According to the opinion of a number of experts, acute biliary pancreatitis should be considered a combination of acute pancreatitis with chronic or acute diseases of the biliary tract. \u0000Aim. To assess intraoperative morphological changes during laparoscopy in patients with acute biliary pancreatitis. \u0000Materials and methods. We treated 65 patients with acute biliary pancreatitis who had isolated cholecystolithiasis. Forty-two patients had acute calculous cholecystitis, twenty-three patients had chronic calculous cholecystitis. All patients underwent laparoscopy, which included diagnostic and treatment stages. \u0000Results. Signs of acute biliary pancreatitis were: serous fluid (6.2 %), fibrinous fluid (10.8 %), hemorrhagic fluid (41.5 %), fatty spots (7.7 %), mesenteric hemorrhagic edema of the large intestine (64.6 %), pancreas edema (23.1 %), omentum edema (50.8 %), vitreous edema of the hepatoduodenal ligament (36.9 %), enlarged gallbladder (60.0 %), destructive acute cholecystitis (9.2 %). When the head of the pancreas was damaged, we observed hemorrhagic fluid, hepatoduodenal ligament edema, an enlarged gallbladder, and acute cholecystitis. When the body of the pancreas was damaged, we observed serous fluid, fatty spots, mesenteric edema of the large intestine, and parapancreatic infiltrate. The proposed work: To improve the results of surgical treatment of patients with acute peritonitis by developing a new aerosol remediation of the abdominal cavity. \u0000Material and methods. A clinical and laboratory examination of 40 patients with acute disseminated peritonitis was carried out, a method of aerosol remediation of the peritoneal cavity with programmed relaparaperties was developed \u0000The results. The developed method of aerosol sanitation of the peritoneal cavity with programmed laparotomy is the method of choice for treating patients with widespread forms of acute peritonitis. It creates opportunities for a long-term local influence on the course of the inflammatory process in the peritoneal cavity, allows to rehabilitate it, remove microorganisms and toxic products before they accumulate and generalize, and exclude their further action on the tissues of the peritoneal cavity. \u0000Conclusions. The terms of programmed laparotomy should be determined individually, depending on the nature of the pathological process and the patient’s condition. However, at the initial stages of treatment, repeated exposures should be carried out no later than 72 hours, since the conditioned effect of ZL is maintained during this time. The purpose of the work was to study the possibility of alternative antibacterial therapy by the type of lymphotropic targeted accumulative antibiotic therapy for injuries of the abdominal cavity. \u0000Materials and methods. To implement it, we studied in an experiment the accumulation of the antibiotic in the tissues of various organs of rabbits after its lymphotropic administration. \u0000Results of the research. The experiment proved, that lymphotropic antibiotic therapy, depending on the place of regional administration of the drug, contributes to the targeted accumulation of the antibiotic in the corresponding organ in an amount sufficient for a persistent antibacterial effect. \u0000Conclusions. The conducted research allowed us to formulate a new diagnostic criterion, with the help of which it is possible to predict its further course in the early stages of the development of AS with the allocation of gradations of the probability of the development of complications \u0000Conclusions: 1. Studying the profile of pro- and anti-inflammatory cytokines, spontaneously and under the influence of TLR ligands, made it possible to distinguish 2 subgroups among the examined patients: favorable and unfavorable. 2. All patients underwent nasogastrointestinal intubation with aspiration of intestinal contents and subsequent prolonged intestinal decompression. \u0000Research results. In the second group, after intestinal lavage with α-aminocaproic acid, a decrease in total proteolytic activity was observed both in the contents of the small intestine and in blood serum. \u0000Conclusions. Burn injury remains one of the serious problems of clinical medicine. For successful spontaneous epithelization of surface burns in the prescribed time frame, it is necessary to create favorable conditions to reduce bacterial insemination and limit additional traumatization of dermal burns. \u0000Goal. Evaluation of the clinical effectiveness of Arederma spray with probiotics for the local treatment of superficial burns. \u0000Materials and methods. The results of complex local treatment of 26 patients with superficial burns are presented. Patients were divided into 2 clinical groups. The main group consisted of 14 patients who, before standard local treatment, underwent additional treatment of burn surfaces with a spray with probiotics. The comparison group consisted of 12 victims who underwent standard local therapy of burn surfaces. In patients of the main group, the burn area on average was (6.4 ± 1.8) % of body surface, in patients of the comparison group (5.9 ± 1.3) % of body surface. The effectiveness of the treatment was carried out on the basis of a microbiological study with the study of the wound process using cytological studies and visual assessment. \u0000Results and discussion. The obtained results