Yudin","doi":"10.23888/hmj202311198-106","DOIUrl":"https://doi.org/10.23888/hmj202311198-106","url":null,"abstract":"INTRODUCTION: Despite the fact that the anatomy of the venous system is considered to be rather variable, the area of saphenofemoral junction has the most permanent structure with rather rare anatomic variants — not more than 0.3%. At the same time, these particular variants create technical difficulties in surgical treatment. It is interesting that the frequency of iatrogenic damages and of occurrence of variant anatomy is comparable. It is also important to note that thromboembolic complications account for 18.3% of complications of the postoperative period, according to different sources. One of factors of these complications may be a long stump of the great saphenous vein, therefore, the stage of resection of the great saphenous vein is of primary importance. In the article, a variant of atypical topography of saphenofemoral junction in a female patient with varicose vein disease is considered. CONCLUSION: It should be said that taking into account probable anatomic variants of vessels, it is always necessary to perform a thorough examination of the patient using instrumental examination in the form of US diagnostics, to avoid intraoperative complications. New concepts and approaches to dental care, comprehensive diagnosis of dental diseases are annually presented at scientific congresses and conferences of various levels. However, a number of traditional methods and approaches to comprehensive diagnosis of the dentoalveolar system remain relevant to this day. The presented article is devoted to the assessment of the relevance and practical application of the occlusiography technique in elderly patients with dentition defects of various extensions. AIM: Determination of occlusiography in elderly patients with various types of dentition defects, and its analysis. MATERIALS AND METHODS: This study is based on the determination and analysis of occlusion-graphy in elderly patients. A total of 150 patients were included in the study, who were divided into three groups. Group 1 — patients with terminal defects (unilateral and bilateral). Group 2 — patients with included defects (unilateral and bilateral). Group 3 — patients with combined defects (terminal and included). There were 50 patients in each group. Computer occlusiography and traditional occlusiography were performed. A proprietary method was used – a program of photographic morphometric analysis of control-diagnostic models. RESULTS: In group 1, high dependence of the results of the proprietary method of digital analysis of occlusiogram on the quality of the photo image was revealed. Due to the absence of a chewing group of teeth, the area of occlusal contact of incisors and canines was considered insignificant. The obtained results revealed a high chewing load on the frontal group of teeth with the absence of chewing teeth. Most of the occlusal contacts led to a complete loss of the wax layer on the occlusiogram. In the 2nd study group, with the presence of teeth limiting the defect, the tightness of occlusal contacts permitted to obtain a true occlusiogram. As a result, 75% of respondents had a small number of supracontacts, 93% of the subjects had supracontacts within the teeth limiting the defect of the dentition. With this, the preserved teeth helped obtain a high-quality imprint of the occlusal relief on an analog wax plate, which permitted to obtain the maximum similarity of the results of the proprietary method and of T-scan computer system (90%). In group 3, the wax template was additionally reinforced with occlusive wax in the area of the terminal defect of the dentition. In 85% of patients, supracontacts were located in the area of dentition defects, significant disorders of the position of the teeth limiting the terminal defect were noted. The reason for this may be vertical loads during chewing. In this group, the average deviation of the results of analog occlusiography from those of the digital one was 87.5%. Laboratory diagnostics, being a unified method for determining the risks of complications and relapses, indicates the need for a comparative analysis of standard laboratory parameters in patients with carbohydrate metabolism disorders (CMD) before revision arthroplasty. AIM: To evaluate changes in the biochemical parameters of blood serum of patients with impaired carbohydrate metabolism before revision arthroplasty. MATERIALS AND METHODS: The data of anamnesis, morphometry and main blood parameters of patients who underwent treatment for osteoarticular pathology and purulent osteology in inpatient facility from 2011 to 2019 were processed and analyzed. The first group included 115 patients (33% of the total amount of examined patients) with revision hip arthroplasty having no disorders in carbohydrate metabolism in history. The second group included 103 patients (30%) who underwent revision hip arthroplasty and had diabetes mellitus, impaired glucose tolerance, and early forms of carbohydrate metabolism disorders as comorbidities. The third group included 73 patients (21%) with periprosthetic infection of the hip joint without history of carbohydrate metabolism disorders. The fourth group included 56 patients (16%) with periprosthetic infection, with