Study subjects: all diagnoses obtained by histological study of biopsies performed on patients from the CMF was included. Type and sample size: 1,211 consecutive cases. Study variables; histological diagnosis, gender and age of the patient. Descriptive statistics with measures of central tendency and dispersion. {"title":"El cáncer gástrico en una institución de tercer nivel: correlación endoscópica, por tomografía computarizada e histopatológica del cáncer gástrico en el Hospital Central Militar","authors":"Gaspar Alberto Motta-Ramírez , Francisco Eduardo Almazán-Urbina , Mariana Aragón-Flores , Javier Bastida-Alquicira , Emmanuel Itzcóatl Luján-Cortés , Raúl Gámez Salas","doi":"10.1016/j.gamo.2015.11.003","DOIUrl":"https://doi.org/10.1016/j.gamo.2015.11.003","url":null,"abstract":"<div><h3>Introduction</h3><p>Cancer is the third cause of death in Mexico, and is the second cause of malignant tumour-related mortality, with an incidence that has not declined over the last decades.</p></div><div><h3>Material and Methods</h3><p>The study included patients who were studied by endoscopy and CT using any protocol due to clinical suspicion of primary gastric tumour lesion in the period from 1 May 2011 to 2 March 2015. Primary gastric tumour diagnosis, as well as other alternative diagnoses, was established by histopathology.</p></div><div><h3>Results</h3><p>The study included 45 patients, of whom, 28 were male (62%) and 17 were female (38%). Ages ranged from 32 to 86 years. The most common clinical reason was constitutional syndrome, either alone or associated with other clinical symptoms in 18 patients (40%). The gastric cancer was histopathologically confirmed in 30 patients (66%), with 16 (53%) being detected late, with peritoneal carcinomatosis (PC) in 13 (43%), 3 of them with liver metastasis (LM), 1 with venous thrombosis (VT), 2 with LM, and 1 with VT. The gastric CT protocol, with and without contrast-enhanced thoracic images, was performed in 12 patients (27%). Gastric folds thickening was detected in 20%, with concentric gastric thickening in 48% and associated with an ulcerated lesion, PC, LM and VT in 32%. No patients were detected early.</p></div><div><h3>Conclusions</h3><p>Gastric distension is critical in abdominal imaging and gastric CT represents the ideal imaging technique for gastric cancer staging and classification. Primary undifferentiated pleomorphic sarcoma of the kidney is a rare variant with a poor prognosis, with about 60 cases described in the literature. The case is presented of a 52-year-old female with a right renal tumour. In peripheral blood, a pattern of leucoerythroblastosis and tear drop cells, as well as elevated levels of pro-inflammatory and angiogenic cytokines is observed.</p><p>There are two types: primary myelofibrosis, which is not associated with any bone marrow condition, and secondary disease, which is associated with myeloproliferative syndrome conditions such as essential thrombocytosis or polycythaemia vera.</p><p>The incidence in the United States ranges from 0.21 to 0.25 cases per 100 thousand inhabitants, it is associated more often with the male gender, and mean age at diagnosis is 67 years.</p><p>The treatment of primary myelofibrosis can be in different forms, depending on the goal of treatment, with curative or palliative intent. A laparotomy was performed, with the final diagnosis of the biopsies being consistent with peritoneal tuberculosis. There were non-specific clinical data, such as night fever, constitutional syndrome, and elevated CA-125. Ultrasound pelvic masses may be common in both conditions. The most effective treatment in primary localised disease is surgery. With imatinib as neoadjuvant treatment in high-risk lesions or metastatic disease, 5-year survival has increased to nearly 80%. The purpose of this work is to analyse prognostic factors that determine recurrence and disease-free survival in this type of tumour.</p></div><div><h3>Material and methods</h3><p>Observational, retrospective, longitudinal study of patients from the Oncology Hospital, CMN Siglo XXI, with histological confirmation and surgical treatment of the primary lesion.</p></div><div><h3>Results</h3><p>Sixty-six patients were identified. The most common tumour site was the stomach (66.6%), followed by small bowel (28.7%), and colon (1.54%). At the time of diagnosis, 7.57% had metastasis: 4.54% (3) in the liver and 3.03% (2) in the peritoneum. The mean size of tumours was 10.84<!--> <!-->cm (2.2 to 38<!--> <!-->cm). Overall 5-year survival was 82%, and 5-year recurrence-free survival was 61%. In the case of overall survival, the site of the lesion was the only factor showing statistical significance, with <em>P</em> <!-->=<!--> <!-->.0054.</p></div><div><h3>Conclusion</h3><p>Tumour location and size, number of mitoses, risk group, and adjuvant treatment with imatinib were statistically significant as prognostic factors of disease recurrence. Several studies have demonstrated that most these cancers are positive for estrogen and/or progesterone receptors, which represent a constant risk for late recurrence. Hormone therapy has been the first systemic treatment that has proven to be effective in patients who express positive hormone receptors. Until recent years, tamoxifen and ovarian suppression were the best hormone therapies available both for adjuvance and advanced disease. Aromatase inhibitors, as well as the new mTOR pathway inhibitors, have driven an important advance in the treatment of breast cancer by improving survival and disease-free interval for these patients. Malnutrition, but not obesity, has been associated with a lower probability of survival and increased risk of relapse.</p></div><div><h3>Objective</h3><p>To assess the impact of obesity on the prognosis of survival and relapse in children with Acute Lymphoblastic Leukaemia (ALL).</p></div><div><h3>Material and methods</h3><p>Children with ALL were included in a cohort. An assessment was made of the nutritional status at diagnosis, according to BMI values in children, searching for an association of obesity and other factors with disease relapse and survival using Kaplan-Meier curves and Cox regression.</p></div><div><h3>Results</h3><p>A total of 161 patients were studied, of which 70% were males with obesity, and of these, 43% had very high risk ALL. Obesity increased the risk for relapse (OR: 3.6; 95% CI: 1.7-7.6, <em>p</em>=.001) and death (OR: 3.4; 95% CI: 1.51-7.48, <em>p</em>=.002), and limited 77-month survival to 52%. An Exp β of 3.35 was obtained in the Cox regression for patients with obesity and a very high risk leukaemia.</p></div><div><h3>Discussion</h3><p>Obesity influences the prognosis of relapse and survival time, due to a considerable increase in toxicity and some growth factors that induce chemoresistance in neoplastic cells.
