It usually involves organs like lungs and/or gastrointestinal (GI) tract [2]. Hepatic involvement is usually associated with miliary tuberculosis which is one of the most characteristic manifestations of tuberculosis. Isolated liver abscess without any primary infection is a rare clinical entity. It is more difficult for a clinician to diagnose as in most cases it is frequently confused with pyogenic or amoebic liver abscess or a hepatoma [3]. Only a few cases have been reported in Bangladesh till date [4-7]. Recently we came across tubercular liver abscess in an immunocompromised patient without any primary foci of infection which was really a diagnostic challenge for us.\u0000\u0000Objectives: The objective of reporting this case is to aware clinicians about the uncommon presentation of a common disease like tuberculosis. This life saving drug has significant side effects. Patients with steroid hazards are frequently encountered in our clinical practice which should be minimized.\u0000\u0000Methodology: It was an Observational, Descriptive, Cross-sectional study. Study period was October 2013 to March 2014 in the department of Medicine & Endocrinology, Sir Salimullah Medical College & Mitford Hospital. Total 100 patients were taken who had history of steroid intake for more than 7.5 mg/day equivalent of prednisolone for more than 3 months by purposive sampling technique.\u0000\u0000Results: The age of patients range from 13 to 82 years. The mean age ±SD was 45.2±9.75 years. Among them 57% was female & 43% was male. Among 100 patients 67% was illiterate & 82% was from low socioeconomic condition. Out of them 80% was taking oral prednisolone for mean duration ±SD was 8 ±4 months and mean dose ±SD was 10±2 mg/day. Frequency of patients with different indications were 29% Rheumatological, 20% respiratory disease, 12% Dermatological, 12% Nephrological, 12% Hematological, 5% Neurological, 4% for mechanical pain, 3% gastrointestinal & 3% had self-intake for gaining weight. Among them 52% patients were receiving according to qualified physician's advice, 41 % overuse steroid beyond prescription & rest of them were taking without indication (abuse). The most commonly observed side effects were Cushingoid appearance (100%), skin changes (94%), weight gain (67%), Diabetes Mellitus (DM) - 64%, increased incidence of infection (56% cases) in which disseminated tuberculosis was 10%, Hypertension (HTN) - 39%, osteoporotic collapse 11%, myopathy 10%, iatrogenic adrenal insufficiency 3% and psychosis 2% patients.\u0000\u0000Conclusion: Due to lack of awareness among patients and health care provider, significant number of people overuse and sometimes abuse steroid in our country. The present study consists of 150 cases of acute appendicitis disease seems to be difficult for laparoscopic appendicectomy selected on the basis of clinical and sonological criteria. They underwent laparoscopic appendicectomy and the results were evaluated on the spectrum of per operative and early postoperative complications, methods of identification of these complications and their managements. The overall intra operative complications in the present study in 8 (5.33%) cases and early postoperative complication in 15 (10%) cases. Laparoscopic appendicectomy results very few intraoperative and postoperative complications. The goal of the study was to compare the efficacy of nebulized salbutamol and nebulized epinephrine in treating children with acute asthma.\u0000\u0000Objectives: To assess the clinical improvement of acute bronchial asthma in children following nebulization with salbutamol and epinephrine.\u0000\u0000Methodology: This random, double-blind clinical trial was conducted in the paediatric department of the Mymensingh Medical College Hospital. Children of either sex with clinical features of acute exacerbation of asthma, an asthma score >5 to 11, an age range from 6 to 12 years, and the ability to measure peak expiratory flow rate (PEFR) were enrolled in this study. Outcomes were observed by measuring PEFR, respiratory rate, pulse rate, and SaO2 and also by determining asthma scores at 15, 35, 55, and 90 min after nebulization at a regular interval at 0, 20, 40, and 60 min. Differences from the base line and differences between both groups were observed.\u0000\u0000Results: A total of 56 patients with acute asthma were enrolled in this study. Among them, 27 children were in the salbutamol group, and 29 patients were in the epinephrine group. Among baseline characteristics, there was no statistically significant difference (p > 0.05) between the two groups. Asthma score and respiratory rate decreased more in the salbutamol group with time after nebulization, but no statistically significant differences existed for the follow-up at 15, 35, and 55 minutes. A statistically significant increase in asthma score and respiratory rate was seen at 90 minutes of treatment in the salbutamol group. On the other hand, percent of predicted PEFR and SpO2 increased with time in both groups, and no statistically significant differences existed except for SpO2 at 35 min of follow-up in the salbutamol group. Sputum can be stained by Ziehl-Neelsen (Z-N) staining and examined by conventional light microscopy. Again it can also be stained with Auramine O stain and examined by light emitting diode (LED) fluorescence microscopy. This study was planned to find the most sensitive, specific and feasible technique for the diagnosis of pulmonary tuberculosis.