The incidence of postoperative nausea and vomiting (PONV) and the severity of pain following paediatric tonsillectomy were examined by comparing concentrations of DEX 0.15 and DEX 0.5 mg/kg, respectively. Aim of the study: The aim of the study is to assess the effect of Dexamethasone in management of postoperative nausea, vomiting and pain after tonsillectomy where the targeted patients are of 3 to 10 years children. Methods: This prospective cross sectional study was conducted in the Department of Paediatric Anaesthesiology and Surgical ICU in Bangladesh Shishu Hospital and Institute, Dhaka, Bangladesh during July 2019 to June 2022. This study included 123 children undergoing elective tonsillectomy. The Institutional Ethics Committee approved this study and informed parental written consent was obtained from the parents before collecting the patients’ information. Results: The prevalence of early PONV was lower in both DEX groups (DEX 0.15: 22%; DEX 0.5: 26%; placebo: 48%; P=0.001). On the second day after surgery, the DEX groups had a lower rate of severe pain (DEX 0.15: 20%; DEX 0.5: 5%; placebo: 47%; P<0.001). The study was not conducted to assess a difference between the two dose groups of Dexamethasone. Conclusions: The frequency of early and late PONV and the level of pain on the second postoperative day were both reduced by an i.v injection of DEX during the induction of anesthesia. However, it is challenging and also rewarding both for patients and treating physicians. BSMMU hospital has given maximum effort for its greater success. The aim of this study was to share our ten years’ experience regarding some aspects of live related renal transplantation. Methods: This retrospective study was conducted in nephrology department of BSMMU hospital from January 2002 to December 2015. Data were collected from hospital records and some previously conducted study on these transplant recipients. Results: A total of 360 live related kidney transplant recipients were evaluated during this period. Recipients Male: Female ratio was 1.57:1. Mean age of recipients were 39.58 ± 10.46 years. The causes of ESRD were chronic glomerulonephritis 220(61.60 %), diabetic nephropathy 58(16.24%), hypertensive nephrosclerosis 22 (6.16 %), chronic interstitial nephritis 11 (3.08 %), SLE 10(2.8%), ADPKD 6(1.68 %), unknown 19(5.32 %). Most of the donors were mother (21.84% %) followed by spouse mostly wife (20.44%) and sister (18.76 %). Almost all recipients were on MHD 352(97.78%), 2 were on CAPD and 06 were pre-emptive transplantation. Triple immunosuppressive protocol Cyclosporine or Tacrolimus, MPA or Azathioprine and Prednisolone were used in each patient. Recipients with poor HLA matching received Baciliximab in standard dose. CMV prophylaxis was given in selected patients and each patient received pneumocystis jirovecii prophylaxis. Common complications during post-operative period were ATN 41(11.48%), DGF 23(6.44%), Acute rejection 50(14%) and infection mainly urinary tract infection 46(12.88%) and RTI 14(3.92) followed by wound infection and other surgical complication. Presence of BK virus infection was studied in 29 transplant recipient and it was found to be positive in 6(20.7%) cases. Protocol biopsy was done in 37 transplant recipient in the 2008-2009 on day 14, and day 90 to see subclinical rejection and early graft dysfunction. On day 14th biopsy report showed 21(56.7%) normal histology, 5(13.5%) had subclinical rejection, 5(13.5%) had clinical rejection, 4(10.8 %) developed ATN, 2(5.2 % ) cyclosporine toxicity, and report at 3 month showed normal histology 18(48.60%), subclinical rejection 7(18.90%), clinical rejection 5(10.80%). Leading cause of chronic allograft dysfunction was chronic allograft nephropathy (CAN) 60(19.80%) followed by chronic cyclosporine toxicity 37(12.21%) and de-novo or recurrent glomerulonephritis. Mean post-transplant hospital stay was 18.46 ± 5.56 days. Mean duration of normalization of serum creatinine after surgery was 7.38 ± 3.88 days. At discharge 74.40% patients had normal renal function with mean serum creatinine 1.10 ± 0.26 mg/dl and 21.34% patients showed gradual improvement of renal function with mean serum creatinine 2.12 ± 0.97 mg/dl. The purpose of this study was to evaluate the metabolic effects of fenugreek seed (FS) and its bioactive compounds such as Fenu pulse (FP), Fenu fiber (FF), Diosgenin (Dsg) in High Fructose High Fat (HFHF) induced diabetic rats.After hyperglycemia was confirmed in HFHF rats compared with controls, further HFHF groups were supplemented with FS, FP, FF, Dsg, and combinations of these compounds. The HFHF diet was given for 4 months which increased the body weight, body fat percentage, and decreased lean body mass (LBM) and fat-free mass (FFM) percentage compared with controls rats. The diet combination was continued for another four months and it did not