Within 6 hours of the injury, emergent surgical debridement of an open tibia fracture is advised by current guidelines. Recently, the 6-hour rule became challenged and delaying in debridement of an open fracture may be unethical.\u0000\u0000Methods: This cross-sectional study was carried out to assess the relationship between time of surgical debridement and rate of infection after open fracture tibia at National Institute of Traumatology and Orthopedic Rehabilitation (NITOR) on 608 patients with open fracture tibia (all Gustilo types) within 24 hours of injury from January 2018 to October 2019.\u0000\u0000Results: The mean age of the 608 patients was 36.2±15.5 years. The foremost mechanism of injury was road traffic accident (72.0%) and among the associated injuries, concomitant soft tissue injury (32.5%) was prevalent. Gustilo III was predominant injury (72.1%) and infection rates among the Gustilo subtype II was predominant (31.3%). After debridement 73 (12.0%) patients were contamination free. A significant association was found in the contamination rate between surveillance culture and post-debridement culture (p=0.000). Positive correlation found between debridement done ≥6 hours and infection present in 3rd culture from ward (r=0.237).\u0000\u0000Conclusion: The study insights that injury characteristics, infection pattern and infection rate of open fracture tibia. Stress at workplace lowers the standard of nursing care and raises the possibility of medical errors.\u0000\u0000Methods: A hospital based cross-sectional study was carried out to the level of occupational stress among nurses working in Bogra 250 bed Mohammad Ali District Hospital and Joypurhat 250 bed District Hospital from January 2022 to December 2022. The ‘Kessler Psychological Distress Scale’ (K10) was used to measures the level of stress.\u0000\u0000Results: The mean age of the respondents was 37.9± 8.8 years. The mean of the working experience was 9.1±6.9 years. The Majority of the nurses (91.4%) had no stress, 5.2% had mild stress and 3.4% was experienced moderate stress in last 30 days. Level of stress among the nurses was statistically significant with ability to take break, sudden change in roaster, confidence in clinical decision making, ability to have healthy meal, missed social events, opportunity to spend time with family and friends, and supports from seniors (p<0.05).\u0000\u0000Conclusion: Study reveals mild to moderate level of stress was found among one out of ten nurses. It has been found that excessive occupational stress lowers the level of nursing care. The CO2 pneumoperitoneum causes changes in the splanchnic microcirculation and can affect liver physiology. Although laparoscopic cholecystectomy offered many advantages over laparotomy, new concerns arose regarding the effects of CO2 pneumoperitoneum on liver function. Knowledge of the effects of CO2 pneumoperitoneum on liver function can help minimize complications while profiting from the benefits of LC without concerns about its safety. It was noticed in many studies that following laparoscopic cholecystectomy (LC), liver function parameters were disturbed.\u0000\u0000Aims: To observe any alteration in liver function by carbon dioxide pneumoperitoneum in laparoscopic cholecystectomy.\u0000\u0000Methods: Ninety-four patients with cholelithiasis having normal liver function enrolled in this study. Test obtained preoperatively including serum bilirubin, alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP) then 24 &72 hour post operatively to compare and proceed. Statistical analyses of the results were obtained by using window-based computer software devised with Statistical Packages for Social Sciences (SPSS22).\u0000\u0000Results: The findings showed that mean duration of pneumoperitoneum was 43.8±9.1minutes. The mean serum bilirubin pre-operatively was 0.75±0.21 (mg/dL), post-operative 24 hours value was 0.77±0.23 (mg/dL) and post-operative 72 hours value was 0.73±0.18 (mg/dL). No significant difference was observed in pre-operative and postoperative values. The mean serum ALT preoperatively was 33.81±7.83 (IU/L), post-operative 24 hours value was 52.51±25.9 (IU/L) and postoperative 72 hours value was 46.3±21.33 (IU/L). The mean serum AST pre-operatively was 33.84±6.83 (IU/L), post-operative 24 hours value was 48.94±19.23 (IU/L) and post-operative 72 hours value was 44.36±15.7 (IU/L). The mean serum alkaline phosphatase pre-operatively was 72.57±25.95 (IU/L), post-operative 24 hours value was 88.26±41.69 (IU/L) and post-operative 72 hours value was 85.32±38.85 (IU/L). There were significant increase of post-operative 24 hours and 72 hours value of liver enzymes when compared to preoperatively.