Psychological and Behavioral Features of Patients with Pharmacoresistant Epilepsy 耐药癫痫患者的心理和行为特征

Burshinov","doi":"10.23888/hmj2022104381-390","DOIUrl":"https://doi.org/10.23888/hmj2022104381-390","url":null,"abstract":"INTRODUCTION: Pharmacoresistant epilepsy (PRE) is diagnosed in 30% of all patients with epilepsy, causing disorders in psychological and social well-being, as well as the development of intrapersonal conflicts and psychological defense mechanisms. The psychotraumatic role of the epilepsy diagnosis is deepened by restrictions in the daily and professional activities of patients. AIM: To analyze the psychological and behavioral characteristics of patients with PRE compared with patients with successfully controlled seizures. MATERIALS AND METHODS: We studied 60 patients diagnosed with epilepsy aged 20 to 75 years, who were divided to 2 groups: 30 patients with pharmacoresistant epilepsy and 30 patients with successfully controlled seizures. The following psychometric methods were used in the study: Eysenck's test; Strelau test; Freiburg Multivariate Personality Inventory (FPI); Toronto alexithymic scale; T. Bant's scale of manipulative attitude; test ‘Tendency to self-destruction’; test of intrapersonal conflicts by V. Lauterbach; questionnaire ‘Types of behavior in conflict’ by K. Thomas – R. Kilman. Statistical differences between the groups were assessed using non-parametric statistics; differences were considered significant at p < 0.05. RESULTS: The patients with PRE showed a higher level of anxiety and alexithymia, a lower level of spontaneous and reactive aggression and tranquility compared to the patients of the control group. Patients with PRE show a higher tendency to manipulative behavior and a lower tendency to self-destructive behavior than patients with successful seizure control. The spectrum of intrapersonal conflicts in patients with PRE is represented by an imbalance between the needs for dominance, achievements, independence and passive avoidance, submission, unrealized opportunities. In interpersonal conflicts, these patients are prone to avoidant behavior, while patients in the control group demonstrate the flexibility of behavior using all possible behavioral strategies. In general, patients with PRE have psychological and behavioral characteristics typical of patients with epilepsy, but expressed to a greater extent. However, in patients with PRE, we found a lower level of aggression and a lower tendency to self-destructive behavior than in patients with successfully controlled seizures, which can be explained by the compensating impact of persistent seizures on high aggressiveness, as well as a behavioral strategy that manifests itself in low compliance with a simultaneous tendency to manipulative behavior when interacting with a doctor. CONCLUSION: The conducted study revealed psychological and behavioral features of patients with PRE, causing significant psychological and social maladaptation requiring correction by medical professionals and psychologists. Dmitriyeva","doi":"10.23888/hmj2022104437-446","DOIUrl":"https://doi.org/10.23888/hmj2022104437-446","url":null,"abstract":"BACKGROUND: Preservation of pulp viability allows to preserve the integrity of tooth structures, the correct relationship with other elements of the dental system, therefore, the improvement of materials aimed at preventing endodontic treatment with preservation of the entire pulp is of great clinical importance. AIM: Comparison of clinical effectiveness of various materials in the vital therapy of permanent tooth pulp. MATERIALS AND METHODS: The study involved 30 patients (20 male and 10 female patients aged 11 to 25 years) divided into 3 groups. Vital pulp therapy of permanent molars and premolars with formed and unformed roots diagnosed as reversible pulpitis was carried out by direct and indirect coating with calcium hydroxide in group 1 (9 patients/18 teeth) (Calasept, with Dycal-isolated surface), with ProRootMTA in group 2 (10 patients/15 teeth) and with Biodentine in group 3 (11 patients/15 teeth). IRM (methyl methacrylate-reinforced zink-oxide-eugenol cement) was used as temporary filling. The research data were processed using nonparametric statistics. RESULTS: After 4 years of follow-up: After the first stage of observation, not in a single of 14 remaining teeth in group 1 (4 teeth were subjected to endodontic treatment), clinical and radiological signs of pulp pathology were found; in 3 teeth (21.4%), pronounced narrowing of the pulp chamber and obliteration of the pulp of canal orifice were noted, which did not require endodontic intervention. In group 2, of 13 teeth (2 teeth previously received endodontic treatment), 1 tooth (7.6%) was diagnosed with irreversible symptomatic pulpitis and 2 teeth (16.6%) — with pronounced narrowing of the pulp chamber, obliteration of the root pulp, 3 teeth (25%) — with denticles in the pulp chamber. In group 3, 2 teeth (13.3% of cases) were subjected to endodontic treatment for irreversible symptomatic pulpitis in 1.5 year after the beginning of the observation, and in 1 tooth (6.6%), the formation of multiple denticles in the pulp chamber was noted. CONCLUSION: Thus, the clinical and radiological effectiveness of use of calcium hydroxide in vital pulp therapy of permanent teeth after 12 