show any further effect on their body composition. There was a significant but marginal effect on fasting glucose and insulin, so the animals did not have overt hyperglycemia, let alone be diabetic. The FS or its components individually or in combination within a diet-induced rat model of diabetes showed no significant antidiabetic effect (glucose tolerance/metabolic parameters), both in the fasting and fed states wherein it elevated the levels of total and HDL cholesterol but not triglycerides (TG) upon continuous feeding for four months. In many studies, hearing loss is described as a consequence of otitis media (OM); the degree of hearing loss can be analyzed by biologically calibrated AA 222 diagnostic audiometer. But in Bangladesh, we have not enough research-based information regarding this issue. Aim of the Study: The aim of the study was to evaluate the correlation of the degrees of hearing loss between male and female patients with otitis media. Methods: This was a prospective observational study and was conducted in the Department of Otolaryngology & Head Neck Surgery, Bashundara Ad-Din Medical College and Hospital, Dhaka, Bangladesh during the period from January 2022 to December 2022. A total of 64 patients with chronic otitis media (COM) suffered for the past three months or more attended the mentioned hospital were enrolled in this study as the study population. Among them 32(50%) were male and other 32(50%) were female patients. Ear drums of patients were inspected on both sides suction under microscope wherever necessary. The patient’s hearing levels in decibel were tested by a biologically calibrated AA 222 diagnostic audiometer. All data were collected, processed and analyzed by using MS Office and SPSS version 23.0 programs as per need. In comparison student’s T-tests were also performed. Results: In this study, in analyzing the hearing loss status of the patients regarding ear involvement we observed that, 6(18.8), 14(43.7) and 12(37.5) male participants on the other hand 10(31.2), 11(34.4) and 11(34.4) female patients right, left and both ears were found involved respectively. In comparing the ear involvement in hearing loss between the male-female groups, we did not find any significant correlation (P=0.427). In comparing the right dB grading, left dB grading, right ear comments and left ear comments of the patients in both groups we did not find any significant correlation; the P- value were 0.932, 0.983, 0.228 and 0.618 respectively. This study aimed to evaluate the survival rate and prosthetic complications of implant-supported overdentures in the edentulous upper jaw. Methods: This prospective study was carried out at Banasree Dental and German Dental implant surgery centers from December 2011 to December 2021 where 78 patients were included in this study and evaluated clinically and radiographically. A total of 20 patients were excluded as they did not agree to or maintain the follow-up schedule. After this procedure, 58 patients (24 male and 34 female) were evaluated for this study. Results: The mean age of the patients was 61.3± 6.8 years whereas the 24 (41.4%) were male. The total survival rate of implants was 94.8% at the 5-year follow-up and 92.2% at the 10-year follow-up. In the 3.0 mm diameter implants, the lowest survival rate was 70.6% at the 5-year follow-up and 52.9% at the 10-year follow-up, whereas in the 3.8 mm diameter implants it was 95.1% and 93.4%, in the 4.2 mm diameter implants it was 97.8% and 96.7%, and in the 4.6 mm diameter implants it was 94.8% and 92.2%, respectively. Most of the 29 implants had prosthetic tooth fractures, followed by 18 premature wear of gaskets, 15 prosthesis loosening/shedding, 9 damaged prosthesis flanges, 4 damaged baskets, and 3 implants with screw loosening. Conclusion: It is concluded that implant-supported maxillary overdentures have a high rate of survival and are a safe and reliable treatment option. CHD is about 25% of all congenital malformations and is the most common type of heart disease among children. It may present in different ages from birth to adolescence. Worldwide, CHD is relatively common with a prevalence ranging from 1.0-6.6 per 1000 live births. Aim of the Study: The aim of this study was to find out the pattern of congenital heart diseases among less than 5 years old Children of a pediatric cardiac center. Methods: This was a retrospective observational study and conducted in the Department of pediatric cardiology of Combined Military Hospital Dhaka, Bangladesh during the period from July, 2018 to July, 2020. A total of 1000 patients from outdoor and indoor patient registry who were diagnosed and treated as a case of congenital heart disease within the mentioned period were included in the study. Result: Majority of our patients was male (58%) compared to female (42%). Among all patients, 29.80% had VSD, followed