disorders of carbohydrate metabolism. RESULTS: The results obtained in the study groups were intercompared and compared with the reference intervals established by the reagent manufacturers for each study method. In the studies, no statistically significant differences were found in the biochemical parameters of blood of patients with aseptic revision arthroplasty, depending on the existing disorders of carbohydrate metabolism. When comparing biochemical parameters of blood of patients with periprosthetic infection, statistically significant differences depending on the presence of CMD, were found in three parameters. Patients with disorders of carbohydrate metabolism had higher blood levels of glucose, creatinine and urea, which, however, remained within the normal range. Two biochemical parameters of the blood of patients of all four groups were beyond the standard limits — C-reactive protein and albumin-globulin ratio. In patients with endoprosthesis instability, bilirubin, creatinine, and albumin values changed depending on the presence of an infectious process in the joint, and creatinine and urea, depending on carbohydrate metabolism disorders. CONCLUSIONS: Depending on the presence of an infectious process or disorders of carbohydrate metabolism, these parameters require closer attention in the preoperative period in patients with endoprosthesis instability. It is known that, despite the published government statistics and information about the new infection, not all the Russian population observe the measures of non-specific prevention of coronavirus infection. One of the reasons is the massive spread of numerous rumors and speculations about COVID-19. AIM: To study the level of awareness of medical workers and medical students of the coronavirus infection COVID-19 specifying the sources of information about this disease. MATERIALS AND METHODS: A survey of 312 respondents from the Ryazan region and the Republic of Bashkortostan was conducted. The study was conducted using a questionnaire based on GoogleForms distributed in social networks and consisting of three blocks of questions. Statistical data processing was carried out using the free computing software environment R (ver. 4.1.2) and the MS Excel 2019 software package in Windows 11. The target significance level p ≤ 0.05. RESULTS: The survey revealed insufficient awareness of students and medical professionals of the transmission routes and symptoms of COVID-19 infection. Only a small part of medical workers (as a rule, people of younger working age) and medical students receive information about COVID-19 from official and reliable sources. Only every 7th respondent gives correct answers about the transmission routes of COVID-19. In general, respondents considered wearing masks to be an effective means of prevention, especially in the group of those who had suffered the disease. When analyzing the spread of various rumors and legends about COVID-19 among medical professionals and medical students, it was found that almost half of them believe various legends about COVID-19. CONCLUSION: It was found that respondents received information mainly from unofficial sources, not trusting federal TV channels, as well as the data from the state statistics service. Often, respondents do not have complete information about the ways of transmission of infection and of its symptoms, but at the same time, most of them observe the self-isolation and the mask regimes. About half of the respondents believe various legends associated with this disease. Approximately one third of patients per year suffering from this disease have indications for a surgical treatment — colectomy with ileostomy. Subsequently, not all patients appear suitable for reconstructive surgery, for which reason they are doomed to a life-long ileostomy. These facts stimulate us to search for new methods of treatment of the rectal stump remaining after colectomy. In this study, 10 patients having been operated for ulcerative colitis and having rectal proctitis, underwent a single session of photodynamic therapy. The effectiveness of photodynamic therapy was controlled before the session and one month after it using rectoscopy with rectal mucosa biopsy. Endoscopy results were evaluated on Shroeder and UCEIS scales. The results of histological examination were evaluated on Geboes scale. Maximum accumulation of Photoditazin in rectal stump affected with ulcerative proctitis, was determined after 1.5–2 hours. Upon achievement of this maximum, fluorescence of the rectal mucosa 10.8 times exceeded the initial level, while in the intact tissues, fluorescence increases only 2.8 times. After a session of photodynamic therapy, the level of fluorescence of the rectal mucosa reduced 3.11 times, which indicates a photochemical reaction (positive photobleaching) in the tissue. In evaluation of effectiveness of photodynamic therapy after a month, 9 patients of 10 showed improvement of endoscopic and histologic picture. In 2 patients, remission was achieved, in 7, the activity of ulcerative proctitis declined from moderate to mild level. In one patient, no changes were revealed on evaluated scales. In 90% of patients, a single session of photodynamic therapy showed a positive effect, but taking into account a small sample size and the absence of