months was 77.7% (14 of 18 teeth), of ProRootMTA — 86.6% (13 of 15 teeth) and of Biodentine — 100%, which absolutely correlates with the data of modern literature sources. The method of osteosynthesis with titanium elastic nails is widely used at the age of 7 to 14 years and has many advantages. At the same time, the elastic-stable osteosynthesis has a number of probable complications and limitations. AIM: To analyze probable complications of the method of intramedullary osteosynthesis with titanium elastic nails in children of different age categories. MATERIALS AND METHODS: The results of treatment of 98 children with fractures of the long bones of the lower extremities were analyzed. All the patients underwent osteosynthesis with titanium elastic nails. The patients were divided to three age groups: 2 to 6 years, 7 to 13 and 14 to 16 years. Clinical and X-ray examinations of the patients were conducted. RESULTS: In young children, excellent results of treatment were obtained in 82.6% and satisfactory results in 17.4% of cases, no poor results were noted. In patients of the group 7 to 13 years, the results of treatment were considered excellent in 82.3%, and satisfactory in 17.7% of cases. In older children, outcomes of treatment were excellent in 62.5%, satisfactory in 29.2%, and poor outcomes that required repeated surgical interventions, were observed in 8.3% of cases. CONCLUSION: The most preferable age for osteosynthesis with titanium elastic nails was found to be from 6 to 14 years. There exist technical difficulties in use of this method in children of 2 to 4 years of age with \"unstable in length\" femoral bone fractures, nevertheless, the displacement of bone fragments was leveled out in all the observed cases. Levit","doi":"10.23888/hmj2022104413-424","DOIUrl":"https://doi.org/10.23888/hmj2022104413-424","url":null,"abstract":"INTRODUCTION: Intraprocedural analgosedation in planned percutaneous coronary interventions is a priority method of anesthesia, since it provides comfort for patient in comparison with when it is absent; it leads to a fast recovery of patients and reduction of the incidence of complications in comparison with the general anesthesia. The question of the optimal sedation method is still open. AIM: To carry out a comparative analysis of sedation with Phenazepam® and Dexmedetomidine in planned percutaneous coronary interventions in patients with stable coronary heart disease. MATERIALS AND METHODS: A prospective randomized study was conducted from September 2021 to January 2022. The study included patients with stable ischemic heart disease (serum troponin T level less than 0.32 ng/ml; absence of anginous pain at the beginning of X-ray endovascular intervention), who had planned percutaneous coronary intervention. The Phenazepam® sedation group included 40 patients, including 30.0% (n = 12) women and 70.0% (n = 28) men. 40 patients were included in the Dexmedetomidine sedation group: 30.0% (n = 12) women, 70.0% (n = 28) men. RESULTS: There was a statistically significant (p < 0.001) reduction of blood pressure and heart rate in patients in the group of sedation with Dexmedetomidine at the stage of stenting of the coronary artery, compared with analogous parameters in the group of sedation with Phenazepam®. Patients in the group of sedation with Dexmedetomidine developed chest pain at the stage of coronary artery stenting reliably 3.13 times less often (p = 0.024) than patients of the group of sedation with Phenazepam® (OR: 0.32; 95% CI: 0.13–0.80). With use of Dexmedetomidine, relief of anginal pain with narcotic analgesics was required 20 times less often (p < 0.001) than with Phenazepam® (OR: 0.05; 95% CI: 0.01–0.38). One can confidently speak about the superiority and safety of analgosedation with Dexmedetomidine in planned percutaneous coronary interventions over sedation with Phenazepam®. CONCLUSIONS: Dexmedetomidine provides deeper sedation and sufficient pain relief in patients in planned percutaneous coronary interventions than Phenazepam®. Uporov","doi":"10.23888/hmj2022104471-490","DOIUrl":"https://doi.org/10.23888/hmj2022104471-490","url":null,"abstract":"INTRODUCTION: The article is devoted to such an urgent problem as the choice of the most effective and safe method of treatment of thrombophlebitis of the saphenous veins of the lower extremities. This pathology is one of the most common in vascular surgery. Left untreated, saphenous vein thrombophlebitis can lead to a life-threatening complication such as pulmonary embolism. The main tasks that a vascular surgeon faces during treatment are to prevent the progression of the disease, relieve symptoms and reduce the likelihood of recurrence of thrombophlebitis of superficial veins. Approaches to the management of patients with this pathology have been changing over a long period of time — from active surgical tactics to conservative therapy and large studies on the effectiveness and safety of drugs. Due to development of modern methods of treating varicose veins, it has become possible to use endovascular methods of surgical intervention for saphenous veins thrombophlebitis of the lower extremities. The article presents the review of the largest and most significant studies devoted to the results of treatment of patients with saphenous vein thrombophlebitis