(Literature review)].","authors":"E F Legkaya, L S Khodasevich, A V Polyakova","doi":"10.17116/kurort202510202161","DOIUrl":"https://doi.org/10.17116/kurort202510202161","url":null,"abstract":"<p><p>The use of information and communication technologies (ICT) in comprehensive rehabilitation and habilitation of patients with autistic spectrum disorder (ASD) in current conditions seems to be actual, however it is not sufficiently illustrated in the literature to date.</p><p><strong>Objective: </strong>To analyze present scientific data on the comprehensive rehabilitation and habilitation of children with ASD by restoring lost or forming absent skills and abilities by adapted physical education using current ICT.</p><p><strong>Material and methods: </strong>A review of publications on cognitive, motor and social disorders' correction in children with ASD using ICT presented in the RSCI, PubMed and Google Scholar databases was carried out. The search was performed by the following keywords: autistic spectrum disorder, rehabilitation and habilitation, adapted physical education, information and communication technologies.</p><p><strong>Results: </strong>ICT in the form of technical teaching aids are used, including remotely, at classes of adapted physical education in patients with ASD to develop gross and fine motor skills, improve movements' coordination, enrich motor experience and increase physical activity, and from the side of higher mental functions - to develop memory, attention, speech and to improve perceptive processes. Shishonin)].","authors":"A Yu Shishonin, V I Pavlov","doi":"10.17116/kurort202510203118","DOIUrl":"https://doi.org/10.17116/kurort202510203118","url":null,"abstract":"<p><p>The influence of the brain on blood pressure and metabolic processes are described by the fundamental concepts of the egoistic brain and the theory of aerobic-anaerobic imbalance of the theory of centralized compensation of aerobic-anaerobic energy balance. In this regard, the study of manual-physical influence (therapy) according to the method of A.Yu. Shishonin on improving the functioning of the brain with a decrease in systemic arterial pressure in patients with essential arterial hypertension is of interest.</p><p><strong>Objective: </strong>To study metabolism in patients with EAG using manual physical therapy according to the method of A.Yu. Shishonin.</p><p><strong>Material and methods: </strong>A total of 134 patients with EAG were examined, divided into 2 groups. The main group included 102 patients of both sexes (58 women and 44 men; average age 59.8±4.9 years), with signs of clinically significant degenerative-dystrophic changes in the cervical spine. The comparison group (control group) consisted of 30 patients (15 men and 15 women; average age 59.4±3.6 years) with EAG of the same degrees and stages, who, for one reason or another, refused both the recommended pharmacotherapy and the proposed alternative non-drug treatment methods. The patients of the main group used the author's method of A.Yu. Shishonin - manual impact on the cervical region to restore vertebral blood flow with subsequent consolidation of the effect using a special patented system of physical exercises. The main method was gas analysis at rest, and the main indicator was the VCO<sub>2</sub>/VO<sub>2</sub> ratio, known as the respiratory exchange ratio (RER).</p><p><strong>Results: </strong>In the main and control groups, the average respiratory quotient was initially 0.77 and 0.78, respectively, which corresponds to the metabolism of fats and carbohydrates in a ratio of approximately 74 and 26%. Then, in the study group, RER reliably increased to 0.79 after the first therapy session. After the last procedure, a pronounced statistically significant increase in RER to 0.87 was observed, which corresponds to the prevalence of glucose metabolism over fatty acids in a ratio of approximately 38% (fatty acids) vs 62% (glucose). In the control group, RER did not undergo significant changes.