testify to the effectiveness of the local application of probiotics in the complex treatment of superficial burns due to the reduction of infection of the burn surface and the positive effect of the drug on epithelization processes, which made it possible to shorten the terms of inpatient treatment of burned patients. \u0000Conclusions. The combined complex application of a neutral wound dressing and a spray containing probiotics optimizes the inflammatory process, reduces the infection of the burn surface with pathogenic microflora, which ensures epithelization of superficial burns in the prescribed time. Traumatism around the world is taking on unmanageable proportions. Suffice it to say that every year on a global scale, several million people die from traffic accidents and industrial and domestic injuries. Some die from the actual damage, and most die from purulent-inflammatory complications. \u0000The urgency of the problem. Treatment of inflammatory complications requires significant funds for antibacterial drugs, but their more considerable amount is insufficient given the resistance of the microflora. \u0000Aim. To substantiate the expediency of antibacterial therapy and methods of its application. \u0000Materials and methods. Several experimental studies were conducted on rabbits to reveal the inhibitory effect of antibacterial drugs that enter the internal organs with various administration methods. \u0000Results and their discussion. It turned out that after intramuscular administration, ceftriaxone accumulates in the organs of the abdominal cavity in an insufficient amount to inhibit the growth of microflora. After intravenous administration, ceftriaxone gets in the appendix in 60 % of the control after 1 hour, and after 2 hours, it decreases to 30 %, which is not very effective. After lymphotropic administration, the antibiotic accumulates after 1 hour in an amount sufficient for antibacterial action (70 %) and after 2 hours — in 95 %, which is evidence of the targeting and accumulation of antibiotics with such administration of the drug. \u0000Conclusions. Taking into account the significant disruption of microcirculation in the impression zone, the «mileage» of the vascular bed, the aggregation of formed blood elements in microvessels, and the formation of microthrombi, the intravenous method of drug administration in traumatic disease cannot be considered optimal. Postoperative abdominal wall wound infevtions became significant problem om modern surgery impacting approximatly 110,800 patients in 2015. \u0000Aim of study. Comparison of postoperative abdominal wall wound complications treatment methods in cancer patients. \u0000Materials and method. 24 cases of patients with purulent-septic complications of postoperative wounds were analised. 10 (41,6 %) — men and 14 (58,4 %) — women. The average age of patients was 63,8±17,1 years. Patients were divided into two groups: a comparative group — 12 patients (routine treatment), and an experimental group — 12 patients, which was treated using the VAC system. Both groups received infusion and antibacterial therapy. \u0000Research results. In cultures of wound contents, the following were found: (1) Staphylococcus aureus — 38,9 %, (2) Staphylococcus epidermidis — 19,5 %, (3) Enterococcus faecalis — 19,1 %, (4) Escherichia coli — 15,4 %, (5) Pseudomonas aeruginosa — 5,8 %, (6) Proteus vulgaris — 0,9 %, as well as (7) gram-positive aerobic and facultatively anaerobic pathogens — 0,4 %. \u0000According to the results of bacteriological examination of the wound contents in the patients of the experimental group, the microbial count decreased statistically significantly (to 105-106, p>0,05) starting from the third day of treatment. In the comparative group — all indicators remained at the previous levels (108-109). \u0000Conclusions. VAC-therapy appeared superior in postoperative abdominal wall wound complications treatment in cancer patients. To study the pathomorphological changes in adhesion formation, and to determine the main critical moments of adhesiogenesis in adhesion disease due to acute inflammatory injuty to the peritoneum. \u0000Materials and methods. The study included 28 cases of adhesion formation in operated patients (average age — 27.91±4.61 yrs). The material was collected intraoperatively in accordance with standard requirements for the production of histological preparations. To obtain differentiated polychromy of tissues, staining of micropreparations with hematoxylin-eosin and the van Gieson method were used. \u0000Results and their discussion. The histological picture varied from immature granulation tissue to signs of degenerative changes in the tissue of the formed adhesions. In some cases, the development of the adhesion process acquired features of combined fibrous and fatty degeneration. The obtained data showed a clear phasic development of adhesiogenesis from the inflammatory process to the formation of powerful connective tissue structures with the phenomena of hyalinosis and secondary degenerative changes. \u0000Conclusions. The study of the patterns of formation of the adhesion process showed that adhesion disease is characterized by a certain phasic development of changes from alternative damage and non-specific inflammation to the formation of sufficiently differentiated connective tissue and changes of a secondary degenerative nature.