control group, further data collection with subsequent statistical analysis is planned. Use of particular methods is substantiated depending on the size of the hernial orifice, the presence of hernia strangulation, diastasis of the abdominal rectus muscles, comorbidity, available technical equipment. Statistical data of complications, recurrences and of their prevention available in the literature are evaluated. The search for information was performed in electronic libraries: Pubmed, Cyberleninka. The inclusion time interval is 10 years. The keywords used in the search: umbilical hernia, IPOM, eTEP, (E)MILOS, SCOLA, tension method of hernioplasty, complications. The most important modern methods using endovideosurgical access are: laparoscopic IPOM(+), eTEP, (E)MILOS, SCOLA. Use of tension plasty can be justified in case of strangulation, but, here, recurrences are more common. The IPOM method has a number of disadvantages (adhesions, chronic pain, chronic seroma, recurrences), which make its use controversial and needing additional research. The eTEP-TAR, eTEP-RS methods are used in pronounced diastasis of the rectus muscles. The eTEP-RS method is convenient in simultaneous surgery of inguinal hernias. The eTEP-TAR method is used in case of large diameter of hernial orifice (5–12 cm), and suggested width of the retromuscular space less than 5 cm. The MILOS, EMILOS methods are used in case of primary, postoperative umbilical and epigastric hernias. With this, the frequency of complications is less relative to Sublay, IPOM. CONCLUSION: Each method of umbilical hernioplasty is based on certain factors taken into: diastasis of rectus muscles, size of hernial orifice, existence of strangulation, availability of equipment and materials, comorbid pathologies. The variety of manipulations determines personal approach in surgery of umbilical hernias. The modern tendencies in this field are minimization of injury rate in surgical approach in use of endovideosurgical access and attempts to perform simultaneous recovery of the muscular aponeurotic apparatus of the abdominal wall through elimination of diastasis of rectus muscle. Evdokimova","doi":"10.23888/hmj202311159-68","DOIUrl":"https://doi.org/10.23888/hmj202311159-68","url":null,"abstract":"INTRODUCTION: Cognitive disorders often do not come to the fore in the interpretation of clinical manifestations in hemodynamically significant stenoses of brachiocephalic arteries. In this regard, the objectification of cognitive disorders based on a battery of tests, as well as of their neurophysiological correlates, is an important task of both angiology and neurology. Endogenous evoked potential P300 is a frequently used neurophysiological phenomenon characterizing the psychophysiological functions of perception, attention, and thinking. AIM: To study the dynamics of cognitive functions and their neurophysiological correlates in patients with hemodynamically significant carotid artery stenosis before and after carotid endarterectomy. MATERIALS AND METHODS: An open prospective study included 60 patients divided to two groups. Group A included 30 ‘symptomatic’ patients with a hemodynamically significant lesion (stenosis) of the internal carotid artery and a history of acute cerebrovascular accident. Group B included 30 ‘asymptomatic patients’ without a history of cerebrovascular events. Within the study, before surgery for carotid stenosis and 6 months after, the following parameters were assessed: cognitive status (according to the MMSE, FAB, MoCA, NIHSS scales), and also clinical and neurophysiological correlates of cognitive functions (P300 evoked potential). RESULTS: In the group of patients with neurological disorders (group A), a reliably greater number of errors (p = 0.048), as well as a decrease in the amplitude (p = 0.006) and increase in latency (p = 0.022) of P2 component of P300 cognitive evoked potential were determined. Based on the parameters of P300 event-related potential, a logit regression model was created that permitted to classify patients to the above-mentioned groups. The cluster analysis of dynamics of cognitive functions was additionally performed before and after surgery. In cluster 1, there were 45% of symptomatic patients, 55% of asymptomatic ones, in cluster 2 — 58% and 32%, respectively, no reliable differences were found (chi-square 0.58; p = 0.445). Cluster 1 was characterized by initially higher level of test performance with some increase in this parameter after surgery; Cluster 2 was characterized by a lower level of these parameters, with the greatest differences found in frontal assessment battery (FAB) characteristics. Reliable differences in the level of P300 were found only after surgery: the greatest differences were recorded in the amplitude of P2, P3, N2 components. CONCLUSION: The factor of cognitive impairment is a separate phenomenon, not closely related to focal neurological syndrome in patients with carotid atherosclerosis. An objective neurophysiological criterion for cognitive impairment in these patients is the endogenous evoked potential P300. Smirnova","doi":"10.23888/hmj202311115-30","DOIUrl":"https://doi.org/10.23888/hmj202311115-30","url":null,"abstract":"BACKGROUND: Medical workers are one of the groups at high risk of contracting a new coronavirus infection (COVID-19), which requires in-depth study of this professional group. AIM: To determine the main directions for the rehabilitation of medical workers with past COVID–19 based on the study of residual clinical manifestations in the early and late recovery periods. MATERIALS AND METHODS: The study was conducted based on the materials of an online survey of employees of medical organizations in three consecutive stages. At the first and second stages, residual clinical manifestations data of the coronavirus infection in the early (up to 6 months) and late recovery periods (6-15 months) were analyzed. At the third stage of the study, the characteristics of affective disorders of COVID-19 convalescents in comparison with employees intact for COVID-19, were given. RESULT: During the first 6 months after the disease, asthenovegetative manifestations persisted in the form of general weakness and increased fatigue, unproductive cough, shortness of breath, anosmia, disorders of a number of cognitive functions. In the late recovery period (from 6 to 15 months), the majority of respondents noted increased fatigue, impaired memory and concentration. Some of the study participants had a change in the structure of their hair and its loss (alopecia). Some of the employees were first diagnosed with arterial hypertension in the long-term after the disease. It was found that the frequency and severity of affective disorders and asthenic manifestations in COVID-19 convalescents were significantly higher than in the group of employees intact for the disease, which is a direct indication for inclusion of the diagnosis of affective disorders and the provision of specialized psychological assistance in the rehabilitation programs of COVID-19 patients. The effectiveness of metoprolol has been proven in numerous clinical studies in the treatment of arterial hypertension, stable angina, myocardial infarction, chronic heart failure. To improve the efficiency and safety of the therapy, it is advisable to carry out therapeutic drug monitoring of metoprolol, which requires a sensitive method for its quantitative determination. AIM: To develop, validate and test a method for the determination of metoprolol in human plasma using high performance liquid chromatography (HPLC) with tandem mass spectrometric detection (MS/MS). MATERIALS AND METHODS: The work was performed on Ultimate 3000 TSQ Fortis HPLC-MS/MS (Thermo Fisher, USA). For sample preparation, acetonitrile was used with fexofenadine hydrochloride at a concentration of 10 ng/ml as an internal standard, which was added to plasma samples in 3:1 ratio. The volume of the injected sample was 5 μl. Separation was performed on UCT Selectra C18 4.6 mm × 100 mm, 3 um, 100 A column with a similar pre-column at 35°C, in gradient elution mode in proportion of 0.1% formic acid solution/acetonitrile: 0 min — 80%/20%, 0.1 min — 45%/55%, 5 min — 10%/90%, 10 min — 80%/20%, at flow rate of 300 µl/min. Detection was performed in positive electrospray ionization mode, electrospray voltage 4000 V, sheath gas 50 arb, auxiliary gas 10 arb, purge gas 1 arb, evaporator temperature 350°C, ion transport tube temperature 300°C, using the multiple reaction monitoring mode (MRM) at argon flow rate 2 mTorr, 268 m/z → 115.5 m/z, Collision Energy 18 V, Tube lens 95 V, 268 m/z → 191 m/z, Collision Energy 17 V, Tube lens 95 V. The blood plasma of healthy volunteers served as matrix. RESULTS: The analytical range of the technique was 2–1000 ng/ml. The developed technique was tested on a patient with arterial hypertension. During the analysis, an equilibrium concentration of metoprolol of 12.0 ng/ml was detected in the patient's blood plasma (previously, the patient took metoprolol tartrate at a dose of 12.5 mg twice a day for a week), and in 2 hours after taking the drug — 31.0 ng/ml. AIM: To perform a comparative assessment of the parameters of physical development of children studying in different types of educational organizations. MATERIALS AND METHODS: The article presents the results of studies of the physical development of children and adolescents studying in the Cossack cadet corps and in a secondary school of Astrakhan. The study involved children and adolescents of male sex aged 10–17 years. As the main parameters, standard anthropometric and physiometric parameters determined by standard methods, were used. A peculiarity of studying in the Cossack corps is a round-the-clock stay in the school territory, a strict observance of work and rest schedule, regular physical activity and rational diet appropriate to the age. RESULTS: The data obtained indicate a higher physical development of cadets in comparison with schoolchildren. However, the anthropometric parameters of the school pupils corresponded to the average age-related values. Among the cadets of the Cossack corps, here were no children with overweight, and a lower amount of children with deficit of body mass in comparison with schoolchildren. Parameters of lung capacity and dynamometry were reliably higher among cadets, which indicates their better physical fitness. CONCLUSION: Evaluation of physical development of children studying in different conditions shows a positive influence of the conditions of the cadet corps on the harmonious development of children and on their physical fitness.