\u0000\u0000Conclusion: There was significant increase in, AST, ALT, ALP post-operative 24 hours and 72 hours after doing laparoscopic cholecystectomy when compared to preoperatively. Many studies have done regarding correlation between cervical traction and cervical spondylosis but still there is lack of data about effect ICT (intermittent cervical traction) on cervical spondylosis and radiculipathy. Methods: This prospective randomized clinical trials was performed at physical medicine & Rehabilitation department, Combined Military Hospital (CMH) Dhaka and BSMMU (Vagabond sheikh mujib medical university), Dhaka over a period of six months. 80cases completed the study after selecting by consecutive sampling according to inclusion & exclusion criteria. The neligible participants allocated into two groups. Group-A were managed by supervised ICT, NSAID's, therapeutic neck exercise, soft cervical collar and ADL advice. Group-B received same as A -group management except (ICT). Evaluation made at initial visit and weekly for six weeks. Assessment of the patient was done by VAS, NPRS, pain frequency score and neck ROM. Student's 't' tests, chi square tests were done to see the level of significance. “P” value < 0.05 considered as significant. Results: Among the 80 patients, mean age was 45 ± 9.25 years, 65% were male and 35% were female. Maximum patients were predominantly table worker 30(37.5%) and mean BMI was 24.95 ± 1.86. After six weeks treatment VAS, NPRS, Pain frequency score shows significant difference of improvement “P” value < 0.05 between two groups. The prevalence of drug abuse has been increasing rapidly all over the world. Recently, in Bangladesh substance abuse is recognized as a grave health and social problem which threatening the whole nation. Methods: A cross-sectional study was directed to assess health related quality of life and relative attributes among 235 substance users of Central treatment center and Brain and Life hospital situated in Dhaka. The questionnaire was prepared by using HRQoLDA scale. Results: The mean age of substance users was 32.29±11.47 year. Two-thirds of the respondents (74.8%) were under graduate and worked in different profession. Most the respondents came from joint families (50.6%) and low income families (45.1%).The majority of the respondents used ganja (79.6%) followed by yaba (45.5%), alcohol (35.7%), injectable drugs(30.0%), fensidil (14.9%), tablet (9.8%) and rest heroine (9.4%). Two-thirds of the respondents started using drug by friends influence (67.2%) followed by family issues (28.1%), depression (21.7%) and rest other causes (10.6%).There were several significant associations were found between the variables as presence of any physical pain, presence of sleeping disturbance, presence of nausea, presence of lack of energy or tiredness, presence of depression, presence of aggressiveness, presence of recalling problem, presence of hallucination (a) auditory (b) Visual , presence of concentration problem, presence of disorientation problem, use of illicit drug to feel better physically, use of illicit drug to feel better mentally, presence of any physical pain which interfere daily activities, felt deterioration of physical condition, felt deterioration of mental condition, getting care from surrounding people, presence of any family problem, Believe if drug use reduce social activities, believe if some people can quit drug and the total mean scores of HRQOL. Age, education, Occupation, family income, duration of using of drug, and number of time of using of drug per day were statistically significant (p<0.05) with the total mean scores of HRQOL. Marital status and with drawal effects such as restlessness, loss of temper, insomnia were also statistically significant (p<0.05) with the total mean scores of HRQOL. Conclusion: This study revealed that the total health related quality of life among substance users were not satisfactory. A total of 16 patients with acute hand injury or pathology in the palm which may create surgical wound, admitted in the study place during the study period were the study population. Where those who had Acute wound in palm with exposed tendon, nerve and bones or degloving hand injury were included in the study. Patient were divided into two group namely Group-A and Group-B. Patient chosen for groin flap were allocated in group-A and for reverse pedicled radial forearm flap were in group-B.