</p><p><strong>Conclusions: </strong>Considering that the main consumer of glucose is the brain, and the impact of the A.Yu. (Literature review)].","authors":"V F Reps, V V Kozlova","doi":"10.17116/kurort202510201166","DOIUrl":"10.17116/kurort202510201166","url":null,"abstract":"<p><p>Application of radon baths with natural and artificial radon-containing waters is one of the common methods of non-drug therapy in the treatment of inflammatory and degenerative diseases of different origin in sanatoriums and wellness centers in many Russian and foreign health resorts. Radon therapy is most often used as an additional method in the structure of multimodal health resort treatment in diseases of the musculoskeletal system. The article presents an analysis of Russian and foreign experience in the application of radon-containing mineral waters and evidence-based approaches in the study of cellular and molecular mechanisms of radon action. Experimental data on the quantification of intake and excretion of radon and its daughter products after the application of mineral water with different radon concentrations are provided on the biokinetic models. According to the literature, radon procedures have analgesic and anti-inflammatory effects, affecting osteoclastogenesis and bone resorption. The authors associate this therapeutic effect of radon with contribution of the immune system and change in the Th17/Treg-cell ratio after receiving balneotherapy. Arthrogenic muscle inhibition, immediately following injury in some cases, leads to quadriceps muscle hypotrophy and weakness, that further embarrasses the athlete's good-quality rehabilitation.</p><p><strong>Objective: </strong>To assess clinical and functional effectiveness of passive electrical myostimulation (EMS) as a muscle preoperative preparation to autotransplantation of knee anterior cruciate ligament.</p><p><strong>Material and methods: </strong>The study involved 47 male athletes with ACL injury. Summarized surface electromyography (EMG) was performed in all participants at admission, as a result of which 28 patients with reduced EMG parameters were selected. Among them, 14 patients received a course of passive EMS of quadriceps on physiotherapeutic <b>INTELECT Advanced</b> (Chattanooga (DJO), USA) hardware. All patients got passive EMS for 2 weeks after surgery and started active muscle training since 3rd week. There is a continuing debate about the sources of this pain and reasonableness of search for them. There is no doubt that discogenic pain prevails over other probable sources of pain in the spinal area and intervertebral hernias play the primary role here. Patients can immediately look for a solution to the problem by a neurosurgeon at the time of «intervertebral disc herniation» diagnosis establishment and in the presence of a pronounced reflex or radicular pain syndrome. It is the «intervertebral disc herniation» wording which often leads patients to a neurosurgeon. There are many methods of intervertebral hernias conservative treatment. This article is focused on the technology of spinal traction therapy, which has a scientific evidence base, including in the question of natural reduction of intervertebral hernias' dimensions. These effects were found in different functional systems, including in central organs of neurohormonal and immune regulation - hypothalamus, epiphysis, thymus. At the same time, the morphofunctional features and general patterns in adaptive and protective reactions in these organs remain unexplored, that is important for development of new effective methods of postradiation damages prevention using drinking sulfate MW.</p><p><strong>Objective: </strong>To perform comparative analysis and generalization of experimental study results in preventive action of drinking sulfate MW on developing adaptive intracellular reactions in neurons of hypothalamic paraventricular nucleus, pineal cells of epiphysis and thymocytes of thymus in radiation exposure to rats.</p><p><strong>Material and methods: </strong>Experiments were conducted on 28 non-linear male rats with a weight of 220-250 gr. The animals of experimental group received drinking sulfate MW with magnesium and calcium (2.6 g/l sulfate concentration, 4.0 g/l mineralization), the animals of control group received main water instead of sulfate MW; intact animals, which were not exposed, were also used. MW and main water were administered to rats intragastrically through a needle with olive at the end by 3 ml once per day for 21 days. The animals of experimental and control groups were irradiated once on «GUBE» apparatus (Institute of General Genetics of the RAS) by <sup>60</sup>Co gamma-rays in dosage of 2 Gy (0.66 Gy/min dosage rate) after watering course. Animals killing was carried out by decapitation 1 month after radiation exposure. The following methods of examination were used: transmission electronic microscopy (study of ultrastructures in neurons of hypothalamic paraventricular nucleus and in pineal cells of epiphysis), biochemical and morphofunctional analysis (study of chromatin in thymocytes nuclei using acridine orange, nuclei quantity and their genome activity), morphometry.