the entire follow-up period, the absence of visualized involvement of the valve apparatus with the presence of blood clots around the pacemaker electrodes, and the inability to remove infected pacemaker electrodes in the acute period for technical reasons. The difficulties of diagnosis and management of such patients in the conditions of the Regional Vascular Center without the technical possibilities for surgical treatment are discussed. At the same time, the problem of perioperative myocardial infarction still exists and is one of the causes of adverse cardiovascular events and mortality, both at the hospital stage of treatment and in the long-term period. A factor leading to such a complication may be acute dysfunction of the coronary bypass graft, the timely detection and elimination of which will reduce the negative consequences of myocardial damage. The relevance of the study results from investigation of the relationship between the angiographic data and the character of ischemic changes on the electrocardiogram of patients with and without proven acute dysfunction of coronary bypass graft. AIM: Identification of the most pathognomonic electrocardiographic signs of dysfunction of coronary bypass grafts in the early postoperative period after planned myocardial revascularization. MATERIALS AND METHODS: The study included 196 (1.86%) of 10,550 patients operated for coronary artery disease in Chelyabinsk Center of Cardiovascular Surgery from 2011 to 2020, who underwent emergency angiography after planned coronary artery bypass graft surgery due to the appearance of signs of myocardial damage in the early postoperative period. Depending on the type of angiographic findings and the chosen treatment tactics, patients were divided to 3 groups. Group I (n = 86) included patients in whom angiography revealed defects in the area of large bypassed coronary arteries, of them 60 patients underwent endovascular treatment, and 26 patients underwent repeated open surgery. Group II (n = 33) consisted of patients in whom bypass graft dysfunction was noted, but, however, due to the small diameter or pronounced diffuse lesion of the bypassed coronary arteries, all the patients underwent conservative therapy. Group III included 77 patients in whom hemodynamically significant defects in grafts and native arteries were not identified. RESULTS: When studying postoperative electrocardiographic data, it was found that significant differences between the groups were obtained only in the occurrence of ventricular arrhythmias (P1–2 = 0.027; P2–3 = 0.01). As for other pathological electrocardiographic signs and their combinations, no statistical differences were found. CONCLUSIONS: An isolated electrocardiographic study in the early postoperative period does not permit to identify patients with dysfunction of coronary bypass grafts. Konovalov","doi":"10.23888/hmj202311131-38","DOIUrl":"https://doi.org/10.23888/hmj202311131-38","url":null,"abstract":"INTRODUCTION: The course and outcomes of the disease, including diseases of the circulatory system, are greatly influenced not only by the quality of medical care, but also by the attitude of patients to their health, as well as their medical activity. AIM: To study the age characteristics of the medical activity of patients with diseases of the circulatory system, taking into account gender and marital status. MATERIALS AND METHODS: On the basis of the Vinogradov City Clinical Hospital, Moscow, using a specially designed questionnaire, 582 patients hospitalized in the cardiological and neurological departments of the hospital for diseases of the circulatory system were interviewed. In the mathematical processing of the obtained data, the methods of variation statistics were used. RESULTS: Only 28.9% of the respondents answered that they regularly see a doctor for diseases of the circulatory system. Women were more active than men in terms of compliance with the dispensary observation schedule and the frequency of consultations with doctors of other specialties. According to the data obtained, 62.4% of the respondents considered it necessary to establish schools for the chronically ill and their family members, but only 17.4% of the respondents attended a school for a patient with diseases of the circulatory system. It was found that 86.1% of the surveyed patients with diseases of the circulatory system kept self-monitoring diaries: 68.7 per 100 respondents monitored blood pressure, 46 — blood sugar level and 33.6 — body weight. At the same time, married men are more likely to keep self-monitoring diaries for blood pressure, blood sugar levels and body weight. The timeliness of seeking medical care is the most important criterion that characterizes the medical activity of patients and largely determines the outcome of treatment and the cost of it. The conducted sociological study revealed a rather high medical activity of patients with diseases of the circulatory system, as well as its significant differences in a number of positions in age, gender, and marriage and family aspects.
