Belhadj","doi":"10.1016/j.neurenf.2024.11.007","DOIUrl":"10.1016/j.neurenf.2024.11.007","url":null,"abstract":"<div><h3>Aim</h3><div>This paper attempts to illustrate the semiology of temporal epilepsy in patients initially considered to be suffering from psychiatric disorders.</div></div><div><h3>Methods</h3><div>We report five clinical vignettes of patients referred to the Child and Adolescent Psychiatry Department of Mongi Slim Hospital (Tunisia) between 2017 and 2021 for psychiatric disorders, in whom the diagnosis of temporal epilepsy was retained.</div></div><div><h3>Results</h3><div>Four boys and one girl, aged three to eleven, were referred for paroxysmal neuropsychiatric features. Family history of epilepsy was noted in three patients. Two children had psychomotor and language delay. Symptomatology was dominated by cognitive and behavioral disorders. Electroencephalogram confirmed the diagnosis of temporal epilepsy by showing local paroxysmal abnormalities. Brain imaging had shown predominantly temporal abnormalities in two cases. The authors used a multidimensional approach to the examination and attempted to weave links between different elements. In addition to inter-individual differences, they found recurring clinical pictures including learning difficulties, interpersonal problems, and poor emotional regulation. On the other hand, similarities in life courses were pointed out both in cognitive profiles and in psycho-affective functioning. The discussion highlights the need to adopt an anamnestic approach, linking linguistic and educational trajectories, migratory pathways, and adverse life events. Corcos","doi":"10.1016/j.neurenf.2025.08.003","DOIUrl":"10.1016/j.neurenf.2025.08.003","url":null,"abstract":"<div><h3>Background</h3><div>The magnitude and nature of sexual violence (SV) are increasingly described in child and adolescent psychiatry, but the mechanisms underlying their impact remain to be understood.</div></div><div><h3>Objective</h3><div>This article describes the characteristics of SV identified among adolescents hospitalized in psychiatry as well as the associated clinical features.</div></div><div><h3>Method</h3><div>The study included 100 patients for whom SV was identified among all patients aged 13 to 17 years hospitalized over a period of approximately four years. The characteristics of SV were correlated with patients’ medical severity, as well as with the number and duration of hospitalizations and the delay in disclosure.</div></div><div><h3>Results</h3><div>Our findings highlight the central role of SV in adolescent psychiatry, with a prevalence of 28.5% and a cumulative length of hospitalization five times longer than average. Correlations were observed between the number of adolescents’ suicide attempts and the number of SV episodes. Medical severity increased when the identified perpetrator was an adult. A 35-item grid was completed by the clinicians involved for each family to document the referral's context, socio-demographic, medical, and psychiatric characteristics, the ICD-10 diagnoses, and the care provided. These features were compared to the 2021 French National Survey on Perinatal Health data (NSPH).</div></div><div><h3>Results</h3><div>Most cases were referred by maternities, with visits starting in the antenatal period for 33% of the families. Situations of social deprivation (i.e., related to housing, income, or social protection access) were over-represented in these families compared to data from the national survey (Odd-Ratios [OR]<!--> <!-->=<!--> <!-->8.2), as well as the contexts of violence during pregnancy (OR<!--> <!-->=<!--> <!-->12.0). Half of these families were naïve about any psychological or psychiatric care. The mean number of visits was 7.79<!--> <!-->±<!--> <!-->8.26, with the participation of different types of professionals (e.g., psychologist, motor therapist, childcare worker). The most common maternal psychiatric disorders were stress and trauma-related disorders, followed by depressive and anxiety disorders. Understanding parental representations related to the diagnosis is seen as a crucial component of therapeutic work. Nine families followed in a mental health center called Centre Médicopsychologique were interviewed through semi-structured sessions with an interpreter, using the McGill Illness Narrative Interview method. It responds to the need to develop a new and original tool used by psychomotricians to guide and refine their clinical observations in this field.</div></div><div><h3>Patients and methods</h3><div>The grid takes up and puts to work preliminary studies on cohorts of 8-month-old babies, but it also involves the networking of several dozen psychomotricians around its use. It is based on a quantitative and qualitative approach.</div></div><div><h3>Results</h3><div>The grid was constructed as a guide for observing specific elements exploring different areas in terms of psychomotricity and is divided into five areas of observation (first observations, the baby's psychomotor discoveries, parent/child interaction and links, the baby's response to the psychomotrician's requests, links between the clinician and the family) with specific items presented in detail for each area. It gives pride of place to issues relating to parents.</div></div><div><h3>Discussion</h3><div>In terms of its experimental use, the grid is original in relation to existing supports and is adapted to the needs of psychomotricians working in this clinical field. The positive feedback received from professionals who have used it testifies to its usefulness.</div></div><div><h3>Conclusions</h3><div>The genesis and initial uses of GAPPEA suggest that it should be used in new ways in the field of clinical approaches to babies.
