Epidermal growth factor receptor is widely overexpressed in this type of cancer. In a metastatic situation, chemotherapy has modest efficacy. Agents targeting this receptor, including Cetuximab, are effective as monotherapy or in combination with chemotherapy. The finding was fortuitous in the anatomopathological analysis of a resection specimen for phimosis. It progressed first to the inguinal lymph nodes 3 months after surgery and then rapidly to the lungs and liver after the first line of chemotherapy. Cetuximab monotherapy in the second line of therapy was well tolerated and showed a complete response after 6 months of treatment. This response has been maintained for 5 years of treatment. Post-dural puncture headache are well- known complications of neuraxial anesthesia in parturients. Objectives: The aim of this study is to assess the Causes, Effects and Management of post dural puncture headache among obstetric patients in a tertiary care hospital an observational study. Methods: This is an observational study. The study used to be carried out in the admitted patient’s Department of anesthesiology, Rangpur Medical College Hospital, Bangladesh. In Bangladesh for the duration of the period from January 2020 to December 2021. Results: This study shows that the according to age of 120 Patients aged 18 to 45 years where, 30(25.0%) were 18-25 years, 65(54.17%) were 26-35 years and 25(20.83%) were 36-45 years. And according to gender 21% were Male, 79% were Female. And according to BMI (n=120). According to BMI distribution the <18.5 (underweight range), 18.5 to <24.9 (Normal), 25-29.9 (Overweight) and ≥30.0 (Obese) were 15(12.5%), 55(45.83%), 35(29.16%) and 15(12.5%) respectfully. Conclusions: We concluded that in the present study about obstetric populace had better prevalence of PDPH in contrast to obstetric population. The nature of poison used varies in different parts of the world and may vary even in others parts of the same country depending on socioeconomic factors and cultural diversity. Self-poisoning accounts for about one-third of the world’s suicides. Objective: To assess the pattern of poisoning among the autopsies. Materials and Methods: This retrospective cross-sectional study was conducted at Dept. Of Forensic Medicine & Toxicology, Dhaka Medical College Hospital, Dhaka, Bangladesh January to June 2022. The study included alleged poisoning deaths, inquest reports, and suicidal notes; PM Reports, hospital records, and FSL reports were scrutinized. The inclusive parameters included Age, sex, socioeconomic status, hospital treatment, motive, and type of poisoning. Exclusion criteria: Highly decomposed cases. Results: The incidence of poisoning was highest in the age group of 31-40 years (21.67%) followed by 21-30 years (20%), and the least was encountered in the elderly age group. Male (51.67%) preponderance was more over female (48.33%). The most common type of poison consumed was Organophosphorus Compound (48.33%) followed by Phosphide (18.33%), Alcohol (15%), and the rest were due to Carbamates (10%) and over-the-counter medications OCT (8.33%). The common reasons for consuming poison were financial constraints (21.4%) followed by ill health (15.55%). The maximum incidence of poisoning was found in the age group of 31 to 40 years. The common reasons for consuming poison were Familial discord (26.66%) followed by Financial matters (16.67%), Unemployment (15%). Out of 60 cases 44 cases were treated in hospital but not survived and the rest died at the site of poisoning or before reaching the hospital.Conclusion: Deaths due to poisoning are on the steep rise due to which there is a huge loss of lives and a devastating impact on communities. In western countries heart disease is the major cause of maternal death. The full spectrum of structural heart disease including congenital heart disease (CHD), valvular heart disease (VHD), and cardiomyopathy (CMP), and also ischemic heart disease (IHD) may be encountered in pregnant women. Aim of the study: The aim of this study was to determine the impact of ischemic heart diseases on maternal adverse outcome during pregnancy. Methods: This was a retrospective cohort study and was conducted in the Department of Gynaecology and Obstetrics of Kushtia General Hospital, Kushtia, Bangladesh during the period from January, 2021 to January, 2022. There was total 100 women in our study. This study was conducted among pregnant women with ischemic heart disease. The study population was categorized into two; namely, Group A -women with IHD(n=50) and Group B - women without IHD (n=50). Result: In total 100 patients from both the groups completed the study. In our study we found the mean ± SD age of pregnant mothers in Group A was (30.01 ± 5.00) and in Group B (27.05 ± 5.04) respectively. The mean± SD of BMI for women in Group A was (27.47 ± 4.40) and in Group B (28.34 ± 5.67) respectively. The prevalence of Hypertension was found in 41(82%) & 21 (42%) and DM was 68% & 36%among Group A & B respectively. Transient Ischemic Attack & Stroke was found 46% & 16 % in Group A respectively. Arrythmia was only found in Group A by 36% & 28%. Conclusion: In our study, we found that the incidence of adverse maternal outcomes was higher among women with IHD. Pregnant women with IHD have a higher risk of maternal and fetal complications compared with women