by 20.4% & 14.9% had ASD & TOF respectively and 72% & 28% of our patients had Acyanotic & cyanotic CHD respectively.VSD & TOF was the most common in patients with acyanotic & cyanotic CHD respectively. Recurrent RTI, pneumonia, heart failure, infective endocarditis, and Cyanotic spell are common complications of congenital heart disease. Down syndrome and congenital rubella syndrome are the main syndromic association in congenital heart diseases. The majority (97%) of our patients improved, and 3% died. Conclusion: Our study focuses on the congenital heart diseases of children of Bangladesh. Among the acyanotic congenital heart disease, VSD is the maximum & among the cyanotic congenital heart disease TOF is the highest in number. We still utilize Halothane, which has a delayed recovery profile, despite the fact that rapid clearance drugs like Sevoflurane have a positive impact on early cognitive recovery, which permits early mobility and reduces post-surgical complications. Objective: The study's goal was to promote the use of sevoflurane over halothane by contrasting the two anesthetics' effects on patients' cognitive functioning after surgery. Method: The prospective randomized comparative study was carried out in the Department of Anaesthesia, Analgesia and Intensive Care Medicine, between the period of September 2021 to June 2022. Study population was the patients of either sex, aged between18-50 years, ASA-I and II, patients undergoing elective surgery by general anaesthesia, lasting for 1 hour or more and remaining 24 hours after surgery. A total number of 100 patients were divided into two groups. 50 patients were in Sevoflurane group and 50 in Halothane group. Results: Group- Sevoflurane had a considerably faster emergence time (10.85 min vs. 15.13 min, P0.001) than Group- Halothane. Group- Sevoflurane had a mean BAMSE score of 29 at baseline and showed no change by the conclusion of the observation period, but Group- Halothane saw their scores both drop and then recover. Patients in Group- Sevoflurane finished the (TMT-A) 30 minutes after regaining consciousness in a considerably shorter amount of time (40. 9 seconds vs. 55. 8 seconds, P 0.001). Furthermore, there was no statistically significant difference between the groups after 1.5 hours, 2.5 hours, or 3.5 hours. Conclusion: So the study concludes that adult patient of sevoflurane group experienced an early post-operative cognitive recovery than halothane group. {"title":"Clinical and Radiology Finding of Patients with Bilateral Optic Disc Swelling","authors":"Kazi Nasimul Hoque","doi":"10.36348/gajms.2023.v05i03.005","DOIUrl":"https://doi.org/10.36348/gajms.2023.v05i03.005","url":null,"abstract":"Introduction: Optic disc swelling (ODS) is a pathological condition with a variety of causes, including optic neuritis (ON), anterior ischemic optic neuropathy, and papilledema. Determining the causes of ODS is critical due to the possibilities of vision-or life-threatening diseases, such as space-occupying lesions. Objective: To assess the clinical and radiology finding of Patients with Bilateral Optic Disc Swelling. Methods: This cross sectional study was conducted in the Department of Ophthalmology, Ad-din Akij Medical College Hospital, Khulna, Bangladesh from January to June 2019. One hundred patients with bilateral disc swelling were selected as study population where bilateral disc swelling due to congenital disc anomaly, pseudo disc edema or need emergency medical care had been purposively excluded from the study. All patients were subjected to detailed ophthalmic examination, including visual acuity (VA), red saturation, bright sensitivity, color vision, and detailed slit lamp examination. All the information’s were recorded in a pre-designed data collection sheet. Results: Total 100 patients included in our study. Most commonly affected age group was between 21-30 years in which 32% case are observed, least common affected age group was 51-60 yr. in which 6% cases are observed. Male patients were 33% and female were 67%. The patients by presenting complaints were headache 71.0% followed by dimness of vision 63.0%. Nausea/ vomiting were present in 42.0% patients and ocular pain had 21 (21.0%) patients. Few (5.0%) had transient loss of vision. Among the patients who had IIH 34 (34%), ICSOL were 25(25%) and grade 4 Hypertensive retinopathy 9% respectively. Among the patients who had bilateral optic neuritis and VKH majority of them 13% and 13% respectively. In right eye, 44 (44.0%) had Visual acuity >0.3 while in left eye, 40 (40.0%) had Visual acuity >0.3. One third had (33.0%) sluggish pupillary response in both eye. Majority of the patients (right: 67.0%, left: 64.0%) had only disc swelling. Seventy-three patients (73.0%) did not have any ocular manifestation other than optic disc swelling while 14 (14.0%) had diplopia, 12 (12.0%) had uveitis and 