One of these factors is cardiac autonomic neuropathy. The possibility of assessing the effect of cardiac autonomic neuropathy on the risk of development of atrial fibrillation after myocardial infarction may help in choosing the management tactics for patients with myocardial infarction and type 2 diabetes mellitus. AIM: To evaluate the effect of cardiac autonomic neuropathy on the risk of development of atrial fibrillation in patients with past myocardial infarction. To develop a method for assessing the risk of atrial fibrillation in 12-month period in patients with type 2 diabetes mellitus. MATERIALS AND METHODS: The study involved 60 individuals with type 2 diabetes mellitus and 60 individuals with newly diagnosed type 2 diabetes hospitalized with anterior and anteroseptal myocardial infarction with Q wave. At the initial stage of the study, the formed groups were homogeneous in age, gender, and all general clinical research methods. The diagnosis of type 2 diabetes mellitus was made on the basis of anamnesis data and available medical documentation confirmed by examination of glycosylated hemoglobin and glycemic profile. The newly found diabetes mellitus was diagnosed based on the data of glycosylated hemoglobin > 6.5%, blood glucose > 11.1 mmol/l recorded two or more times, according to national clinical guidelines. On the 3rd day, in 3 and 12 months after hospitalization, a set of tests for detecting cardiac autonomic neuropathy was performed; the presence of atrial fibrillation, the dynamics of left ventricular hypertrophy were assessed by electrocardiography and echocardiography. Statistical processing of the obtained results was performed using Microsoft Office Excel 2019, SPSS Statistics Standart ver. 28 programs for Windows. In order to choose the method of analysis, the distribution was estimated using Kolmogorov–Smirnov test. Comparison of the groups was carried out using Mann–Whitney test. The risk of atrial fibrillation was assessed using logistic, multifactorial and ROC analysis. RESULTS: Prognostic models were formed to determine the risk of development of atrial fibrillation in patients with type 2 diabetes mellitus. The model for diabetes mellitus had determination coefficient (R2 = 0.7), Hosmer–Lemeshow criterion 0.83, and the area under the curve 0.93. The model for newly diagnosed diabetes mellitus had R2 coefficient 0.72, Hosmer–Lemeshow goodness-of-fit test 0.85, and the area under the curve 0.95. Prosvirin","doi":"10.23888/hmj2022103299-310","DOIUrl":"https://doi.org/10.23888/hmj2022103299-310","url":null,"abstract":"INTRODUCTION: Treatment of intra-articular fractures of the proximal tibial metaepiphysis can often cause difficulties in accurate restoring of the congruence of the articular surfaces of the knee joint. Such fractures may be accompanied by subsidence of the articular surface and the formation of a subchondral defect. In the absence of displacement of bone fragments and preservation of congruence of the articular surfaces of the knee joint, such fractures can be treated conservatively. In the opposite case, there is a need for a complete restoration of the articular surface of the tibia. It should be noted that among all fractures of the tibial plateau, more than half are fractures of the lateral condyle, also accompanied by the presence of an impression. A clinical case of surgical treatment of a 62-year-old patient, BMI (body mass index) 39, with an intra-articular multi-comminuted impression fracture of the lateral condyle of the tibia with displacement of bone fragments is presented. The patient gave her written consent to the use of her clinical data for scientific purposes. Upon admission to hospital, a closed intra-articular fracture of the external condyle (Schatzker II) was diagnosed with fragmentation and articular surface impression of more than 5 mm, and the presence of a spongy bone defect with a volume of about 4 cm3 as a result of the impression. Surgical treatment was performed that consisted of open reposition and restoration of the congruence of the articular surface, with simultaneous plasty of the defect of the external condyle with an autograft taken from the iliac crest, followed by osteosynthesis of the external condyle with a plate and screws. On the follow up examination 9 months after the surgical treatment, the patient developed post-traumatic osteoarthrosis of the knee joint with severe pain. Some probable errors in the preparation of the patient for surgical treatment, preoperative planning, surgical treatment, recovery in the postoperative period, as well as functional and radiological results of treatment are presented for discussion. CONCLUSION: The development of post-traumatic osteoarthrosis in the treatment of tibial condylar fractures still remains an important problem in modern traumatology. Special attention in the treatment of such fractures should be given to preoperative planning, surgical technique and implant selection. In patients with HS, morphologically transformed erythrocytes — microspherocytes — appear in blood in different quantities. Erythrocyte lysis rates in patients with HS are significantly higher than in individuals with no hematologic diseases in history, however, there are no studies demonstrating the relationship between