without HD. The patency of these lines is maintained by continuous flushing, usually with the addition of the anticoagulant heparin to the flushing solution. Normal saline solution can maintain patency of arterial and central venous pressure monitoring catheters. Aim of the Study: The aim of this study was to evaluate the effect between normal saline and heparin solution in arterial line flushing on platelet count. Methods: This was a prospective cross- sectional study conducted in the Department of Cardiac surgery of National Institute of Cardiovascular Disease (NICVD), Sher-E-Bangla Nagar, Dhaka, Bangladesh during the period from July 2016 to June 2017. This study included sixty patients who underwent single valve replacement surgery and shifted to the ICU with arterial line catheter. The patients who fulfilled the inclusion criteria were divided into two groups- Group A (with heparin solution) and Group B (with normal saline). Result: In total 60 patients from both the groups completed the study. In our study we found the mean ± SD of age among group A & B was (36.3±6.7) & (35.7±10.3) respectively. We found the mean± SD of platelet count at 1st POD was (206530±64441) & (212543±48768); at 3rd POD was (226517±60185) & (245957±52826); at 7th POD was (240517±57379) & (257713±53655); at 14th POD was (245850±52680) & (244337±56796) in Group A & B respectively. Maternal weight gain in pregnancy can offer a good means of assessing the wellbeing of the pregnant mother and her baby. Inadequate prenatal weight gain is a significant risk factor for intrauterine growth restriction, preterm delivery and low birth weight in infants. Obesity and excessive weight gain on the other hand can lead to adverse maternal and fetal outcomes. Interestingly, to get a good fetomaternal perinatal outcome mode of delivery decision is also changed with BMI. Objective: The aim of the study was to evaluate the effect of maternal BMI on the mode of delivery. Methods: This cross-sectional study was carried out department of obstetrics and gynaecology at Bangabandhu Sheikh Mujib Medical University. A total 100 population of purposive sampling was the methods of choice to select the sample from the hospital admitted patients during the period from August 2016 to December 2017. Results: The mean age of 100 mothers were 28.4 (± 6.2) years. The maximum mothers attended from 3rd trimester were overweight (55.36%) whereas 44.64% from the same trimester were normal BMI mothers. Maximum mothers (27%) were from 25-29 years age group and the minimum mothers (4%) were from >= 40 years. Forty Seven Percent mothers came as primigravida. Primigravid women and multigravida with LSCS and without LSCS showed different mode of deliveries. The more the gravida with history of caesarean section the more frequency of present occasion of CS (p=0.006). The higher BMI showed more frequency of caesarean section than NVD or assisted delivery (p=<0.00001) Multiparous women showed more BMI than the p nulliparous. (p=0.005). Multiparity, maternal age and pre-existing medical conditions revealed more complicated labor. Transmission of the mite Sarcoptes scabiei var hominis generally occurs by skin-to-skin contact, but with crusted scabies it may also occur through fomites, such as infected clothing or bedding. Diagnosis is usually clinical. Objective: To find out prevalence and life cycle in patients with scabies infection. Materials and Methods: Scope and Delimitation of the Study. This study focused the management of scabies. This study was conducted among the OPD patients in Kumudini Women’s Medical College Hospital, Tangail, Bangladesh and BSMMU, Dhaka, Bangladesh. The study was conducted from July 2021 to August 2022. The diagnoses of scabies of the potential respondents were based on the set criteria. Only those diagnose clinically with scabies were included in this study. There were no scraping procedures (acarus test) done to confirm the diagnosis of scabies. Other skin diseases aside from scabies were not included in the study. Results: In our study 110 scabies patients it appears that maximum population affected with Scabies infection were aged between 0-6 years (16.3%) and 7-12 years (20.9%) and minimum were between 43-48 years (prevalence 3.6%) and 49-54 years (prevalence 3.6%). The highest prevalence 13.63% in female were in the 7-12 years and lowest prevalence 0.95% in 49-54 years. The highest prevalence 6.36% in male were in the 7-12 years and lowest prevalence 0.95% in 25-30 years and 31-36 years. November (29.09%), December and January (34.54%) show the highest prevalence in scabies infection. In Bangladesh, this three months represent a winter season. On the other hand, July to October represent almost hot condition. So from my result, it is concluded that scabies infection is more prevalent in winter than summer. Scabies infection is not serious problem for our country because the highest prevalence 50% of severity of scabies infection limited with in Moderate (second stage of classical scabies infection) category. Classical scabies is more active in Bangladesh; on the other hand crusted scabies is limited person in Bangladesh. Conclusion: Scabies is more common where overcrowded conditions prevail; it can affect any individual irrespective of social status, personal hygiene, profession, gender, age or ethic origin. These