</p><p><strong>Results: </strong>Preventive use of drinking sulfate MW had a cytoprotective effect, which was manifested in the reduction of postradiation damages intensity in studied organs, enhancement of adaptive reactions in the form of increased stability of membranes and cellular structures. The higher level of preventive adaptation and resistance to radiation has been found in neurons of hypothalamic paraventricular nucleus, lower - in cells of the epiphysis and thymus. These adaptive changes were caused by antioxidant, membrane-stabilizing and immunomodulating effects of MW on different regulatory and functional systems of the body, as well as on membrane structures of cells directly. Nevertheless, they are the most effective and safe for improvement of aerobic capacity in case of individually adjusted intensity of the load and with a strict pulse control during the exercises.</p><p><strong>Objective: </strong>To evaluate the effectiveness and safety of aerobic cyclical trainings on cycle ergometers in children in the long term after surgical correction of CHDs and to study the features of short-term adaptation of the child's body to exercises on stationary bicycles in setting of rehabilitation center depending on the type of CHD.</p><p><strong>Material and methods: </strong>The study included 80 children with CHDs aged between 10 and 17 years in the long term after Fontan procedure, aortic valve replacement, surgical correction of aortic coarctation or septal defects hospitalized for rehabilitation course in 2023. Intensity of exercises on stationary bicycles was determined based on clinical examination results in the absence of contraindications. The trainings were performed under the control of electrocardiography, tonometry and oxygen saturation; the intensity of exercises was automatically adjusted depending on the reaction of the cardiovascular system to the load.</p><p><strong>Results: </strong>Anthropometric and anamnestic data of the studied group were analyzed. Low physical activity was characteristic of most children with CHDs, that has affected the results of exercise tolerance test. A number of trainings equal from 6 to 9 (Me=6) were performed during the course of cardiac rehabilitation. A statistically significant increase in the load in training sessions during hospitalization, comparing the first exercise both with the last training (<i>p</i>=0.05) and with one with the maximum capacity (<i>p</i><0.001), has been found. The dynamics of load in trainings according to the type of CHD was analyzed, including a deeper analysis of subgroups of children after Fontan procedure, as well as individual clinical examples of examinations and trainings of children with different CHDs in case of re-hospitalization were presented. No adverse events were noted during the study.</p><p><strong>Conclusion: </strong>Aerobic cyclical trainings on cycle ergometer are safe for children with CHDs with adequately selected intensity of trainings. {"title":"[Static delay exercises for improvement of proprioception and movements control in children with impaired intelligence (on the example of walking pattern correction)].","authors":"D V Fedulova, K A Berdyugin","doi":"10.17116/kurort202510201132","DOIUrl":"10.17116/kurort202510201132","url":null,"abstract":"<p><p>Children with mental retardation, due to the inertia of cortical processes, have changes in the motor sphere development, which are reflected in both mechanisms of motion control and postural disorders of the musculoskeletal system.</p><p><strong>Objective: </strong>To evaluate the effectiveness of static delay exercises in improving motor control and development of proprioception in children with mental retardation.</p><p><strong>Material and methods: </strong>The study involved 80 children aged 12-14 years. For comparison, 4 groups of 20 patients in each were formed: the 1st, the 2nd groups - children with a mild degree of impaired intelligence; the 3rd, the 4th groups - healthy children. Completion of two courses of exercises throughout the academic year was proposed to experimental groups. The emphasis of static exercises performed with rubber expander was focused on strengthening the core muscles as a main segment, providing the body's stability and participation in locomotions.