of the lower extremities. CONCLUSION: Despite numerous studies and opinions of reputable experts in the field of vascular surgery, some issues of choosing the most safe and effective treatment method remain debatable at the present time. Thus, for example, it is not clear what is more effective — active surgical tactics with the existing indications or 45-day conservative therapy under continuous US control. There is no consensus in the opinions on the terms of phlebectomy for prevention of recurrence of venous thromboembolic events. The most promising methods of treatment with the active use of physical factors affecting the purulent focus are in the conditions of modern increasing resistance of microorganisms. AIM: To determine the effect of local ultra-low temperatures on the inflammatory and reparative processes in an experimental septic wound of soft tissues for planning a subsequent clinical study of the effectiveness of debridement using cryotherapy. MATERIALS AND METHODS: The experiment was conducted on 40 Wistar–Kyoto male rats of 9-month age, weighing 200 to 230 grams. The wound was created by excising an oval skin flap 2.0 × 1.5 cm in the withers region, followed by injection of 1.5 ml of Staphylococcus aureus solution at a concentration of 109 CFU/1 ml into the hypodermis. The animals were divided to 4 groups: a control group with the natural course of the healing process, and the main groups, which included a group with single daily treatment of the wound with Levomekol ointment, a group with a single daily treatment of the wound with vapor-drop jet of liquid nitrogen, and a group with single daily treatment with a combination of the above methods. The treatment was carried out during the exudative phase of the wound process. With the disappearance of exudation and of paravulnar edema and hyperemia, debridement in the main groups was stopped. In the dynamics, a comparative observation and study of planimetric changes in the wound were carried out. RESULTS: A more rapid decline of the severity of hyperemia, paravulnar edema and exudation, accelerated filling the wound with granulation tissue, accelerated epithelialization were seen in treatment with cryotherapy compared with treatment with Levomekol. These effects led to increase in the rate of wound healing due to the shortening of all phases of the wound process by at least 2 days. There were no statistically significant side effects, healing and survival occurred in all animals. CONCLUSION: In was found in the study that the use of cryotherapy leads to a rapid subsidence of symptoms, activates the processes of granulation and epithelization. Mutual potentiation of the positive effects of debridement using ultra-low temperatures and antibacterial agents has been established. Petrov","doi":"10.23888/hmj2022104447-458","DOIUrl":"https://doi.org/10.23888/hmj2022104447-458","url":null,"abstract":"INTRODUCTION: Bezoars are foreign bodies formed in the digestive tract as a result of ingestion of some indigestible substances: plant fibers, hair and animal dander, chemical compounds. They are mainly found in animals. But although this is an extremely rare pathology of humans, it can be complicated with such threatening conditions as acute intestinal obstruction, decubitus ulcers of the digestive tract, gastrointestinal bleeding and perforation with the development of peritonitis. Bezoars are the cause of acute intestinal obstruction in only 2–4% of cases, but mortality, according to the literature, can reach 30%. Difficulties in the clinical diagnosis of bezoar disease lie in the absence of specific symptoms: it manifests itself as an abdominal syndrome, like a rather wide range of various diseases. Therefore, the importance of informing physicians of various profiles both about the factors provoking the formation of bezoars and about the methods of their diagnosis and treatment is undeniable. The article presents a case report of an 86-year-old female patient with acute intestinal obstruction caused by diospyrobezoar. CONCLUSION: The clinical interest of the presented case lies in its highly illustrative character. One of factors of the unfavorable course of chronic forms of ILD is progressing fibrosis of the lung tissue. While idiopathic pulmonary fibrosis is a classic fibrosing ILD with the poorest prognosis, diffuse pulmonary fibrosis can also occur in other ILDs. The progression of the disease leads to restrictive alterations of breathing and to impairment of the diffusion capacity of lungs, which results in respiratory failure, reduced tolerance to physical exercise and disability of the patient. This impedes routing of such patients and limits access to specialized medical care. Restrictive measures introduced in 2020–2021 in connection with a new coronavirus infection, and for elderly people with chronic diseases as well, produced an additional negative impact on the possibility of patients to receive qualified medical care of specialists. In these conditions, the relevance of standard telemedicine healthcare services increased, and there appeared an urgent need for developments of a digital nursing system for chronic patients providing a possibility of active remote communication of a doctor and a patient. This review presents the experience in use of a digital nursing program for