Aruna","doi":"10.15380/2277-5706.jcsr.14.033","DOIUrl":"https://doi.org/10.15380/2277-5706.jcsr.14.033","url":null,"abstract":"Background: Delirium, a common manifestation in critically ill patients is a strong predictor of prolonged intensive care unit (ICU) stay, higher morbidity and mortality. Methods: One hundred patients admitted to medical ICU (MICU) at our tertiary care teaching hospital with delirium were evaluated using confusion assessment method. Their Simplified Acute Physiology score II (SAPS II) at presentation, socio-demographic details, risk factors for development of delirium were recorded; aetiology and factors affecting outcome were identified. Results: Their mean age was 48.1 ± 16.6 years; there were 63 males. Most of them (87%) belonged to lower socioeconomic status. Infections (65%) followed by intentional poisonings (19%), metabolic causes (10%) and substance abuse (6%) were the important aetiological causes. Hyperactive delirium (48%) was the most common, followed by mixed (27%) and hypoactive delirium (25%). Univariate analysis showed that non-survivors had a significantly higher SAPS II (p<0.001) and a higher burden of chronic kidney disease (p<0.001) compared to survivors. We report the case of a patient with HIV1 infection who presented with chronic diarrhoea that led to hypokalemia induced rhabdomyolysis and acute kidney injury (AKI). A 32 -year-old man, known to be HIV-1 seropositive who was on treatment with tenofovir, lamivudine, lopinavirand ritonavir, presented to the emergency department with 6 months history of diarrhoea and sudden onset of weakness of all 4 limbs of two days duration. On examination limb power was grade 3/5 with hypotonia and diminished reflexes. Laboratory investigations showed severe hypokalemia, low urinary potassium, normal anion-gap metabolic acidosis, markedly increased creatine kinase and mildly increased serum creatinine. He was diagnosed as having diarrhoea induced hypokalemic myopathy leading to rhabdomyolysis and AKI. With potassium supplements, antiprotozoal treatment and adequate hydration he improved significantly and is on regular follow-up. We report the rare case of accidental ingestion of kerosene by a previously healthy 20-year-old woman resulting in left lower lobe pneumonitis with minimal pleural effusion. Pulmonary parenchymal lesions were seen only through computed tomography of the chest. Pain is an unpleasant sensation in the post-operative period. Methods: Fifty patients of American society Anesthesiologists (ASA) grade I and II, scheduled to undergo laparoscopic surgery, were randomized into butorphanol group (Group B) (n=25) and fentanyl group (Group F) (n=25). Four minutes before induction of anesthesia, Group B received inj. butorphanol 40 μg/Kg intravenously while Group F received Inj. fentanyl 2 μg/kg intravenously. All patients received general anaesthesia with controlled ventilation. Heart rate, systolic blood pressure (SBP), diastolic blood pressure (DBP), end tidal carbon dioxide (ETCO2), and oxygen saturation were monitored at different intervals. Results: The demographic data was comparable in both groups (p<0.05). There was a significant difference between the two groups in pulse rate after 12 minutes after intubation that persisted till the 2nd hour of post-operative period. There was no significant difference in DBP till 9 min after intubation, and then onwards significant changes were noted till 4th hour of post-operative period. Butorphanol provided prolonged analgesia when compared to fentanyl. She was known to have epilepsy since childhood. Anticipating a scar dehiscence or rupture uterus she was posted for emergency laparotomy under spinal anaesthesia. Intra-operatively, 180o uterine torsion was evident. The uterus was untwisted and a lower segment caesarean section was performed to deliver the dead foetus. As the uterus remained flabby and did not retracted after correction of torsion, subtotal hysterectomy was done. She recovered well after surgery. We report the case of a patient with squamous cell carcinoma of the larynx presenting with idiopathic thombocytopenic purpura (ITP). A 67-year-old lady presented with multiple ecchymotic patches and petechiae all over the body and bleeding from oral cavity was found to have severe thrombocytopenia diagnosed as ITP with bone marrow evidence of peripheral destruction without infiltration of bone marrow. Five months later she was diagnosed to have squamous cell carcinoma of larynx.