</p><p><strong>Results: </strong>Significant differences in the changes of the indicators of dynamic and biomechanical disorders of movement patterns in children with impaired intelligence, primarily related to the improvement of movement coordination and motor act regulation mechanisms were identified.</p><p><strong>Conclusion: </strong>Static delay exercises are effective for improving proprioception and movements control in children with impaired intelligence. Impact of shockwave therapy (SWT), high-intensity laser therapy (HILT) and pharmacopuncture (PP) and combinations of these physiotherapeutical factors were assessed according to the criteria of the International Classification of Functioning, Disability and Health (ICF).</p><p><strong>Results: </strong>Dynamics of 15 domains of ICF, including the dynamics of the leading ICF domain in DA - b28013 «Pain in back», was studied. Reliable rehabilitation predictions for the positive dynamics of ICF b28013 «Pain in back» domain in patients with DA have been established in the form of equations of SWT, HILT and PP or their combined application impact regression. Effects of the studied physiotherapy variants were compared.</p><p><strong>Conclusion: </strong>SWT is the most effective of the applied monotherapy in relation to the dynamics of b28013 «Pain in back» domain, HILT - on the second place and PP - on the third place. {"title":"[Influence of physical therapy on intestinal microbiota, blood pressure control and systemic inflammation level: a randomized controlled study of 84 patients with arterial hypertension].","authors":"T I Petelina, K S Avdeeva, L L Valeeva, S G Bykova, E V Zueva, E F Dorodneva","doi":"10.17116/kurort202510202111","DOIUrl":"10.17116/kurort202510202111","url":null,"abstract":"&lt;p&gt;&lt;strong&gt;Objective: &lt;/strong&gt;To analyze the clinical and organ-protective effects of different 12-month therapeutic intervention schemes, including a balanced diet enriched with dietary fibers, course application of prebiotics, PT courses, in presence of basic antihypertensive therapy in patients with arterial hypertension (AH) of the 1st-3rd degree.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Material and methods: &lt;/strong&gt;The study included 84 patients with AH of the 1st-3rd degree, randomized to 4 groups. Basic antihypertensive pharmacological therapy was prescribed in the 1st group (control; 22 patients with AH; mean age 51.82±11.89 years); balanced diet with dietary fibers in addition to antihypertensive therapy - in the 2nd group (21 patients; mean age 52.90±9.97 years; balanced diet with dietary fibers and prebiotic (Plantago ovata Forssk) in addition to antihypertensive therapy - in the 3rd group (20 patients; mean age 47.65±11.56 years); pharmacological therapy and balanced diet with dietary fibers and PT course - in the 4th group (21 patients; mean age 53.52±9.50 years). The groups were comparable in age, degree of AH, pharmacological therapy and presence of concomitant pathology. We conducted a laboratory study of the biosamples of blood and faeces at the time of hospitalization. The following parameters were evaluated: metabolites of intestinal microbiota - short-chain fatty acids (SCFA) in the faeces (gas chromatography (GC) with mass spectrometry /GC with plasma ionization detection), endotoxin (chromogenic method based on enzymatic reaction, LAL test set); level of proinflammatory markers - proinflammatory cytokines (interleukin (IL)-1β, IL-6, IL-8, FNO-α (enzyme-linked immunoassay, «sandwich»-method, IMMULITE 2000 (USA)), hsCRP, homocysteine (Stat Fax 4200, USA). We performed: 24-hour monitoring of blood pressure (BP) (BPLaB apparatus, OOO «Petr Telegin», Russia) according to the standard protocol; study of elastic properties of the vascular wall by sphygmography (Vasera VS-1000 Series apparatus, Fukuda Denishi, Japan) with assessment of the following indicators: PWV - pulse wave velocity and CAVI - parameter of the vascular wall rigidity stiffness by the arteries of elastic type.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Analysis of the obtained data has shown that there was a significant reduction in waist and hips volume in patients of the 2nd, 3rd and 4th groups (&lt;i&gt;p&lt;/i&gt;=0.049, &lt;i&gt;p&lt;/i&gt;=0.001, &lt;i&gt;p&lt;/i&gt;=0.025 and &lt;i&gt;p&lt;/i&gt;=0.007, &lt;i&gt;p&lt;/i&gt;=0.001, &lt;i&gt;p&lt;/i&gt;=0.051, respectively).