the amount of microspherocytes and erythrocyte lysis rates. AIM: Was to establish correlation between erythrocyte lysis rates and the amount of microspherocytes in patients with HS. MATERIALS AND MATERIALS: Study material was venous blood of 15 patients with HS (the main group) and of 14 “healthy” individuals (control group). Venous blood was taken into test tubes with K3EDTA as an anticoagulant for calculation of morphometric parameters of erythrocytes, and into test tubes with Li-heparin for determination of erythrocyte lysis rates. Morphometric parameters of erythrocytes were calculated using VideoTesT-Morphologia hardware and software complex. For each erythrocyte, average surface area, average diameter, thickness and sphericity index were determined Erythrocyte lysis rate was determined with use of glycerol reagent (glycerol test) on Vitalon-400 semi-automatic biochemical analyzer, and was assessed by reduction of optical density of the incubation medium and expressed as the difference between the initial and final optical densities multiplied by 1 000 (standard units). Statistic data processing was carried out using parametric and non-parametric statistical methods, and Spearman correlation analysis was performed by Spearman method using Sigma-Plot 11.0 software. RESULTS: Erythrocyte lysis in glycerol test occurred faster in patients with HS than in “healthy” persons due to instability of membrane of microspherocytes which make a significant proportion of erythrocytes in peripheral blood of patients with HS. CONCLUSIONS: A proportional dependence was established between the amount of microspherocytes and increase in the rate of erythrocyte lysis, which indicates their instability typical of HS. The most common causes of the acute coronary syndrome in young individuals are anomalies in the development of coronary arteries. The article is devoted to the description of a clinical case of acute myocardial infarction in the anterior septal region of the left ventricle in the perioperative period after surgery of spinal injury in a patient without severe somatic pathology. During the surgery, unstable hemodynamics was noted with the development of arterial hypotension, which required infusion of norepinephrine. On the basis of clinical, laboratory and instrumental examinations, in the postoperative period the diagnosis: “Q-positive myocardial infarction of the anterior septum” was made. Coronary angiography visualized the presence of an intramural passage in the middle segment of the anterior descending artery with up to 30% stenosis in systole. The resulting arterial hypotension at the operational stage, with the myocardial bridge in the anterior descending artery, was the cause of circulatory disorders in the anterior septal region with the development of myocardial infarction and a decrease in the pumping function of the heart with subsequent hemodynamic disorders. CONCLUSION: Myocardial muscle bridges are a common anomaly of coronary arteries, which is usually asymptomatic. Clinical manifestations, if present, are blurred and atypical. Nowadays, a doctor can select the optimal material for each specific clinical case and to achieve the maximal functional and esthetic result. Thus, the most common method of restoration of the tooth crown is direct restoration using composite materials. In the last decade, the demand for conservative restorative dentistry has sharply increased. In turn, esthetic characteristics of composite materials are of key importance both in filling of teeth of the anterior group, and in restoration of the lateral teeth. Discoloration of the dental filling material is a common cause of its replacement. Therefore, to achieve a positive long-term result, it is important to prevent the change of composite color over time. This requires a clear understanding of the causes and conditions of discolorations. AIM: To systematize the scientific data accumulated in recent years concerning the study of the effect of coloring substances of human diet on the aesthetic characteristics of composite materials for direct dental restoration. CONCLUSION: Various pigments present in food and beverages have an effect on composite materials, reducing their microhardness, increasing the rate of wear and changing the color. It depends on the composition of the composite matrix and its chemical structure. Nowadays, the clinical and epidemic features of the disease are the subject of debate and have not been adequately studied. Studies on the effect of coronavirus on the cardiovascular system deserve special attention. They are of significant interest because myocardial damage is the second most frequent cause of death in COVID-19. The damaging effect of the virus on the cardiovascular system is multifactorial and is realized both through the direct effect of the virus on its elements and indirectly. In the long term, COVID-19 patients with myocardial lesions retain an increased risk of cardiovascular events and require monitoring by a cardiologist. CONCLUSION: Development of preventive measures, improvement of diagnosis and treatment tactics of cardiovascular diseases in COVID-19 is the main task of further long-term studies. BCRP plays an important role in the pharmacokinetics of drugs, therefore all new drugs are recommended