The purpose of this review is to raise awareness of the increase in the incidence of malignant tumours in adults during 2014, compared with 2013. In addition to the conventional oral examination, it identifies areas with histological changes that are not identified with conventional white light.</p></div><div><h3>Objective</h3><p>To determine whether fluorescence facilitates resection with negative margins in patients diagnosed with squamous cell carcinoma of the tongue.</p></div><div><h3>Material and methods</h3><p>Patients diagnosed with invasive tongue squamous cell carcinoma were evaluated with a conventional oral examination and fluorescence. To determine whether the threshold of injury coincided in both tests, the limits of section were identified and histologically evaluated.</p></div><div><h3>Results</h3><p>The study included 30 patients, 18 women and 12 men; 10 T1, 15 T2, and 5 patients with T3. The neoplastic margin evaluated with conventional light coincided with fluorescence in 17 patients (56%), and in 13 (44%) fluorescence identified a larger tumour. Surgical margins were negative in 27 (90%), and 3 (10%) positives that were all in the tongue thickness and with bulky tumours (T3).</p></div><div><h3>Conclusions</h3><p>Fluorescence identifies larger tumours than those identified with conventional oral examination in 44% of patients, and ensures a longer surgical resection with free surgical margins in 90% of cases. {"title":"Tumores limítrofes de ovario (experiencia con 50 casos)","authors":"Alfonso Torres Lobatón ,&nbsp;Cecilio Alberto Jacobo González ,&nbsp;Fred Morgan Ortiz","doi":"10.1016/j.gamo.2015.12.012","DOIUrl":"https://doi.org/10.1016/j.gamo.2015.12.012","url":null,"abstract":"<div><h3>Introduction</h3><p>Borderline ovarian tumors are tumors with cell proliferation and nuclear atypia without stromal invasion, but with the ability to develop tumor implants, and recurrence probability up to 5 or more years after diagnosis.</p></div><div><h3>Objective</h3><p>To show the clinical and pathological aspects of diagnosis and treatment of these tumors through an institutional review of casuistry.</p></div><div><h3>Methods</h3><p>Retrospective analysis of clinical records from borderline ovarian tumors managed with oncologic criteria in the Department of Oncology at Hospital General de Mexico “Dr. Eduardo Liceaga”, for a period of 5 years.</p></div><div><h3>Results</h3><p>We treated 50 cases of 502 patients with epithelial ovarian cancer (9.9%); with a mean age of 45.8 years; the predominant clinical manifestations were pain and increased abdominal volume. Thirty-nine patients (78.0%) had serous tumors, 10 (20.0%) mucinous tumors; and 1 (2.0%) endometrioid tumor. Forty-one (82%) were classified in stage I; and 9 (18%) with stage II and III. Seventy one % of stage I and 88.8% of the stage II and III, had elevated Ca-125 (p<!--> <!-->&gt;<!--> <!-->.05).</p><p>In 18 of 33 patients (54.5%) the correct diagnosis was obtained by intraoperative study. Thirty seven patients of the study were treated with radical surgery (74%) and 13, with conservative (but comprehensive) surgical staging (26%). Only one case of nine (11.1%) with restaging showed substratification; and 1 out of 30 lymphadenectomies (3.3%) had lymph node involvement. Twelve cases (24%) showed tumor implants. In this series, there was no tumor recurrence during a mean follow-up of 32.6 months.</p></div><div><h3>Conclusions</h3><p>Management with oncologic criteria of these tumors corroborated that the majority corresponds to Stage I, and helps reduce the numbers of recurrences. They have an incidence of 0.5 to 1.6/100,000 cases per year. There are two histological types: adult (ACGT) and juvenile (JGCT), which show different histological and clinical characteristics. The juvenile variety is a rare entity, accounting for 5% of GCT, and usually occurs in premenarchal women and under 30 years of age. Two cases are presented of women under 30 years old with a diagnosis of juvenile granulosa cell tumour. The patient in Case no. 1 presented with a tumour of 20<!--> <!