Formerly, microalbuminuria was used as the first marker, but recently, high-sensitivity c-reactive protein (hs-CRP) may be useful for early diagnosis. Aims and Objectives: This study aimed to compare microalbuminuria and hs-CRP in type 2 diabetic patients and test their efficiency in diagnosing early diabetic nephropathy. Methods: This is a cross-sectional observational study with 75 patients with type 2 DM. Demographic variables, microalbuminuria, and hs-CRP were measured and recorded. Microalbuminuria and positive or negative CRP were used to stratify patients into groups of MG, LG, and control. The t-test was used to test for differences in hs-CRP levels between the groups. Results: As for renal markers, microalbuminuria was elevated at 60% among these patients. The hs-CRP levels were higher in patients with microalbuminuria compared to patients without it (3. 1 ± 1. 2 mg/L versus 1. 5 ± 0. 8 mg/L, respectively; p = 0.001). Patients with microalbuminuria were older, their diabetes duration was longer, HDL was lower, and triglyceride levels were higher in comparison to patients without microalbuminuria. These observations were consistent with the CRP-positive group compared with the CRP-negative group. Conclusions: The research suggested a relationship between increased hs-CRP concentrations and microalbuminuria among patients with type 2 diabetes. This indicates that hs-CRP could be a potential biomarker for detecting diabetic renal disease early enough. Therefore, screening for hs-CRP in combination with screening for microalbuminuria could increase the early detection of kidney complications in type 2 diabetes. More large-cohort longitudinal follow-up studies are warranted to determine the diagnostic value of hs-CRP for predicting the development of diabetic nephropathy and to define optimal cut-off levels.","PeriodicalId":397187,"journal":{"name":"Global Academic Journal of Medical Sciences","volume":"45 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141663794","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Understanding the risk factors associated with early-onset CRC is crucial for developing targeted prevention and intervention strategies, particularly in limited-resource settings. Objective: This study aimed to identify and analyze the risk factors contributing to the development of early-onset CRC in a population from a limited-resource country. Method: A comparative study was conducted among 178 CRC patients admitted to the National Institute of Cancer Research & Hospital (NICRH), Dhaka. Patients were divided into two groups: Group-A (age 20-50 years) and Group-B (age >50 years). Demographic and clinical data were collected, and statistical analyses were performed to determine associations between various risk factors and early-onset CRC. Results: The mean age of Group-A subjects was 43.6±7.1 years, significantly younger than Group-B subjects (59.2±9.3 years). Male patients constituted 69.1% of the cohort, with a statistically significant male-to-female ratio of 2.2:1. Higher BMI (≥25) was strongly associated with early-onset CRC (p<0.01). Occupations such as business and middle socioeconomic status were significantly linked to early-onset CRC. Regular exercise (>30 minutes daily) demonstrated a protective effect against CRC development. Consumption of red meat and fast food was associated with a higher risk of CRC, while regular consumption of vegetables was protective. Familial adenomatous polyposis (FAP) presence was significantly associated with early-onset CRC. Conclusions: Male sex, high BMI, specific occupations, dietary factors, and presence of FAP emerged as key risk factors for early-onset CRC. Encouraging regular exercise and promoting healthy dietary habits, particularly reducing red meat and fast-food intake, are crucial in mitigating the risk of early-onset CRC in similar populations.","PeriodicalId":397187,"journal":{"name":"Global Academic Journal of Medical Sciences","volume":"106 36","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140380601","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a case of a 43-year-old immunocompetent woman who experienced unilateral acute retinal necrosis, with Varicella-Zoster Virus (VZV) infection as the etiology. A unilateral vesicular facial rash accompanied by rapidly declining visual acuity over 7 days prompted consultation. Intravenous antiviral treatment and panretinal photocoagulation laser (PPR) were initiated, followed by antiviral prophylaxis. The patient developed ophthalmic zona lesions, peripheral retinal ischemia, and unilateral macular serous retinal detachment (SRD). Treatment included valacyclovir, corticotherapy and PPR laser. Knowledge of this condition, diagnosed clinically, is crucial due to its rapid progression and severe complications. Early intervention appears to be a significant prognostic factor, emphasizing its therapeutic urgency.","PeriodicalId":397187,"journal":{"name":"Global Academic Journal of Medical