include increased number of household members, presence of other family members with scabies, overcrowding, poverty and low educational attainment. They are benign, non-encapsulated, highly vascular tumors with a high incidence of persistence and recurrence from a histological standpoint, but they are also locally invasive tumors. Methods: This retrospective cross-sectional study was conducted by the Otolaryngology & Head Neck Surgery division at Bangabandhu Shiekh Mujib Medical University, Dhaka Medical College Hospital, and Shaheed Suhrawardy Medical College Hospital in Dhaka. The study was conducted from February through November of 2012. The total sample size for the study was 30. Result: The majority of individuals (54%) who have nasopharyngeal angiofibroma are between the ages of 16 and 20. The middle class people (40%) are more sufferers from nasopharyngeal angiofibroma. People living in rural area (53.33%) are more sufferers from nasopharyngeal angiofibroma. The majority of patients have similar symptoms such epistaxis, nasal blockage, nasal discharge, anemia, and masses in the nasal cavity. It is important to know what a given total score or a change score from baseline on one scale means in relation to the other scale. Objective: The aim of the study was to determine the depression symptoms rating scale of Beck Depression Inventory Scale (BDI-II) verses Hamilton Anxiety Rating Scale (HAM-A) among patients of Cancer Center of Combined Military Hospital Dhaka. Materials and Methods: It was a cross-sectional study. A total of 150 samples were selected by convenient sampling technique. The patients were studied after histopathological confirmation of diagnosis of cancer. Socio-demographic and other data were collected by face-to-face interview using semi structured questionnaire. The BDI-II and HAM-A was used to screen out the patients with depression and anxiety. The diagnosis was confirmed by a psychiatrist using DSM-5 criteria. Data analysis was done by Statistical Package for Social Sciences (SPSS) version 26.0 and results were presented with appropriate graphs and texts. Results: The mean age was 46.5±9.5 years. 65.3% were male and 34.7% respondents were female. Male: female ratio 1.9:1. 40.7% patients had moderate depression symptom followed by 30.0% had mild depression symptom, 17.3% patients had severe depression symptom and 12.0% patients had minimum depression symptom that can be considered as no depression. The overall mean score of Beck Depression Inventory (BDI-II) was 22.1±8.4 with range 5 to 45. Regarding anxiety of cancer patients, 45.3% patients had moderate to severe anxiety symptom followed by 29.3% patients had mild to moderate anxiety symptom, 25.3% patients had minimum anxiety symptom that can be considered as no anxiety symptom. The overall mean score of Hamilton Anxiety Rating Scale (HAM-A) was 21.6±6.7 with range 7 to 30. Comparatively most of the patients of 31-40 age groups had severe depression level symptoms and among above 50-60 year’s patients had the most severe anxiety symptoms. According to BD-II and HAM-A rating scale, among stage 4 patients 43.8% had severe depression symptom and 87.5% had severe anxiety symptom. Conclusion: Study shows that according to the rating scales both depression and anxiety symptoms are significantly higher in cancer patients. Moreover, frequency and severity of depression and anxiety symptom is much more significantly raised among older age group specially stage 4 cancer patients. The majority of epidemiological data refers to workplace accidents which account for a significant share of such accidents in adults. Exposure to electric shock has been associated with an increased risk of developing immediate and delayed cardiac arrhythmias. Objective: To evaluate the prevalence of cardiac arrhythmias and different symptoms in patient with high voltage and low voltage electrical injury. Methods: This was observational prospective study was carried out at the Dept. of Cardiology, Bangabandhu Sheikh Mujib Medical College Hospital, Faridpur, Bangladesh from January to June 2021. 51 consecutive patients with electrical injury who were admitted. Admission criteria were age ≥15 years, loss of consciousness, cardiac arrest, electrocardiographic abnormalities, soft tissue damage and burns. Patients were categorized into high and low voltage injury group and their variables were compared. Results: The mean age of the patients was 32.3±10.4 years among them 41 (82.3%) were male. Patients who sustain high voltage electrical injury (>1000V) were 18 (35.2%) and low voltage injury (<1000V) were 33 (64.7%).Cardiac arrhythmias like sinus tachycardia (11.1% vs 6.0%, p=0.054), sinus bradycardia (11.1% vs 3.0% p=0.254), ventricular premature beats (5.6% vs3.0%, p=0.674), atrial fibrillation (11.1% vs 0%, p=0.054) were observed in high voltage and low voltage group. The commonest presenting symptoms in both groups were pain (77.8% vs 81.8% p=0.560) and fatigue (55.6% vs 39.3%, p=0.328). Conclusion: In this study few non-fatal cardiac arrhythmias were observed in both high and low voltage electrical injury group. Parameters considered to be risk factors such as known structural heart disease, loss of consciousness, high voltage electric shock, burn and soft tissue injuries were also not significant predictors of the occurrence of arrhythmias.