to be tested for belonging to its substrates, inducers and inhibitors. The influence of substances on the activity of this transport protein in vitro is evaluated by a change of transmembrane transfer of its substrates, one of which being quercetin. This, in turn, requires the development and validation of a method for its quantitative analysis in an appropriate matrix. AIM: To develop a method for the quantitative determination of quercetin using high performance liquid chromatography (HPLC) with tandem mass selective detection (MS/MS) with full validation. MATERIALS AND METHODS: The method was developed on Ultimate 3000 HPLC equipped with TSQ Fortis (Thermo Fisher, USA) MS/MS detector. The conditions of chromatographic analysis were as follows: column UCT Selectra C18 4.6 mm*100 mm 5μm, 100 A, Selectra C18 Guard Cartridges SLC-18 GDC46-5UM. The analysis time was 11 min at separation temperature 35°С, flow rate 0.5 ml/min, injected sample volume 5 μl. Gradient elution regime was used: on 0th minute, the ratio of 0.1% formic acid solution and acetonitrile was 70% and 30%; 0.3 min — 30% and 70%; 4 min — 1% and 99%; 9 min — 70% and 30%. Quercetin retention time was 3.91 minutes. Detection conditions: negative ionization mode, 301 m/z → 150.9 m/z with collision energy 22 V, 301 m/z →178.9 m/z with collision energy 17 V, source fragmentation 5 V, electrospray voltage 3 000 V, CID gas pressure 1 mTorr, Sheath gas 50 Arb, Aux gas 10 Arb, Sweep gas 10 Arb, ion transfer tube temperature 300°C, vaporizer temperature 350°C. The matrix was transport medium after incubation for 3 hours with Caco-2 cells overexpressing BCRP. In this medium, the transport of quercetin through the cell monolayer was further evaluated in vitro. Precipitation of protein and isolation of quercetin from the transport medium was realized by a mixture of water and acetonitrile in a ratio of 1:1. RESULTS: The developed method was validated by the following parameters: selectivity, linearity, accuracy, precision, limit of quantification, sample transfer, matrix effect, sample stability. The analytical range of the technique was 5–500 nmol/l. In this case, the correlation coefficient was more than 0.99. The limit of detection and the lower limit of quantification of quercetin (LLQQ) were 1 and 5 nmol/l, respectively. The calculation of intra- and inter-cycle accuracy and precision showed that these parameters do not exceed 20% for the concentration corresponding to the lower limit of quantitative determination, and 15% for other concentrations. The analyte demonstrated stability in triple freeze-defreeze cycle at -80°C, in storage at -80°C for 60 days, after sample preparation and being kept in the autosampler for 24 hours. The need for donor material in special clinical situations is reflected. The main stages of the historical development of the use of homografts are listed. The experience of foreign and domestic authors in reconstructive interventions on the main vessels and aorta using homografts is demonstrated in the historical perspective of the XXth century. Historical milestones in the development of tissue banks in the world are considered on an example of America, Europe, Africa, Asia and Russia. The legal regulation of use of homografts is at present especially relevant for the Russian Federation. The use of homografts as a material for reconstruction is stated to be within the legal framework of the Russian Federation. A large number of modern studies in the 21st century are carried out with choice of cadaveric homografts, both cryopreserved and of wet storage, as the material for reconstruction of the main arteries. Critical ischemia is a probable indication for the use of cadaveric homographs, but their implantation shows ambiguous results. Infection of a permanent vascular access for hemodialysis can also be considered as an indication for the use of a venous homograph as the material for access reconstruction. AIM: To analyze the state and problems of providing geriatric care to the population of Mongolia. MATERIALS AND METHODS: A content analysis of the scientific literature characterizing the development of the geriatric service in Mongolia in the historical aspect, was carried out. Using a specially designed evaluation card-questionnaire, an expert opinion was obtained from 36 profile specialists (doctors and nurses) from geriatric departments (offices) on the state and problems of providing geriatric care to the population. RESULTS: It was found that 36.1% of respondents are not satisfied with the material and technical base of their medical organization, 33.3% rather no than yes, only 30.6% of respondents gave a positive answer. The shortage of gerontological equipment was most often noted, especially of equipment for the rehabilitation and safety of the elderly and senile. According to 55.6% of geriatric specialists who took part in the survey, it is necessary to rework and expand the existing standards for the provision of geriatric care. The expediency of holding a school for elderly and senile patients on an outpatient basis was reported by 51.6% of respondents, 48.4% of respondents believed that this should be done in a hospital. Geriatric specialists were found not to be actively involved in continuing professional education.