Sciences","volume":" 43","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140385594","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to compare outcomes between using the right internal mammary artery (RIMA) and the great saphenous vein (GSV) as secondary conduits in Coronary Artery Bypass Grafting (CABG), focusing on early and mid-term postoperative results. Methods: A retrospective analysis of 270 patients who underwent CABG between November 2015 and January 2023 at Mohamed VI University Hospital was conducted. Patients were divided into RIMA and GSV groups for comparative analysis. Results: There were no significant differences in the preoperative characteristics of the study population. Patients in the RIMA group had lower cardiopulmonary bypass time and aortic cross-clamping time (110.70±34.08 vs 127.45±31.17: p<0.00 and 77.67±26.31 vs 82.93±23.66: p<0.00, respectively). The use of GSV was associated with more significant bleeding compared to RIMA (570.46±403.66 vs 761.40±647: p<0.00). However, there were no significant differences in the occurrence of deep sternal wound infection (3.24% vs 4.31%: p=0.8945) between the two groups. Regarding early mortality rates, there was no significant difference observed (5.19% vs 2.58%: p=0.4458). Mid-term results showed no difference in the recurrence of angina and dyspnea between the groups. Conclusion: The study indicated that RIMA used as a secondary graft in CABG wasn't linked to sternal wound infection or bleeding, suggesting its superiority as a graft choice. The resulting defect poses more complex repair challenges. We report a case of chondrolysis of the nasal cartilages as a sequel to rhinophyma treatment. Our objective was, based on this case and literature data, to present our therapeutic approach. This concerned a 65-year-old patient, presented with a voluminous rhinophyma evolving for 15 years, causing not only aesthetic impairment but also nasal obstruction. The excision was performed in one piece with an electric scalpel, hemostasis achieved using a bipolar coagulating forceps while preserving the nasal cartilages. The excision site was left for directed healing. Postoperative outcomes for our patient were marked by transfixing apexo-alar nasal substance losses following chondrolysis of the nasal cartilages. Faced with this defect, a repair surgery was undertaken not only to fill the substance loss but also to restore an appearance as normal and aesthetic as possible, ensuring functional reconstruction.","PeriodicalId":397187,"journal":{"name":"Global Academic Journal of Medical Sciences","volume":"85 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139171522","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An estimated 80% of all children have had at least 1 episode of OME by the age of 10 years, with a peak of prevalence in the first 2 years of life. Objectives: The aim of the study was to evaluate the effects of COVID-19 on otitis media with effusion in a tertiary care hospital. Methods: This cross-section observational study Rangpur Medical College & Hospital Rangpur, Bangladesh. The duration of the period from July 2021 to July 2022. A total of 80 children, Age <6 months or >12 years with Otomicroscopic evidence of tympanosclerosis, choesteatoma, eardrum perforation, or complete stenosis or atresia of the external auditory canal were included in the study. Severely ill children and not willing to participate were excluded. After collection, the data were checked and cleaned, followed by editing, compiling, coding and categorizing according to the objectives and variable to detect errors and to maintain consistency, relevancy and quality control. Statistical evaluation of the results used to be obtained via the use of a window-based computer software program devised with Statistical Packages for Social Sciences (SPSS- 24). Results: Before pandemic 9.52% of the respondents had <3 years of age, 23.80% were 3-7 years, 57.14% were 7-10 years and 10% had 10-12 years of age. During pandemic 11.42% of the respondents had <3 years of age, 19.04% were 3-7 years, 61.90% were 7-10 years and 7.62% had 10-12 years of age. No significant difference in mean age before and during pandemic. Before pandemic 65% of the respondents were male and 35% were female. During pandemic 40% of the respondents were male and 60% were female. Before pandemic 28.57% respondents were underweight, 57.14% were in normal range and 14.28% were overweight. During pandemic 26.67% were underweight, 55.23% were in normal range and 18% were overweight. The prevalence of OME was 40.6% before pandemic and 2.3% during pandemic. Children with chronic OME had a higher rate of disease resolution during pandemic (95%) than those examined before pandemic (5%, P .001). Conclusion: The findings of our study, in our opinion, have important clinical implications, implying that keeping children at home for as little as 2 months may allow for the resolution of most cases with severe and refractory OME.