monitoring and self-monitoring of chronic non-infectious patients, including those with respiratory diseases. The article presents a clinical case of joint management of a comorbid patient with a severe damage to pulmonary interstitium and the underlying rheumatoid arthritis by pulmonologists and rheumatologists through telenursing. Positive aspects of the remote monitoring are discussed. However, the development of restenosis of the reconstruction zone often leads to a need for repeated operations. AIM: To evaluate the effectiveness of combined antioxidant therapy in reducing the risk of restenosis after endovascular interventions. MATERIALS AND METHODS: The study included 84 patients with IIB–III stage peripheral arterial disease (PAD). Group A included 45 patients who underwent endovascular interventions, group B included 39 patients who were prescribed combined antioxidant therapy (Vitamin E, Actovegin®) a week before endovascular intervention and within a week after. On admission to hospital, in the first hours of stay, in 1, 7 days after surgery, the amount of Bax and platelet growth factor (PDGF BB) biomarkers were determined in the blood serum of all the patients. On control visits, the patients underwent duplex scanning of the arteries of the lower extremities to measure the thickness of the neointima and to detect restenosis of the intervention area. RESULTS: When comparing Bax values in the first hours and days after surgery in groups A and B, its growth was noted in patients of the first group (p < 0.01). At the same time, in patients of group A, the increase in PDGF BB was more pronounced (p < 0.01) on the 7th day. In patients of group A with restenosis of the reconstruction zone, the values of Bax marker in the first hours after surgery exceeded those in patients of group B (р < 0.01). On the 7th day, the amount of PDGF BB in these patients of group A was increased relative to group B (р < 0.01). The changes in the number and dynamics of the studied markers in group B patients with restenosis of the reconstruction zone led, firstly, to decrease in the percentage of stenosis in the area of the previous surgical intervention, secondly, to decrease in the severity of the clinical manifestation of this complication, thirdly, to occurrence of restenosis of the reconstruction zone in the later period, and, fourthly, to decrease in the percentage of patients with this complication. Dmitriyeva","doi":"10.23888/hmj2022103277-288","DOIUrl":"https://doi.org/10.23888/hmj2022103277-288","url":null,"abstract":"INTRODUCTION: Hyaluronic acid (HA) is a natural glycosaminoglycan which is an integral component of both the matrix of articular cartilage and synovial fluid with its unique rheological properties that are lost in gonarthrosis, which leads to a decrease of viscoelastic characteristics of the synovia and destruction of the cartilage. AIM: To evaluate the effectiveness and safety of the use of Hyapro sterile biopolymer synovial fluid prosthesis in a disposable syringe for intra-articular injection at a concentration of 1.5% — 2 ml in treatment of gonarthrosis. MATERIALS AND METHODS: From September 2020 to April 2022 at settings of the orthopedic department of Ryazan Regional Clinical Hospital, an open prospective non-comparative study of the effectiveness and safety of Hyapro sterile biopolymer synovial fluid prosthesis at a concentration of 1.5% — 2 ml in 30 patients with 2 to 3 stage gonarthrosis according to Kellgren–Lawrence scale was carried out. Prior to the treatment, and also 3 and 6 months after, the severity of clinical symptoms on VAS, WOMAC, Leken scales, the quality of life according to SF-36 questionnaire were registered; the biochemical markers namely cartilaginous oligomatrix peptide (COMP), metal matrix proteinase 3 (MMR 3) were determined with the use of StatFax enzyme-linked plate analyzer (Awareness Technology, USA). RESULTS: In general, the studied drug was well tolerated by patients: there were no typical for intra-articular injections cases of clinically important adverse reactions like prolonged pain at the injection site, expansion-like sensation, an elevation of local temperature, and changes in the color of skin or changes of the shape of the knee joint. Just transient local tenderness in 6.7% of injection cases was noted. We revealed a statistically significant decrease in serum MMR-3 levels by 6 months of follow-up, which reflects a reduction of the inflammatory component in osteoarthritis of the joint studied. Besides, a gradual increase in COMP levels by the 6th month of the study was found, which may be an evidence of activation of the protein after the course of treatment to replenish the cartilage matrix. The results on VAS, WOMAC scales, and Leken algofunctional index demonstrated similar positive dynamics: a statistically reliable decrease in such parameters as pain, stiffness, and impaired functioning by the 3rd month of follow-up compared with the initial level; this trend was remaining in 6 months. After 3 months of treatment, there was a reliable improvement of the quality of life on the subscale of the physical component of health compared with the initial level, which persisted until the end of the study. On the subscale of the psychological component of health, no changes were found during the period.