1 (1.0%) had ptosis. Out of 100 patients, 70 patients (70.0%) did not have any space occupying lesion while 9 (9.0%) had meningioma and 6 (6.0%) had Cerebellopontine (CP) angle tumor. Dermatological lesions are commonly encountered in all countries and it encompasses a wide spectrum, varies from country to country and various regions within a country. Skin diseases are much common in developing countries. The spectrum varies according to geographic distribution, gender, age, and coexisting disorder. We conducted this study to find out the prevalence of different skin lesions and to evaluate their frequency and site of distribution. Aim of the Study: The aim of this study was to find out the prevalence of different skin lesions and to evaluate their frequency and site of distribution. Methods: This was a descriptive study and was conducted in the Department of Histopathology of Bangladesh Specialized Hospital, Dhaka, Bangladesh during the period from January 2019 to December 2021.We included 133 skin biopsies in our study. Result: Among 133 skin biopsies examined, non-infectious vesicobullous and vesicopustular disease were found in 53 (48.18%) cases followed by microbial disease in 20 (18.18%) and non-infectious erythematous papular and squamous disease in 28 (21.05%) cases. Lichen simplex chronicus was the most common vesicobullous disease seen in 08 (15.09%) cases. Dermatophytes were the commonest microbial disease found in 10 (50%) cases. The commonest non- infectious erythematous papular and squamous disease was lichen planus and psoriasis 08 (15.09%) cases. The commonest neoplastic lesion was keratinocytic tumor seen in 13 (56.53%) cases. The commonest tumor of the skin was seborrheic keratosis and squamous cell carcinoma in 13 (56.53 %) cases. Upper extremities were the most frequently involved site by skin lesions. Conclusion: In our study prevalence of lichen simplex chronicus was higher. We observed a wide spectrum of skin lesions ranging from dermatitis to malignant neoplasm. The importance of specific histomorphological features lies in distinguishing various skin lesions and play a major role in making the final diagnosis of these diverse skin lesions. {"title":"Quality of Life in Children with Minimal Change Nephrotic Syndrome","authors":"Tanzina Parveen, Tasmina Parveen, Nandalal Sutradhar","doi":"10.36348/gajms.2023.v05i03.003","DOIUrl":"https://doi.org/10.36348/gajms.2023.v05i03.003","url":null,"abstract":"Introduction: Nephrotic syndrome (NS) in children is a common, yet challenging, relapsing, and remitting renal disorder and exhibits a heterogeneous clinical phenotype ranging from a single episode, infrequently relapsing, frequently relapsing to steroid-resistant disease. Although 80% of these children are corticosteroid responsive, nearly half of them demonstrate a frequently relapsing or steroid-dependent course, often resulting in multiple complications, hospitalizations, or even chronic renal failure. Objective: To assess the quality of life in patients with minimal change nephrotic syndrome. Methods: This cross-sectional, comparative, questionnaire-based study was conducted in Dhaka Shishu Hospital, Dhaka, Bangladesh July to December 2022. The cases included children with MCD, attending the pediatric nephrology unit and outpatient clinic of nephrology unit. 100 children were selected then they were randomly subdivided into 2 groups, group 1 included 50 known cases of MCD aged 2–18 years and group 2 included 50 age matched children attending the general pediatric outpatient clinic and other pediatric subspecialty clinics. We distributed two questionnaires to 32 outpatients with MCNS. We also used the Self-Care Behavior Scale for patients with chronic kidney disease (CKD), which consists of 31 questions with 4 subscales. Results: Total of one hundred studied nephrotic patients with MCD and an equal number of controls with other chronic diseases aged 2.2–15 year and 3.5–13 years, respectively, were included in the study. Table 1 illustrated the demographic details of the studied cases and controls. Among children with MCD, 32% had first attack or infrequently relapsing variant while 68% had difficult to treat clinical phenotypes (frequently relapsing, steroid– dependent and steroid-resistant varieties). The SF-36v2 social functioning subscale was most impaired and bodily pain was least affected in patients with MCNS. The self-care subscales of information/communication and positive behavior had positive correlations with the QOL subscales of mental health (𝑃 < 0.05) and vitality (p<0.05). The correlation between social functioning and information/communication was close to significant (𝑃 = 0.051). Conclusion: In conclusion, our findings suggest that patients with MCNS have lower QOL based on low social functioning and that QOL is related to the positive behavior and thoughts of the patients.