\u0000\u0000Results: During the study, complete flap survival was 100% in both group A and B. At all the follow-ups hand grip strength was more in group B and the difference was statistically significant (p<0.05, obtained by unpaired t-test). At final follow-up (12 weeks) hand grip strength was 22.38 (SD±5.26) kg in Group A and 30.13 (SD±10.15) kg in Group B.There is no(0.0%) excellent result of index and middle finger in group A patient compared to 3 (37.5%) and 6 (75.0%) patients respectively in group B. Ingroup A3 patient had excellent outcome in ring fingercompared to 7 (87.5%) patients in group B. Majority (87.5%) patient‘s little finger was excellent in group A and 8(100.0%) in group B.The difference of index finger and middle finger were statistically significant between groups (p<0.05 obtained by Fisher Exact test).\u0000\u0000Conclusion: From this study we can say that, both the handgrip strength and TAM score was better in patients who underwent radial forearm flap and the difference was statistically significant (p<0.05). It can be done through abdominal, vaginal and laparoscopic routes 1,2.The American college of Obstetricians and Gynaecologists recommends the vaginal approach as the route of choice of hysterectomy for benign disease whenever feasible 9 .Laparoscopic hysterectomy is a preferable alternative to open abdominal hysterectomy for those patients in whom vaginal hysterectomy is not indicated or feasible. Objective(S): To compare the surgical outcome of total abdominal hysterectomy (TAH) and non-descend vaginal hysterectomy (NDVH). Methods: This is a cross sectional and comparative study that was conducted from January 2014 to December 2015 in Obstetrics and Gynaecology Department of Sylhet Shaheed Shamsuddin Ahmed Hospital, Sylhet. A Total of 120 patients with benign gynaecological disorder who underwent hysterectomy were included. Among them 60 cases underwent NDVH and 60 cases underwent TAH for similar indication. Main Outcome measures are: i) Duration of operation (min) ii) intra and post-operative complications iii) blood loss (ml) during operation iv) Analgesic requirements v) post-operative hospital stay. Results: Most of the patients in both NDVH and TAH belonged to age group of 41-50 years. Majority of them were multiparousin both the groups. Medical co morbidities were Diabetes mellitus, Hypertension, Hypothyroidism and most common surgical co morbidities were history of lower segment cesarean section (LSCS), bilateral tubal ligation, laparotomy for benign ovarian pathology. Common indication for hysterectomy in NDVH was abnormal uterine bleeding (AUB) (37%) and in TAH was (18%), fibroid uterus 35% cases in NDVH and 37% in TAH. Mean operative time of NDVH group was 51.08±12.59 minutes while that of women in TAH group was 69.25±11.12 minutes. This difference was found to be statistically significant (p<0.05 when df 118). The mean blood loss in NDVH group was 61.08±18.82 ml while in TAH group was 105.50±38.42 ml, this is also statistically significant (p<0.05). Analgesic required in NDVH group 5.91±0.97 doses and in TAH was11.58±2.67 doses which is statistically significant(p<0.05).11(18%) patients developed wound infection in TAH group, but none of the patients of NDVH group developed wound infection which is significant. Post-operative hospital stay in NDVH group was 3.02±1.43 days and those in TAH group was 7.7±3.40 days this is also statistically significant. Conclusions: NDVH is safe, feasible and effective procedure and associated with less blood loss during surgery, decrease operative time, less intra-operative and post-operative complications with shorter hospital stay as compared total abdominal hysterectomy. This was bi-center study and study places were the OT complexes of Sylhet Women’s Medical College Hospital (SWMCH) and Al Haramain Hospital Pvt. Ltd. of Sylhet. A Total 24 samples were collected from 12 OTs. Air and wall swab both types of samples were collected from each OT. Out of these 12 OTs 4 had laminar air flow system. Half of the air samples (50%) and 8.3% of wall samples were infected, whereas overall organism isolation rate was 29.17%. Among all the infected samples 25% were from laminar air flow OTs and the rest 75% were from non-laminar air flow OTs. Commonest isolated bacteria was staphylococcus epidermidis (62.5%) followed by bacillusspp (25%) and staphylococcus aureus (12.5%). staphylococcus epidermidis was predominantly found in General Surgery, Obstetrics and septic OTs.