-->cm in the left iliac fossa, extending to upper quadrant of the same side. She underwent exploratory laparotomy with left salpingo-oophorectomy and an intra-operative study of the tumour. The patient in Case no. 2 received outpatient care, with our institution just performing a close monitoring. One year after oncological follow up, she obtained a healthy, live baby, without complications. It is concluded that the GCT remain a rare entity. In patients of childbearing age, conservative surgery such as unilateral oophorectomy can be performed, prior to excluding any extension outside the ovary and endometrial cancer. Mammography remains the reference standard in screening and the initial assessment for this neoplasia. However, it is far from being a perfect method. Positron Emission Mammography (PEM) is an emerging nuclear method, highly sensitive, and specific for malignant lesions. It detects smaller tumours, and in earlier stages than other conventional imaging methods. It is has a great value, not only for staging the disease, but also for surgical planning. Around two-thirds are myeloid type, and it is characterised by a fast and lethal progression without early treatment. After early diagnosis, the chemotherapy should be established immediately to ensure the life of the mother. Although there are a few cases reports of good outcomes in second trimester, there are many adverse effects in the foetus in first trimester, making it a huge therapeutic dilemma. The patient will require interdisciplinary management, including haematology, obstetrics, and oncology. In particular they will also need to be informed about the importance in the timing of early chemotherapy and the inherent risks in pregnancy, as well as the risks of deciding not to interrupt it. They are rare and account for only 0.1% of all metastatic bone lesions. Endometrioid adenocarcinoma (EA) is the most frequent in the female genital tract cancer in developed countries. Bone metastases are rare in EA.</p></div><div><h3>Case report</h3><p>A 57 year old female 57 years presenting with trans-vaginal bleeding (TVB). The ultrasound and endometrial biopsy reported a moderately differentiated AE. This was initially treated with a hysterectomy, bilateral pelvic and para-aortic lymphadenectomy plus 50.4<!--> <!-->Gy external radiotherapy, and intracavitary brachytherapy. Six months later she complained of pain in left ankle and inability to walk. Computed tomography showed an expansive lytic lesion, predominantly in the talus. As the bone biopsy reported a moderately differentiated metastatic endometrioid adenocarcinoma, treatment is initiated with radiotherapy of 30<!--> <!-->Gy. Support was deferred and palliative chemotherapy started with paclitaxel and carboplatin in six cycles, as well as monthly application of zoledronic acid. Currently, after twelve months follow-up the patient has made good progress, with pain in left ankle, and tolerating walking.</p></div><div><h3>Conclusion</h3><p>The treatment strategy in patients with isolated acrometastasis remains controversial, and there are few cases described in the literature. External radiotherapy is usually considered as treatment of choice for these lesions. The pain was non-mechanical in character, treatment resistant, constant, and with night rest difficulty. Several tests were performed, with osteomyelitis being the primary diagnosis, and antibiotics were started. There was no improvement, and the patient was evaluated again, with the final diagnosis of left lower limb acrometastasis as the first clinical sign of lung adenocarcinoma.</p><p>Acrometastasis in lower limbs due to lung adenocarcinoma is uncommon and usually arises from colorectal or urogenital cancer. However, the morbidity and mortality associated with the procedure limit its use.</p></div><div><h3>Objectives</h3><p>To describe clinical characteristics, toxicity, and survival of adult Mexican patients undergoing ASCT using busulfan with a reduced dose of cyclophosphamide (BUCY 2) as a conditioning method.</p></div><div><h3>Methods</h3><p>A prospective cohort study was conducted from October 1999 to December 2014, by performing an analysis of clinical characteristics, complications, and survival of 35 patients using this conditioning regimen.</p></div><div><h3>Results</h3><p>The study included 35 patients undergoing ASCT with reduced BUCY 2. All of them had an HLA-matched related donor, with a median age of 33 years (range 16-49), and 60% were male. The patients had the following underlying diseases: myelodysplastic syndrome in 14 patients (40%), chronic myeloid leukaemia in 9 (25.7%), acute leukaemia in 10 (23.5%), and paroxysmal nocturnal haemoglobinuria in 2 (5.7%). The median of transfused CD34<!--> <!-->+<!--> <!-->cells was 2.04<!--> <!-->×<!--> <!-->10<sup>6</sup>/kg. The median time to neutrophil and platelet recovery was 21 and 18 days, respectively. The most common toxicity was mucositis (91.4%).