\u0000\u0000Materials and Method: This cross-sectional analytical study was conducted in the department of Medicine in collaboration with the department of Microbiology, Sylhet M.A.G. Osmani medical college, Sylhet from 1st January, 2019 to 30th June, 2019. All clinically suspected patients with pulmonary tuberculosis attending both outpatient and inpatient department of Medicine Sylhet M.A.G Osmani medical college hospital, Sylhet during the study period were the study population. Total 380 patients were recruited as study sample after fulfilling the inclusion and exclusion criteria by purposive sampling method. All the patients were referred to department of Microbiology, Sylhet M.A.G Osmani medical college for sputum for AFB examination. All the samples were divided in to two portion and then one portion was marked as group-A-and another portion as group-B. In group-A, conventional Ziehl Neelsen (Z-N) staining with light microscopy and in group-B, Auramine staining with LED fluorescent microscopy were done.\u0000\u0000Result: Among 380 patients, 47 (12.4%) patients and 52(13.7%) patients were diagnosed as pulmonary tuberculosis by Z-N method and LED fluorescence microscopy respectively but this difference was not significant (*Z=-0.532; p>0.05). Paucibacillary (scanty and 1+) cases were observed more in LED 34 (8.9%) method in comparison to Z-N 31 (8.2%) method. But this difference again did not reach the level of significance (*Z=0.345; p>0.05). The liver stiffness could be used as predictors of oesophageal varices in cirrhotic patients because portal hypertension is related to liver fibrosis.\u0000\u0000Objectives: This study aimed to evaluate the diagnostic accuracy of transient elastography for the presence and grade of oesophageal varices (EV) in patients with liver cirrhosis.\u0000\u0000Methods: This cross-sectional study was conducted in the Department of Medicine and Department of Gastroenterology Sylhet MAG Osmani Medical College Hospital, Sylhet between July 2018 and June 2019. Seventy two consecutive cirrhotic patients (mean age 47.21 ± 14.02 years, 73.6% males) were enrolled. Patients with hepatocellular carcinoma, liver cirrhosis with moderate or massive ascites, acute liver failure, previous variceal bleeding, treatment with ß blockers, sclerotherapy or band ligation of oesophageal varices, transjugular intrahepatic portosystemic shunt or surgery for portal hypertension, liver transplantation, portal, splenic or hepatic vein thrombosis revealed by the abdominal ultrasonography, Spontaneous bacterial peritonitis, extrahepatic cholestatsis, congestive cardia failure, BMI 30 or above were excluded . All patients underwent fibroscan (transient elastography) and upper GI endoscopy. The diagnostic performance of the methods was assessed using sensitivity, specificity, positive predictive value, negative predictive value, accuracy and receiver operating characteristic curves.\u0000\u0000Results: Oesophageal varices were found in 86.1% with grade-I in 22.2%, Grade-II in 31.9%, Grade-III in 31.9% and no oesophageal varices 13.9% of patients. A significant positive correlation revealed between liver stiffness measured by fibroscan and presence (r=0.568; p<0.001) and grade (r=0.783; p<0.001) of oesophageal varices. The best cutoff value of liver stiffness measurement was 14.45 kPa in predicting the presence of oesophageal varices with the sensitivity of 98.4%, specificity of 90.0%, positive predictive value (PPV) of 98.4%, negative predictive value (NPV) of 90% and area under the curve (AUROC) of 0.967; and 41.55 kPa for large oesophageal varices (grade-II and III) with the sensitivity 95.7%, specificity 92.3%, PPV 95.7%, NPV 92.3% and AUROC 0.965.\u0000\u0000Conclusion: Liver stiffness measurement by fibroscan is correlated with presence and grading of oesophageal varices in liver cirrhosis. Measurement of liver stiffness by fibroscan is reliable for predicting the presence and larger grade of oesophageal varices by endoscopy in patients with liver cirrhosis. But at times the syndrome manifests with a wide and variable range of ocular and endocrine features. Central retinal vein occlusion (CRVO) is in and of itself a rare cause of blindness, and its presence in the context of primary empty sella is even rarer. In this article, the authors report a case of a hypertensive man in his late thirties presenting to the medicine inpatient with headache and blindness of the left eye. Investigations came up with the diagnosis of PES syndrome with CRVO with macular edema. {"title":"Diabetes Mellitus Associated with Outcome of COVID-19 Patients.","authors":"Islam GMM, Rahman MM","doi":"10.47648/jswmc2023v13-02-80","DOIUrl":"https://doi.org/10.47648/jswmc2023v13-02-80","url":null,"abstract":"Introduction: COVID-19 is a highly infectious disease spreading all over the world since its appearance causing enormous sufferings and loss of human life specially among those who have suffered from Diabetes Mellitus.