Rapidly adopted in the USA with remarkable effectiveness, 7-valent PCV was then introduced in Europe and the UK. In France, implementation has been more gradual, and the impact of this vaccination has been less obvious due to rapid serotype replacement, despite the efficacy of the vaccine serotypes. Here, we present efficacy results for France, based on time-series studies of pneumococcal meningitis, invasive pneumococcal infections, pneumonia and pleuropneumonia, acute otitis media and invasive infections in adults.</div><div>© 2024 The Author(s). Varon","doi":"10.1016/S0987-7983(24)00153-1","DOIUrl":"10.1016/S0987-7983(24)00153-1","url":null,"abstract":"<div><div>The nasopharyngeal microbiota represents a complex ecosystem made up of several hundred bacterial species, f ve of which are frequently associated with paediatric infections: <em>Streptococcus pneumoniae</em> (Sp), <em>Haemophilus influenzas</em> (Hi), <em>Moraxelle catarrhalis</em> (Mc), <em>Streptococcus pyogenes</em> and <em>Staphylococcus aureus</em> (SA), and will be discussed here.</div><div>The introduction of pneumococcal conjugate vaccines, however, represented a veritable ecological revolution in the respiratory tract microbiota, altering the carriage of pneumococci, causing vaccine serotypes to be replaced by other, less pathogenic serotypes, and reducing both invasive and non-invasive infections.</div><div>The impact of vaccination programs with conjugate vaccines, carriage in healthy children and in children with acute otitis media are reported here through a study of over 20,000 children over the past 25 years.</div><div>© 2024 The Author(s). However, serotypes 3, 19A, and 19F persist at low levels in children, and new non-vaccine serotypes (NVTs) now predominate in IPD. Among the new pneumococcal vaccines approved in Europe, PCV20 covers more than 40% of invasive isolates, compared to less than 10% for PCV15. Some NVTs like 24F, not included in PCV20, remain problematic. The choice of future vaccines will depend on their ability to cover these replacement serotypes.</div><div>©2024 The Author(s). However, some serotypes are more frequently observed in pneumococcal infections than in carriage, suggesting an higher disease potential. The spectrum of disease induced by each serotype varies considerably: some serotypes are primarily associated with pneumonia, while others are more frequently implicated in meningitis or other specific infections. Respiratory viruses play a crucial role in triggering pneumococcal infections, particularly respiratory syncytial virus and influenza viruses. Once again, virus-bacteria cooperation varies according to serotype and viruses.</div><div>© 2024 The Author(s). Différents modèles multidimensionnels de l’intelligence sont proposés, mais en première instance, il est indispensable de recueillir les éléments d’anamnèse du comportement au sein de l’environnement familial, scolaire, social et des loisirs qui étayeront le profil HPI. Si le seuil requis est au moins un QI Total de 130, il peut être cependant diminué lors de trouble du neurodéveloppement ou autres anomalies avec la présentation d’un profil d’indices hétérogènes. Il convient que le praticien psychologue clinicien puisse à tout le moins calculer l’IAG, et surtout effectuer une interprétation clinique très fine du profil des indices, plutôt que de se contenter d’un « ticket de loto » à remettre. En effet, une différence de plus de 29,5 entre l’indice le plus fort et le plus faible met en exergue un profil hétérogène chez le HPI qui doit être pris en considération afin de ne pas passer à côté de l’existence d’une comorbidité ou co-occurrence de troubles à investiguer avec d’autres évaluations standardisées, selon les fonctions cérébrales concernées, sans omettre l’évaluation du domaine moteur et corporel, trop souvent négligé. Ce dernier étant plutôt en avance de maturation dès le plus jeune âge, mais force est de constater qu’il existe beaucoup de préjugés sur l’identification du HPI qui serait en dyssynchronie entre différents domaines, alors que c’est un fait de recrutement biaisé par un recueil d’enfants HPI issu de consultations cliniques.&lt;/div&gt;&lt;/div&gt;&lt;div&gt;&lt;div&gt;The notion of High Intellectual Potential (HIP) is closely linked to the development of IQ assessments, although there is no true international consensus. Various multidimensional models of intelligence are proposed, but initially, it is essential to gather anamnesis elements of behavior within the family, school, social, and leisure environments to support the HIP profile. While the required threshold is generally a Total IQ of at least 130, it can be lowered in cases of neurodevelopmental disorders or other anomalies, presenting a profile with heterogeneous indices. The clinical psychologist practitioner should at least be able to calculate the General Ability Index (GAI) and especially perform a very fine clinical interpretation of the index profile, rather than merely providing a “lottery ticket”. Indeed, a difference of more than 29.5 points between the highest and lowest index highlights a heterogeneous profile in the HIP that must be taken into account to avoid overlooking the existence of comorbidity or co-occurrence of disorders, which should be investigated with other standardized evaluations, depending on the brain functions concerned.