\u0000\u0000Material & method: This retrospective cohort study was conducted among purposively selected 133 COVID-19 patients in which 39 were exposed to DM and 94 were not-exposed to DM who were admitted in CMH, Dhaka from July 2020 to June 2021 with the objective to find out the association of Diabetes Mellitus with the mortality and morbidity outcome of COVID-19 patients within 90 days of discharge from Hospital. The data were collected by telephonic interview and from medical record.\u0000\u0000Result: The study revealed that the mean age of the respondents were 48.01 years, 80.5% were male. It was seen from the study that among all the respondents 95.5% were survivor and 4.5% were non-survivor. Among the exposed 87.2% were survivor and among the non-exposed 89.9% were survivor. Among the exposed more than half (51.3%) were ⩾60 years age group,48.7% were military retired personnel, 61.5% had family income TK< 25,000. On 14th and 28th day of admission among exposed 94.9% and 30.8% and among non-exposed 83.0% and 21.3% were not cured respectively. Before attaining 28th day of admission among exposed 12.8% and among non-exposed 1.1% were non-survivor. Regarding ICU support among exposed 30.8% and among non-exposed 10.6% required ICU support during treatment, whereas among exposed 33.3% and among non-exposed 6.4% were critical patients. Among exposed 48.7% required oxygen, 17.9% needed ventilator support whereas among non-exposed 23.4% required oxygen and 4.3% needed ventilator support. Among exposed HTN, CHDs and CKDs were 61.5%,7.7% and 12.8% whereas among non-exposed it was 21.3%, 3.2% and 1.1% respectively. The risk exposure outcome among ⩾60 years age group were higher among exposed (RR:2.05, 95% CI 1.33-3.17). Morbidity outcome among exposed on 14th 28th and 90th day were higher (RR:2.05, 95% CI 1.33-3.173), (RR: 1.485, 95% CI 0.847-2.603) and (RR:1.190, 95% CI 0.371-3.820) respectively. Need of Oxygen were higher among exposed (RR:1.445, 95% CI 1.064-1.961). Association in regards to HTN, CHDs and CKD were also higher among exposed which were (RR:1.87, 95% CI 1.321-2.656), (RR: 1.433, 95% CI 0.639-3.214) and (RR:4.394, 95% CI 0.732-26.376) respectively.\u0000\u0000Conclusion: Health education measures should be taken to prevent and control the morbidity and mortality outcome of COVID-19 patients with special attention to those who are aged, and suffering from DM, HTN and CKD. There are two widely utilized procedures to produce pneumoperitoneum closed and open techniques. Both have advantages and disadvantages.\u0000\u0000Aims: Comparing open and closed techniques of creating a pneumoperitoneum for laparoscopic cholecystectomy is the goal.\u0000\u0000Methods: The Surgery Department of Sylhet MAG Osmani Medical College Hospital conducted this prospective comparative observational study from September 2018 to August 2019. This study comprised a total of 138 hospitalized patients, of whom had symptomatic gallstone disease and required cholecystectomy. The patients were split into two groups; 69 were chosen for the closed approach and were referred to as Group A, while the remaining 69 were chosen for the open method and were referred to as Group B.\u0000\u0000Results: The average age was 44.88 ±12.61 in Group A and 47.12 ±11.62 in Group B. In groups A and B, the male-to-female ratio was 1:2.3 and 1:1.6, respectively. The difference between the two groups was statistically insignificant (p>0.05). The average access time was 7.43 minutes for Group A and 3.14 minutes for Group B. The average access time was substantially longer in group A (p <0.05). 27 patients (39.1%) in group A and 38 patients (55.1%) in group B had gas leaking. The difference was not statistically significant (p>0.05) between the two groups. 15 (21.7%) patients in group A and 5 (7.2%) patients in group B had extraperitoneal insufflation. Extraperitoneal insufflation in group A was considerably (p<0.05) greater. The average surgery was 81.83± 20.21 minutes for Group A and 53.42± 11.63 minutes for Group B. The mean time frames spent on wound closure in groups A and B were 6.61± 1.49 and 2.55 ±0.8 respectively. The mean duration of the procedure and the mean time to close the wound were considerably (p<0.05) higher in group A. No group had any cases of visceral damage, conversion need, haematoma, seroma, subcutaneous emphysema, gas embolism, or port site hernia. In groups A and B, respectively, one (1.4%) patient each had wound discharge and wound infection. The difference between the two groups was not statistically significant (p>0.05).\u0000\u0000 Conclusion: It was preferable to create pneumoperitoneum openly. G6PD deficiency is an X-linked disorder, and one of its important signs is the presence of hemolytic anemia. It is a worldwide important cause of neonatal jaundice and causes life threatening hemolytic crisis in childhood. Here a 2- year- old boy was admitted into department of pediatrics of Sylhet Women’s Medical College Hospital with fever for 2 days & passage of dark color urine for 1 day. There was history of accidental naphthalene ingestion present prior to develop symptoms. The boy was clinically febrile, severely pale, anicteric. No organomegaly was present. It was confirmed by G6PD level assay. Symptomatic & supportive treatment was given with packed cell volume transfusion and avoidance of triggering factors was advised.
